Some aspect of psychosocial criteria is commonly utilized by most transplant programs in assessing candidates' acceptability for transplantation. However, regardless of the assessment methodology, information obtained in pretransplant assessments may be limited given the evaluative nature of the assessment as well as the sensitive nature of the contents. Indeed, transplant candidates may present themselves in a favorable fashion, minimizing any negative traits or psychological dysfunction which they perceive might prevent transplantation. Unfortunately, there are limited data addressing the extent to which transplant candidates may present themselves in an overly positive light. This investigation surveys the prevalence of social desirability in lung transplant candidates as well as its association with self-reports of perceived stress. Further, the relationship between social desirability and interviewer ratings of transplant candidacy is examined. Subjects included 24 patients in end-stage organ failure being evaluated for lung transplant candidacy. Subjects completed the Perceived Stress Scale and a brief version of the Marlowe-Crowne Social Desirability Scale. Additionally, subjects were interviewer-rated on the Psychosocial Assessment of Candidates for Transplantation. Results indicate social desirability is a prevalent phenomenon in lung transplant candidates, with more than half of the sample scoring at or above the 84th percentile on the social desirability measure. Further, self-reports of perceived stress are moderately and inversely associated with social desirability (r=–.55,p .01). Social desirability was unrelated to interviewer-ratings of transplant candidates acceptability (r=.13,p .56). Future research might include larger samples of subjects, other organ transplant candidates, and more detailed assessments of symptom distress.  相似文献

Adolescent inpatients with a diagnosis of alcohol abuse (n=28), nonalcohol substance abuse disorders (n=15), or psychiatric disorders without substance abuse (n=46) were examined on measures of attentional, intellectual, and personality functioning. High scores on a personality measure, the Substance Abuse Proclivity Scale (SAP), was the best predictor of substance abuse in general. Alcohol abusers were differentiated from other substance abusers by their increased impairment on the Continuous Performance Test. Adolescents with alcohol abuse did not manifest the types of cognitive impairments associated with chronic alcoholism. These data suggest that certain attentional impairments may be associated with early onset alcohol abuse and that earlier conceptions of the role of attentional impairments in risk prediction for alcohol abuse are supported.This research was supported by the Four Winds Foundation.  相似文献

Without a transplant, end-stage liver disease is associated with significant morbidity and mortality. Transplant candidates endure physical and psychological stress while awaiting surgery, yet little is known about the relationship between physical health and psychological resilience during the wait-list period. This study examined predictors of psychological resilience and mediators of the relationship between physical health and psychological resilience in liver transplant candidates. Wait-listed candidates (N?=?120) from a single Northeast transplant center completed assessments of physical functioning, coping, perceived social support, and resilience. Findings revealed that physical functioning, active coping, and perceived social support were positively associated with resilience; maladaptive coping was negatively associated with resilience. Perceived social support and active coping partially mediated the relationship between physical functioning and resilience. Transplant center care providers should promote active coping skills and reinforce the importance of effective social support networks. These interventions could increase psychological resilience among liver transplant candidates.  相似文献

Male alcoholic veterans (N=194) were divided into four groups on the basis of their scores on the MacAndrew scale (MAC) of the Minnesota Multiphasic Personality Inventory (MMPI) and compared on psychological and sociodemographic measures and self-reported alcohol, drug, and legal histories. Nineteen subjects scored in the nonalcoholic (NA) range of the MAC (i.e., false negatives, MAC23). Remaining subjects, scoring in the alcoholic range of the MAC, were divided into thirds by MAC scores (lowest third, L-MAC=24–28,n=53; middle third, M-MAC=29–31,n=63; highest third, H-MAC>31,n=59). Groups did not differ on age, any important sociodemographic variables, or scores on the Profile of Mood States, State-Trait Anxiety Inventory, Symptom Checklist-90, or Rod and Frame Test. Significant group differences were obtained on several MMPI scales. Lower MAC scores were associated with higher scores onL, K, D, andR, and lower scores onMa. There was also a tendency for subjects with lower MAC scores to score higher onSi. NA subjects began drinking and heavy drinking later than other subjects, although groups did not differ on duration, quantity, or frequency of drinking. Subjects with higher MAC scores more often reported drinking in bars, drinking liquor straight, alcohol-related job disruptions, and previous use of marijuana, hallucinogens, and barbiturates. Subjects with higher MAC scores had more often been arrested, convicted, fined, and jailed for alcohol-related offenses but not for other offenses. Results are examined with respect to MacAndrew's distinction between primary and secondary alcoholics and with respect to the possible relations of MAC to important person variables such as sensation seeking.The authors wish to thank Craig MacAndrew for his helpful critical reading of an early draft of this paper. The aim of this study was to assess the psychological state of women who have undergone surgery for breast cancer or cardiac surgery, including examination of the role of social support in both groups. The study included 48 women (mean age: 66.04?±?8.3 years). They were divided into two groups according to diagnosis: 23 women (mean age: 69.2?±?8.6 years) who underwent heart surgery (cardiac group, CG) and 25 women (mean age: 63.2?±?7.0 years) treated for breast cancer and associated with the Women After Mastectomy Club (oncology group, OG). In addition to the assessment of socio-demographic variables, the following self-report questionnaires were administered: Satisfaction with Life Scale, Acceptance of Illness Scale, Beck Depression Inventory, Spielberger State-Trait Anxiety Inventory as well as Berlin Social Support Scale. In the CG, the severity of depressive symptoms was two times higher than in OG (p?=?.003). In both groups, there was a high percentage (80%) of women with severe symptoms of anxiety (p?=?.37). In both groups, the level of life satisfaction was similar (p?=?.58), but OG was characterized by a higher level of acceptance of the disease (p?=?.003). The correlation analysis showed that in both groups, social support was related differently to the parameters of emotional state. Women treated for breast cancer were in a better mental condition than women treated for heart disease. The support coming from other women in similar circumstances (Women After Mastectomy Club) seems to be more effective than the support coming from the patient’s immediate environment. The results for social support ought to be interpreted not only through the prism of mean values of received support, but also with regard to the information on the sources of support.  相似文献

