Population-Wide Parenting Intervention Training: Initial Feasibility   总被引:1,自引:0,他引:1  
A population-level approach to deliver parenting and family support is a necessary but neglected approach needed to reduce the high prevalence of emotional/behavioral problems in children, decrease inadequate and potentially abusive parenting practices, and to provide improved parenting support to all parents within a specified population. We examined the initial feasibility of a large-scale professional training regimen to prepare existing service providers to implement an evidence-based preventive intervention in the realm of parenting and family support. Data from the U.S. Triple P System Population Trial are used to illustrate how a parenting and family support intervention can be successfully disseminated to a large, multidisciplinary workforce. We discuss lessons learned from this dissemination effort as well as implications for population-based approaches to child and family well-being.  相似文献

With adequate therapeutic support, the ideal of co-operative shared parenting could become a reality for the majority of separated, divorced and remarried families. A therapeutic/interventionist approach to family mediation (as contrasted to the more short-term, future-focused, neutralist mainstream model, structured strictly towards the resolution of issues under dispute) may offer the most effective and efficient means to do this. Within this approach, mediation is used to introduce separating parents to shared parenting as a viable alternative, reduce their anxiety about shared parenting as a living arrangement deviating from the norm, enable them to consider a range of shared parenting options, help them work through the development of a shared parenting plan, and support them in the transition to post-divorce parenting. In its emphasis on parental responsibility and shared parental authority in the resolution of post-separation parenting disputes, the model reflects an orientation consistent with the philosophy of the Children Act 1989, and is ideally suited for use by family therapists working with parents during the divorce transition.  相似文献

Parent involvement in type 1 diabetes (T1DM) care leads to improved adherence; however, the manner in which parents approach illness management interactions with children must also be considered. It was hypothesized that greater use of an authoritative parenting style and less parenting stress would be associated with greater behavioral adherence and better metabolic control. Ninety-five primary caregivers of preadolescents (ages 8-11) with T1DM completed questionnaires assessing parenting style, pediatric parenting stress, and child behavioral adherence. Caregivers primarily self-identified as using an authoritative parenting style. Greater authoritative parenting was associated with greater behavioral adherence and less difficulty with pediatric parenting stress; no differences in metabolic control were observed. Greater engagement in authoritative parenting behaviors may contribute to increased age-appropriate child behavioral adherence and less pediatric parenting stress. Interventions highlighting diabetes-specific authoritative parenting techniques may enhance health outcomes and improve overall family functioning.  相似文献

This paper presents a comprehensive review of research linking single-parent family structure to parenting processes, maternal well-being, and child developmental outcomes among African Americans. The approaches used to study these families, related methodological and conceptual concerns, and the factors linked to maternal well-being, effective parenting processes, family functioning, and child outcomes are addressed. Much work remains to be done on conceptualizing and assessing parenting processes among African Americans in general and single African American mothers in particular. Researchers must examine more carefully the circumstances that foster or impede successful parenting among these mothers. Studies also are needed to disentangle the interactions between economic stress and parenting behavior and to determine the extent to which the findings can be applied to middle-income single African American parents.  相似文献

The Nurtured Heart Approach to parenting (NHA; Glasser & Easley, 2008) is summarized and evaluated in terms of its alignment with current theoretical perspectives and empirical evidence in family studies and developmental science. Originally conceived and promoted as a behavior management approach for parents of difficult children (i.e., with behavior disorders), NHA is increasingly offered as a valuable strategy for parents of any children, despite a lack of published empirical support. Parents using NHA are trained to minimize attention to undesired behaviors, provide positive attention and praise for compliance with rules, help children be successful by scaffolding and shaping desired behavior, and establish a set of clear rules and consequences. Many elements of the approach have strong support in the theoretical and empirical literature; however, some of the assumptions are more questionable, such as that negative child behavior can always be attributed to unintentional positive reinforcement by parents responding with negative attention. On balance, NHA appears to promote effective and validated parenting practices, but its effectiveness now needs to be tested empirically.  相似文献

