This article offers a theory about the evolution of small group systems through a series of stages. It discusses the relationship of these stages to and their effect on the self-other and therapeutic interventive spheres in the long-term group treatment of children. The developmental unfolding is seen as universal to small groups and as operating on an interpersonal and collective level, distinguishable from individual psyches and close dyadic bonds but influenced by them. On an isomorphic level the group stages contain emotive, cognitive, and interactive themes and processes that parallel and evoke those of the bonding and work processes. As such, the group dynamic provides an enabling matrix and a mirror for the other goings on in the member-therapist encounter. Its pacing and its progressions and regressions appear to correspond with those of the development of the transitional space and therapeutic culture of the group. Like the other articles in the symposium, this one offers specific examples in which the three spheres intersect and are integrated.  相似文献

During adolescence the peer group plays a major role in facilitating developmental processes, since it not only serves as a replacement for childhood love-objects and attachments, but offers the adolescent allies in the struggle to separate and become emancipated from parents. For those adolescents who, because of interpersonal and intrapsychic difficulties, have no peer group or have chosen an inappropriate peer group, providing a therapeutic group can make it possible for development to proceed. This paper presents not only a theoretical basis for the utilization of groups of peers in the treatment of adolescents, but also an elaboration on the dynamic processes which make possible improvement of ego-functioning and resolution of core conflicts related to the past, as well as to this particular developmental phase.  相似文献

Psychodynamic group therapy, by definition, offers group members an opportunity to revisit some of the earliest developmental stages. The authors argue that conflicts around separation and individuation are stimulated with each beginning and ending of an intimate group. The leader of that group is similarly challenged around these primary conflicts. The question of group contagion, difficulties around the capacity to be alone, and the problems of projective identification are explored as they have an impact on leaders in a group. Case examples are offered to illustrate leadership problems along these dimensions.  相似文献

ABSTRACT

The study explores the therapeutic factors in two types of group interventions–-film therapy (n = 69) and affective support therapy (n = 48) for Arab adolescents (ages 13–14) in Israel. Film therapy is a relatively new method for groups, aimed at enhancing adolescents’ engagement in the process of group counseling. Six-hundred students from 11 schools were screened for the level of parent–adolescent conflict, based on a self-report measure. The 117 students who met the cutoff point constituted our sample. They were randomly divided into 12 small groups (6 in each type of intervention). A previous study reported the outcomes of these two interventions, showing equal progress in both groups (compared to control). The current study explores process variables, specifically therapeutic factors, in an attempt to enhance our understanding of those outcomes. Results indicated common therapeutic factors across both types of intervention. The factors of relationship–climate and other- versus self-focus were ranked the highest and, contrary to expectations, were significantly higher in affective support therapy than in film therapy.  相似文献

Abstract

Background: Transgender and non-binary children and young people and their parents in England, UK are poorly served across a range of healthcare settings. Whilst UK equalities legislation and international guidance on transgender healthcare pathways protects this group from discrimination and mandates an affirmative approach, services in England are not keeping pace.

Aims: This study aims to draw on the experiences of transgender and non-binary children, young people and their parents in a support group in England in order to investigate their experiences of healthcare provision, and to develop some ideas for improvement.

Method: Data was collected with participants in a family support group which offers a parent helpline service, social groups for children and parents, and training for schools and other organizations. 65 parents and children from 27 families from the family support group attended participatory workshops where they were given a range of briefs: “health,” “family,” “friends,” and “education.” Their participation involved being asked to define their own interview questions and collect data by interviewing each other. Their interview notes constituted the raw data. Data was coded inductively by the author with respondent checking as a second stage.

