Living in Ambiguity: Mission Formation as Living among Counter‐communities

This article has been developed from a short response given to Prof Néstor Míguez's paper, “Mission Formation for Transforming Discipleship” (featured in this issue of IRM) at the Commission for World Mission and Evangelism's Consultation on Mission Formation, held in Matanzas, Cuba, from 10 to 15 September 2016. The article sets out to explore Míguez's concept of living in ambiguity as a call to mission formation to create transformative counter‐communities, which can equip people to live with the struggle of justice promised and justice denied. The paper explores this through a number of lenses. The letter of Jeremiah to the exiles in Jeremiah 29 offers a biblical context for creating communities which are counter to dominant and imperial values. The work of French philosopher Michel Foucault on heterotopias offers a philosophical framework for such communities.     

Psychiatry and religion circa 1978: Analysis of a decade,part I

Prior tensions between science and religion have dissolved and coalesced into new alliances based on ideology and philosophy. This general cultural pattern is reflected in the realignment of interests and interactions between psychiatry and religion. There are increasing numbers of mental health professionals with devout religious commitments and involvement in religiously oriented mental health activities; while clergy have developed new organizational structures to reflect many diverse mental health interests, including pastoral counseling, community mental health chaplains, hospital chaplains, and expanded parish ministries. Clinical and research literature has continued to rapidly proliferate, while a unique genre of pastoral care and counseling literature has emerged. A decade ago there was hope for an amicable alliance between specialists in psychiatry and in religion. Medical psychotherapy consultation: psychoanalytic psychiatry for the patient and professional

An NHS Mental Health Trust Medical Psychotherapy Consultation Service using psychoanalytic psychiatry to help the patient and professional is described. The Consultation Service established in 2000 is offered to secondary acute and community mental health teams and primary care. The service was evaluated as a basis for regional and national development. Between 2006 and 2013, 87 consultations from 210 were sampled to ascertain demographic and diagnostic profiles and outcomes of the consultation process. We conducted an online survey of local consultant psychiatrists’ views about the service, and undertook a thematic analysis of the free text comments. We also conducted a survey of members of the Royal College of Psychiatrists’ Medical Psychotherapy Faculty to ascertain whether similar consultation services existed elsewhere in the UK and had been evaluated. The Leeds model of psychoanalytic consultation – a ‘consultation sandwich’ – is described. From a psychoanalytic perspective, the work of consultation is seen as an extension of the dynamic field of the analytic situation. This paper develops the concept of a bastion – an omnipotent reserve in and between the patient and professional derived from adhesive identifications leading to stuck relationships. The adhesive identification in the patient and professional acts like a ‘grievance glue’ – a mutual manifestation in a last bastion of painful limitations not faced, losses not grieved.     

The element of hostility in parish work

This article might well be read in conjunction with the question on Volunteers in Parish Work which is discussed in the Consultation Clinic of this issue. It was written at our request in order to explore more fully at least one facet of parish work. Other articles will follow. We will welcome our readers' comments as to the helpfulness of this material and suggestions for further exploration.     

Outpatient groups for chronic psychiatric patients

Long-term support groups have been utilized at the University of Alberta Hospitals since 1977 in order to meet the needs of chronic psychiatric patients who are unwilling or unable to commit to insight-oriented therapy. These groups meet once weekly for one hour and are intended to be drop-in groups, which patients can use regularly or only while in crisis. It is hypothesized that the institution or style of therapy practiced at an institution, rather than the specific therapists, provides a source of stability and support to these patients. Our experience suggests these groups are beneficial and cost-efficient, but research is necesssary to evaluate them to determine components of therapeutic effectiveness.Linda McAuley, M.A., was a staff therapist at the Psychiatric Walk-in Clinic and is currently in private practice.An earlier version of this paper was presented at a meeting of the Canadian Group Psychotherapy Association in Banff, Alberta, October, 1987.     

What is the question you need to answer? How consultation can prevent professional systems immobilizing families

Consultation to an outside agency is an important part of the work of departments of child and adolescent psychiatry. This article describes and illustrates one particular approach to such consultations based on the principles of structural family therapy. It is premised on the belief that outside agencies have the capacity to arrive at a decision which is in the best interest of their clients. To do this the agency needs to define what is the question they need an answer in order to make a decision regarding their client. The consultants' role is to clarify what decision the consul tee agency needs to make by following up the question, What is the question you need to answer? How this question is followed up is discussed.     

Basic Personality Inventory clinical and validity scales: Stability and internal consistency

A sample of 71 incarcerated adult male inmates was used to investigate the stability and internal consistency for the clinical and validity scales of the Basic Personality Inventoiy. One month test-retest stability coefficients for the clinical scales ranged from .53 to .77. KR-20 reliabilities ranged from .61 to .86 on each occasion. With the exception of the longer perseveration scales, stability was also reasonable for the validity scales. Overall, the Basic Personality Inventory clinical and validity scales demonstrated favorable stability and reliability in an inmate population.This research was supported by a STEP grant from the Alberta Government. Early rejected status and later social adjustment: A 3-year follow-up

Sociometric measures and rates of social interaction were obtained from 186 kindergarten children. Subjects were classified into popular, rejected, and average status groups on the basis of sociometric data. Teachers' ratings on the Bristol Social Adjustment Guides were obtained when the children were in grade 2 and again in grade 3. Analyses of variance showed rejected children to be significantly more acting-out than popular or average children by grade 3. Rejected children were rated as less physically attractive, doing less well in reading and in arithmetic. Negative peer nomination in kindergarten was more predictive of later acting-out in grades 2 and 3 than rate of interaction.Research was funded by Alberta Mental Health Advisory Council, and Alberta Advisory Committee for Educational Studies. A Review of Gerald Caplan's Theory and Practice of Mental Health Consultation

Gerald Caplan's Theory and Practice of Mental Health Consultation (1970), considered to be a classic seminal work in the field of consultation, is reviewed. First, the author presents Caplan's general definition of consultation along with what may be considered the general principles or procedures of consultation. Then the author presents Caplan's procedures and principles as relevant to each type of consultation. Next, each of Caplan's four models of consultation is described. Last, these four models are compared in terms of the professional role of the consultee, the primary goal of consultation, and the target of consultation intervention.     

