创伤后应激障碍(PTSD)是由高情绪刺激引起延迟出现和长期持续的精神障碍。闯入、闪回、病理性重复体验、持续性警觉性增高和回避等症状严重损害其认知功能。PTSD患者记忆形成了情绪网络。情绪记忆具有强烈、持久的特点, 在记忆中具有优势地位。回顾PTSD患者情绪记忆优势的行为及脑成像研究发现, PTSD患者情绪记忆优势研究结果并不完全一致, 受到诸多因素的影响, 这些影响因素需进一步探讨。  相似文献

个体经历严重创伤性事件后可能会形成创伤后应激障碍(posttraumatic stress disorder, PTSD)。在创伤经历中形成的情绪记忆是以后发展为PTSD的重要病理机制。PTSD的形成涉及到情绪记忆的过度巩固, 而去甲肾上腺素能神经信号可增强情绪记忆的巩固和再巩固。因此, 在创伤记忆的巩固和再巩固期间阻断去甲肾上腺素能神经信号, 而在创伤记忆的消退期间增强去甲肾上腺素能神经信号, 可能会破坏和或抑制病理性的情绪记忆, 从而预防或治疗PTSD。  相似文献

创伤后应激障碍(Posttraumatic stress disorder,PTSD)患者通常伴随着明显的认知功能损伤。例如,存在注意保持缺陷,对与创伤相关的信息表现出更多的注意偏向和应激反应;并存在对创伤信息的记忆偏向,在陈述性记忆和自传体记忆中表现出一定的缺陷。近年的研究积累了更多类似的证据,表明PTSD的认知损伤可能与执行功能缺陷密切相关,但这一解释是否正确仍有待进一步的研究。本综述对5·12汶川大地震后长期的心理干预工作,尤其是认知功能损伤人群的筛选、恢复以及相关科学研究具有现实意义和理论启发作用  相似文献

以往研究认为认知控制在闯入记忆的发展中起一定作用。研究以83名大学生本科生为被试,采用创伤电影范式和问卷调查结合的方式,旨在考察创伤个体注意控制、焦虑及情绪对闯入记忆的影响。结果显示,注意控制、焦虑和情绪与闯入记忆相关;注意控制和观看创伤电影前的消极情绪对闯入记忆有一定的预测作用;注意控制与焦虑、情绪相关;高低注意控制组分别在焦虑、情绪上差异显著。注意控制、焦虑、创伤前负性情绪与闯入记忆相关;注意控制、创伤前负性情绪显著影响闯入记忆。 关键词:闯入记忆;注意控制;焦虑;情绪;创伤  相似文献

过度泛化现象是区分普通心理问题和严重心理问题的重要指标。创伤后应激障碍(posttraumatic stress disorder, PTSD)患者在创伤事件后会出现非适应性的泛化。现有研究结果表明非适应性泛化(maladaptive generalization)可能是PTSD发生发展的一个重要因素, 但也有研究支持PTSD是导致了泛化从适应性向非适应性化转变的原因。虽然泛化现象的变化与PTSD症状的发展有相同趋势, 且具有共同的生理基础:记忆相关神经结构, 但二者之间的关系尚无明确定论。阐明PTSD与非适应性泛化之间的关系对PTSD的预测、诊断和治疗均有推进作用。  相似文献

采用三因素混合实验设计,结合情绪标识范式考察创伤后应激障碍(PTSD)个体对创伤相关图与负性情绪面孔的情绪标识效应。结果发现:PTSD组的情绪标识任务抑制了创伤相关图对之后奇偶判断任务的干扰,表现出显著的情绪标识效应;而对负性情绪面孔的情绪标识效应不显著。该研究表明将谈话疗法用于PTSD患者的心理危机干预是有效的。  相似文献