Results: Results constitute the views of a small group of people, so cannot be generalized. However, they do illustrate some of the issues which may arise. Participants’ experiences elicited five key themes: professionals’ perceived lack of clinical and therapeutic knowledge; mental distress caused by excessive waiting lists; professionals’ stereotyped gender assumptions; direct discrimination within healthcare settings; and a lack of attention to parent and child voice, especially in terms of school-based experiences and where a patient had a diagnosis of autism.  相似文献

Video records of five different sessions of a single therapy group were scored for the occurrence of laughing, smiling, and talking. Successive five-minute intervals were also scored for therapeutic level through ratings derived from the Hill Interaction Matrix. Humorous remarks were categorized according to humor target: self, other in group, and generalized other. Results indicated that the vast majority of humorous remarks were directed at some specific target, and that over 50% of these remarks were negative in tone. Results also revealed that remarks targeted at others in the group tended to decrease therapeutic effectiveness whereas remarks targeted at individuals or institutions not presently in the group were found to increase therapeutic effectiveness. Selftargeted remarks were found to produce inconsistent effects. These findings are discussed in terms of their significance for a more general analysis of group humor as well as in terms of their more specific implications for therapeutic interventions.  相似文献

This paper argues that Foucault's late, unpublished lectures present a model for evaluating those ethical authorities who claim to speak truthfully. In response to those who argue that claims to truth are but claims to power, I argue that Foucault finds in ancient practices of parrhesia (fearless speech) a resource by which to assess modern authorities' claims in the absence of certain truth. My preliminary analytic framework for this model draws exclusively on my research of his unpublished lectures given at the Collège de France between 1982–84. I argue that this model proceeds in three stages: the truth‐teller is first established as independently authoritative, he is subsequently tested under conditions of risk, and the encounter concludes by generating trust and a relation of ‘care’ with the audience. Foucault's model results in an ‘aesthetics of existence’ organized around a set of ethical practices, and thus offers an alternative to other forms of ethical subjectivity. In so doing, this model also critiques the place for risk in liberal political institutions.  相似文献

Combined therapy is defined as concurrent dyadic and group therapy with the same therapist. This paper focuses on a neglected aspect of combined therapy: broaching and exploring this question with one's individual, patient. The author shows how raising this question: (1) alters the therapeutic field whether or not the patient joins; (2) can sharpen the therapist's understanding of the indications and contraindications for group membership; (3) offers untapped therapeutic approaches and opportunities to clarify issues in the individual treatment; and (4) stirs up countertransference issues related to issues of profitability, the transference of the patient in individual treatment, and the transference of the group-as-a-whole. ABSTRACT

Part 1: Theoretical Models of Right Brain Therapeutic Action. The first part of this article on the central role of the right brain in group psychotherapy offers evidence-based theoretical models of therapeutic action cocreated by the group members and the group leader. It describes how recent advances in interpersonal neurobiology and neuropsychoanalysis allow for a deeper understanding of the underlying nonverbal right brain change mechanisms beneath the words in individual psychotherapy. It then expands this model to the group context, specifically focusing on the theoretical constructs of cohesion, attachment, transference-countertransference dynamics, and implicit affect regulation, all of which are right brain functions. Part 1 concludes with a discussion of the fundamental role of these right brain mechanisms in synchronized group regressions and reenactments of attachment trauma that allow for new beginnings in emotional and relational development. Part 2: Clinical Case Analyses of Group Right Brain Regressive Enactments. The second part of this article offers case examples and commentary on working with early dysregulated attachment histories and the affect blunting defense of dissociation. Clinical vignettes demonstrate how the group reenacts attachment dynamics in transient regressions into an earlier stage of preverbal development, outside of the domain of language. Such emotionally shared regressions of attachment trauma, rupture, and repair allow the group members and leader to companion each other into and out of enactments. In this manner, regulated reenactments of preverbal emotional experiences potentially allow the cohesive group to expand adaptive right brain capacities to regulate and communicate a broader range of affectively charged subjective self states, thereby cocreating new ways of being with others.  相似文献

