One-hundred and thirty-five children between the ages of 7 and 18 years were evaluated clinically. Their diagnoses included Fetal Alcohol Syndrome (FAS) or Effects, Schizophrenia, Bipolar Mood Disorder, various neurological diseases, Attention Deficit Hyperactivity Disorder (ADHD), Conduct Disorder, Oppositional-Defiant Disorder and learning disabilities. As part of a comprehensive neuropsychological assessment, the children were given the Word Memory Test (WMT; Green, Allen, & Astner, 1996; Green & Astner, 1995), containing various subtests which measure, respectively, effort and verbal memory. Although age and verbal intelligence are known to affect scores on most ability tests, they were not found to be significant determinants of WMT effort scores. Younger children did not score any lower on the effort subtests than older children. The children scored as well as a group of parents seeking custody of their children and they scored higher than adult patients with mild head injuries. The computerized WMT requires some basic reading skills and some children with lower than a grade 3 reading level scored at a relatively low level on the effort subtests. The current data suggest that most children with at least a grade 3 reading level can pass the WMT using the adult criteria. It is concluded that the WMT is potentially useful in the evaluation of effort during pediatric neuropsychological evaluations. Further research is needed to replicate these findings and to develop child norms for the memory subtests.  相似文献

Assessing effort level during neuropsychological evaluations is critical to support the accuracy of cognitive test scores. Many instruments are designed to measure effort, yet they are not routinely administered in neuropsychological assessments. The Test of Memory Malingering (TOMM) and the Word Memory Test (WMT) are commonly administered symptom validity tests with sound psychometric properties. This study examines the use of the TOMM Trial 1 and the WMT Immediate Recognition (IR) trial scores as brief screening tools for insufficient effort through an archival analysis of a combined sample of mild head-injury litigants ( N = 105) who were assessed in forensic private practices. Results show that both demonstrate impressive diagnostic accuracy and calculations of positive and negative predictive power are presented for a range of base rates. These results support the utility of Trial 1 of the TOMM and the WMT IR trial as screening methods for the assessment of insufficient effort in neuropsychological assessments.  相似文献

Six same-different matching tests, both verbal and nonverbal, in three modalities, along with a set of reading tests, were administered to 120 Israeli children in second, third, and fourth grade. The main effects of all S variables, except sex (grade, socioeconomic level, and ability) were significant, as were the test factors of modality (visual, auditory, cross-modal) and form (verbal-nonverbal), but interactions between S and test factors were small. Multiple regression analysis revealed that overall matching test scores accounted for 35% of the variance in reading scores, although the additional contribution of specific subtests was negligible. Performance on the visual-visual tests was virtually perfect. Auditory-auditory matches were more difficult than auditory-visual matches with nonverbal stimuli, while the reverse was true with verbal stimuli.  相似文献

The assessment of effort is a fundamental component of test performance analysis, since effort determines whether a psychological evaluation is valid. The assessment of effort in children has proven problematic. This may be related to the variable and inconsistent nature of children’s developing self-regulatory systems, and the fact that measures commonly used to assess effort were standardized on adults. If one uses effort measures designed for adults to assess children, then one must presume that the maintenance of effort in children is comparable to the same behavior in adults. However, because children’s executive functioning, including their abilities to self-regulate, attend, concentrate, and to engage in various cognitive activities improve with time (Barkley, 1997, pp. 209-234), our hypothesis is that young children’s effort regulation is dissimilar to that of adults, and the presumption of similarity is implausible. The purpose of this study was to determine whether age is a significant influence upon young children’s performances on the Computerized Assessment of Response Bias (CARB) and Word Memory Test (WMT). Statistical analysis suggests that younger children (those under 10 years of age) tended to produce poorer performance on these instruments. Younger children’s scores differed significantly from children ages 10 and older. Children 11 years and older produced CARB and WMT results similar to adult participants, suggesting a viability for adult normative comparisons with children in this age range. The current investigation concluded that children’s maintenance of effort appears to be significantly related to age and reading ability level. Consequently, the use of current adult-based norms with the CARB and WMT, without regard for a child’s developmental status and other contextual factors such as the child’s ability to read, appears ill-advised especially with children under 11 years of age.  相似文献

