I describe the therapy of a 20-year-old woman who believed that her difficulties in concentrating and remembering were caused by her 'ME' (Myalgic encephalomyelitis, Chronic fatigue syndrome, or CFS). She had been fathered by a man who never left his own wife. Work with her dreams revealed a within-body drama in which she was locked in an unspeakable fight to the death with her mother. Her symptoms improved after parallels between a dream and an accident showed her own self-destructive hand in her story. Another dream, reflecting her first 'incestuous' affair, showed her search for her original father-self as someone separate from mother, and a later affair provided a between-body drama, helping her to own the arrogant and abject traits she had before seen only as her mother's. I show how we worked in the area of Winnicott's first 'primitive agony' as experienced by a somatizing patient, stuck in a too-close destructive relationship with her mother-body. I discuss how analytical work can be done with the primitive affects and conflicts against which the ME symptoms may be defending.  相似文献

The noted American impressionist, Mary Cassatt is remembered for her intimate portrayal of women, children, and the mother-child relationship. In this paper I have attempted to highlight some of the psychological forces impinging upon the artist, feeling that the artist's work is highly overdetermined. Mention was made of some of the difficulties that a psychobiographical study engenders. Nevertheless, it is hoped that such a study leads to enhanced understanding and appreciation of the artist, her work, and her rich inner world. An examination of the artist's life indicates that difficulties in the family of origin impinged upon her and deeply influenced her work. The loss of several siblings during critical developmental subphases may have produced intense survival guilt in Mary, motivating her to "recreate" her siblings on canvas and to devote her life to care of survivors. Lack of confirmation of Mary's talents by her father may have hindered her development, propelling the child toward a profoundly libidinalized and enmeshed relationship with the mother. Mary's intense relationship with her mother may have led the artist to develop particular stylistic nuances in her productions, contributed to her inability to become a wife and mother herself, and led to frequent episodes of depression. A case was made that Mary suffered from narcissistic disturbance, never completing the recognition of herself as a person outside of the orbit of her mother. Finally, the role of Edgar Degas in the artist's life was described. He seems to have played a major role in the evolution of Mary's style as well as being an important influence in her making a partial separation from her mother in adulthood. In spite of Mary's deep personal suffering, she was able to epitomize in her paintings the most tender and nurturant of relationships. By painting the mother-child theme, she sublimated her own wishes to become a mother as creator of art. Within her family system, she appropriated the position of mother; as the artist, she became the interpreter of this experience. By developing her talent, she communicated her wish to be a mother, and expressed the need to find, if only on canvas, a more truly empathic mother. In essence, her work allowed her to conceive of a life different from the one external reality imposed upon her. This paper explores Bessie Head's writing as a survival strategy through which she transformed her lived experience into imaginative literature, giving meaning and purpose to a life under permanent threat from the dominant group first in South Africa and later in Botswana. This threat included the destructive effect of the many fixed labels imposed upon her including: a 'Coloured' woman, the daughter of a woman designated mad, an exile, a psychotic, a tragic black woman, and a Third World woman writer. Her endeavours to avoid and defeat such limited, static definitions produced work characterised by contradiction and paradox, through which she asserted her right to survive and determined, like Makhaya in When Rain Clouds Gather, to establish 'a living life' in place of the 'living death that a man could be born into' (Head 1989, 136). Through a combination of Head's personal letters and papers and her published work, it can be seen how her particular preoccupations and experiences including her life in exile, her beliefs about her origins, her relationship to her absent mother, her distress, her madness and her need for love and for work were transformed into writing which expresses not only the destructive circumstances of her life but also its life-affirming aspects. Her writing was also a means by which she could create identities to express the dangers she encountered from the all-pervasive power structures which influenced her life and her sense of self, as well as ways to transcend them, enabling her to say in the last years of her life 'I am no failure' (20.2.1986 KMM BHP).  相似文献

This paper examines the dynamics of a 26-year-old prostitute who entered psychotherapy with a number of symptoms. The patient had undergone a series of traumas in her life and was reenacting them in her present behavior. Repetition compulsion, as described by Freud, was a primary form of resistance manifested by this patient. The repetition of the sexual trauma reenacted itself in her choice of profession; namely, prostitution. Additionally, this woman had lived her early years falsely believing that her parents were caring for her, when in fact, they were her grandparents. This early family secret led to the development of a false self within the patient, as well as a strong sense of entitlement, greed, and need for revenge.  相似文献

