Psychophysiological models of panic hypothesize that panickers focus attention on and become anxious about the physical sensations associated with panic. Attention on internal somatic cues has been labeled interoception. The present study examined the role of physiological arousal and subjective anxiety on interoceptive accuracy. Infrequent panickers and nonanxious participants participated in an initial baseline to examine overall interoceptive accuracy. Next, participants ingested caffeine, about which they received either safety or no safety information. Using a mental heartbeat tracking paradigm, participants' count of their heartbeats during specific time intervals were coded based on polygraph measures. Infrequent panickers were more accurate in the perception of their heartbeats than nonanxious participants. Changes in physiological arousal were not associated with increased accuracy on the heartbeat perception task. However, higher levels of self-reported anxiety were associated with superior performance.  相似文献

Anxiety sensitivity (AS) is the fear of sensations associated with autonomic arousal. AS has been associated with the development and maintenance of panic disorder. Given that panic patients often rate cardiac symptoms as the most fear-provoking feature of a panic attack, AS individuals may be especially responsive to cardiac stimuli. Consequently, we developed a signal-in-white-noise detection paradigm to examine the strategies that high and low AS individuals use to detect and discriminate normal and abnormal heartbeat sounds. Compared to low AS individuals, high AS individuals demonstrated a greater propensity to report the presence of normal, but not abnormal, heartbeat sounds. High and low AS individuals did not differ in their ability to perceive normal heartbeat sounds against a background of white noise; however, high AS individuals consistently demonstrated lower ability to discriminate abnormal heartbeats from background noise and between abnormal and normal heartbeats. AS was characterized by an elevated false alarm rate across all tasks. These results suggest that heartbeat sounds may be fear-relevant cues for AS individuals, and may affect their attention and perception in tasks involving threat signals.  相似文献

Two experiments were conducted to study selective memory bias favoring anxiety-relevant materials in patients with anxiety disorders. In the 1st experiment, 32 patients with generalized anxiety disorder (GAD), 30 with social phobia (speaking anxiety), and 31 control participants incidentally learned GAD-relevant words, speech anxiety-relevant words, strongly pleasant words, and words with a neutral valence. Participants did not show any explicit memory bias for threatening materials. Thirty patients suffering from panic disorder (PD) with agoraphobia and 30 controls took part in the 2nd experiment. The design was similar to the 1st experiment. This time a highly specific selective memory bias for threatening words was found. Words describing symptoms of anxiety were better recalled by PD patients. Results are consistent with previous findings but are inexplicable by existing theories.  相似文献

A computerized Stroop color-naming paradigm was used to investigate attentional biases for selectively processing threat information in panic-disordered patients. Subjects named the colors of neutral words (e.g. TYPICAL), fear words (e.g. PANIC), bodily sensation words (e.g. HEARTBEAT), and catastrophe words (e.g. HEART ATTACK). To control for familiarity with threat concepts, we used clinicians who treat panic disorder as normal control subjects. In contrast to normal controls, panic patients exhibited greater Stroop interference for all threat words, especially those associated with catastrophe. Stroop interference waned during the course of the experiment, thereby indicating habituation to the semantic content of the cues. These findings suggest that panic disorder, like other anxiety disorders, is associated with an attentional bias for processing threatening information.  相似文献

The current study tested whether "suffocation sensations" (respiratory loads) are automatically evaluated in a negative way by people fearing these sensations. It was found that, after having been primed with a slight respiratory load, participants with high suffocation fear (n=15) reacted more quickly to suffocation words and more slowly to positive words than participants with low suffocation fear (n=21). However, the effect was present only in participants who had noticed the primes. The findings are relevant to the cognitive model of panic disorder because automatic negative appraisal of sensations may play a role in initiating a panic attack.  相似文献

