BACKGROUND: Comorbidity amongst anxiety disorders is very common in children as in adults and leads to considerable distress and impairment, yet is poorly understood. Multivariate genetic analyses can shed light on the origins of this comorbidity by revealing whether genetic or environmental risks for one disorder also influence another. We examined the genetic and environmental influences on the comorbidity between three common childhood anxiety disorders: Specific Phobia, Separation Anxiety and Social Phobia. METHODS: Using a two-phase design 4,662 twin-pairs were screened in the first phase and 854 pairs were assessed in the second phase by maternal-informant diagnostic interview using DSM-IV criteria. RESULTS: Multivariate genetic analysis revealed significant shared environmental over-lap between Specific Phobia and Separation Anxiety and significant familial and non-shared environmental over-lap between Specific Phobia and Social Phobia. CONCLUSIONS: Familial influences, especially shared environment, are central to the comorbidity between Specific Phobia and both Separation Anxiety and Social Phobia.

While social phobia is typically diagnosed in adolescence, the roots of social phobia are found in childhood experiences of shame. When shame occurs because of a failure to meet a child’s need this creates a fundamental uncertainty which can become internalized in the form of self-contempt. Emotions such as sadness are sometimes forbidden by caregivers, and the expression of natural sexual drives are sometimes prohibited. Such experiences create shame and may keep the person from feeling emotions and drives because shame has displaced the real emotion. The fractures in early relationships can be restored through the healing presence of another. The pastor has access to persons with social phobia who may not be willing to seek psychiatric care and is in a unique position to bring the resources of the church to bear in the lives of persons with social phobia. To do so, she moves away from the extroverted frame of reference within which the church frequently operates.Philip Browning Helsel, a graduate of Princeton Theological Seminary, serves as a Chaplain Resident in the Clinical Pastoral Education program at Yuma Regional Medical Center in, Yuma, AZ

Anxiety disorders, including social phobia, occur often in children with autism spectrum disorders (ASD; Gillott, Furniss, & Walter, 2001; Leyfer et al., 2006; Simonoff et al., 2008); however, little is known about the conceptualization and treatment of social phobia in this population. The current study presents the case of “James,” a 6-year-old male with comorbid Asperger's disorder and social phobia. A cognitive-behavioral treatment (CBT) package, including parent training, psychoeducation, graduated exposure, and reinforced practice, was implemented to decrease avoidance behaviors in social settings. Results showed that James's avoidance behaviors were reduced significantly following intervention, suggesting that a CBT approach may be effective in treating social phobia in children with ASD. The significant influence of CBT on decreasing avoidance behavior in comorbid social phobia and ASD highlights the need for continued investigation of adapting CBT for use with children with ASD.

The present study examines the attentional bias hypothesis for individuals with generalised social phobia (GSPs). Socially phobic individuals were hypothesised to exhibit attentional bias towards threat stimuli relevant to interpersonal situations. This hypothesis was tested using the face-in-the-crowd paradigm. GSPs and nonanxious controls (NACs) detected an angry, happy, neutral, or disgust target face in a crowd of 12 distracter photographs. Results indicated that, compared to NACs, GSPs exhibited greater attentional biases for angry than for happy faces in a neutral crowd. GSPs were more slowed down in their performance by happy and angry versus neutral distracters; NACs did not exhibit such sensitivity to distracter type. Finally, GSPs were faster in detecting anger than disgust expressions; NACs detected both types of faces equally quickly. Implications of these findings for the maintenance of social phobia are discussed.

This study's primary goal was to examine relations between symptoms of specific social phobia (SSP), generalized social phobia (GSP), avoidant personality disorder (APD), and panic and depression. Past research has suggested a single social phobia continuum in which SSP displays less symptom severity than GSP or APD. We found SSP symptoms correlated less strongly with depression but more strongly with panic relative to both GSP and APD symptoms. These findings challenge a unidimensional model of social phobia, suggesting a multidimensional model may be more appropriate. These findings also inform current research aimed at classifying mood and anxiety disorders more broadly by identifying that the different factors of fear versus distress appear to underlie different subtypes of social phobia.

The present investigations examined the factor structure and psychometric properties of two new self-report measures of social phobia, the Social Interaction Anxiety Scale (SIAS) and the Social Phobia Scale (SPS). A confirmatory factor analysis in Study I provided support for the fit of a two-factor model of the SIAS and SPS. Internal consistency estimates were high for the original two scales with a sample of 200 undergraduates. Also, using an item parceling procedure, the obtained internal consistency reliability indices for each parcel were acceptable. Results of the CFA in Study II provided support for the factorial stability of the model identified in Study I. Furthermore, multisample analyses showed invariant patterns for factor loadings and factor correlations across 138 men and 272 women. Gender differences were not observed in the mean SIAS and SPS scale and item scores. Both scales correlated negatively and significantly with measures of social desirability. Concurrent validity was established for the scales. The SPS was less specific than the SIAS to symptoms of social phobia.