A sample of 437 patients completed self-report measures of quality of life and social support while they were being evaluated for bone marrow transplantation (BMT) at The Johns Hopkins Oncology Center. Generally, the candidates showed reasonably high levels of quality of life (QOL) on the Satisfaction with Life Domains Scale (SLDS), their present ranking on the Cantril Self-Anchoring Ladder of Life, and their scores on the Bradburn Positive Affect Scale. The level of QOL of these candidates for transplant was significantly related to their level of social support. Both availability and adequacy of social support for these transplant candidates were found to be significantly related to QOL as measured by the SLDS. Availability of social support as measured by patient membership in religious and other organizations was significantly related to Positive Affect but not Negative Affect. The Family APGAR and Relational Support Scales measures of social support were significantly correlated with both Positive and Negative Affect.  相似文献

Patterns and correlates of comorbidity, as well as differences in manifest depressive profiles were investigated in a sample of depressed adolescents. A sub-sample of the youth were characterized as belonging to either a Pure depression group, an Internalizing group (depression and co-occurring internalizing disorders), or an Externalizing group (depression and co-occurring externalizing disorders). Item response theory (IRT) and differential item functioning (DIF) were used to assess whether the depressed adolescents from the different comorbidity groups presented with different depressive symptoms. Results indicated that the comorbidity groups were meaningfully distinct in terms of psychosocial correlates as well as showed differences in depressive symptom profiles as informed by DIF analyses. In particular, the comorbidity groups differed in terms of presentation of psychomotor changes and cognitive impairments. Implications for assessment are discussed.

This article discusses group therapy as part of a multidisciplinary approach to the management of the various psychological and physical rehabilitation concerns posed by kidney and liver transplant candidates and recipients at Howard University Hospital in Washington, DC. The group's history, format, intervention foci, and roles of clinicians and patients attending the group are described and evaluated. Given the relative paucity of research literature in this area, the authors offer recommendations for empirical evaluation of the benefits of multidisciplinary group psychotherapy in ameliorating physical and emotional suffering and prolonging life among transplant patients.  相似文献

Prior research has shown that exposure to alcohol‐related images exacerbates expression of implicit racial biases, and that brief exposure to alcohol‐related words increases aggressive responses. However, the potential for alcohol cue exposure to elicit differential aggression against a Black (outgroup) relative to a White (ingroup) target—that is, racial discrimination—has never been investigated. Here, we found that White participants (N = 92) exposed to alcohol‐related words made harsher judgments of a Black experimenter who had frustrated them than participants who were exposed to nonalcohol words. These findings suggest that exposure to alcohol cues increases discriminatory behaviors toward Blacks.  相似文献

