Recent and dramatic changes in the demand for and reimbursement of psychological services and expertise in health care delivery systems have radical implications for the preparation of psychologists at the predoctoral, internship, and postdoctoral levels. In order to respond to these changes effectively and advance the profession, training programs must realize the expanded and potential role of psychological expertise in evolving health care delivery systems. In this paper, we review several limitations and unfortunate consequences of traditional training programs that have confined the scope of research and practice to the realm of mental health. We propose that future psychologists be recognized and trained from a broader perspective as behavioral scientists, prepared to operate at the highest levels of health care delivery systems. Specific recommendations for training and education are discussed.  相似文献

The author narrates a history of the development of her interest in integrating behavior into heath research and health care. Using diabetes as an exemplar, she describes some of the challenges of integrating behavior into health research and health care: (1) becoming a member of interdisciplinary research and health care teams; (2) acquiring funding for behavioral research with medical populations; (3) communicating scientific findings; and (4) acquiring reimbursement for psychological services rendered. In anticipation of the growing need for adequate mental health care for older adults in residential aged care facilities, psychologists are challenged to overcome several barriers that impede the uptake and delivery of their services in such settings. Information and communication technologies (ICT) have strong potential to overcome some of these barriers by supporting the delivery of evidence-based psychosocial treatments for common psychogeriatric issues. This paper presents two case studies that illustrate when and how psychologists can use various ICT applications (e.g., tablet devices, web-based applications) integrated with cognitive behavioural and reminiscence-based therapies. Both case studies demonstrate that ICT can effectively support the therapeutic alliance, enhance therapeutic engagement, and individualize treatment delivery to accommodate the needs of elderly patients. It is hoped that these case studies will encourage clinicians to consider using ICT to augment therapy with their elderly patients.  相似文献

Emotion regulation has been studied in diverse ways within different subdisciplines of psychology—most notably social development, personality and individual differences, social psychophysiology, interpersonal relationships, and stress and coping. The goal of this Special Issue is to demonstrate the value of integrating these diverse approaches for building multilevel, lifespan models of emotion regulation. Recent changes to the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders highlight the importance of identity dysfunction within several psychiatric diagnoses. Despite a long-standing tradition of identity research and theory in the developmental literature, there is limited work establishing intersections between clinical and developmental conceptualizations of identity problems. The relative lack of integration between decades of clinical and developmental work is unfortunate, and likely limits progress in both areas. In this commentary, the authors argue for greater interdisciplinary collaboration and highlight contributions from developmental and clinical theories, which, if integrated, could enhance identity scholarship. The developmental psychopathology perspective is introduced as an ideal framework to promote these goals.  相似文献

Epistemology mirrors reality but notperfectly, and in the process molds reality butnot exactly as intended or anticipated. Horizontal interconnections also exist betweenand among epistemology, ideology, theory andpraxiology. However, these relations areneither deductive nor deterministic in naturebut are merely resonant, and then unclear,ambiguous and confounded. In this paper, thepoint is made that we need a grand reflectionon both our paradigms of reality and ourpredicaments of life as lived, to deal with thediscontent of humanity at this moment of thehistory of our civilization, and to engage inpraxis ``to act on our world and to reconstructit' to make it moral, just and more humane. Toundertake such a grand reflection, we need apractical philosophy of knowledge – that is, apractical epistemology – to bridge across themultiple epistemological approaches nowcrowding our discussions. An epistemictriangle formed by systems thinking,constructivist thinking, and dialecticalthinking is offered to serve as the startingpoint of crystallization for building a commonground for epistemologies. It is shown how suchan epistemological diagram could be first usedto develop a minimum ideological consensus andthen to bring orderliness to the chaos ofcontemporary themes of our civilization andissues of education and culture within andacross nations. Finally, an agenda foreducational leadership for educational practiceis offered to fit the needs of the globalcommunity of nations as we enter thetwenty-first century.  相似文献

Rapidly occurring changes in the healthcare arena mean time is of the essence for psychology to formalize a strategic plan for training in primary care settings. The current article articulates factors affecting models of integrated care in Academic Health Centers (AHCs) and describes ways to identify and utilize resources at AHCs to develop interprofessional educational and clinical integrated care opportunities. The paper asserts that interprofessional educational experiences between psychology and other healthcare providers are vital to insure professionals value one another’s disciplines in health care reform endeavors, most notably the patient-centered initiatives. The paper highlights ways to create shared values and common goals between primary care providers and psychologists, which are needed for trainee internalization of integrated care precepts. A developmental perspective to training from pre-doctoral, internship and postdoctoral levels for psychologists in integrated care is described. Lastly, a call to action is given for the field to develop more opportunities for psychology trainees to receive education and training within practica, internships and postdoctoral fellowships in primary care settings to address the reality that most patients seek their mental health treatment in primary care settings.  相似文献

