This study describes the case of a global alexic patient with a severe reading deficit affecting words, letters and Arabic numbers, following a left posterior lesion. The patient (VA) could not match spoken letters to their graphic form. A preserved ability to recognize shape and canonical orientation of letters indicates intact access to the representation of letters and numbers as visual objects. A relatively preserved ability to match lowercase to uppercase letters suggests partially spared access to abstract letter identities independently of their visual forms. The patient was also unable to match spoken letters and numbers to their visual form, indicating that she could not access the graphemic representations of letters from their phonological representations. This pattern of performance suggests that the link between graphemic and phonological representations is disrupted in this patient. We hypothesize that VA’ residual reading abilities are supported by the right hemisphere.  相似文献

This study examined the letter-writing practices in a child and family psychology service. Utilizing a grounded theory approach to data collection and analysis, six clinical psychologists (including one pilot study) were interviewed about their letter-writing practices in relation to direct clinical work and in correspondence with other professionals. Analysis of the data yielded five themes: typical practices, use of clinical and micro skills, letters addressing the relationship with the family, messages to other professionals, and beliefs about the family. Findings suggest that letters are used in a variety of ways including being an aide-mémoire of the contents of face-to-face meetings, directly addressing the therapeutic relationship with the client, demonstrating respect, and informing other professionals of psychological practices and views regarding clients and their presenting problems. It emerged that letter-writing is seen to exist in the wider context of social relationships, between the referrer, clinician and client. The results are discussed in relation to clinical practices and future areas of study.  相似文献

Recently, the Eriksonian midlife personality strength of generativity has been described as existing in many forms and in persons of differing ages (McAdams, 2001). In this longitudinal study, narratives of life “turning points” and significant accomplishments were examined for generative themes in 32 adolescent/emerging adults (aged 16 and 20 years). We also explored these emerging adults’ volunteering behavior, prosocial reasoning, and parental influence as potential factors in generativity. Several parenting factors when adolescents were 16 (parents’ autonomy-encouraging practices, their emphasis on caring in stories of family value teaching, and adolescents’ reports of authoritative parenting style) were associated with more generative themes in narratives at age 20. Adolescents’ levels of prosocial reasoning, volunteering behavior, and personal value choices were also associated as expected with generative theme usage at age 20 in life stories, supporting the meaningfulness of this construct in emerging adulthood.  相似文献

It has long been known that the number of letters in a word has more of an effect on recognition speed and accuracy in the left visual field (LVF) than in the right visual field (RVF) provided that the word is presented in a standard, horizontal format. After considering the basis of the length by visual field interaction two further differences between the visual fields/hemispheres are discussed: (a) the greater impact of format distortion (including case alternation) in the RVF than in the LVF and (b) the greater facilitation of lexical decision by orthographic neighbourhood size (N) in the LVF than in the RVF. In the context of split fovea accounts of word recognition, evidence is summarised which indicates that the processing of words presented at fixation is affected by the number of letters to the left of fixation but not by the number of letters to the right and by the number of orthographic neighbours activated by letters to the left of fixation but not by the number of orthographic neighbours activated by letters to the right of fixation. A model of word recognition is presented which incorporates the notion that the left hemisphere has sole access to a mode of word recognition that involves parallel access from letter forms to the visual input lexicon, is disrupted by format distortion, and does not employ top-down support of the letter level by the word level.  相似文献

In this paper, we consider how recent social changes in the United States may be affecting vocational decision making. While adolescents often maintain high educational and occupational aspirations, the transition from school is characterized by few institutional supports, the prolongation of education, and a multitude of options with respect to the combination of school, work, and family. In such a context, what themes characterize decision making about schooling and occupational careers? We draw on qualitative interviews collected as part of the Youth Development Study (n=1000), a longitudinal study of work through adolescence and early adulthood. Multiple themes were identified including unfulfilled expectations, the postponement of decisions, turning points that crystallized decisions, and resources and obstacles including, among others, family, work, school counseling, and teachers. These themes characterize contemporary occupational decision making and thus would be appropriate focal points for future research. They also suggest that social policies may need to be modified to facilitate the young people's quest for vocational identity and work.  相似文献

