Pain-related correlates of MMPI profile subgroups among back pain patients

The MMPI profiles of 96 male and 218 female patients attending a back pain clinic in a private university-affiliated, orthopedic hospital were analyzed by a hierarchical clustering procedure. The clustering procedure produced four male and four female profile subgroups. The subgroups were compared with one another on the basis of patients' responses to the Cornell Medical Index and revised McGill Pain Assessment Questionnaire. Within the male and female patient cohorts it was found that profile subgroups featuring elevated clinical scales showed greater disruptions of daily activities than did subgroups with relatively unelevated profiles. However, profile subgroups with elevations primarily on the neurotic triad scales reported greater affective disturbance and disruption of daily activities than did subgroups with elevations on both the neurotic triad and relatively psychotic scales. In addition, profile subgroups with subclinical elevations on the neurotic triad scales appeared to have adjusted to their pain experience more poorly than did subgroups that featured scores on nearly all clinical scales that were within one standard deviation of the mean. Suggestions are provided for the use of the MMPI in assessing chronic pain patients and future research regarding cluster analyses of patients' MMPI profiles.

This paper addresses various training issues of psychotherapy training in mental hospitals. It discusses some particular attitudes about the use of the Psychoanalyst as a Psychotherapist as well as the Institutions' (i.e. psychiatric establishments) desire often for a superficial teaching in Psychotherapy. It describes a training for NHS staff in psychodynamics and in particular in use of the counter-transference from Balint's model of general practitioner training groups.

Training groups most usually seen in GP seminars, with their central use of counter-transference by the analyst, can be extended to train medical students, nurses, social workers, etc. It is a form of training that uses the analytic stance rather than something of lesser alloy, and allows the Psychoanalyst to be himself rather than function at the level of a Psychotherapist alone.     

Some properties of the ego-resiliency subtle scale of the MMPI.

The Ego Resiliency-Subtle (ER-S) scale was developed by Block (1965) as a social desirability free measure of the first factor of the MMPI. The purpose of the present experiments was to determine whether or not the 40-items comprising the ER-S scale remain neutral regarding social desirability and if the subject's score on the scale remains constant when taken out of the context of the other MMPI items; if so, the ER-S scale could be used as a screening device. Experiment I demonstrated that the items were not neutral regarding social desirability when taken out of context, but Experiment II indicated that this did not influence the responses subjects gave to the items. Because a memory factor may have been operating in Experiment II, a third experiment was conducted to minimize this factor resulting in a male-female vs. embedded-alone interaction and an order of presentation vs. embedded-alone interaction.     

Musculoskeletal status and disability of MMPI profile subgroups among patients with low back pain

The Minnesota Multiphasic Personality Inventory (MMPI) profiles of 205 male and 157 female patients with acute and chronic low back pain (LBP) were studied for replicable homogeneous subgroups using three cluster-analysis procedures. Two normal and three clinically elevated profile subgroups were identified. The two normal subgroups were characterized by relatively normal musculoskeletal condition and were least disabled, but they differed somewhat from each other in duration of pain, presence of physical abnormalities, and daily functioning. The third subgroup had the following characteristics: an elevated neurotic triad, greatest duration of symptoms, most abnormal physical findings, greatest limitation of motion, most pain complaints during physical examination, and the fewest number of intact functional activities. The fourth subgroup, with an elevated neurotic triad plus subclinical elevations on other scales, had somewhat less musculoskeletal impairment but had extensive work disability, financial compensation, and previous surgeries. The fifth and most pathologic profile subgroup surprisingly consisted largely of acute-pain patients whose musculoskeletal condition and daily functioning were similar to those of the normal subgroups. Patients in the three abnormal MMPI subgroups were exposed to more LBP physical-risk factors in the workplace. Implications for psychological treatment with these different profile types are discussed.     

Subtle-obvious scales of the MMPI: indicators of profile validity in a psychiatric population.

This study explored the usefulness of the Minnesota Multiphasic Personality Inventory (MMPI) Subtle-Obvious scales as profile validity indicators with a inpatient psychiatric population. Some 292 MMPI profiles were utilized and divided into overreporters, underreporters, and standard reporters, based on their Subtle-Obvious scale scores. Reporting style was shown to be unrelated to actual patient pathology because of the lack of relationship between reporting style and diagnostic categorization according to the Diagnostic and Statistical Manual of Mental Disorders (3rd ed., rev. [DSM-III-R]; American Psychiatric Association, 1987) Axis I and II. Comparisons of MMPI profiles of the three groups revealed that overreporters endorsed more pathology on the MMPI clinical scales than did either underreporters or standard reporters. The same pattern of response style was demonstrated by subjects on another objective measure, the Beck Depression Inventory, whereas on a projective measure, the Rorschach Inkblot Test, there were no differences between groups. These findings suggest that clinicians may want to utilize the Subtle-Obvious scales to gain information about MMPI profile validity. Specifically, profiles of patients identified as overreporters should be interpreted with caution so as to not overstate their level of pathology.     

Some uses of simulation in an in-service counselling training programme

In an in-service, pre-professional training programme offered in Gwent, a variety of simulation techniques are used to promote an understanding of the core facilitative conditions of counselling and to enable participants to experience their importance in counselling situations. In this paper, eight specific simulation techniques are described in terms of their objectives and procedures. The advantages and disadvantages of each technique are explored. Teaching and ethical issues raised through the use of such simulations in training programmes at this level are also discussed.     

Some reflections on "MMPI short forms: a literature review"

Stevens and Reilley (1980) have suggested that recent changes in the methodological approaches for developing short forms of the MMPI make them an expanding and fruitful area of research. Since Stevens and Reilley did not address several important issues in the development and use of Short forms, these issues are reviewed and suggestions are presented for future research with short forms.     

Family sculpting: II. Some practical examples

This paper reviews a number of practical examples of family sculpting to illustrate possibilities for its use described in a general way in an earlier companion article (Journal of Family Therapy (1981) 3: 341–352). Examples are considered first in relation to generic practical possibilities for sculpting: structural analysis and diagnosis, the inescapability of the concrete, the portrayal of ambiguity. A second set of examples is considered in relation to major practical contexts of sculpting: with children, with students, with workers. The paper concludes with a discussion of the complexity of family sculpting and a comparison of the structural interpretation of sculpting offered here with Papp et al. 's notion of choreography.