In this article, I analyse a narrative by Rohima(1), a Bangladeshi woman now living in the UK who was diagnosed in adulthood as having NF1 (neurofibromatosis). While some themes in her narrative account resemble those of persons with NF1 in the general European and American population, other themes relate specifically to Bangladeshi cultural issues and practices. These particularly concern gender, sexual identity and marriage. As a young woman, Rohima's dark skin coloration and the tumors or lumps associated with her condition led her parents and relatives to regard her as unmarriageable and in effect, despite her evident intelligence and competence, to deny that she was a woman. Bangladeshi men and women alike found her appearance a bar to any social acceptability. Even after her marriage (to a non-Bangladeshi man), her family have been unwilling to accept her and her children fully into their kinship network. The article explores the consequences of genetic disorders such as NF1 in cultures where social identity and concepts of personhood, particularly for women, are inextricably related to appearance and to judgements regarding marriageability.  相似文献

Poetry as a tool for research reflectivity is used in this article to explore the individual and personal experience of a black woman researcher-practitioner, working and studying in two white male-dominated organisations—the British Prison Service and a university institution. The discussion will trace how she reconciled her multiple identities and how these identities shaped her perception of her gender-racial group, prison culture, and university culture. An interpretive creative poem that reflects the author’s attempt at capturing the essence of her experience as a prison service employee is presented. The author then examines an excerpt from her journal alongside an in-depth qualitative interview extract and discusses her interpretive comments. The article discusses the issues related to the author’s outsider-within position, a result of her gender and racial minority status, and highlights how poetry allowed her to articulate the anxiety and tension caused by her multiple identities.  相似文献

The life and work of the artist Louise Nevelson are examined through the lens of her ideas about light and shadow. Nevelson developed several ways of dealing with a basic fault established in childhood. Some ideas of Christopher Bollas are used in understanding the artist's relationships with art dealers who served her as transformational objects. The artist became the grandmother of environmental sculpture in America and saw her work as a bridge between the third and the fourth dimension--an idealized realm and a popular concept in twentieth-century art that she related directly to light and shadow. Colette Roberts, a dealer with whom she worked, allowed Nevelson to arrive at a signature style. Arne Glimcher, who represented her for twenty-four years, provided the material and psychological comfort that allowed Nevelson to explore new materials and new styles and to arrive at aesthetic solutions of remarkable power and sophistication through her eighty-ninth year. From the time she met Roberts in 1953 until the end of her life thirty-five years later, Nevelson's ideas about light and shadow expressed not only her thoughts about visual perception but her experience of her mother as her original transformational object and unthought known.  相似文献

A biracial client's recovery from posttraumatic stress disorder (PTSD) through the use of eye movement desensitization and reprocessing (EMDR) is discussed to illustrate the interaction between ethnicity and phenotype as well as diagnosis and treatment considerations. This case explores a woman's experience of discrimination in and out of her home and her vulnerability to complex PTSD, and it documents the importance of the therapy focusing on experiences of discrimination and prejudice as well as abuse. It shows how the client structures her environment in a personally creative fashion to include representative features of various aspects of her identity, by her choice of where and who she teaches as well as how and with whom she spends her free time.  相似文献

Reflecting on the events that culminated in her receiving the Award for Distinguished Contributions to Applied Psychology as a Professional Practice, Canter, an independent practitioner, discusses the road she traveled to become a clinical psychologist and to become involved in professional organizational activities. She believes that this award was given to her because of her contributions to psychology over her lifetime as an effective and hardworking leader, mentor, and role model in her home state of Arizona and nationally. She addresses some of her ideas about effective leadership and mentoring in the American Psychological Association (APA), providing many examples from which she has learned. Throughout the 1980s Margaret Thatcher dominated British and global politics. At the same time she maintained an active Christian faith, which she understood as shaping and informing her political choices and policies. In this article I argue that we can construct from Thatcher's key speeches, her memoirs, and her book on public policy a cultural “theo‐political” identity which guided her political decisions. Thatcher's identity was as an Anglo‐Saxon Nonconformist. This consisted of her belief in values such as thrift and hard work, care for the family and local neighbor, and charitable generosity; her belief in the renewal of the national British Christian spirit; and her notion of morality as the opportunity for free choice. Without a recognition of the centrality of her theo‐political identity, it is difficult to understand the values and beliefs which were central to her political life. The methodological issues raised by the construction of this theo‐political identity are examined in this article. The aim of the proposed methodology is to develop theological insights into a political phenomenon like Thatcher rather than make policy judgments or recommendations.  相似文献