Objective

Treatment decision‐making in bipolar II disorder is complex due to limited evidence on treatment efficacy and potentially burdensome side‐effects of options. Thus, involving patients and negotiating treatment options with them is necessary to ensure that final treatment decisions balance both clinician and patient preferences. This study qualitatively explored clinician views on (a) effective treatment decision‐making, unmet patient needs for (b) decision‐support and (c) information.

Method

Qualitative semi‐structured interviews with 20 practising clinicians (n = 10 clinical psychologists, n = 6 general practitioners, n = 4 psychiatrists) with experience treating adult outpatients with bipolar II disorder were conducted. Interviews were audiotaped, transcribed verbatim and analysed thematically using framework methods. Self‐report professional experience, and clinician preferences for patient decision‐making involvement were also assessed.

Results

Qualitative analyses yielded two inter‐related themes: (a) challenges and barriers to decision‐making and (b) facilitators of clinician decision‐making. Symptom severity, negative family attitudes, system‐based factors, and information gaps were thought to pose challenges to decision‐making. By contrast, decision‐making was supported by patient information, family involvement and patient‐centredness, and a strong therapeutic relationship. Clinician views varied depending on their professional background (medical vs clinical psychologist), patient involvement preferences, and whether the clinician was a bipolar specialist.

Conclusions

Whilst clinicians uniformly recognise the importance of involving patients in informed treatment decision‐making, active patient participation is hampered by unmet informational and decision‐support needs. Current findings inform a number of bipolar II disorder‐specific, clinician‐endorsed strategies for facilitating patient decision‐making, which can inform the development of targeted patient decision‐support resources for use in this setting.  相似文献

Two key issues for models of visual word recognition are the specification of an input-coding scheme and whether these input-coding schemes vary across orthographies. Here, we report two masked-priming lexical decision experiments that examined whether the ordering of the root letters plays a key role in producing transposed-letter effects in Arabic—a language characterized by non-concatenative morphology. In Experiment 1, letter transpositions involved two letters from the root, whereas in Experiment 2, letter transpositions involved one letter from the root and one letter from the word pattern. Results showed a reliable transposedletter priming effect when the ordering of the letters of the root was kept intact (Experiment 2), but not when two root letters were transposed (Experiment 1). These findings support the view that the order of the root letters is allowed only a minimum degree of perceptual noise to avoid the negative impact of activating the “wrong” root family.  相似文献

The optimal stimulation theory proposes that hyperactive children are less tolerant of lower levels of arousal than nonhyperactive children and should thus derive greater gains from stimulation added to repetitive copying tasks than do comparisons. To test this hypothesis, 16 adolescents, rating high on attention and behavior problems, were matched on the basis of age and poor handwriting performance to 16 controls. Matched pairs were randomly assigned to treatment order (high-stimulation colored letters followed in 2 weeks by low-stimulation black letters or the reverse order) and to level of information (color added to difficult letter parts or added to randomly selected letters), counterbalanced for treatment order and level of information within each order. Errors and activity were subjected to a mixed-design analysis of covariance, with IQ the covariate. The major findings indicated that attention-problem adolescents performed better with high-stimulation task stimuli than with low, relative to the opposite performance pattern of controls. The media creates and sustains knowledge about socially relevant issues such as crime, mental illness, and policing. Past research suggests that while the media portrayal of the police is undoubtedly complex, their portrayal of people experiencing mental illness remains mainly negative. The depiction of their encounters in the mass media is of considerable interest because of the influential role the media has on public opinion and debate; however, there is a lack of current research in this area. This study aimed to examine media items in the major newspapers across Australia between January 2006 and December 2007 (n = 77) to investigate their portrayal of this interface. The dominant themes encountered in the initial analysis depicted people experiencing mental illness as dangerous and a threat to the public, where the mental health system has failed to manage them adequately. Further analyses found that people experiencing mental illness were stigmatised in a third of the media items reviewed and were represented significantly more negatively in these encounters than the police. Implications of these findings and some suggestions regarding possible strategies that could improve socially responsible media reporting and public discourse on mental illness are proposed.  相似文献