为探讨乳腺癌患者反刍这一认知情绪调节方式在体象和创伤后应激障碍(PTSD)、创伤后成长(PTG)之间的中介作用,采用体象量表(BIS)、认知情绪调节问卷中文版(CERQ-C)、创伤后应激障碍量表(PSS)和创伤后成长问卷(PTGI)对150名乳腺癌术后患者进行研究。结果发现:(1)乳腺癌患者的体象可以正向预测PTSD和PTG;(2)乳腺癌患者的反刍在体象和PTSD的闯入性症状、回避性症状中起中介作用,在体象与PTG中的中介作用不显著。结果表明,体象既可以正向预测乳腺癌患者的PTSD和PTG,也可以通过反刍的中介作用影响PTSD。  相似文献

创伤后应激障碍的双重表征理论(dual representation theory of PTSD,DRT)基于多重记忆系统的观点,强调特定加工通路(即视觉加工腹侧通路和背侧通路)在侵入性表象形成和提取中的作用。DRT认为,两类不同性质的竞争性认知任务(即言语任务和视觉空间任务)分别占用特定加工通路的认知资源从而影响创伤信息加工和创伤记忆再巩固;工作记忆模型(working memory model,WMM)则强调竞争性认知任务对整体认知资源的占用。目前,DRT和WMM均得到了一些研究的支持。未来的研究应考虑个体的工作记忆能力;开展更多干预研究,并尝试改进研究方法,以及设计与文化相符、更加生活化的竞争性认知任务。此外,在加强模拟干预研究的同时也要增加临床应用研究,适当开展真实创伤情境的干预研究,还要加强相关信息加工能力训练的研究。  相似文献

记忆的顿悟优势效应指, 与加工非顿悟问题相比, 加工顿悟问题能够产生更优的记忆保持量。该效应在顿悟的两种操作性定义以及不同实验材料中稳定出现。基于顿悟阶段观, 研究者提出两种理论解释记忆的顿悟优势效应的认知机制:认知的生成效应和表征转化理论认为, 顿悟加工过程中的认知努力、适当迁移加工以及顿悟的表征转换加工促进记忆的保持效果; 情绪的“啊哈”体验理论认为, 加工顿悟问题后的情绪对后续的信息提取具有促进作用。未来的研究可从优化顿悟的指标、巩固记忆的顿悟优势效应、评估其持续性以及更加系统探讨记忆的顿悟优势效应的认知机制等四个方面展开。  相似文献

采用创伤暴露程度问卷、生活事件量表、安全感量表、情绪调节策略问卷和修订后的DSM-5的PTSD核查表对汶川地震8.5年后的1156名中学生进行调查,考察其重复创伤暴露、安全感和认知重评与PTSD之间的关系。结果发现,在控制初次地震暴露后,重复创伤暴露可以直接正向预测PTSD,也可以通过确定控制感和人际安全感分别间接地正向预测PTSD,还可以通过人际安全感经认知重评的多重中介作用来正向预测PTSD。不过,重复创伤暴露不能直接经过认知重评对PTSD发挥显著的预测作用,也不能通过确定控制感经认知重评来对PTSD发挥显著的多重间接预测作用。  相似文献

Posttraumatic stress disorder (PTSD) and depression frequently co-occur following a traumatic event. Differences in the processing of autobiographical memory have been observed in both disorders in the form of overgeneralised memories and negative intrusive memories. The current study examined how symptoms of PTSD and depression influence the phenomenological characteristics of trauma memories. Undergraduate students who had experienced a traumatic event (n?=?696) completed questionnaires online including measures of PTSD and depressive symptom severity. They rated their trauma memory on several phenomenological characteristics using the Memory Experiences Questionnaire [Sutin, A. R., &; Robins, R. W. (2007). Phenomenology of autobiographical memories: The memory experiences questionnaire. Memory.]. Moderated multiple regression was used to examine how PTSD and depressive symptom severity related to each phenomenological characteristic. Symptoms of PTSD and depression were related separately and uniquely to the phenomenological characteristics of the trauma memory. PTSD severity predicted trauma memories that were more negative, contained higher sensory detail, and were more vivid. In contrast, depressive symptom severity predicted trauma memories that were less accessible and less coherent. These findings suggest that depressive and PTSD symptomatology affect traumatic memory differently and support a distinction between these two disorders.  相似文献