This study tests the hypothesis that social anxiety and fear of bodily sensations are associated with the development of agoraphobic avoidance behavior in panic disorder patients. Twenty patients with panic disorder were compared to 20 patients with panic disorder with agoraphobia, matched by sex and duration of disorder. The two groups did not differ on measures of fear and frequency of assertive social responses. However, the agoraphobics scored higher on measures of interpersonal sensitivity, depression, feelings of inadequacy, and hostility. They also reported higher fear of bodily sensations. Although definitive conclusions need to be postponed until prospective studies have been conducted, there is evidence suggesting that the development of agoraphobia in panic patients is associated with hypersensitivity to bodily sensations and interpersonal situations.This research was supported by Grant 560-268-009 of the Dutch Organization for Scientific Research.  相似文献

Beck and Clark's 1997 information processing model of panic was presented and evaluated. In general, studies using the Stroop task have shown panic patients have a cognitive bias toward negative and personally relevant information. Several studies have also shown that panic patients tend to have more catastrophic thoughts and are more likely to misinterpret bodily sensations. Further, cognitive-behavioral therapy is more effective than drug therapies. The limitations of the cognitive model and cognitive-behavioral therapy were discussed, and an extension of current models was proposed. The extension of current models combines cognitive-behavioral and psychodynamic concepts and techniques and suggests that exposure in vivo used for agoraphobic situations and interoceptive exposure used for panic should also be applied to the underlying themes of panic disorder.  相似文献

In a controlled functional magnetic resonance imaging (fMRI) study the effect of psychodynamic short-term psychotherapy on cerebral activation of panic disorder patients was determined. In a linguistic go-no go paradigm behavioral measures and cerebral activation to affective word stimuli with threatening, neutral and positive content were assessed pretreatment and post-treatment in 9 patients with panic disorder. 18 healthy controls were scanned twice at the same intervals. In an additional long-term follow-up after approximately 3 years post-treatment the effect of brain activation changes during treatment on the long-term outcome of core symptoms (panic-related cognition and fear of body sensations) were determined. In accordance with the hypotheses increased limbic (hippocampus and amygdala) activation accompanied by low prefrontal activation to negative words were found. At treatment termination, panic-related symptoms had improved significantly and fronto-limbic activation patterns were normalized. Reduction of activation of the limbic system during treatment was predictive of a favorable long-term outcome. The results indicate that short-term psychodynamic treatment leads to changes in fronto-limbic networks, similar to previous findings on cognitive-behavioural treatment.  相似文献

Previous work reported in this journal suggested that the cognitive capacities of high dissociators are impaired under conditions of focused (selective) attention, but not under conditions of divided attention, compared with the cognitive capacities of low dissociators. Using a directed-forgetting paradigm, the current study demonstrated that under divided-attention demands, high dissociators have impaired memory for words associated with trauma (e.g., incest) but not for neutral words, as compared with low dissociators. In addition, high dissociators reported significantly more trauma history and significantly more betrayal trauma (abuse by a caregiver) than low dissociators. These results are consistent with the proposal that dissociation may aid individuals with histories of betrayal traumas to keep threatening information out of awareness.  相似文献

Psychology distinguishes between a bodily and a narrative self. Within neuroscience, models of the bodily self are based on exteroceptive sensorimotor processes or on the integration of interoceptive sensations. Recent research has revealed interactions between interoceptive and exteroceptive processing of self-related information, for example that mirror self-observation can improve interoceptive awareness. Using heartbeat perception, we measured the effect on interoceptive awareness of two experimental manipulations, designed to heighten attention to bodily and narrative aspects of the self. Participants gazed at a photograph of their own face or at self-relevant words. In both experimental conditions interoceptive awareness was significantly increased, compared to baseline. Results show that attention to narrative aspects of self, previously regarded as relying on higher-order processes, has an effect similar to self-face stimuli in improving interoceptive awareness. Our findings extend the previously observed interaction between the bodily self and interoception to more abstract amodal representations of the self.  相似文献