The Social Phobia and Anxiety Inventory (SPAI) is an empirically derived self-report inventory developed as a specific measure of social phobia. The current investigation included two studies. The first examined the correlation of the SPAI with daily social behavior of a clinic sample of social phobics. The results indicated that the SPAI provides a reasonable indication of the distress experienced during daily social encounters in three dimensions: behavior, cognitions, and overall distress. The second study examined the validity of the SPAI with reference to the somatic response and avoidance behavior of social phobics. The results indicated that the somatic items of the SPAI are related to the somatic response of social phobics and that performance on the SPAI is associated with avoidance behavior in an anxiety-producing task.

The present study aimed to examine the relevance of age of onset to the psychopathology of social phobia using a large clinical sample of 210 patients with social phobia. The two most common periods of onset were during adolescence (ages 14–17) and early childhood (prior to age 10). Structural regression modeling was used to test predictions that early onset social phobia would be associated with greater severity of the disorder, stronger current symptoms of depression and anxiety, greater functional impairment, and more pronounced levels of emotional disorder vulnerabilities (e.g., neuroticism/behavioral inhibition, extraversion, perceptions of control). Logistic regression was used to evaluate relationships between age of onset and the presence of acute and chronic stress at the time of onset. Results showed that earlier age of social phobia onset was associated with stronger current psychopathology, functional impairment, and emotional disorder vulnerabilities, and that later age of onset predicted the presence of an acutely stressful event around the time of disorder emergence. These results are discussed in regard to their clinical implications and congruence with prominent etiological models of the emotional disorders.

The objectives of the present study were to adapt and analyze the factor structure, reliability, and validity of the Social Phobia and Anxiety Inventory for Children (SPAI-C; Beidel, Turner, & Morris, 1995) in a Spanish population. The SPAI-C was applied to a sample of 1588 children and adolescents with ages ranging from 10 to 17 years. The confirmatory factor analysis (CFA) showed a four-factor structure: Public performance, Assertiveness, Fear and avoidance/escape in social encounters, and Cognitive and psychophysiological interferences. Internal consistency was high (.90) and test-retest reliability was moderate (.56). Significant differences were found in the variables sex and age, although the effect size was small in both variables and their interaction. Overall, the increase of the age value was inversely proportional to that of social anxiety measured with the SPAI-C; in participants of the same age, values were higher for girls than for boys. Results suggest that the Social Phobia and Anxiety Inventory For Children is a valid and reliable instrument to assess social anxiety in Spanish children and adolescents.

We examined perfectionism, interpersonal sensitivity and dysfunctional cognitions in patients with Social Phobia (SP). The sample consisted of a clinical group with a diagnosis of SP (n?=?30) and a non clinical group (n?=?30), matched for age and gender. Both groups were assessed on Frost??s Multidimensional Perfectionism Scale (FMPS), Interpersonal Sensitivity Measure (IPSM), and Dysfunctional Attitudes Scale (DAS). The clinical sample was also assessed on Liebowitz Social Anxiety Scale (LSAS), Beck??s Depression Inventory-II, Eysenck??s Personality Inventory-Neuroticism sub-scale (EPI- N). The two groups were compared on FMPS, IPSM and DAS using t-test. Associations between FMPS, IPSM, DAS and predictors of social anxiety and depression were examined. The clinical group scored higher on perfectionism, interpersonal sensitivity and dysfunctional cognitions. There was no significant association between total scores on FMPS and IPSM. However subscales of IPSM and FMPS were correlated. Doubt about Actions was associated with Separation Anxiety (r?=?0.520) and Timidity (r?=?0.407). Organization was related to Interpersonal Awareness (r?=?0.371) and Separation Anxiety (r?=?0.407). Parental criticism was negatively associated with fragile inner self. DAS was positively correlated with FMPS and IPSM. Interpersonal sensitivity emerged as a significant predictor of social anxiety. Discriminant Functional Analysis indicated that concern over mistakes, organization, fragile inner self, separation anxiety discriminated between the clinical and non-clinical groups.