Addressing affect dysregulation may provide a complementary alternative or adjunctive approach to the empirically supported trauma memory processing models of cognitive behavior therapy (CBT) for posttraumatic stress disorder (PTSD). A CBT designed to enhance affect regulation without trauma memory processing—trauma affect regulation: guide for education and therapy (TARGET)—was compared to present centered therapy (PCT) and wait-list (WL) conditions in a randomized clinical trial with 146 primarily low-income and ethnoracial minority mothers with PTSD. TARGET achieved statistically and clinically significant improvement on PTSD and affect regulation measures compared to WL, with more consistent and sustained (over a 6-month follow-up period) evidence of improvement than PCT. Drop-out rates (~ 25%) were comparable in TARGET and PCT, similar to those previously reported for trauma memory processing CBTs. Symptom worsening was rare (2–8%) and transient. Affect regulation-based CBT without trauma memory processing warrants further research as a potentially efficacious therapy for victimization-related PTSD.  相似文献

Information processing theory suggests that cognitive changes following trauma are common and hypothesized to have an impact on attention, memory, and intrusive thoughts. There is an ever-expanding empirical literature where cognitive features of posttraumatic stress disorder (PTSD) are being explored. However, it can sometimes be difficult for front-line clinicians to stay abreast of this literature and how it impacts the treatment s/he provides. The goal of this paper is to provide an overview of some recent basic and applied research on information processing in PTSD and the implications of these findings for cognitive-behavioral clinicians. In particular, we explore recent findings regarding attention, memory, intrusive thoughts/thought suppression, and acceptance as they relate to clinical work in patients with PTSD.  相似文献

Peri-traumatic information processing is thought to affect the development of intrusive trauma memories. This study aimed to replicate and improve the study by Holmes, Brewin, and Hennessy (2004, Exp. 3) on the role of peri-traumatic verbal processing in analogue traumatic intrusion development. Participants viewed an aversive film under one of three conditions: counting backwards in 3s (“verbal interference”), verbalising emotions and thoughts (“verbal enhancement”), or without an extra task. A dual-process account of PTSD would predict that verbal interference would increase intrusion frequency compared to no task, whereas verbal enhancement would lead to a decrease. In contrast, mainstream memory theory predicts a decrease in intrusion frequency from any concurrent task that diverts attention away from the trauma film. The main finding was that the verbal interference task led to a decrease in intrusive memories of the film compared to the other two conditions. This finding does not support a dual-process account of PTSD, but is in line with general theories of memory and attention.  相似文献

基于模拟创伤研究范式, 采用道路交通事故影片对159名健康大学生进行模拟创伤, 考察PTSD易感性人格特质(特质焦虑、特质分离和神经质)、工作记忆能力(注意控制和工作记忆容量)和创伤期间认知加工(数据驱动加工和概念加工)对模拟创伤后不同时段闪回的影响。运用结构方程模型分析数据, 结果发现:(1)数据驱动加工正向预测不同时段闪回(即时闪回、一周闪回和触发闪回)的数量、生动性、痛苦度以及一周侵入频率; (2)神经质正向预测触发闪回痛苦度, 注意控制和工作记忆容量负向预测一周侵入频率; (3)概念加工在注意控制与即时闪回痛苦度之间起中介作用。研究结果提示:数据驱动加工是创伤后不同时段闪回的主要影响因素; 概念加工主要作用于早期急性应激反应, 注意控制是其保护因素; 神经质和工作记忆能力对闪回具有较长期的影响, 且工作记忆能力具有一定的保护作用。  相似文献