This study examined how mindfulness and integrative self‐knowledge were related to health‐related issues. Men in general population (n = 103) and coronary heart disease samples (n = 101) completed the Mindful Attention Awareness Scale, the Integrative Self‐knowledge Scale, the Type 2 subscale of the Interpersonal Reactions Inventory, the Perceived Stress Scale, and the Anxiety and Depression Scales. In both samples, there was a moderate positive correlation between mindfulness and integrative self‐knowledge and they were negatively correlated with all health‐related variables. However, only integrative self‐knowledge explained independent variance in health‐related variables. Specifically, in both samples, the relationship between mindfulness and health‐related variables was mediated by integrative self‐knowledge. Mindfulness and integrative self‐knowledge are related domains of self‐awareness that are associated with a range of health‐related variables. These relationships are robust across samples drawn from general population and patients with coronary heart disease. The finding that integrative self‐knowledge explained additional variance in the health‐related variables after the contribution of mindfulness had been accounted for suggests that reflective self‐awareness in integrative self‐knowledge may make a unique contribution to the explanation of individual differences in health variables.  相似文献

The Child Behavior Checklist for Ages 6–18 (CBCL/6-18) possesses newly developed DSM-Oriented Scales, constructed through expert clinical judgment to match selected categories for behavioral/emotional problems as described in the DSM-IV. The present investigation examined the basic psychometric properties for all six DSM-Oriented Scales (i.e., Affective, Anxiety, Somatic, Attention-Deficit/Hyperactivity, Oppositional, and Conduct Scales) in a large clinical sample of children and adolescents (N = 673). Findings from the present study provide strong evidence for the reliability, as well as convergent and discriminative validity, of these scales. It appears that the DSM-Oriented Scales may provide accurate supplementary information that may be considered when formulating clinical diagnoses.

The present study aims to estimate the relationship between child alcohol problems, sociodemographic characteristics, externalizing behavior, parental and peer alcohol use, and family violence by using a cross-sectional community sample of 320 Ukrainian children (9–16 years of age, 50% boys) and their parents. Participants answered questions from the Drinking and Drug History and Current Use Patterns Questionnaire, the Revised Conflict Tactics Scales, the Alabama Parenting Questionnaire, and the Child Behavior Checklist. Fifty-two percent of children reported alcohol use within the past year and 32% experienced alcohol-related problems. The average number of reported alcohol problems was 11.19 (SD?=?63.65). Five robust regression models examined correlates of early problem drinking in Ukraine. The final model indicated older child age (β?=?0.21, p?<?0.001), more symptoms of externalizing behavior (β?=?0.17, p?<?0.01), and higher peer alcohol use (β?=?0.23, p?<?0.001) were significantly and positively associated with child alcohol problems. Results of the final model explained 32% of the variance in child alcohol problems, F(8, 311)?=?10.76, p?<?0.001. In conclusion, the findings suggest that older age, exposure to high-risk alcohol permeated peer environments, impulsivity, and rule-breaking behaviors are linked with the trajectory of early alcohol abuse among Ukrainian children. Mechanisms that reduce the harmful influence of these risk factors on alcohol consumption need to be in place.  相似文献

Objective: The majority of psychological studies with organ transplant recipients have examined negative psychological effects. This study aimed to further investigate the positive effects of organ transplantation and to construct a specific measurement instrument.

Design: The initial pool of 14 items for the Positive Effects of Transplant Scale (PETS) was derived from organ recipient interviews. A cross-sectional postal study included 87 heart, 46 lung and 193 liver transplant recipients. The PETS was subjected to principal components analysis (PCA) using varimax rotation, and associations with other measures investigated.

Main outcome measures: PETS and an open-ended item about positive effects.

Results: Coding of the open-ended item revealed that the majority of recipients attributed positive life changes to the transplant experience. PCA of the PETS indicated three factors that accounted for 58.82% of the variance. The 12-item questionnaire assesses improvements in: (1) life philosophy, (2) gratitude and (3) health. The total PETS scores exhibited adequate internal consistency and validity.

Discussion: Most transplant patients report positive psychological effects, which suggests this may be an understudied area. The initial development of an assessment tool provides researchers and clinicians a way to assess the degree and nature of these life changes.  相似文献