Drawing on a qualitative study based on in-depth interviews with 30 rural and urban Australian women, we analyze their experiences of a hysterectomy for conditions other than cancer. Women were recruited via advertisements in community newspapers and the network sampling technique. The study aimed to identify the personal, social and medical factors that influenced their decision to undergo surgery and on their understandings of self following this procedure. Three common themes emerged: a negative response to a hysterectomy, ambivalence about it and a perceiving it as having a positive impact on the woman’s self. The latter pattern predominated, indicating that contrary to the medical rhetoric, women’s own accounts are different and relevant when counseling women about a hysterectomy.  相似文献

This paper argues that ideas about personhood and relationships vary cross-culturally and that although it is not always obvious or easy to discover, this variation is particularly salient to family therapy. This argument is supported by a dual definition of culture emphasizing both continuity of cultural themes (generative aspect) and the reconstitution, redefinition and change of such themes through interaction and communication (interactive aspect). The effectiveness of interventions which are isomorphic with ideas which families themselves hold about personhood is demonstrated through work with two families, one Punjabi and one Bangladeshi.  相似文献

The present research investigated the relationship between Rapid Automatized Naming (RAN) performance, letter-string reading measures of sight vocabulary (SV) and phonetic decoding (PD), and lexical decision. Criterion-based naming rates were obtained from three types of RAN tasks: digits, letters, and letter sounds. Latency measures were obtained from the naming of regular words, exception words, nonwords and pseudohomophones; as well as button press and verbal lexical decision tasks. Regression analyses supported the hypotheses that RAN-Letters latency reflects SV processing in that its variance is uniquely accounted for by exception word naming latency and button press lexical decision latency, and that RAN-Letter Sounds latency best reflects PD processing in that its variance is uniquely accounted for by pseudohomophone and nonword naming latency. Findings are discussed in light of what the RAN tasks are measuring, implications involving visual word recognition models of reading, and the utility of the new RAN-Letter Sounds task with respect to diagnostic and remediation applications.  相似文献

This paper outlines recent conceptual and methodological developments in the assessment of triadic and family group process during infancy and toddlerhood. Foundations of the emerging family group process are identified, and conditions specific to the assessment of the family during the early phases of family formation are summarized. Both microanalytic and global approaches to evaluating mother–father–child interactions are discussed. We highlight both similarities and differences in the strategies and methods employed by several different investigators who have been studying the group dynamics of families with infant and toddler children, and underscore several important family patterns and emerging themes that appear to be cutting across these different methods and measurement strategies. Preliminary evidence for the validity and clinical significance of family-level assessments is summarized, and directions currently being pursued by researchers engaged in studies of the family triad are outlined. We close by identifying several conceptual and clinical issues that remain to be addressed by subsequent work.  相似文献

In the general population, people with comorbid mental health (MH) and alcohol and other drug (AOD) disorders (comorbidity) have great difficulty accessing appropriate services, and poor outcomes. Little is known about comorbidity in resettled refugees in Australia. This study was designed to identify risk factors and patterns of comorbidity development in young people from refugee backgrounds living in a disadvantaged urban region of Adelaide, South Australia. This qualitative study utilised in‐depth semi‐structured interviews (n = 30) with resettled refugee youth and workers from MH, AOD, and refugee support services. Thematic analyses were conducted to investigate the aetiology of MH and AOD disorders in young refugees. Interviews with both groups revealed how the interrelated nature of risk factors may place young people from refugee backgrounds at heightened risk of experiencing MH and AOD problems. The situations and conditions described by both groups are discussed under six main themes: pre‐migration experiences of torture and trauma; familial factors of intergenerational conflict; post‐migration adjustment difficulties in terms of language, culture, education, and employment; exposure to and availability of substances; maladaptive coping strategies and self‐medication; and access to information and services. Implications for psychologists and MH professionals are identified, emphasising the need for clinicians to understand the complexities surrounding the aetiology of comorbidity in these youth. The initial assessment needs to be comprehensive, including pre‐ and post‐settlement experiences and cultural and family dimensions of their current situation. Treatment may often need to simultaneously address multiple contributing factors and involve culturally sensitive psycho‐education.  相似文献