The author describes how her own internal change was a vital part of transformation between herself and two patients. She draws on Loewald's work as she discusses how change in her own internal relationship with her father was part of a lifelong emotional reorganization of Oedipal relations. She describes a process of mutual change whereby her and her patients' unconscious growth each stimulated the other. A female physician who was serving as a first-year medicine resident in Manhattan in September 2001 writes this paper. It details her experience of signing up for military service as a result of the September 11th attack on the United States. She lays out the surroundings, atmosphere, and reactions of those around her during the attack and details her own personal motivations for joining the military, her need to take control and help those in need heal while also trying to heal herself. Grateful, yet haunted by her experience, she provides an intimate glimpse into her time serving as a combat physician at a trauma hospital in Balad, Iraq during the 2007 military surge. A trained geriatrician and palliative care physician she recounts the stories of several patients that have forever shaped her life and explores the contradictions and ethical challenges she faced while caring for them ultimately struggling with the uncertainty of whether what she was truly doing was good for those she served or herself.  相似文献

In this article the author charts the beginning of a journey, a weaving together of personal insights and experiences related to schizophrenia, mostly unwanted, including observations from decades of study as a psychologist and Jungian-oriented therapist/student and dream work from her own analysis. The shock of her mother's death, in conjunction with her adult son's plummet into the world she most dreaded, forced her into a reckoning with a topic she was raised to avoid: a re-examination of her personal history and that of her extended family, and an inescapable invitation to question her precious images, notions, and plans for the ideal life. The function of the mother's reverie, as described by Bion, is viewed as a psychological parallel, following birth, to the physical function of the placenta.

Clinical material from psychotherapy with a 3 1/2-year-old girl illustrates that, as long as she maintained the delusion of being attached to her mother's body by a physical placenta, she was hindered from learning to experience herself as a separate individual and from developing normal feelings of attachment and grief over loss.

She was referred soon after her parents' separation when she showed a lack of reaction to the loss of her father. The material suggests that at the time of separation from her father she regressed to an earlier stage where there had already been a failure to work through infantile separation and individuation from her mother

The material reveals stages of her emergence from an illusory state of union, into one in which she could increasingly tolerate her separate existence. There followed an enrichment of her psychological means of communication through the use of imagination and symbolic play. She also became more receptive to her therapist's communications and could increasingly use psychological support to share and face inner pain.  相似文献

Four interaction-perception perspectives of the fictitious target of a double-bind conflict were provided by four groups of subjects. The perspectives included perceptions of a daughter about herself and her father (the double-bind source), and her estimates of his perceptions of both himself and her. As predicted, the target saw herself as weak but “good”, and as cooperative and highly frustrated, while attributing all the opposite characteristics to the source, who was seen as strong but “bad”, and uncooperative and not frustrated. Further, it was found that she believed he saw himself as strong and very good, but frustrated and moderately cooperative. These observations are consistent with a hypothesis that double-bind experiences result in frustration and mixed feelings toward one's self and the source of the dilemma. It was also concluded that the target's belief that her father would not recognize her weakness, nor her cooperativeness, nor her frustration, and would disinterestedly evaluate her as neither good nor bad resulted from the inconsistent and contradictory communications which had contributed to the double bind.  相似文献

This paper examines the emotional impact of Munchhausen syndrome by proxy on a child aged 6 who had been poisoned by her mother. It describes her treatment in child psychotherapy over twenty months at the Cassel Hospital where the family worked towards her successful rehabilitation home. The work with her showed her experience of dangerous relationships in her family that left her distrustful of any new relationship, and doubtful of her capacity to be close without being destructive. Like her parents, she was terrified of facing the murderous feelings in the family, retreating into denial when life was too painful. The paper describes three phases of treatment: fear of separation and preoccupation with the trauma of poisoning, ambivalence about facing the destructiveness in the family and the move into more appropriate latency behaviour as she prepared to go home with her parents. It concludes with an overview of the impact of Munchhausen syndrome by proxy on the developing child, as shown in this treatment: the delay in emotional and cognitive development, the interference with the capacity to relate, the retreat into illness under stress, and the fear of permanent damage caused by the emotional and physical abuse.  相似文献

Although she experienced, as did the many psychoanalysts before her, the countertransferential dynamics of disgust towards a patient presenting great deficiency in terms of mentalization capacity, the author strives to understand what urged her to tolerate the patient, to invest in her and lead her to develop a capacity for internalization, after 6 years of analytic work filled with transgressive‐transferential acting‐out. Fourteen years later, she becomes aware of the way in which the revival of some grief related to traumatic traces in her own lived experience, sustained, without her knowledge, a saviour/countertransference, creating a specific transferential‐countertransferential spiral which rescued a situation whose advent could have been compromised. This could happen thanks to what she refers to as the unique inter‐relation between her and this particular patient.  相似文献
