ObjectiveFunctional neuroimaging may provide a viable means of assessment and communication in patients with Guillain-Barré Syndrome (GBS) mimicking the complete locked-in state. Functional neuroimaging has been used to assess residual cognitive function and has allowed for binary communication with other behaviourally non-responsive patients, such as those diagnosed with unresponsive wakefulness syndrome. We evaluated the potential application of functional neuroimaging using a clinical-grade scanner to determine if individuals with severe GBS retained auditory function, command following, and communication.MethodsFourteen healthy participants and two GBS patients were asked to perform motor imagery and spatial navigation imagery tasks while being scanned using functional magnetic resonance imaging. The GBS patients were also asked to perform additional functional neuroimaging scans to attempt communication.ResultsThe motor imagery and spatial navigation task elicited significant activation in appropriate regions of interest for both GBS patients, indicating intact command following. Both patients were able to use the imagery technique to communicate in some instances. Patient 1 was able to use one of four communication tasks to answer a question correctly. Patient 2 was able to use three of seven communication tasks. However, two questions were incorrectly answered while a third was non-verifiable.ConclusionsGBS patients can respond using mental imagery and these responses can be detected using functional neuroimaging. Furthermore, these patients may also be able to use mental imagery to provide answers to ‘yes’ or ‘no’ questions in some instances. We argue that the most appropriate use of neuroimaging-based communication in these patients is to allow them to communicate wishes or preferences and assent to previously expressed decisions, rather than to facilitate decision-making.  相似文献

Memories of Near-Death Experiences (NDEs) most often are recounted as emotionally positive events. At present, no satisfactory explanatory model exists to fully account for the rich phenomenology of NDEs following a severe acute brain injury. The particular population of patients with locked-in syndrome (LIS) provides a unique opportunity to study NDEs following infratentorial brain lesions. We here retrospectively characterized the content of NDEs in 8 patients with LIS caused by an acute brainstem lesion (i.e., “LIS NDEs”) and 23 NDE experiencers after coma with supratentorial lesions (i.e., “classical NDEs”). Compared to “classical NDEs”, “LIS NDEs” less frequently experienced a feeling of peacefulness or well-being. It could be hypothesized that NDEs containing less positive emotions might have a specific neuroanatomical substrate related to impaired pontine/paralimbic connectivity or alternatively might be related to the emotional distress caused by the presence of conscious awareness in a paralyzed body.  相似文献

言语想象不仅在大脑预处理机制方面起到重要的作用,还是目前脑机接口领域研究的热点。与正常言语产生过程相比,言语想象的理论模型、激活脑区、神经传导路径等均与其有较多相似之处。而言语障碍群体的言语想象、想象有意义的词语和句子时的脑神经机制与正常言语产生存在差异。鉴于人类言语系统的复杂性,言语想象的神经机制研究还面临一系列挑战,未来研究可在言语想象质量评价工具及神经解码范式、脑控制回路、激活通路、言语障碍群体的言语想象机制、词语和句子想象的脑神经信号等方面进一步探索,为有效提高脑机接口的识别率提供依据,为言语障碍群体的沟通提供便利。  相似文献

A number of rating systems are available to evaluate emotional communication in a single modality. The main purpose of this study was to develop procedures to train human raters to evaluate posed expressions of emotion across three different channels of communication, i.e., facial, prosodic/intonational, and lexical/verbal. These procedures were used to evaluate posed emotional expressions produced by individuals with unilateral brain lesions from stroke. Posers in this preliminary report were two right brain-damaged, two left brain-damaged, and two normal control right-handed adults who were matched on demographic and neurological factors. Eight emotional expressions, both positive and negative, were produced in three channels and rated for intensity, pleasantness, and category accuracy. 15 normal adults served as raters, five per channel. The rating procedures were comparable across channels, with analogous properties, and yielded substantial interrater agreement. In this small sample of posers, it was observed that the expressions of the right brain-damaged group were rated as the least accurate and those of the left brain-damaged group as the most intense. When patterns of individual performance across the channels were examined, performance was quiet consistent for the normal controls yet variable for the right brain-damaged persons. These observations are in keeping with the notion that patients with right hemisphere pathology have difficulty in emotional communication. In summary, these findings suggest that comparison of emotional expressions across multiple channels is feasible.  相似文献

The triangular ISIS model for describing the operation of human factors in complex sociotechnical organisations or systems is applied in this research to a large international air traffic control system. A large sample of senior Air Traffic Controllers were randomly assigned to small focus discussion groups, whose task was to identify problems occurring at the interfaces of the three major human factor components: individual, system impacts, and social. From these discussions, a number of significant interface problems, which could adversely affect the functioning of the Air Traffic Control System, emerged. The majority of these occurred at the Individual-System Impact and Individual-Social interfaces and involved a perceived need for further interface centered training.  相似文献

We are developing an electroencephalographic (EEG)-based brain-computer interface (BCI) system that could provide an alternative communication channel for those who are totally paralyzed or have other severe motor impairments. The essential features of this system are as follows: (1) EEG analysis in real time, (2) real-time conversion of that analysis into device control, and (3) appropriate adaptation to the EEG of each user. Digital signal processing technology provides the speed and flexibility needed to satisfy these requirements. It also supports evaluation of alternative analysis and control algorithms, and thereby facilitates further BCI development.  相似文献

