The associations between sociodemographic variables, psychological factors, and changes in dietary fat consumption over 4 months were assessed in a randomized controlled trial of behavioral counseling versus standard advice. Patients were 141 men and 150 women, with an average age of 52.1 years and total cholesterol level of 7.27 mmol/l (278 mg/dl). Smokers, younger patients, and those with greater body mass index had higher fat intake at baseline. Behavioral counseling led to greater reductions in fat intake than did standard advice. Self-efficacy and ratings of benefits of low-fat diets were related to fat consumption at baseline, and changes in these measures were correlated with changes in fat intake. Family support, baseline anticipated regret, and (for the behavioral counseling group only) baseline behavioral intentions predicted reductions in fat intake. The results indicated that psychosocial variables associated cross-sectionally with fat consumption do not necessarily predict change and that factors involved in the process of change and the prediction of change need to be differentiated.  相似文献

ABSTRACT. Severely obese men and women (body mass index ≥ 35 ≤ 55 kg/m2; Mage = 44.8 years, SD = 9.3) were randomly assigned to a 6-month physical activity support treatment paired with either nutrition education (n = 83) or cognitive-behavioral nutrition (n = 82) methods for weight loss. Both groups had significant improvements in physical activity, fatigue, self-regulation for eating, and fruit and vegetable intake. Compared to those in the nutrition education group, participants in the behavioral group demonstrated greater overall increases in fruit and vegetable intake and physical activity. These group differences were associated with changes that occurred after Month 3. Increased physical activity predicted reduced fatigue, β = ?.19, p =.01. A reciprocal relationship between the mediators of that relationship, which were changes in self-regulation and fruit and vegetable intake, was identified. There was significantly greater weight loss over six months in the behavioral nutrition group when contrasted with the nutrition education group. Self-regulation for eating and fruit and vegetable intake were significant predictors of weight loss over both three and six months. Findings enabled a better understanding of psychosocial effects on temporal aspects of weight loss and may lead to more effective behavioral treatments for weight loss.  相似文献

Maintaining loss of weight beyond an initial 6–9 months remains problematic, with research indicating little recent progress. A poor understanding of how and when prior weight loss, behavioral changes, and psychosocial predictors are associated with long-term weight changes persists. To better-inform behavioral treatments for long-term success with weight loss, women with obesity (N = 86; Mage = 48.6 years) volunteered for research incorporating community-based weight-management treatments. They were assessed at months 6, 12, and 24 on theory-based psychosocial and behavioral factors, and over 24 months on weight. Considering the included times and temporal ranges, it was found that change in weight from month 6–24 was the strongest predictor of 24-month weight change, and physical activity and fruit/vegetable intake at month 24 best predicted that change in weight. Self-regulation, self-efficacy, and mood at month 24 best predicted both physical activity and fruit/vegetable intake at month 24, with body satisfaction also a significant predictor of physical activity. From these data, mediation analyses found that the predictions of long-term weight loss by scores of self-regulation, self-efficacy, body satisfaction, and mood at month 24 were significantly mediated by fruit/vegetable intake and physical activity in separate equations. Findings indicated both psychosocial and behavioral targets, and timing for those targets, most indicative of long-term success with weight loss.  相似文献

In cognitive-behavioral treatments for obesity, self-regulation is thought to be a strong predictor of behavioral change, but it is rarely directly measured in intervention research. Thus, how self-regulation interacts with other psychological variables regarding treatment effects is largely unknown. In this preliminary field study, self-regulatory skills were directly measured and were found to be significantly associated with both volume of exercise and fruit and vegetable consumption in severely obese adults (N=116) enrolled in a behavioral weight management program. Significant partial and complete mediation of the relationship between self-regulation for physical activity and physical activity, and self-regulation for appropriate eating and fruit and vegetable intake, respectively, were found by reported negative mood. Self-efficacy was not found to be a significant mediator of these relationships. The bivariate relationship between baseline scores of self-regulation for physical activity and self-regulation for appropriate eating was significant (r = .46), which supported the premise that self-regulation is a trait-like personal characteristic. Volume of exercise and fruit and vegetable consumption significantly predicted weight loss over 6 months (R2 = .35). Results were consistent with the few laboratory-based findings available and, after replication, may extend theory related to obesity treatment.  相似文献