Previous studies have examined the concurrent relationship between posttraumatic stress disorder (PTSD) and a range of psychophysiological variables, including respiratory sinus arrhythmia (RSA). However, there is a lack of research examining the prospective development of trauma symptomatology, and the directionality of the association between RSA level and PTSD has yet to be determined. The current study is the first prospective study to examine whether RSA level and RSA reactivity are risk factors for PTSD symptoms in children. Assessments were conducted both prior to (Time 1) and following (Time 2) a natural disaster (i.e., Hurricane Katrina). Participants were 36 children who were 3–6 years-old during the Time 1 assessment. Structured diagnostic interviews were used to assess PTSD symptoms at both Time 1 and Time 2. RSA level during a neutral stimulus, RSA reactivity to emotional video stimuli (distress, joy, and trauma videos) and RSA reactivity to memory stimuli (remote happy memory, trauma memory, mother’s recall of the trauma memory) were also collected at both time points. Time 1 RSA level during a neutral stimulus was a significant predictor of Time 2 PTSD symptoms (controlling for age, Time 1 PTSD symptoms, Time 2 neutral RSA level), such that lower RSA during a neutral condition was related to higher PTSD symptoms. Also, Time 1 RSA reactivity in response to memory (but not video) stimuli, in the form of relatively less vagal withdrawal, was a significant predictor of more Time 2 PTSD symptoms (controlling for age, Time 1 PTSD symptoms, Time 2 RSA reactivity). This unique prospective study provides evidence for level of RSA and RSA reactivity as pre-existing clinical markers of stress sensitivity that predict psychopathology following a trauma.  相似文献

Cognitive models of posttraumatic stress disorder (PTSD) assert that memory processes play a significant role in PTSD (see e.g., Ehlers & Clark, 2000). Intrusive reexperiencing in PTSD has been linked to perceptual processing of trauma-related material with a corresponding hypothesized lack of conceptual processing. In an experimental study that included clinical participants with and without PTSD (N = 50), perceptual priming and conceptual priming for trauma-related, general threat, and neutral words were investigated in a population with chronic trauma-induced complaints as a result of the Troubles in Northern Ireland. The study used a new version of the word-stem completion task (Michael, Ehlers, & Halligan, 2005) and a word-cue association task. It also assessed the role of dissociation in threat processing. Further evidence of enhanced perceptual priming in PTSD for trauma stimuli was found, along with evidence of lack of conceptual priming for such stimuli. Furthermore, this pattern of priming for trauma-related words was associated with PTSD severity, and state dissociation and PTSD group made significant contributions to predicting perceptual priming for trauma words. The findings shed light on the importance of state dissociation in trauma-related information processing and posttraumatic symptoms.  相似文献

Most people are psychologically effected by an extreme trauma and for a significant minority the debility is long term, often expressed as post-traumatic stress disorder (PTSD). A cognitive-contextual approach is described in which the prime focus is upon teaching the client how to interact adaptively with the memory of the trauma by: a) facilitating a switch from a primarily perceptual to a more conceptual level of processing the trauma itself; and b) placing the trauma in the context of past life experiences and determining its relevance for the future. The approach is illustrated by the treatment of a PTSD diagnosed motor vehicle accident victim.  相似文献

Participants with a lifetime history of posttraumatic stress disorder (PTSD) and trauma-exposed controls with no PTSD history completed an emotional working memory capacity (eWMC) task. The task required them to remember lists of neutral words over short intervals while simultaneously processing sentences describing dysfunctional trauma-related thoughts (relative to neutral control sentences). The task was designed to operationalise an everyday cognitive challenge for those with mental health problems such as PTSD; namely, the ability to carry out simple, routine tasks with emotionally benign material, while at the same time tackling emotional laden intrusive thoughts and feelings. eWMC performance, indexed as the ability to remember the word lists in the context of trauma sentences, relative to neutral sentences, was poorer overall in the PTSD group compared with controls, suggestive of a particular difficulty employing working memory in emotion-related contexts in those with a history of PTSD. The possible implications for developing affective working memory training as an adjunctive treatment for PTSD are explored.  相似文献

Information processing theories of post-traumatic stress disorder (PTSD) state that intrusive images emerge due to a lack of integration of perceptual trauma representations in autobiographical memory. To test this hypothesis experimentally, participants were shown an aversive film to elicit intrusive images. After viewing, they received a recognition test for just one part of the film. The test contained neutrally formulated items to rehearse information from the film. Participants reported intrusive images for the film in an intrusion diary during one week after viewing. In line with expectations, the number of intrusive images decreased only for the part of the film for which the recognition test was given. Furthermore, deliberate cued-recall memory after one week was selectively enhanced for the film part that was in the recognition test a week before. The findings provide new evidence supporting information processing models of PTSD and have potential implications for early interventions after trauma.  相似文献