A variety of neurodevelopmental impairments related to fetal alcohol spectrum disorder (FASD) diagnoses have been consistently documented. However, it is not clear whether such variables are predictive of a diagnosis. The purpose of the present study is to use logistic regressions to identify predictors of FASD in neuropsychological assessment. Charts of 180 children and adolescents with prenatal alcohol exposure (PAE) who underwent psychological and diagnostic assessment for FASD were retrospectively reviewed. A total of 107 received an FASD diagnosis (the PAE-FASD group) and 73 did not (the PAE group). Following preliminary analyses, direct logistic regressions were performed to assess the contribution of different neuropsychological testing measures on the likelihood of a child or adolescent receiving an FASD diagnosis. The results indicate that the classification accuracy of the PAE-FASD and PAE groups is clinically significant across models of intelligence, academic achievement, memory, and executive functioning. Classification rates across the various models range from 67.1% to 75.5%, with models incorporating 10 intelligence subtests or 3 academic subtests emerging as superior to those using broad indices of intelligence and/or individual subtests of memory or executive functioning. A “test battery” model incorporating verbal intelligence, verbal/auditory working memory (digit span), basic reading and spelling skills, math calculations, delayed story recall, and spatial planning and problem-solving yielded a classification rate of 74.7%. These results suggest that neuropsychological testing is a critical component of FASD assessment and help guide decisions to maximize the efficiency and efficacy of the diagnostic process and treatment recommendations.  相似文献

The current study reports on an intervention study that investigated the relative contributions of 5‐week English language training (ELT) and 5‐week working memory training (WMT) to improvements in English language knowledge and working memory capacity in university learners of English as a foreign language. The participants were allocated to one of the following three experimental groups and a control group: (i) ELT only; (ii) WMT only; and (iii) both WMT and ELT. The results showed that intervention effects were pronounced in the WMT + ELT group, who improved scores on the reading span test and all tests of verbal and visuo‐spatial short‐term and working memory. The gains on verbal short‐term (Japanese) and working memory (Japanese and English) were maintained 3 months later. None of the interventions, single or combined, were associated with gains in foreign language knowledge, however. The sustained effects of the dual approach and its implications are discussed. The present study evaluated rates of forgetting on verbal and nonverbal memory tests in children diagnosed with Attention Deficit Hyperactivity Disorder (ADHD). Although children with ADHD are known to have poor organizational and attentional problems that may impair memory performance, we hypothesized that savings scores would remain intact, because poor recall reflects difficulty with initial learning versus memory per se. Fifty-seven children diagnosed with ADHD were administered tests of nonverbal memory (Rey-Osterreith Complex Figure) and verbal memory (Story Memory, Wide Range Assessment of Memory and Learning) as part of a comprehensive neuropsychological evaluation. Subjects showed no evidence of rapid forgetting of information across the delay on either nonverbal memory (mean savings score=93%) or verbal memory (mean savings score=88%) tests. Cognitive remediation for children with ADHD should address ways to initiate and execute organizational strategies so that more information is learned upon immediate presentation.  相似文献

This study examined immediate memory processes in specific reading disability subtypes. Three subgroups (n = 15 in each subgroup) of reading disabled children were examined: (a) perceptual-motor disorder, (b) verbal disorder-general, and (c) verbal disorder-specific (memory). The three groups were matched for age and full scale IQ. All children received a memory-for-colors task (Color Span Test) designed to evaluate intra- and intermodal serial memory functioning. Comparison of memory profiles for the three reading disability subtypes revealed that patterns varied depending on mode of stimulus presentation or response. Although all three groups performed considerably below normative levels on each of the four subtests of the Color Span Test, all subjects performed significantly better on verbally presented items than on visually presented items. The findings were interpreted to suggest that these reading disabled children may not consistently utilize verbal strategies for coding or retrieval of information when stimuli are visually presented.  相似文献

