Is it permissible for a doctor or nurse to knowingly administer a placebo in a clinical setting? There is certainly something suspicious about it: placebos are typically said to be ‘sham’ treatments, with no ‘active’ properties and so giving a placebo is usually thought to involve tricking or deceiving the patient who expects a genuine treatment. Nonetheless, some physicians have recently suggested that placebo treatments are sometimes the best way to help their patients and can be administered in an honest way. These physicians conclude that placebo treatments are a perfectly acceptable, and ethically unproblematic, mode of treatment. While I grant the common idea that placebos are deceptive is correct, I argue that widespread misunderstandings concerning why this is so has led proponents of placebo treatments to respond to the charge of deception in a way that misses the mark entirely. My goal in this paper, then, is to develop a precise conception of what makes something a placebo, which in turn will clarify the central charge concerning the ethics of placebo treatment, viz. that it is deceptive.  相似文献

One is to undergo a deep-seated, qualitative change toward becoming the person one is capable of becoming. The related other goal is to be free of the worrisome, fearful scenes or situations of being crazy, out of one's mind, psychotic. Bryden, McManus, and Bulman-Fleming′s (1994) meta-analysis of studies examining the Geschwind-Behan-Galaburda hypothesis indicates that the original data set supports the hypothesis significantly more than the body of replication attempts. We present data on 2256 subjects that clearly fail to support the hypothesis and describe practical and statistical reasons suggesting why the original data cannot be easily replicated. A power analysis suggests that to find the effect in a general sample, one should plan to have 17,000 to 40,000 subjects.  相似文献

Attachment theory is a comprehensive, empirically supported theory of emotional, relational, and neurophysiological development. Modern psychoanalysis is a theory of technique that addresses early psychological deficits and conflicts considered beyond the reach of traditional psychoanalysis. Both orientations have influenced the theory and practice of group psychotherapy and emphasize personality maturation as a treatment goal. This paper explores their potential synergies when treating preoedipal, insecurely attached emotional states, disrupted emotional self-regulation, and impaired mentalization. Theoretical and technical applications are suggested that may enhance the treatment of disordered attachment in group psychotherapy. By addressing insecure attachment as resistance, modern psychoanalytic techniques may engage the emotional substrates of the attachment process to facilitate the expansion of relational capacities, mentalization, self-regulation, the differentiation of self/other representations, and epistemic trust.  相似文献

The study of ethnic and national identity, owing to the complex interaction between individuals, groups, and systems, has not remained confined to traditional disciplinary boundaries. In particular, a branch of social-psychological research has provided insight into the most fundamental aspect of this phenomenon, subjective bonds between individuals and groups. Here, data derived from a Q-methodological study of Basque national identity are used to show that claims advanced within the existing literature appear to be restricted in their ability to explain subjective attachment to the nation; that is, they cannot adequately account for the types of bonds that emerged from the Basque case study. Primarily through Q methodology, an alternative explanation and method for uncovering the ties between individuals and groups is offered. In the place of previously vague and restrictive a priori categories of collective attachment, Q is shown to offer a means by which to explore the subjective nature of ethnic and national identification.  相似文献

Compassion fatigue is a phenomenon that occurs when a caregiver feels overwhelmed by repeated empathic engagement with distressed clients (Figley, 2002). Research demonstrates its existence among nurses, physicians, and mental health professionals, but to date no published study has specifically investigated the nature and prevalence of compassion fatigue among genetic counselors. The present study was an initial attempt to identify and describe the phenomena in genetic counseling by conducting focus group interviews with 12 genetic counselors. Data analysis yielded several themes: a) compassion fatigue occurs and may compromise professional and personal functioning; b) prevalent triggers include delivering bad news and difficult patient issues (e.g., terminal illness, anger, psychopathology); c) effective coping strategies include consulting with colleagues, setting boundaries, and humor; and d) risk factors include personality characteristics and traumatic memories. Current theories of parsing suggest a wide variety of mechanisms by which modifiers, such as relative clauses, may be related to constituents that offer more than one potential attachment site. Some, like the tuning hypothesis, are based on the premise that people's parsing performance is shaped by prior exposure to language. Others (e.g. garden-path theory and construal theory) play down any potential role of past linguistic experience, stressing instead the varying influences of structural characteristics of the sentence in question. The two views encourage differing expectations about cross-linguistic variation in parsing preference. A questionnaire study and two on-line experiments were carried out to investigate attachment preferences in Dutch. The results pose a number of problems for the majority of the existing parsing models and are clearly inconsistent with some of the traditional theories. In contrast, the findings are compatible with models incorporating parsing mechanisms that are tuned by language experience. The results highlight the need for further corpus studies to subject these accounts to more searching scrutiny.  相似文献

