This article provides an overview of 20 years of professional experiences with developing and implementing a model for integrating behavioral health services into primary care. The Primary Care Behavioral Health (PCBH) model is designed to provide immediate access to behavioral care for a large number of primary care patients by positioning a behavioral health consultant in the exam room area to function as a core member of the primary care team. In an initial era of discovery, the authors were directly involved in developing and testing a variety of new approaches to providing behavioral health services in general medicine. In a second era focused on feasibility, the authors worked with Kaiser Permanente, the United States Air Force and Navy, the Veteran’s Administration, and the Bureau of Primary Care to system test this innovative model of integrated care. Now in an era devoted to dissemination, the authors review the various roles formal research, system level quality improvement initiatives and stakeholder analysis play in promoting integrated care. The authors also describe current efforts to (1) create a tool that helps systems develop integration targets and (2) use the PCBH model as a platform for teaching medical residents and behavioral health providers to work together in a redesigned primary care team model.  相似文献

In this article, the authors expand the view of parental consultation to include advocacy and empowerment as important contextual considerations. A brief review of existing approaches to parent consultation is provided. Selected advances in multicultural parent consultation are also presented. An advocacy‐and empowerment‐focused perspective is offered that describes the power of consultation with parents from marginalized backgrounds. Public school districts frequently seek professional consultation to improve educational services. This article is an overview of the focus, scope, and practice of behavioral consultation to public schools. A four-stage process of consultation is described, followed by a discussion on the expanding role of behavior support intervention. Next, the involvement of consultants in the design of individual-student, classroom-wide, and whole-school programs is considered. Contemporary approaches to consultation are presented such as positive behavior support, functional behavioral assessment, and efficacy evaluation that targets natural data sources. A concluding section sets forth recommendations for the successful practice of public school behavioral consultation.  相似文献

The purpose of this article is to provide potential consultants with a broad introduction to the learning, gestalt, systems, organizational, psychoanalytic, and chaos theories of consultation. The goal of this article is to demonstrate how the consultant can map each of these theories onto the consultation environment to produce a unique perspective that each theory alone cannot provide. Each theory is described as to how it views and frames the consultation problem and how interventions are then based on these frames. In this study, desktop video teleconferencing was used to facilitate behavioral consultation at a distance with three parents of school‐age children with autism spectrum disorders. Parents were supported via desktop video teleconferencing as they conducted functional analyses and learned to implement an antecedent manipulation strategy, functional communication training, and a consequence‐based strategy. Parents then conducted a brief multi‐element treatment comparison to evaluate the preliminary effectiveness of each intervention strategy on their child's challenging behavior. The results of this study provide additional support to previous research suggesting that a telehealth model of behavioral consultation may be an effective way to assist parents of children with autism spectrum disorders address challenging behavior that disrupts family routines. Edgar Schein (1969, 1978, 1989, 1990) has proposed three models of consultation based on assumptions inherent in different helping styles. The first two models, purchase-of-expertise and doctor-patient, focus on the content of organizational problems. The client gives the problem to the consultant to find and implement solutions. The process consultation model focuses on how organizational problems are solved. The client and consultant collaborate throughout the problem-solving effort to find workable solutions. Because of the nature of organizations' cultures and the underlying assumptions that determine how they operate, Schein has suggested that it is most efficacious for consultants to begin in the process consultation mode and involve the client in the investigation of the problem.  相似文献

All 216 doctoral and specialist level school psychology training programs were mailed a seven question survey to determine how much behavioral consultation and behavior analysis course work school psychology students are offered. One hundred and twenty-eight surveys were returned, yielding a return rate of 58.26%. The results indicated that almost all programs offer didactic courses in consultation (98.79%) and behavior analysis/behavior modification (91.52%). The percentage of practicum devoted to behavioral consultation and behavior analysis/modification was considerably less (about 25% for each) than other subject areas. The results are discussed within the context of training needed to meet the needs of practitioners in the areas of behavioral consultation and behavior analysis/behavior modification.  相似文献

Whileprocess andaction are fundamental notions, in ubiquitous use, they lack satisfactory logical treatment in two critical respects: in analyses of the fundamentals themselves and in logical development. For what treatment they have so far received, under classical systematisation, leaves significant lacunae and induces much paradox. A relevant logical relocation, carried through in detail here, removes such problems, and provides solid ground-work for a satisfactory treatment.Firstly, as to fundamentals: processes should be explicated, so it is argued, as certain sorts of (time) directed functions (from inputs to outputs); thus they can be represented through certain ordered pairs of relations. Significant logical structures they can enter into are investigated: notably, process lattice and coupled logics, and a generalized category theory (tolerating nonassociativity of composition).Actions are types of processes, agent-ascribed process. As stock analyses of the differentia, operators and agency, through intentionality, rationality and so on, demonstrably fail, new causal analyses are proposed.Secondly, as to logical developments: for the most part, the apparently diverse offering of process and action logics to be encountered in the literature are but multiple modal logics: modal logics enriched with further functors of interesting modal sorts. Some, for example, like advertised process logics are dynamic logics (themselves basically multiple modal logics) enriched by tense logical functors, themselves modal in character. In a way that is now becoming nonstandardly standard, these modal enterprises can be reworked on relevant logical bases. A main point to such exercises resembles that of other relevant reworkings: namely, the search for correctness, for adequacy to pre-analytic and linguistic data, and therewith removal of paradoxes and anomalies that accumulate under modal analyses.Logical components from a properly expanded Humean model of action are supplied with relevant logics and semantics, in particulardoing, trying andstriving, intention andmotivation. The difficult question of formalising practical inference is then addressed.Relevant dynamic logics, paralleling modal developments, are built up piece by piece, relevant theory change is considered within a dynamic framework, and work on relevant temporal and process logics of programming cast, including functors such asbefore, during andthroughout, is initiated. The present state of logical play is assessed.I have been much encouraged in this work by Krister Segerberg, who deserves and has my considerable thanks.