Abstract

Progressive decline in language has been described in neuropsychological studies on Alzheimer’s disease and Mild Cognitive Impairment (MCI). We investigate this decline in its early stages, as well as its manifestations and the linguistic differences between participants at risk of developing MCI and without impairment. Participants were 36 older adults, all aged over 65. They were assessed using 31 subtests grouped into eight areas: fluency, comprehension, oral expression, reading, writing, memory, perceptual organization and processing speed. Our results showed significant differences between participants at risk of developing MCI and without impairment in verbal tests but not in manipulative tests. All participants at risk of developing MCI scored lowest in these eight areas. Our findings illustrate the relevance of investigating language skills in people at risk of developing MCI. Investigating these neuropsychological changes will help design appropriate intervention programmes that affect areas with higher levels of impairment.  相似文献

This study examines the validity of the measures of verbal and nonverbal working memory on the Stanford-Binet Fifth Edition (SB5). The validity evidence included Rasch-based, criterion-referenced item mapping, correlations with other clinical measures of memory, and prediction of reading and mathematics scores. The item mapping clearly demonstrates a parallel between increasing item difficulty and a progression of item characteristics that placed increasing demands on verbal and nonverbal working memory. The higher correlations of the SB5 verbal and nonverbal working memory subtests with other measures of verbal and nonverbal memory, respectively, and lower correlations with nonverbal and verbal memory measures, respectively, clearly show convergent and divergent validity. The higher correlations between SB5 verbal working memory and reading skills and between SB5 nonverbal memory and mathematics skills are consistent with past research.  相似文献

This study investigates the role of linguistic and visuospatial attentional processes in predicting reading fluency in typical Italian readers attending primary school. Tasks were administered to 651 children with reading fluency z scores > ?1.5 standard deviation to evaluate their phonological awareness, rapid automatized naming (RAN), verbal short-term memory, vocabulary, visual search skills, verbal-visual recall, and visual-spatial attention. Hybrid models combining confirmatory factor analysis and path analysis were used to evaluate the data obtained from younger (first and second grade) and older (third–fifth grade) children, respectively. The results showed that phonological awareness and RAN played a significant role among younger children, while also vocabulary, verbal short-term memory, and visuospatial attention were significant factors among older children.  相似文献

It examined the performance of children aged 48 to 67 months on tests of attention/executive function, language, memory, and sensorimotor abilities, as measured by the NEPSY and Conners' K-CPT. Two hundred thirty-seven children were divided into four subgroups based on mothers' report of behavior using rating scales and a diagnostic interview: hyperactive only (HYP), oppositional-defiant only (OD), hyperactive and oppositional-defiant (HYP/OD), and nonproblem. Children in the HYP/OD group scored significantly worse than nonproblem children on four of nine subtests on the NEPSY, including one test of executive function, one test of language comprehension, and both tests of short-term verbal memory. However, only the test of executive function (Statue) showed significant predictive power, and, while specificity of this subtest was good, sensitivity was poor. On the K-CPT, a continuous performance test, children in both the HYP and HYP/OD groups performed worse than children in the OD and nonproblem groups. When the NEPSY Statue subtest and the K-CPT were used together, overall predictive power was .74. Results suggest that neuropsychological deficits can be observed among preschool children with hyperactivity, particularly when comorbid oppositional-defiance is present; however, moderate predictive power suggests that these tests should be used in conjunction with other methods of assessment.  相似文献

During the school years, psychological test norms may be indexed by age or by grade. A number of studies have shown that using age-based norms appears to produce biases associated with grade assignment. Cahan and Cohen [Child Dev. 60 (1989) 1239-1249] showed that the effect of one grade was over twice the effect of 1 year of age for most verbal cognitive ability subtests in Grades 4-6. A higher ratio might be expected for more curriculum-related areas such as reading tests. Analysis of a representative sample of 4257 Grade 1 and 2 children in Tasmanian government schools (which use relatively strict age assignment to grades) for three subtests of the Woodcock Reading Mastery Test—Revised (WRMTR): Word Identification, Word Attack, and Passage Comprehension, showed that the grade effect is about twice the age effect. This data shows that using age-based norms instead of grade-based norms for reading and other verbal ability tests may produce bias in the early school years. Psychologists should thus be primarily concerned with children's educational history, as once children have entered school other developmental factors indexed by age have less influence on children's verbal performance.  相似文献

