Abstract

Group-based interpersonal psychotherapy (IPT-G) was provided to nine male Vietnam veterans with posttraumatic stress disorder (PTSD) to reduce interpersonal difficulties. Standardized measures of posttraumatic stress, depression, interpersonal problems, and functioning were administered pre- and posttreatment and at 2- and 4-month follow-ups. Individual (reliable change indices) and group analyses (repeated measures ANOVAs) indicated improvements in interpersonal and global functioning (not maintained at follow-up), as well as for PTSD and depressive symptoms (maintained at follow-up). Qualitative feedback indicated reduced levels of anger and stress as well as improved relationships. IPT-G for Vietnam veterans shows promise in improving interpersonal functioning and reducing psychological distress. However, since not all improvements were maintained over time, future studies may need to explore relapse prevention strategies.  相似文献

Social traumatization is targeted at entire victim groups and occurs in a societal context. The classification of posttraumatic stress disorders in DSM-5, which intentionally fails to differentiate between different types of traumatic events, ignores the social factors of traumatogenic pathologies. Whereas clinical practice unanimously embraces the thorough exploration of patients’ individual life experiences and history of suffering, taking the relevant social and environmental factors into account, in the classification systems (DSM-5 and ICD-10) it has been decided to address posttraumatic disorders as individual pathologies and not as social phenomena. Even equating social trauma with a disease may foster this partial exclusion, which in turn can contribute to perpetuation of the posttraumatic condition.  相似文献

Trauma is usually understood as reaction to an exceptional disorganizing event. Instead, it can be recognized as the underlying fabric of everyday experience, entailing reality’s fundamental unpredictability, constant change, and the inevitable erosions wrought by sickness, aging, and death. This paper draws on Ferenczi’s psychoanalytic thinking to develop this view of reality as trauma, and to suggest a shift in interpersonal self-regard as central to psychoanalytic change.  相似文献

Victims of traumatic events who experience re-traumatization often develop a highly ambivalent relationship to God and all religiosity as extremely conflictual. On the one hand, they may choose to blame God for not having protected them, for having left them to feel so alone, for having been indifferent to them or they may even turn their wrath upon God, as the source of cruelty. Often though, the traumas experienced by individuals prompt them to turn to God and religion in search of help. This gives reason for the need of new and up-to-date research that can help elucidate why some people choose to seek help in religion and others turn away from it.  相似文献

In this paper, I follow the evolution of the concept of trauma within psychoanalytic theory, and some of the effects of this evolution. Starting out from the point where Freud gives up the theory of trauma as the cause of neurosis in 1897, and the alleged reasons for such a change, I analyze the theoretical and clinical consequences of this change within psychoanalysis. I proceed to develop Ferenczi's ideas on trauma, their relevance to present-day psychoanalytic theorizing and practice, as well as for the understanding of the psychic processes in children, tracing the development of some of these ideas in contemporary psychoanalytic thinking.  相似文献

Ausgehend von der Beobachtung, dass der Missbrauch psychotroper Substanzen bei traumaassozierten psychischen Erkrankungen geh?uft anzutreffen ist, werden m?gliche wechselseitige Zusammenh?nge zwischen Suchterkrankungen und Traumafolgest?rungen er?rtert. Selbstverletzendes Verhalten stellt kein einheitliches, sondern ein für die klinisch psychologische Forschung sehr komplexes und noch unzureichend erkl?rtes Ph?nomen dar. Selbstverletzendes Verhalten h?ngt zumeist mit frühen traumatischen Erlebnissen zusammen, auf die nicht angemessen reagiert werden konnte. Aufbauend auf den Ergebnissen einer Stichtagserhebung am Anton-Proksch-Institut wird ein ressourcenorientiertes Behandlungsmodul für traumatisierte, suchtkranke Frauen vorgestellt, welches Stabilisierung und die Entwicklung von Selbstfürsorge umfasst.  相似文献