In two studies, we compared intentional and non-intentional measures of emotional well-being in terms of reliability and validity. In the first study, we expanded on the findings of ?im?ek (2011 ?im?ek, Ö. F. (2011). An Intentional Model of Emotional Well-Being: The Development and Initial Validation of a Measure of Subjective Well-Being. Journal of Happiness Studies, 12, 421442. [Google Scholar]) by showing that an intentional measure of emotional well-being, namely the Emotional Well-Being Scale, accounted for unique variance in both negative and positive mental health indicators above and beyond the variance accounted for by Larsen and Diener's Scales (1992), which measures all dimensions of affect circumplex. The correlated traits-correlated uniqueness model results showed that the original-intentional version of the Emotional Well-Being Scale explained more variance in the measurement model than the non-intentional version. The second study showed that the results were similar for the intentional vs. non-intentional versions of the Positive and Negative Affect Schedules (Watson, Clark &; Tellegen, 1988 Watson, D., Clark, L. Healthy college-age males and females classified as Type A or Type B were randomly assigned to an alcohol (N=24) or a no-alcohol condition (N=24). Subjects were exposed to a verbal stress quiz while blood pressure, heart rate, peripheral vascular response (PVR), and self-reported anxiety indices were monitored. Results indicated that alcohol effected a reduction in resting levels of systolic blood pressure and a tonic reduction in the peripheral vascular response. Alcohol attenuated the systolic blood pressure, heart rate, and PVR to stress and efffected a decrease in anxiety following the Stressor. When analyzed relative to the participants' drinking experience, data indicated that the strongest stress-modulating effect of alcohol was evidenced by Type A subjects who have been identified as long-term drinkers. During recovery from stress Type A subjects identified as short-term drinkers maintained high levels of PVR. Types A and B subjects who did not ingest alcohol evidenced high PVR levels during stress. The findings are discussed in terms of the protective action of moderate chronic alcohol use on cardiovascular disease risk reduction in persons evidencing the coronary-prone behavior pattern.  相似文献

Introduction

Self-discrepancies (the distances between the perceived self and the ideal or the socially prescribed selves) are a hallmark in psychological distress. However, a clinical tool evaluating these discrepancies is lacking.

Objective

To investigate the validity, the psychometric characteristics and the clinical relevance of the Self-Discrepancy Scale, an instrument designed to assess with multiple indices discrepancies between mental representations of the self: the actual self, on the one hand and ideal or socially-prescribed selves, on the other hand.

Method

The Self-Discrepancy Scale has been administered to a large community sample, together with measures of depression, anxiety, self-esteem, and self-efficacy. It was also proposed to an additional clinical sample composed of clients with a diagnosis of mood or anxiety disorders seeking psychotherapeutic help.

Results

A factor analysis evidenced three underlying dimensions to self-discrepancies: the size of the discrepancies, the resulting distress and the presence to unwanted traits. Test-retest consistency is in the acceptable range. Different profiles of self-discrepancies distinguished clinical groups suffering from different disorders.

Conclusions

The data suggest that the Self-Discrepancy Scale is a valid measure of self-discrepancies and a valuable predictor of emotional vulnerability, especially with regards to abstract global judgments of discrepancies and of discrepancy induced distress. It is concluded that the Self-Discrepancy Scale offers a valuable help in clinical settings.  相似文献

Secretarial and management students were compared on characteristics that have differentiated between traditional and nontraditional career women. Fifty-five female secretarial students and 55 female management (bachelor of commerce) students completed the following measures: (a) the Work and Family Orientation Questionnaire (WOFO); (b) the Attitudes toward Women Scale (AWS); (c) the Powerful Others, Personal Control, and Chance Scales (I-E Scale); (d) the Personal Attributes Questionnaire (PAQ); and (e) a measure of career choice considerations. The groups differed on two aspects of the WOFO (competitiveness and desire to master difficult intellectual challenges), and on Personal Control, sex role orientation (PAQ), and career choice considerations. The results are discussed with respect to sex role socialization and the denigration of secretarial work.  相似文献

This study used latent variable mixture modeling (LVMM) with a combined sample of undergraduates and internet users who endorsed non-suicidal self-injury (NSSI; N?=?440) to identify profiles of non-suicidal self-injurers. LVMM results revealed four distinct subtypes (“Experimental NSSI,” “Mild NSSI,” “Multiple Functions/Anxious,” and “Automatic Functions/Suicidal” groups) that were similar to those found in a prior study of undergraduates Klonsky and Olino (Journal of Consulting and Clinical Psychology 76:22-27, 2008), with an additional fifth “Multi-method.” ANOCOVAs and logistic regressions showed the first four groups differed on aspects of psychopathology in a similar fashion to the previous study. Specifically, the Experimental and Mild NSSI groups had the lowest levels of psychopathology, and the AF/Suicidal group had higher levels of psychopathology than the first three groups except for anxiety, which was higher for the MF/Anxious group. The additional Multi-method group was found to have higher levels of psychopathology than all other groups, but not significantly different from the MF/Anxious group. With regard to alcohol use and drinking motives, the MF/Anxious group had the highest level of hazardous drinking and drinking motives related to coping with depression and anxiety, enhancement, and conformity. The other four groups were not differentiated from one another on any drinking variable. Findings suggest that NSSI may be better conceptualized as a heterogeneous syndrome in clinical and research settings, with one sub-group being more at-risk for hazardous alcohol use.  相似文献