The Social Phobia Inventory (SPIN) is a widely used measure in mental health settings and a 3-item version (mini-SPIN) has been developed as a screening instrument for social anxiety disorder. In the present study, we examined the psychometric properties of the SPIN and developed a brief version (mini-SPIN-R) designed to assess social anxiety severity using item response theory. Our sample included 569 individuals with social anxiety disorder who participated in 2 clinical trials and filled out a battery of self-report measures. Using a nonparametric kernel smoothing method we identified the most sensitive items of the SPIN. These 3 items comprised the mini-SPIN-R, which was found to have greater internal consistency, and to capture a greater range of symptoms compared to the mini-SPIN. The mini-SPIN-R evidenced superior convergent validity compared to the mini-SPIN and both measures had similar divergent validity. Thus, the mini-SPIN-R is a promising brief measure of social anxiety severity.

Current models divide social phobia into specific (SSP) and generalized (GSP) subtypes and suggest strong overlap between GSP and avoidant personality disorder (APD). Meanwhile, other research suggests reclassifying anxiety and mood disorders as fear and distress disorders. To unify these separate lines of research, this study was designed to test the hypothesis that SSP is more related to fear disorders (e.g., panic and phobias), whereas GSP and APD are more related to distress disorders (e.g., depression and generalized anxiety). Confirmatory factor analysis suggested the best-fitting model had symptoms of GSP, APD, and depression loading on one factor, and symptoms of SSP, panic, and specific phobias loading on a second factor. Key components of this model were (a) the inclusion of GAD symptoms reduced model fit and (b) GSP and APD symptoms significantly predicted SSP symptoms; this is consistent with conceptualizations of individuals with both GSP and SSP reporting performance anxiety.

After an intense panic attack at school this 14-year-old male became virtually housebound and did not attend school for five months prior to therapy. Treatment, consisting of self-monitoring, relaxation, thought stopping imaginal desensitization, and "in vivo" exposure, ended after 10 weakly sessions when the boy returned to school. At 18 months follow up, school records showed nearly perfect attendance and improved grades to a "B+" from a pretherapy "C" average. MMPI changes from pretherapy to 12 months post-therapy andan interview at 12 months follow-up showed additional improvements in overall personality adjustment and social behavior. Results illustrate hypotheses about choice of treatment methods proposed in a recent review of densensitization with children.

ABSTRACT— "Holding warm feelings toward someone" and "giving someone the cold shoulder" indicate different levels of social proximity. In this article, we show effects of temperature that go beyond these metaphors people live by. In three experiments, warmer conditions, compared with colder conditions, induced (a) greater social proximity, (b) use of more concrete language, and (c) a more relational focus. Different temperature conditions were created by either handing participants warm or cold beverages (Experiment 1) or placing them in comfortable warm or cold ambient conditions (Experiments 2 and 3). These studies corroborate recent findings in the field of grounded cognition revealing that concrete experiences ground abstract concepts with which they are coexperienced. Our studies show a systemic interdependence among language, perception, and social proximity: Environmentally induced conditions shape not only language use, but also the perception and construal of social relationships.

Social phobia is a debilitating anxiety disorder that often goes undetected in young children, but can be effectively treated with cognitive-behavioral interventions. For children and adolescents, treatment usually includes education, social skills training, cognitive restructuring, relaxation training, and exposure. However, for very young children who present with social phobia, it is necessary to adapt treatment to the developmental level of the child. A case illustration demonstrates the way in which cognitive behavioral treatment (CBT) was modified for the treatment of a five year-old girl with social phobia. Several modifications were made, including utilizing novel exposure techniques and emphasizing parent management training in order to promote generalization of treatment gains outside of session.

The Social Relations Model: An integrative method for personality research
As outlined by Snyder and Ickes (1985), the study of personality can be undertaken using one of three research approaches dispositional, situational, and interactive We show how the Social Relations Model provides an integrative method to estimate simultaneously dispositional, situational, and interactive effects Reviewed are component approaches to the study of personality The Social Relations Model is shown to be a component model (a special case of generalizability theory) applied in a social interaction context In the model, dispositional, situational, and interactive effects are termed actor, partner, and relationship effects, respectively The Social Relations Model can be used to answer a number of important issues in personality research The model can be used to assess reliability, measure the validity of self-ratings, and validate self-report inventories The model requires special designs in winch each person interacts with multiple partners Empirical examples are presented in which social anxiety, sex role inventories, and self-disclosure are studied

This study explored the relations between 5- and 6-year-olds' (N = 40) past social experiences, social knowledge, and interpretation and recall of social situations. Children were read stories depicting a fictional child's behaviors, and path analyses related children's impressions of the story character and story recall to their expectations about social situations and parent reports of their behavioral histories. Children's behavioral histories predicted their social expectations, which predicted their impressions of the character, and their impressions shaped their memories of the character's behaviors. These links, however, varied according to the complexity of social knowledge. The findings suggest that individual differences in children's interpretation and recall of identical events may be partially explained by variations in past experience and background knowledge.