Prospective memory involves the self-initiated retrieval of an intention upon an appropriate retrieval cue. Cue identification can be considered as an orienting reaction and may thus trigger a psychophysiological response. Here we present two experiments in which skin conductance responses (SCRs) elicited by prospective memory cues were compared to SCRs elicited by aversive stimuli to test whether a single prospective memory cue triggers a similar SCR as an aversive stimulus. In Experiment 2 we also assessed whether cue specificity had a differential influence on prospective memory performance and on SCRs. We found that detecting a single prospective memory cue is as likely to elicit a SCR as an aversive stimulus. Missed prospective memory cues also elicited SCRs. On a behavioural level, specific intentions led to better prospective memory performance. However, on a psychophysiological level specificity had no influence. More generally, the results indicate reliable SCRs for prospective memory cues and point to psychophysiological measures as valuable approach, which offers a new way to study one-off prospective memory tasks. Moreover, the findings are consistent with a theory that posits multiple prospective memory retrieval stages.  相似文献

Abstract

This article challenges Honneth’s reading of Hegel’s Philosophy of Right in The Pathologies of Individual Freedom: Hegel’s Social Theory (2001/2010). Focusing on Hegel’s method, I argue that this text hardly offers support for the theory of mutual recognition that Honneth purports to derive from it. After critically considering Honneth’s interpretation of Hegel’s account of the family and civil society, I argue that Hegel’s text does not warrant Honneth’s tacit identification of mutual recognition with symmetrical instances of mutual recognition, let alone his subsequent projection of symmetrical forms of mutual recognition onto the various spheres of the Philosophy of Right as a whole. I conclude by indicating an alternative way in which Hegel’s text might be used to understand contemporary society.  相似文献

The single most common referral problem in middle childhood and adolescence is academic underachievement and learning failure. Yet, the term “learning disorder” lacks specificity as a diagnostic entity and offers no guidelines for psychotherapy. An appreciation of the developmental processes by which the undifferentiated, unstructured, self-less and objectless newborn becomes a more fully developed, self-sufficient individual, capable of adaptive functioning and formal academic learning is encompassed in contemporary psychoanalytic conceptions of the separation-individuation process. This theory provides an organizing, theoretical base for diagnosis and treatment of learning disorders. Based on these conceptions, we discuss therapeutic techniques which are appropriate to the level of developmental arrest at each of the phases of the separation-individuation process.  相似文献

Most psychotherapists are aware of the many social issues and cultural factors that influence development throughout each individual life. Human nature, being a social nature, exists within relationships to others, the community, and a culture, sometimes a mixture of cultures. One social parameter, social class, redefined here as psychosocial class, has not received the same attention from psychotherapists as gender issues or ethnicity, two other important sociocultural spheres of influence on everyday life. It is suggested that group therapists are in a unique position to develop a cultural psychotherapy that addresses psychosocial class and cultural aspects of mental health. This paper includes a preliminary model of cultural psychotherapy that highlights the psychology of social class and culture for psychodynamic group psychotherapy.  相似文献

This article presents a cultural adaptation of a group therapeutic approach that is being offered in the Bahamas. “The Family: People Helping People” project was designed as an intervention to improve socialization in New Providence, the Bahamian capital and its most heavily populated city. “The Family” group model offers support and training to improve communication in relationships and to encourage constructive emotional expression. This article will provide an overview of “The Family,” address key elements of this approach that are culturally tailored, and offer clinical examples and note implications for group therapy training. This cultural adaptation offers helpful insights for addressing community problems, such as violence and societal fragmentation, and may inform the development of community-based group interventions in other settings.  相似文献

The Clown Club is a short term therapeutic group that makes use of structured fantasy and creative drama as the vehicle for the expression and resolution of conflict. In this group, children are invited to create a pretend identity, a clown character which can become a metaphorical expression of themselves. Each child creates a life story, and the group as a whole constructs a tale of how they came together and how they get along interpersonally. This story encompasses all group stages, from its beginning, through the central task of planning and performing a circus show, to its termination.