Cognitive theories of panic disorder suggest that the catastrophic misinterpretation of bodily sensations is the trigger for a panic attack. A challenge to cognitive theories is the suggestion that dyspnea (shortness of breath) is central to the development of panic and that negative cognitions are by-products of panic. To examine these seemingly contradictory theoretical perspectives, the present study investigated panic symptomatology in a sample of patients with chronic shortness of breath (i.e. pulmonary patients). Past studies have shown an increased prevalence of panic in pulmonary patients, a finding that may be useful in elucidating panic etiology. The current sample of pulmonary patients (N = 28) confirmed previous reports of high prevalence rates of panic in this population. Based on self-report of panic symptomatology, a total of nine patients (32%) met DSM-IV criteria that were consistent with panic disorder. Multivariate comparison of participants with and without panic symptomatology revealed that panickers had significantly higher levels of anxiety, depression and agoraphobic cognitions. However, these groups showed no significant differences on physiological measures of pulmonary functioning. The authors conclude that dyspnea alone is inadequate in predicting panic development. High levels of panic symptomatology in pulmonary samples may reflect increased opportunities for these patients to misinterpret bodily sensations and, in particular, their pulmonary symptoms.  相似文献

In the present study, a modified semantic priming paradigm was used to test whether panic patients more strongly associate catastrophes with anxiety symptoms than nonclinical subjects. Subjects named catastrophic target words (e.g. infarction) and target words neutral to anxiety themes (e.g. weekend) that followed auditive prime sentences immediately (i.e.0ms) or with a delay (i.e. 1500ms). Prime sentences described the perception of anxiety symptoms (e.g. You feel tense) or sensations neutral to anxiety (You feel relaxed). Consistent with an earlier study [Schniering C.A., & Rapee, R.M. (1997). A test of the cognitive model of panic: Primed lexical decision in panic disorder. Journal of Anxiety Disorders, 11, 557-571] the two groups did not differ if semantic priming effects were calculated in the usual way, i.e. by averaging across identical stimuli for all subjects. As expected, however, panic patients demonstrated stronger semantic priming effects for catastrophes immediately following prime sentences if priming effects were calculated for idiographically selected stimuli. The latter result indicated stronger automatic associations between idiographic anxiety symptoms and catastrophes in panic patients consistent with the cognitive model of panic disorder (Clark, D.M. Cognitive treatment of panic attacks is based on the hypothesis that panic results from the catastrophic misinterpretation of bodily sensations, and that changing such misinterpretations will block the occurrence of panic. The treatment normally involves an integrated set of cognitive and behavioural techniques. In a consecutive series of panic patients, a multiple baseline across subjects design was used to investigate whether a modified form of treatment involving only cognitive procedures could reduce panic attack frequency. The results provide preliminary evidence that cognitive procedures directed at changing misinterpretations of bodily sensations can reduce panic attack frequency, and also that cognitive procedures which do not target misinterpretations may not reduce panic.  相似文献

Slower colour-naming reaction times (RTs) for negatively valenced Stroop words (e.g. SELFISH printed in green ink) than for positively valenced words (e.g. TOLERANT printed in blue ink) have been taken as evidence of an automatic vigilance mechanism that directs attention towards undesirable events and objects. These findings were replicated when words were shown at fixation but not when they were shown away from the locus of fixation. The failure of negative word valence to capture spatial attention is compared with the capture that has been found with angry and threatening facial expressions. From an ecological view of social perception both findings are unsurprising. Unlike words, facial expressions have evolved to provide the species with adaptively important information.  相似文献

The present study investigated the relationship between respiratory function, catastrophic thoughts about anxiety, and panic in 48 Chronic Obstructive Pulmonary Disease (COPD) patients. During a routine office visit which included respiratory function tests (Forced Vital Capacity, FVC; Forced Expiratory Volume--first second, FEV1) patients completed a battery of questionnaires which assessed history of panic, days with shortness of breath, general activity level, agoraphobic cognitions, perception of bodily sensations, anxiety and depression. Thirty-seven percent of the sample reported experiencing a panic attack. Subjects showed a significant impairment in respiratory functioning. Patients with a history of panic did not differ from those who had not experienced panic on demographic, physiologic, or activity variables. Patients who experienced panic reported significantly more agoraphobic cognitions and greater concern with bodily sensations than did patients who did not experience panic.  相似文献