Rates of overweight in youth have reached epidemic proportions and are associated with adverse health outcomes. Family-based programs have been widely used to treat overweight in youth. However, few programs incorporate a theoretical framework for studying a family systems approach in relation to youth health behavior change. Therefore, this review provides a family systems theory framework for evaluating family-level variables in weight loss, physical activity, and dietary approaches in youth. Studies were reviewed and effect sizes were calculated for interventions that manipulated the family system, including components that targeted parenting styles, parenting skills, or family functioning, or which had novel approaches for including the family. Twenty-one weight loss interventions were identified, and 25 interventions related to physical activity and/or diet were identified. Overall, family-based treatment programs that incorporated training for authoritative parenting styles, parenting skills, or child management, and family functioning had positive effects on youth weight loss. Programs to improve physical activity and dietary behaviors that targeted the family system also demonstrated improvements in youth health behaviors; however, direct effects of parent-targeted programming is not clear. Both treatment and prevention programs would benefit from evaluating family functioning and parenting styles as possible mediators of intervention outcomes. Recommendations are provided to guide the development of future family-based obesity prevention and treatment programs for youth.  相似文献

Using a sample of low‐income mothers enrolled in Early Head Start (n = 65), this study tested the hypothesis that parenting stress is affected by social support and self‐efficacy, in addition to family risk status and family income. Specifically, it was proposed that social support and self‐efficacy are psychological resources that are associated with lower parenting stress levels, and would moderate the impact of family income on parenting stress. A significant proportion of variance in parenting stress was explained by self‐efficacy, family risk, and the interaction of self‐efficacy and family income; family income alone was not a significant predictor of parenting stress levels. Mothers higher in self‐efficacy had lower levels of parenting stress, and income was less associated with parenting stress levels for mothers high in self‐efficacy. Social support was not associated with lower parenting stress levels, nor did social support moderate the effect of income on parenting stress. Family risk was also a strong and reliable predictor of parenting stress, suggesting that family circumstances are perhaps better predictors of parenting stress levels than income alone. These findings suggest that parenting stress among low‐income parents should be viewed as a function of psychological, as well as financial, resources.  相似文献

This study examined the effects of the Boys Town In-Home Family Program (BT-IHFP) on parenting skills, family functioning, and child behavior for at-risk families involved with child protective services. The BT-IHFP is designed as a home-based, family-centered alternative to out-of-home placement with the goal of improving family functioning, keeping families together, and preventing further involvement with child protective services. The model uses a hands-on cognitive-behavioral approach, promotes family engagement, provides 24/7 services to families, and links families to community resources. While it is essential to examine ultimate, long-term outcomes such as family preservation, it is also important to investigate the proximal and intermediate outcomes that address the key changes in behavior that support successful long-term outcomes. The goals of this study were to (1) examine the pre-post change of proximal and intermediate outcomes and (2) examine if improvements in parenting skills, parental roles, and parental stress (proximal outcomes) will be related to improvements in child behavior at home, child behavior at school, and family functioning (intermediate outcomes). Inspecting intake and discharge data for 44 families, 94% were intact at discharge and significant pre-post improvements were found in proximal and intermediate outcomes. These findings suggest that this six-week intensive BT-IHFP holds promise for teaching at-risk families skills to promote positive family functioning.  相似文献

Three-generation households that include parents and grandparents raising children together have become increasingly common in China. This study examined the relations among depressive symptoms, parenting stress, and caregiver–child relationships in the mother–grandmother dyadic context. Participants were mothers and grandmothers from 136 three-generation households. Results from Actor–Partner Interdependence Mediation Modeling indicated that mothers’ depressive symptoms were indirectly related to mother–child conflict/closeness through own parenting stress; grandmothers’ depressive symptoms were indirectly related to grandmother–child conflict through own parenting stress. Mothers’ depressive symptoms were indirectly related to grandmothers’ conflict with children through grandmothers’ parenting stress, and grandmothers’ depressive symptoms were indirectly related to mothers’ conflict/closeness with children through mothers’ parenting stress. The relation between mothers’ parenting stress and mother–child closeness was stronger than the relation between grandmothers’ parenting stress and grandmother–child closeness. Findings highlight the implications of using a family system perspective and the dyadic approach in understanding and improving family functioning in Chinese three-generation households.  相似文献

Child and family researchers have long recognized parenting as making an important contribution to child development. However, little is known about the factors that shape parenting itself, particularly factors outside the family. This article examines the literature on variables that comprise the context in which parenting occurs, and summarizes what is known about how three contextual factors (ethnicity/culture, family socioeconomic status, and neighborhood/community) influence parenting practices. Implications for future research on parenting, clinical interventions designed to improve parenting, and policy decisions affecting parenting are discussed.  相似文献