This study examined gender differences intransformational leadership from multiple perspectives.The sample was employees of a large international bank inAustralia. Ratings were obtained from branch managers' (n = 120 female and n = 184 male), theirsuperiors (n = 32) and subordinates (n = 588). Thefindings showed that superiors evaluated female managersas more transformational than male managers. Consistent with the superior observations, at the globallevel, female managers rated themselves as moretransformational than males, however, at the morespecific, behavioral level of analysis, significantgender differences were noted only for those subscaleswhich are more interpersonally-oriented. Quality Improvement (QI) is a health care interprofessional team activity wherein psychology as a field and individual psychologists in health care settings can and should adopt a more robust presence. The current article makes the argument for why psychology’s participation in QI is good for health care, is good for our profession, and is the right thing to do for the patients and families we serve. It reviews the varied ways individual psychologists and our profession can integrate quality processes and improve health care through: (1) our approach to our daily work; (2) our roles on health care teams and involvement in organizational initiatives; (3) opportunities for teaching and scholarship; and (4) system redesign and advocacy within our health care organizations and health care environment.  相似文献

The authority to prescribe could differentiate psychologists from other non-physicians in primary care, achieving a significant step for the growth of our profession. Psychologists are currently prescribing in primary care settings in a number of public service venues, as well as medical school training facilities. In this Special Section of the Journal of Clinical Psychology in Medical Settings, results of a survey of physicians working with a prescribing psychologist indicate that the psychologist is highly valued and seen as a competent prescriber. A series of papers providing case examples demonstrate how psychologists are evolving in these new clinical roles, as well as participating in behavioral and psychopharmacology training of primary care physicians.  相似文献

Implementation experts suggest tailoring strategies to the intended context may enhance outcomes. However, it remains unclear which strategies are best suited to address specific barriers to implementation, in part because few measurement methods exist that adhere to recommendations for reporting. In the context of a dynamic cluster randomized trial comparing a standardized to tailored approach to implementing measurement-based care (MBC), this study aimed to (a) describe a method for tracking implementation strategies, (b) demonstrate the method by tracking strategies generated by teams tasked with implementing MBC at their clinics in the tailored condition, and (c) conduct preliminary examinations of the relation between strategy use and implementation outcomes (i.e., self-reported fidelity to MBC). The method consisted of a coding form based on Proctor, Powell, and McMillen (2013) implementation strategy reporting guidelines and Powell et al.’s (2012) taxonomy to facilitate specification of the strategies. A trained research specialist coded digitally recorded implementation team meetings. The method allowed for the following characterization of strategy use. Each site generated 39 unique strategies across an average of six meetings in five months. There was little variability in the use of types of implementation strategies across sites with the following order of prevalence: quality management (50.00%), restructuring (16.53%), communication (15.68%), education (8.90%), planning (7.20%), and financing (1.69%). We identified a new category of strategies not captured by the existing taxonomy, labeled “communication.” There was no evidence that number of implementation strategies enacted was statistically significantly associated with changes in self-reported fidelity to MBC—however, financing strategies were associated with increased fidelity. This method has the capacity to yield rich data that will inform investigations into tailored implementation approaches.  相似文献

A heuristic model is presented concerning leadership behaviors in youth sports. The cognitive-behavioral model specifies individual difference variables, situational factors, and cognitive processes assumed to mediate overt coaching behaviors and athletes' reactions to them. Initially, the status of leadership research is reviewed, focusing on the potential relevance of behavioral assessment to this area of inquiry. Next, the athletic environment is characterized as a largely untapped naturalistic setting for psychological research, and a rationale is developed for the study of leadership in youth sports. Following this, consideration is given to the basic components of a mediational model of coach-athlete interactions and some preliminary research relative to the model. An expanded model is then presented, along with suggestions and hypotheses for future research. Finally, the sport-specific model and associated research is related to the larger context of theoretical advances and empirical work in the area of leadership.  相似文献