In this paper we describe the background that led to the development of the Clown Club. Details of how the group is designed and carried out are presented, along with clinical examples and an appraisal of the model at this stage.  相似文献

Abstract

It is a well-established fact that forming a mature and coherent political identity is one developmental task in adolescence and young adulthood. However, given different degrees of commitment on the regional, national, and European level, the question remains whether young people’s identification varies among those spheres? Drawing on data from the European Catch-EyoU-project, it was the goal of this study to examine whether young people can be classified according to their identification toward their home country and Europe and how these types are associated with age, gender, country as well as political interest, tolerance, and political participation. The study is based on adolescents and young adults from the Czech Republic, Germany, Great Britain, Greece, Estonia, Italy, Portugal, and Sweden (N = 9339; Mage = 19.62; 59.1% female). Cluster analysis revealed five types of young people’s identification with country and Europe which showed significant associations between group membership and tolerance, political interest, and participation. The implications of distinguishing types of identification and their associations with political outcomes are discussed.  相似文献

Depression and anxiety are common during adolescence. Whilst effective interventions are available treatment services are limited resulting in many adolescents being unable to access effective help. Delivering mental health interventions via technology, such as computers or the internet, offers one potential way to increase access to psychological treatment. The aim of this systematic review and meta-analysis was to update previous work and investigate the current evidence for the effect of technology delivered interventions for children and adolescents (aged up to 18 years) with depression and anxiety. A systematic search of eight electronic databases identified 34 randomized controlled trials involving 3113 children and young people aged 6–18. The trials evaluated computerized and internet cognitive behavior therapy programs (CBT: n = 17), computer-delivered attention bias modification programs (ABM: n = 8) cognitive bias modification programs (CBM: n = 3) and other interventions (n = 6). Our results demonstrated a small effect in favor of technology delivered interventions compared to a waiting list control group: g = 0.45 [95% CI 0.29, 0.60] p < 0.001. CBT interventions yielded a medium effect size (n = 17, g = 0.66 [95% CI 0.42–0.90] p < 0.001). ABM interventions yielded a small effect size (n = 8, g = 0.41 [95%CI 0.08–0.73] p < 0.01). CBM and ‘other’ interventions failed to demonstrate a significant benefit over control groups. Type of control condition, problem severity, therapeutic support, parental support, and continuation of other ongoing treatment significantly influenced effect sizes. Our findings suggest there is a benefit in using CBT based technology delivered interventions where access to traditional psychotherapies is limited or delayed.

There is today a convergence between politics and therapy, and disillusion with both. As awareness of boundaries increases, the question of how to both comprehend society and manage oneself becomes acute. We need always to connect the socio- with the psycho-. Study of the large group offers one such means. Life in the large group matches that in society and individual experience. The two basic defenses found in the large group (polarizing and institutionalizing) need not necessarily be defenses. They indicate another connection, namely, to the family. The family may be regarded as the place where we first learn about projections and have our first experience of an institution. The large group links individual, society, and family, especially when we see that the dynamics of the large group and the very small group (family sized) are congruent. Study of the large group is especially important for those whose primary work is with individuals. It may be confusing and exasperating, but that way lies learning.He has written a number of books on theology and pastoral practice employing the basic conceptual framework which underlies the group relations studies of the Tavistock Institute and the A. K. Rice Institute. He has extensive experience in such work. With E. R. This study applies Kohut's self-psychology toward an understanding of the self-functions that membership in a religious cult group (Divine Light Mission) provides for the narcissistic personality. It is proposed that there exists a psychosocial fit between the appeal of the cult group's structure and process and the needs of the narcissistic personality. The cult group offers reparative and substitutive functions to the follower who seeks an idealized selfobject to stabilize a defective sense of self. The special relationship of the follower to the Guru bears a close resemblance to the “idealizing transference” which arises between certain narcissistic patients and their group therapist. The therapeutic use and misuse of the “idealizing transference” in group therapy is explored and suggestions are made for its appropriate clinical management.  相似文献