The quality of parenting children receive affects a diverse range of child and youth outcomes. Addressing the quality of parenting on a broad scale is a critical part of producing a more nurturing society. To achieve a meaningful population-level reduction in the prevalence rates of child maltreatment and social and emotional problems that are directly or indirectly influenced by parenting practices requires the adoption of a broad ecological perspective in supporting families to raise children. We make the case for adopting a multilevel, whole of population approach to enhance competent parenting and describe the essential tasks that must be accomplished for the approach to be successful and its effects measurable. We describe how a theoretically integrated system of parenting support based on social learning and cognitive behavioral principles can be further strengthened when the broader community supports parental participation. Implications for policy and practice are discussed.  相似文献

This case study provides an in-depth example of a comprehensive therapeutic assessment with an adolescent (TA–A) and his parents. The TA–A addressed parental concerns about their son's drug experimentation as well as the adolescent's own private questions about his distinctiveness from others, all set against a backdrop of ongoing parental conflict and poor communication. The TA–A process and how it is specifically tailored to balance the needs of adolescents and their parents is discussed. Subsequently, each step of TA–A is illustrated through the case study. Research findings at the conclusion of the assessment and at follow-up indicated significant decreases in internalizing symptomology and school problems, increases in self-esteem and self-reliance, and improved family functioning as reported by the adolescent. At follow-up, the father spoke of developing a more assertive parenting approach and successful follow-through on recommendations. This case study provides a template for clinicians interested in conducting TA–A.  相似文献

In this article, we discuss the successful implementation of an adapted evidence‐based parenting intervention for families affected by two decades of war in Northern Uganda. The adaptation and adoption of such interventions to support mental health and family functioning is widely endorsed by prevention scientists and considered a priority in global mental health. The preparation and early adoption phases of engaging with a highly vulnerable community affected by war trauma are documented in this paper along with a discussion of the steps taken to adapt a parenting intervention for cultural and contextual fit. This study is a component of an overall program of research aimed at reducing the long‐term negative effects of war on parenting practices and childhood outcomes, which have considerable implications for preventing mental, neurological, and substance‐use disorders. The processes described here cover a 4‐year period culminating in the implementation of the nine‐session Enhancing Family Connection intervention piloted with a group of 14 mothers. The lessons in cultural adaptation have been valuable and the feasibility results promising for further testing the intervention.  相似文献

Childcare advice has a long history. This article comments on parenting programmes, a recent form of this advice. It argues that the use of parenting programmes has grown with support from three sectors. Firstly, those who argue from the evidence‐based approach suggest that there is strong research evidence for the use of such programmes with children with conduct problems. Secondly, they are supported by theoretical models that emphasize the causal relationship between parental behaviour and child outcomes. Finally, the dominant rhetoric of social exclusion in current UK policy supports action at the level of individual parents' behaviour. The example of parenting programmes employed in parenting orders is used to open debate across the boundaries of disciplines. Tensions between this use and the evidence and theories presented in favour of parenting programmes are discussed, highlighting the difference between a therapeutic focus on family problems and a policy focus on problem families.  相似文献

Family-based interventions targeting parenting factors, such as parental monitoring and parent–child communication, have been successful in reducing adolescent offenders’ substance use and delinquency. This pilot, exploratory study focuses on family and parenting factors that may be relevant in reducing juvenile offenders’ substance use and sexual risk taking behavior, and in particular examines the role of family emotional involvement and responsiveness in young offenders’ risk-taking behaviors. Participants included 53 juvenile drug court offenders and their parents. Results indicate that poor parent–child communication is associated with marijuana use and unprotected sexual activity for young offenders; however, family affective responsiveness is also a significant unique predictor of unprotected sexual activity for these youth. Findings suggest that interventions focused on improving parent–child communication may reduce both marijuana use and risky sexual behavior among court-involved youth, but a specific intervention focused on improving parents and young offenders’ ability to connect with and respond to one another emotionally may provide a novel means of reducing unprotected sexual risk behaviors.  相似文献