Three experiments investigated the impact of five lexical variables (instance dominance, category dominance, word frequency, word length in letters, and word length in syllables) on performance in three different tasks involving word recognition: category verification, lexical decision, and pronunciation. Although the same set of words was used in each task, the relationship of the lexical variables to reaction time varied significantly with the task within which the words were embedded. In particular, the effect of word frequency was minimal in the category verification task, whereas it was significantly larger in the pronunciation task and significantly larger yet in the lexical decision task. It is argued that decision processes having little to do with lexical access accentuate the word-frequency effect in the lexical decision task and that results from this task have questionable value in testing the assumption that word frequency orders the lexicon, thereby affecting time to access the mental lexicon. A simple two-stage model is outlined to account for the role of word frequency and other variables in lexical decision. The model is applied to the results of the reported experiments and some of the most important findings in other studies of lexical decision and pronunciation.  相似文献

Therapeutic foster care (TFC) offers a promising community-based treatment option for children with serious emotional and behavioral disorders in the child welfare system. Family involvement is believed to contribute to achieving the goal of family reunification in TFC, but there has been little attention to family involvement in TFC. I present findings of a qualitative study of child welfare professionals' and TFC providers' perspectives on family involvement. Respondents' views of parent-child contact, parent-professional communication and information sharing, and family involvement in decision making were examined. Values and attitudes toward family involvement, practices related to family involvement, barriers to involvement, and strategies to promote involvement emerged as themes. Professionals in this study believed in the value of family involvement, but there were challenges at the organizational level and related to some TFC providers' lack of training to work with families.  相似文献

ABSTRACT

Background: Transgender children/youth demonstrate the best possibility for resilience and positive mental health when they are part of an affirming and supportive family. To optimize families in supporting transgender children/youth, parents/caregivers need to be supported. Transgender children/youth and their families regularly navigate a myriad of challenges through society.

Aims: Within the extant literature on transgender children and youth, relatively little attention is focused on the experiences of parents/caregivers and how their close family relationships are affected. The present qualitative study addresses this gap in knowledge through its exploration of the experiences, identities, and views of parents/caregivers of transgender children/youth.

Method: The study is longitudinal and this article represents the first wave. Fourteen parents of 12 transgender children/youth, aged 6–17, participated in in-depth semistructured interviews. Participants were recruited via social media and at an on-site conference for transgender youth and their families. All children/youth had socially transitioned. Those who were eligible for puberty suppression and/or hormone therapy were receiving those. The interviews were inductively coded for themes by two coders using the constant comparative method.

Results: Participants' reports on their experiences yielded three themes: (1) transgender issues as the family's focus; (2) proactivity, child-focused: preemptive actions to prevent adverse consequences; and (3) self-care. These themes included subthemes on effective coping strategies to mitigate struggles. Three themes related to participants' identity and views emerged: (1) identity reformation, (2) self-evaluation, and (3) views of future. These themes included subthemes that reflected how participants viewed themselves, their child, and the future.

Discussion: A transgender identity in one family member affects all household members. Parents/guardians may experience guilt and self-doubt over decisions. Factors that may improve outcomes with extended family and schools include families' preemptive dissemination of information on transgender identities and explicating expectations of respect and nonintrusion. Ingrained traditions may force life-altering decisions.  相似文献