This article articulates some of the problems that surround the use of the term trauma in psychoanalytic theory and suggests that the key element for a theory of pathogenesis and mental functioning is not the either/or of external versus internal causation or trauma versus drive. Rather, it is an understanding of whether, or to what extent, the raw data of existential experience is or is not transformed into psychological experience. From this perspective, trauma is whatever outstrips and disrupts the psyche’s capacity for representation or mentalization. Absent the potential for mental representation, these events and phenomena are historical only from an external, third-person perspective. Until they are mentalized, they remain locked within an ahistorical, repetitive process as potentials for action, somatization, and projection.  相似文献

The meaning of trauma within psychology has for a long time been viewed mostly from a ‘pathologising’ standpoint. Viewed as pathology, people's trauma may become understood using a singular lens thereby leaving out broader contextual (social, environmental, political, historical, economical etc.) aspects that have been shown to play a role in the subjective experience of various forms of suffering. Psychology has for a long time been quick to diagnose victims of traumatic experiences from an individualistic perspective, almost always positioning the ‘problem’ within the person. As a result of the Vietnam War, World Wars I and II and the Holocaust, the rise in the diagnosing of PTSD gave way to the labelling of trauma. Within psychology, post‐traumatic stress disorder has become the way to operationalise suffering, and I would like to argue that this occludes how suffering lives in a family, a community and in gendered bodies. The gendering of trauma continues to be a silenced space. There is a need to highlight and acknowledge the gendered nature of suffering which begs for trauma to be understood in context and not as an isolated act or occurrence affecting the individual. Women have remained, and in many ways continue to remain, silenced with minimal opportunities to name the world for themselves. This paper seeks to provide a critical analysis of the notion of trauma (and its intersection with gender) that arises from conflict and post‐conflict situations.  相似文献

The events of the Armenian Genocide of 1915, during which more than 1.5 million Armenians were massacred by the Ottoman Turkish Empire, has left a deep, painful scar on this small but prominent culture. Those who lived through the Armenian Genocide survived death marches, rapes, drownings, physical mutilation, and other such heinous crimes. As they have passed, it is being recognized that subsequent generations continue to fight for justice, manifesting their ancestral pain, sadness, and mourning for the loss of their family members. As sociocultural trauma has a multigenerational impact within families and communities and affects the feeling of membership and belonging within that community, understanding how previous traumas shape future generations of that group is important for clinicians. This article reviews the scarce research on intergenerational trauma in the Armenian community within the United States after the Armenian Genocide, using the constructivist self-development theory to provide clinical implications and suggestions.  相似文献

Peri- and posttraumatic emotional responses have been understudied, and furthermore, have rarely been compared among trauma types. The current study compared college students' retrospective self-reports of peri- and posttraumatic responses of fear, shame, guilt, anger, and sadness among four types of traumatic events: sexual assault, physical assault, transportation accident, and illness/injury. Overall emotional responding was generally high for all trauma types, and for those in the sexual assault group, emotion increased sharply from the peri- to posttraumatic time point. Generally, fear was higher during the trauma compared to after the trauma, whereas the other emotions tended to remain stable or increase posttrauma. The sexual assault group tended to report higher levels of posttrauma emotion than the other trauma type groups.  相似文献

The author approaches the topic through infant observation. At the age of eight months the baby moves in the borderland between the physical and the symbolic. When the baby hurts herself, she needs the mother's arms to protect her: at the same time, her budding spoken symbol "mama" is comforting in itself, it carries the memory of the mother's arms. Can speech - tone of voice, rhythm, pauses - and the moments of loss, absence, the losing of self also be intertwined? The author discusses the question of how trauma starts to live in speech through two patients. She talks about "a silent language" which speaks in between the words and sentences - changes the rhythm of speech, makes lose words, tears a hole in speech. This "silent language" talks about trauma, reaches for its very essence - a bodily experience and meaning to the wholeness/disruption of self.  相似文献