With the rapid progress and considerable promise of nanobiotechnology/neurosciences there is the potential of transforming the very nature of human beings and of how humans can conceive of themselves as rational animals through technological innovations. The interface between humans and machines (neuro-digital interface), can potentially alter what it means to be human, i.e., the very idea of human nature and of normal functioning will be changed. In this paper, I argue that we are potentially on the verge of a paradigm shift in terms of the ends and goals of techno-science and its applications in the biomedical sciences. In particular, the development of brain-computer interfaces could reconceptualize the very notion of what it means to be human. Hence, we should not limit our reflections of applications in terms of therapy and enhancement but also include an examination of applications aiming at the alteration of human nature. To this end I will first delineate the potential paradigm shift and then map out four distinct clusters of concerns in relation to the brain-computer interface. Finally, I argue that our moral and philosophical reflections should follow a procedural model based on managed consensus due to our pluralistic context.

Translational research is most prominently represented by the search for biomarkers and preclinical research. Aside from generating such new measures and methodologies, translational research additionally refers to translation of integrated knowledge. This strategy involves synthesis, exchange, and dissemination of available knowledge, with the goal of improving health services and health care systems. For stress-related disorders, such as depression and anxiety disorders, this strategy meets numerous challenges, as the great majority of these patients are treated by family physicians. Here, we introduce Neuropattern, a new diagnostic tool, which allows translation of psychobiological knowledge to this stress "bedside." Neuropatterns are conceptualized endophenotypes of the activity and reactivity status of neurobiological interfaces, which participate in the crosstalk between the brain and peripheral organs under stressful conditions. Neuropattern can easily be implemented in routine clinical work, and helps the physician to individualize those therapeutic interventions that are already available.  相似文献

Modern experiments in the behavioral sciences frequently employ several items of electronic equipment such as computers, monitoring devices, stimulus presentation equipment, and response collection systems. In many cases it would be advantageous for these items to communicate directly with each other. Such communication may facilitate greater automation of experiments (i.e., reduced experimenter influences during the experiment), more precise experiment control (i.e., superior timing and synchronization capabilities of electronic devices), and greater accuracy of data collection (i.e., reduced ambiguity of participant responses). Many electronic experiment devices already provide external interfaces through which communication with other devices can be implemented. The most common is based on the RS-232 protocol, which is also found in all standard PCs. Therefore, Microsoft Windows based computers can be programmed to control experiments by communicating directly with electronic experiment devices. We show how to implement this RS-232 interconnection between devices and a Windows PC using currently available software tools.  相似文献

This paper is a report of a study of congruity between verbal and nonverbal maternal communication with sons during two 30-minute sessions. The instructions to the mothers varied for the two sessions by increasing the time the mother was asked to spend on a brain teaser game and by decreasing the number of play materials available for the sons in the second session. The two sessions were videotaped through a one-way mirror and rated independently for maternal verbal and nonverbal communicaiton and child compliance. Mothers differed for the two sessions in their communication with their sons. The study demonstrated that maternal nonverbal communication, as well as verbal communication, can be reliably measured through use of videotape recording.  相似文献

In this perspective, we describe how developmental improvements in youth executive function (EF) are supported by hierarchically organized maturational changes in functional brain systems. We first highlight evidence that functional brain systems are embedded within a hierarchical sensorimotor-association axis of cortical organization. We then review data showing that functional system developmental profiles vary along this axis: systems near the associative end become more functionally segregated, while those in the middle become more integrative. Developmental changes that strengthen the hierarchical organization of the cortex may support EF by facilitating top-down information flow and balancing within- and between-system communication. We propose a central role for attention and frontoparietal control systems in the maturation of healthy EF and suggest that reduced functional system differentiation across the sensorimotor-association axis contributes to transdiagnostic EF deficits.  相似文献

This is a study of the relationship between improved conditions of attention and communication and ability to abstract in patients with schizophrenia and organic brain syndromes. Groups of 20 schizophrenics, 20 organics, and 20 normals, equated for intelligence, were given the Similarities subtest on the WAIS and a multiple choice version which was designed to increase the focus of attention and to facilitate communication. The main results supported the hypothesis that special conditions facilitating concentration and communication do not necessarily enhance schizophrenics' abstract functioning more than that of organic patients. Moreover, it was concluded that a deficit in abstract functioning is not a general or invariable consequence of schizophrenia.  相似文献

Numerous theories discuss the neuropsychological functions of the frontal lobes, most based on some concept of supramodality, and an extensive literature presents the phenomenology and semiology of language and communication deficits after focal brain lesions involving the frontal lobes. Despite this, few attempts have been made to link the clinical phenomenology to a theory. This paper presents (1) a general theory of frontal functions; (2) a brief summary of experimental and anatomical literatures in support of defined frontal functional systems; (3) clinical observations that delineate these functional systems for the specific modalities of language and communication; (4) a review of the available literature supporting the idea of specific modal and supramodal language and communication capacities; (5) hypotheses about the distributed anatomy of these functional systems; and (6) implications for traditional clinical notions of aphasia, particularly in relation to a general theory of frontal lobe functions.  相似文献
