As we work through our theories, we rejoice in discovering the logic of concepts and their articulation in clinical experience. Such joy hides other movements, in which the only stakes are demands for love, sexual curiosity and the satisfaction of hate, transferred from infancy to others worlds, idealised or despised. When we participate in our psychoanalytical institutions, we feel pride or even vanity to be participating in the great historical movement of psychoanalysis. This conceals the working of transference in our institutional life; transference which is crudely sexual at its origins, and implies maternal, paternal or fraternal objects. This is its constraining power. We should be fully aware of this, as Strachey or Balint have already shown, in order to attenuate the violence of transference and stimulate our creativity. Anna Freud and Melanie Klein's controversies had a reality of their own. This reality was quickly forgotten and trapped in various conflicts, as those existing between Jones and Freud or between Glover and the rest of the British Psychoanalytical Society of his times. These re-elaborations gave rise to ideological elements, which have ever since pervaded the history of psychoanalytical theory, influencing psychoanalytical methods and clinics. It is important for us today to elucidate these ideologies in order to improve our achievements.  相似文献

The development of combined individual and group psychotherapy is split between the “separationists” and the “integrationists” with regard to therapists' handling of the interface between the individual and group sessions. To bridge the impasse, a high-benefit–low-risk series of guidelines is offered for therapists that includes techniques for analyzing patients' “cross-references” to the other component modality. The four guidelines provide for a harmonious balance that preserves the confidentiality of the individual sessions vis-à-vis the group, while enhancing the group interaction rather than draining it. The therapist guidelines maximize patient initiative in integrating parts of the self expressed in the individual and group sessions, while employing the least intrusive methods necessary to best integrate the processes into a unified combined modality.  相似文献

With the rapid progress and considerable promise of nanobiotechnology/neurosciences there is the potential of transforming the very nature of human beings and of how humans can conceive of themselves as rational animals through technological innovations. The interface between humans and machines (neuro-digital interface), can potentially alter what it means to be human, i.e., the very idea of human nature and of normal functioning will be changed. In this paper, I argue that we are potentially on the verge of a paradigm shift in terms of the ends and goals of techno-science and its applications in the biomedical sciences. In particular, the development of brain-computer interfaces could reconceptualize the very notion of what it means to be human. Hence, we should not limit our reflections of applications in terms of therapy and enhancement but also include an examination of applications aiming at the alteration of human nature. To this end I will first delineate the potential paradigm shift and then map out four distinct clusters of concerns in relation to the brain-computer interface. Finally, I argue that our moral and philosophical reflections should follow a procedural model based on managed consensus due to our pluralistic context.

If a patient comes for psychotherapy or psychoanalysis, why medicate him with psychotropic drugs? The answers to this question are explored and addressed from several points of view. The central hypothesis of this paper suggests that most of the reasons for medicating patients have to do with therapist's unresolved countertransference issues. Furthermore, medicating a patient may have deleterious effects on his introjected sense of self, and impacts the transference relationship in a significant way. The issues surrounding medication as adjunctive therapy are elucidated and clarified in terms of rationale, efficacy, and the impact on the psychotherapeutic relationship. An ongoing controversy in the field of group psychotherapy is whether transference regression is greater in groups than in individual treatment. There appears to be a general consensus that group behavior may be understood as operating on at least three levels—the conscious–rational, the oedipally determined transference, and the preoedipal maternal transference level. The more rational levels are associated with transference dilution, whereas the more primitive levels accompany transference intensification. Dilution occurs as a result of reality demands and inputs of the group situation and because of multiple targets of displacement. Transference intensification is a product of mutual stimulation, contagion effect, frustrating inputs, and support of the group theme. Certain patients benefit most from the dilution features of a group, and others profit most from intensification. The therapist's technique and the patient's pathology are the main determinants of which aspect gets emphasized and utilized.  相似文献

Transference as a theoretical construct within the conceptual framework of psychoanalysis is examined, pointing out the limitations this imposes on its use. After discussing why the term transference cannot be applied to psychological testing, the ways in which it has been misused are presented. The concept of externalization is introduced as an important means of understanding the structural dynamics of patients being tested, and its relevance to transference is stressed.  相似文献