T he present study of three generations' families living together during a period of marked economic dislocation investigates whether supportive grandparents can ameliorate the negative effects of economic stress on parenting and on teen outcomes. The study focuses on harsh parenting and adolescent depression, which has been shown to be especially responsive to economic stress. Three hypotheses were investigated: (1) in extended families there is a link between economic adversity and adolescent depression that is mediated by harsh parenting; (2) grandparent support diminishes the relationship between economic pressure and harsh parenting; and (3) grandparent support diminishes the relationship between harsh parenting and adolescent depression. Data were collected from a community sample of 62 extended families with an adolescent during a period of marked economic dislocation in Bulgaria. Mothers, fathers, and adolescents completed questionnaires about perceived economic pressure, parents' harsh parenting, and perceived support of grandparents. Adolescents also completed a measure of self‐reported depression. Additional interviews were conducted with every eligible family member about family relations. Results showed that even though there was significant economic stress reported by parents, adolescents reported moderate levels of depression and low levels of harsh parenting by mothers and fathers. Correlation and regression analyses revealed that whenever respondents experienced support by grandparents, it made a difference for family members. More specifically, despite economic stress, mothers who experienced the support of grandparents tended to exercise less harsh parenting practices than mothers who lacked such support. Furthermore, when grandparents were supportive, harsh parenting was not associated with adolescent depression. These findings are discussed in the light of existing studies of families under economic pressure with a special focus on extended family relations and the role of cultural context.  相似文献

We investigated whether parenting and child behavior improve following psychosocial treatment for Attention-Deficit/Hyperactivity Disorder, Predominantly Inattentive Presentation (ADHD-I) and whether parenting improvements mediate child outcomes. We analyzed data from a randomized clinical trial investigating the efficacy of a multicomponent psychosocial intervention (Child Life and Attention Skills, CLAS, n = 74) in comparison to Parent-Focused Treatment (PFT, n = 74) and treatment as usual (TAU, n = 51) for youth with ADHD-I (average child age = 8.6 years, range 7–11 years, 58 % boys). Child and parent/family functioning were assessed prior to treatment, immediately following treatment, and at follow-up into the subsequent school year using parent and teacher reports of inattention, organization, social skills, academic competency (teachers only), parenting daily hassles, and positive and negative parenting behaviors (parents only). Both treatment groups improved on negative parenting and home impairment, but only CLAS families also improved on positive parenting as well as academic impairment. Improvements in positive and negative parenting mediated treatment effects on child impairment independent of improvements in child inattention, implicating parenting as an important mechanism of change in psychosocial treatment for ADHD-I. Further, whereas parent-focused training produces improvements in negative parenting and impairment at home for children with ADHD-I, a multicomponent approach (incorporating child skills training and teacher consultation) more consistently produces improvements at school and in positive parenting, which may contribute to improvements in social skills into the next school year.  相似文献

Recent studies have found significant but relatively modest associations in parenting across generations, suggesting additional influences on parenting beyond experiences in the family of origin. The present prospective, cross-generational study of at-risk men (Oregon Youth Study) focuses on fathers' negative discipline practices with their 2- to 3-year-old children. The theoretical model is based on a dynamic developmental systems approach to problematic family functioning, which points to the importance of developmental systems, including family risk context and key influential social interactional systems, and emphasizes influence that is directly pertinent to the outcome of interest. Path modeling indicated that the men's poor and harsh discipline practices were predicted by partners' problem behavior (substance use and antisocial behavior) and negative discipline practices, as well as by poor discipline experienced in the family of origin; men's own problem behavior, ages at which they became fathers, and family socioeconomic status were controlled. Findings indicate the importance of focusing on influence dynamics across parents.  相似文献

The mental health of refugees has been an increasingly researched area, but has been criticised for having an individualised and symptom-focused approach to understanding the experience of forced migration. This paper attempts to respond to calls to address this culturally limited and incomplete way of conceptualising responses to experiences of persecution and terror bound up within global hegemony and power inequalities. Interpretative phenomenological analysis was employed to analyse semi-structured interviews undertaken with six refugee mothers, with the aim of exploring how participants made sense of, and created meaning around parenting and family life in the UK. Three main themes emerged from the data analysis (a) loss as a constant companion to parenting; (b) a shifting view of the self as a mother; and (c) taking the good with the bad in family life. Methodological limitations, as well as implications for future research and clinical practice are discussed.  相似文献