From elementary through high school, Japanese children are required to memorize a large number of distinct visual forms, i.e., roughly 2,000 Chinese characters, and tremendous effort is expended in learning to read and write them. We hypothesized that early training in memorizing Chinese characters and the use of these characters in daily life shapes brain development and facilitates recall of visual forms in general, including geometrical figures in visual recall tests. To demonstrate whether the learning of Chinese character is correlated with specific aspects of memory, we administered the Wechsler Memory Scale-Revised (WMS-R) to a representative sample of the normal Japanese population (316 persons, 100% Japanese) and compared their scores with data previously obtained from a representative sample of the normal U.S. population (316 persons, 82.5% Caucasian). The WMS-R has 12 memory subtests, each measuring a different aspect of memory and includes two visual recall subtests (Visual Reproduction I and II). Compared to the Americans, the Japanese group obtained significantly higher scores on these two visual recall subtests in each of the six age groups tested (16 to 74 years old). Further, the scores of Japanese on the two visual recall subtests did not decline across age groups as much as scores for the U.S. group. Japanese also scored significantly higher on the Visual Memory Span subtest in three age groups.  相似文献

The aim of the present study was to (a) examine the prevalence of ADHD and the comorbid difficulties in a sample of 114 children, 3.6 to 17.6 yr. of age (89 boys, 25 girls) referred to our Unit and (b) evaluate the discriminative ability of the WISC-III scores for children with Attention Deficit Hyperactivity Disorder (n = 22), Learning Disability (n = 50), and Language Disorder (n = 42). Analysis showed only 18% of 114 children had an Attention Deficit Hyperactivity Disorder diagnosis. Multivariate analysis of variance and stepwise discriminant function analysis were applied. Vocabulary and similarities were the best predictors for distinguishing between language disorders and the other two groups. Moreover, the Language Disorder group scored significantly lower on all the subtests while the Attention Deficit Hyperactivity Disorder and Learning Disability groups scored lower on coding and information, respectively. Children with Attention Deficit Hyperactivity Disorder and Learning Disability could not be accurately identified from the WISC-III test or their ACID profile.  相似文献

The purpose of this study was to investigate verbal working memory processing both before and after providing semantically elaborated training sentences designed to enhance memory for symbol-word (visual-verbal) pairs. Abilities of 20 children diagnosed with Reading Disorder (RD) and 20 age-matched peers who were normally achieving in reading (NA) were compared (M = 10 years old). Results demonstrated RD children experienced significantly more difficulties on measures of complex auditory-verbal working memory than their NA peers. The best predictor of reading performance was word recall ability measured after students were provided with semantic training sentences. Findings have important implications for identifying young children with potential reading impairment.  相似文献

This study compared the Wechsler Intelligence Scale for Children-III (WISC-III) scores of traumatized youth with posttraumatic stress disorder (PTSD) to the scores of trauma-exposed and nonexposed comparison groups without PTSD. All groups were free of additional major childhood psychiatric disorders. The PTSD group scored significantly lower than the comparison groups on verbal subtests, but not on performance subtests. The scores of the trauma-exposed PTSD negatives and nontrauma exposed controls were not significantly different. Accordingly, PTSD and not a history of trauma exposure in the absence of PTSD was associated with lower verbal IQ.  相似文献

Borderline personality disorder (BPD) is often linked to neuropsychological deficits, yet few studies have examined BPD or its features and concomitant neuropsychological dysfunction in childhood. This study examined children with borderline features (n = 21) using the Coolidge Personality and Neuropsychological Inventory for Children and compared them to controls (n = 21) with features of at least one personality disorder, but not BPD. As hypothesized, the BPD group scored significantly higher than the control group on the Attention Deficit/Hyperactivity Disorder, Executive Functions Deficits, Mild Neurocognitive Disorder, Conduct Disorder, and Oppositional Defiant Disorder scales. It appears that behavioral disturbances associated with BPD are linked strongly with neuropsychological dysfunction. Because none of the children with BPD features in the present sample had a history of traumatic brain injury (TBI), it appears likely that TBI in the histories of adult BPD patients may not be the cause of BPD, but traits such as anger and impulsivity in BPD may cause TBI. Clinical implications and future research directions are discussed.  相似文献