Sentence-type variables as aural concept formation dimensions

An aural conjunctive concept formation experiment used as stimuli sentences varying systematically in Voice (active/passive), Mood (declarative/interrogative), Modality (affirmative/negative), tense, and lexical content. Target classes were the eight sentence types defined by all combinations of the first three syntactic variables. Aural processing was more difficult than visual, but higher education level facilitated concept acquisition for males and females equally (cf. Bakeret al., 1973). The 64 undergraduate subjects tended to avoid syntactic analysis in depth, classifying sentences on as cursory a basis as the task allowed. The simple, unequivocal syntactic signals of Mood and Modality meaning were readily apparent, but the discrimination of Voice was complicated by ambiguous syntactosemantic associations and lack of discourse context. Voice is thus not seen as a determinant of utterance type, but as a context- and content-dependent realization of agent or object focus in transitive messages.This report is based on an M.Sc. thesis completed at the University of Alberta (Edmonton, Canada), in 1972     

Career metaphors and their application in theory and counseling practice

This article, first given as an address to the January 2002 National Consultation on Career Development, Ottawa, Ontario, draws attention to the inevitability of metaphor in human thought, discourse, and theory, including that concerning careers. The article (a) explores the benefits and the dangers of metaphorical thinking and advocates the use of “multiple metaphor” as a means of understanding careers; (b) looks particularly at metaphors in the context of new, more flexible careers; (c) introduces 10 archetypal career metaphors; and (d) considers how career counseling practice can be assisted through the facilitation of metaphorical thinking.     

Group Psychotherapy—The Psychoanalytic Approach by S. H. Foulkes and E. J. Anthony

Group therapists often lead consultation groups. For effective leadership it is important to distinguish between consultation groups and psychotherapy groups. A consultation group is one in which a leader serves as consultant to the members. Group process variables of task, contract, role of the leader, and effect of the larger system should be understood by the consultant. The process of defining, testing, and maintaining these variables encourages exploration of resistance and parallel process, and is an important aspect of the ongoing learning. Consultation group leaders should understand the importance of serving as group leader but not group therapist.     

Consulting With Teachers in Two Areas: Grief and Mourning: Relaxation Techniques

Consultation with teachers remains untried by many counselors despite mandates, role definitions, and models for consultation. Barriers include the preference for the status quo and the lack of information and preparation. Involvement and training can help overcome these barriers. Two possible consultation areas are grief management and relaxation training. Grief is the normal reaction to any kind of loss. Being aware of the various stages is the first step in helping someone through the grief process. Relaxation techniques can be learned with proper guidance. Before tying any new ideas, however, counselors and teachers must have sufficient information and training.     

Evangelism from the Margins: Experiences of the Ironic in Evangelism in Cardiff,UK

This article arises from a presentation to the CWM/WCC Consultation, Explorations in Evangelism which took place in Sydney from 5 to 13 September 2015. There I shared the street art evangelism of my own congregation. However, in this article I want to explore how this street art evangelism is pointing us to the need for an appreciation of the ironic nature of evangelism, and consider evangelism from the margins. I explore this here as an aspect of the liberation missiology of the WCC's new ecumenical affirmation of mission: Together towards Life. But also show how it describes the historical roots of evangelism in the early church and captures the possibilities of evangelism now in a post‐Christian context like the UK. The article explores ideas and artwork as they invite an ironic appreciation of the countercultural nature of Christ's call to life, a call that questions the empires of Caesar and the church.     

The French model at work: indication and the Jean Favreau Centre for Consultation and Treatment

The goal of this paper is to present the way in which an analyst trained in the French Model at the Paris Psychoanalytical Society and working at its policlinic, the Jean Favreau Centre for Consultation and Treatment (CCTP), approaches the question of indication. After a brief examination of Freud ’s 1905 text on indication I will discuss certain concepts, such as representation and tertiary processes, which form the basis of our listening to today’s patients. After presenting certain elements of the French Model I will then discuss a period which was a turning point in French psychoanalysis. Psychodrama, a psychoanalytically inspired treatment modality developed in France, which is useful when the capacity to represent is limited will be introduced. Two clinical illustrations of consultations at the CCTP will then follow.     

Ethics Consultation Quality Assessment Tool: A Novel Method for Assessing the Quality of Ethics Case Consultations Based on Written Records

Although ethics consultation is offered as a clinical service in most hospitals in the United States, few valid and practical tools are available to evaluate, ensure, and improve ethics consultation quality. The quality of ethics consultation is important because poor quality ethics consultation can result in ethically inappropriate outcomes for patients, other stakeholders, or the health care system. To promote accountability for the quality of ethics consultation, we developed the Ethics Consultation Quality Assessment Tool (ECQAT). ECQAT enables raters to assess the quality of ethics consultations based on the written record. Through rigorous development and preliminary testing, we identified key elements of a quality ethics consultation (ethics question, consultation-specific information, ethical analysis, and conclusions and/or recommendations), established scoring criteria, developed training guidelines, and designed a holistic assessment process. This article describes the development of the ECQAT, the resulting product, and recommended future testing and potential uses for the tool.