Many patients suffering from panic disorder do not receive adequate care. Advances in the conceptualization and treatment of panic disorder encourage innovative strategies for targeting core fears (fears of anxiety sensations) that underlie this disorder. In this article, we discuss the use of exercise as a potential strategy for therapeutic exposure to these feared sensations, and the role that exercise may play as an acceptable intervention to aid the dissemination of exposure-based approaches to the treatment of panic disorder. Evidence for the efficacy of exercise for the treatment of anxiety and panic is presented; along with strategies to enhance the role exercise can play as an interoceptive (internal sensation) exposure procedure. Finally, issues of comorbidity and exercise acceptability are discussed. Our conclusion is that exercise-based interventions are promising strategies for improving the utilization of exposure-based interventions for panic disorder. Clinical guidelines and future research directions are discussed.  相似文献

Anxiety sensitivity, a trait characterised by fear of anxiety-related body sensations, has been linked to heightened attention to pain, appraising body sensations as threatening, and remembering threat-related information. We assessed whether individuals with greater anxiety sensitivity overestimate in remembering pain. We also assessed whether emotion regulation strategies that direct attention away from pain (distraction), or alter appraisals of pain (reappraisal), alleviate memory bias. Participants (N = 137) were randomly assigned to one of two emotion regulation conditions or to a control condition before taking part in a cold pressor task. Greater anxiety sensitivity was associated with overestimation in remembering pain. Engaging in reappraisal mitigated this memory bias but engaging in distraction did not. This is the first study to examine the relations among anxiety sensitivity, emotion regulation and memory for pain. The findings suggest that health-care practitioners can encourage reappraisal to promote more positive memories of procedural pain, particularly in patients high in anxiety sensitivity.  相似文献

Effects of emotional valence on attention have been shown to occur even when stimuli are presented outside awareness. The impact of negative valence on stimulus processing has been demonstrated to be particularly salient in anxiety. Therefore, it has been hypothesized that compared to nonanxious individuals, anxious individuals have an enhanced ability to detect the valence of negative stimuli. However, it remains unclear whether anxious individuals are better at identifying the valence of threatening stimuli or, rather, more likely to label ambiguous stimuli as threatening. To investigate these hypotheses, high and low anxious participants categorized lexical stimuli as "safe" or "dangerous." Stimuli were presented at durations that allowed for both conscious (unmasked) and nonconscious (masked) processing. Results show that on masked trials, anxious individuals evidenced an enhanced ability to correctly classify threatening information, whereas nonanxious participants demonstrated an enhanced ability to correctly classify neutral or positive information. Signal detection analyses indicated results were explained by a response bias, whereby anxious individuals were more likely than nonanxious individuals to categorize masked words as threatening and nonanxious individuals were more likely to categorize masked words as nonthreatening. No group differences for nonword stimuli emerged, suggesting that anxiety-related response bias tendencies are activated only after detection of a weak semantic signal.  相似文献

Alterations in the perception of body signals (i.e., interoceptive awareness [IA]) are considered crucial for the development and maintenance of somatoform disorders (SFDs). However, competing theories come to different conclusions about whether IA is increased or decreased in SFDs. The present study investigated IA in 23 patients with SFDs (as defined by the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition) and in 27 healthy controls. IA was reliably assessed with two well-established heartbeat perception paradigms (heartbeat discrimination task and mental tracking task). The results of both paradigms showed no evidence for increased IA in patients with SFDs. Correlational analyses revealed that having a higher number of somatoform symptoms was significantly linked to lower (rather than higher) IA in SFDs. These findings are in line with recent cognitive approaches to SFDs that stress the importance of biased schema-guided processing of interoceptive information. Efficient processing of complex social and biological stimuli associated with threat is crucial for survival. Previous studies have suggested that threatening stimuli such as angry faces not only capture visual attention, but also delay the disengagement of attention from their location. However, in the previous studies disengagement of attention was measured indirectly and was inferred on the basis of delayed manual responses. The present study employed a novel paradigm that allows direct examination of the delayed disengagement hypothesis by measuring the time it takes to disengage the eyes from threatening stimuli. The results showed that participants were indeed slower to make an eye movement away from an angry face presented at fixation than from either a neutral or a happy face. This finding provides converging support that the delay in disengagement of attention is an important component of processing threatening information.  相似文献