Key findings and conclusions from a landmark study conducted by a National Research Council Committee are discussed. The 12‐year study was divided into 4 phases that covered topics of individual, small group, and organizational performance. Concentrating primarily on techniques with strong claims for enhancing performance, the committee found little support for some (e.g., sleep learning, meditation, parapsychological techniques, hypnosis, total quality management) and stronger support for others (e.g., mental practice, expert modeling, cooperative learning, team training, practice to optimize transfer of cognitive and social skills). Insights were also obtained about the roles of expectancy effects, belief perseverance, emotions, thought suppression, and organizational cultures. The article highlights the continuing contributions made by committee members in such areas as transfer, expert modeling, self‐efficacy, mental practice, organizational design and enhanced performance, and conflict resolution.  相似文献

This article reports findings from a longitudinal field study on multilevel processes in teams and examines the role of individual team-goal specification and team-goal clarity with regard to individual performance. It is hypothesized that individual team-goal specification predicts change in individual performance over time, particularly when team-goal clarity is low. Multisource data gathered in 31 project teams supported the hypotheses. Overall, the findings suggest that team members can improve their individual performance when engaging in teamwork processes that are relevant for the team as a whole.  相似文献

This paper provides an in-depth, qualitative analysis of two Christian youth groups. Researchers conducted personal interviews with participating youth and their youth ministers and collected extensive field notes on participant observations of youth group events. Findings indicate the presence of youth group cliques with clear delineations about which participants are youth group insiders and which are outsiders. Results show that insider participants are typically socioeconomically-advantaged youth, while outsiders are typically youth from disadvantaged backgrounds. Despite the existence of these exclusionary practices within both groups, leaders in the two cases demonstrate differing levels of awareness of and response to these dynamics. This paper explores the specifics of both cases, illuminating who is targeted for exclusion within these groups and how this targeting varies across the two congregational contexts. Implications of these findings are that religious youth groups may perpetuate broader socioeconomic divisions. Specifically, the analysis reveals that rather than forming benign cliques, youth group members may be discriminating against others based on their relative socioeconomic advantage.  相似文献

Although situational judgment tests (SJTs) have been in use for decades, consensus has not been reached on the best way to score these assessments or others (e.g., biodata) whose items do not have a single demonstrably correct answer. The purpose of this paper is to review and to demonstrate the scoring strategies that have been described in the literature. Implementation and relative merits of these strategies are described. Then, several of these methods are applied to create 11 different keys for a video‐based SJT in order to demonstrate how to evaluate the quality of keys. Implications of scoring SJTs for theory and practice are discussed.  相似文献

The assessment of attachment security in infancy and adulthood is well-studied, but middle childhood has been relatively neglected in the literature. The Child Attachment Interview (CAI) represents a promising assessment of attachment in middle childhood; recent evidence supports its reliability and validity in restricted samples. We assessed the construct validity of the CAI among a diverse sample of 104 8–12 year-old children (M age  = 9.80 years; 50 boys; 49 % family income <$40,000; 38 % Latino/a). We evaluated associations among CAI variables and self-report measures of attachment (convergent validity). Further, we evaluated whether CAI variables uniquely relate to theoretically-relevant constructs (internalizing symptoms) above and beyond self-reported attachment (incremental validity) and a theoretically-distinct construct, child temperament (discriminant validity). Our findings support the CAI’s validity, and have important implications for assessing and understanding attachment processes in middle childhood, as well as their development across the lifespan.  相似文献

Liberian women's coping with refugee resettlement in the United States was examined. The authors analyzed interviews of 10 women according to van Manen's ( 1990 ) hermeneutic phenomenology. Nine themes, grouped within 3 coping categories (adopting culturally sanctioned attitudes, engaging with a new environment, and situating oneself in a narrative), were identified. Implications include the need for counselors to use holistic and advocacy‐based counseling approaches and facilitate coping by cultural meaning making of experiences.  相似文献