Baseline data from the Health and Behavior in Teenagers Study (HABITS) were used to investigate associations between stress and dietary practices in a socioeconomically and ethnically diverse sample of 4,320 schoolchildren (mean age = 11.83 years). Male (n = 2,578) and female (n = 1,742) pupils completed questionnaire measures of stress and 4 aspects of dietary practice (fatty food intake, fruit and vegetable intake, snacking, and breakfast consumption) and also provided demographic and anthropometric data. Multivariate analyses revealed that greater stress was associated with more fatty food intake, less fruit and vegetable intake, more snacking, and a reduced likelihood of daily breakfast consumption. These effects were independent of individual (gender, weight) and social (socioeconomic status, ethnicity) factors. Stress may contribute to long-term disease risk by steering the diet in a more unhealthy direction.  相似文献

In this study the authors examined psychosocial variables as mediators for fruit and vegetable (FV) intake in a clustered, randomized effectiveness trial conducted in African American churches. The study sample included 14 churches (8 intervention and 6 control) with 470 participants from the intervention churches and 285 participants from the control churches. The outcome of FV intake and the proposed mediators were measured at baseline and at 6-month follow-up. Structural equation modeling indicated that the intervention had direct effects on social support, self-efficacy, and autonomous motivation; these variables also had direct effects on FV intake. Applying the M. E. Sobel (1982) formula to test significant mediated effects, the authors confirmed that social support and self-efficacy were significant mediators but that autonomous motivation was not. Social support and self-efficacy partially mediated 20.9% of the total effect of the intervention on changes in FV intake. The results support the use of strategies to increase social support and self-efficacy in dietary intervention programs.  相似文献

Adults (N?=?163) aged 20-65 were randomised into four groups: implementation intentions (II group), self-efficacy (SE group), combination of II?+?SE group) and a control group receiving written information on nutrition. Study variables were measured at baseline, post-intervention and at 3-month follow-up. At follow-up, compared to the control group, FVI increased significantly in the II and II?+?SE groups (1.5 and 1.9 servings per day, respectively). Most psychosocial variables significantly increased compared to the control group, with the exception of SE for vegetable intake (VI). Moreover, at 3-month follow-up, change in FVI was mediated by changes in fruit intake (FI) intention and VI action planning. In conclusion, II interventions were efficient to increase FVI, with or without consideration for the development of SE. Thus, future studies should favour the adoption of this approach to bridge the intention-behaviour gap for FVI.  相似文献

Exercise is a robust predictor of long-term success with weight loss, and research based on social cognitive theory suggests that exercise program-induced changes in self-regulation, mood, and self-efficacy transfer to improved eating. These relationships were tested in adults with morbid obesity (overall Mage = 43 years; 86% female; Mbody mass index = 45 kg/m2) participating in a 6-month treatment of cognitive-behaviorally supported exercise paired with either standard nutrition education (n = 87) or cognitive-behavioral methods for controlled eating (n = 89). Based on multiple mediation analyses, improvements in self-regulation and self-efficacy were significantly associated with increased fruit and vegetable intake and reduced body mass index (BMI). The cognitive-behavioral nutrition methods were associated with greater improvements in fruit and vegetable intake and BMI, however, within both models, complete mediation was found after simultaneous entry of changes in self-regulation, mood, and self-efficacy, and exercise volume as mediators. Only the indirect effect of change in self-regulation was a significant (or marginally significant) independent mediator. Generalization of previously identified relationships between exercise program-induced improvements in psychosocial variables and improvements in nutrition and BMI were supported for individuals with morbid obesity. Based on these relationships, implications for behavioral treatments were discussed.  相似文献

Prior research suggests that fruit and vegetable intake predicts psychological well-being (WB) when controlled for demographic variables such as age, income and education. Using multiple-item measures and including additional diet and health variables as covariates, the current study assessed self-reported well-being in the past week and daily fruit and vegetable consumption over the past 4 weeks for 1270 university students. Mean positive affect increased linearly as a function of number of daily servings of fruits and vegetables; the pattern of this relationship did not differ significantly for males and females. This association remained statistically significant after controlling for demographic variables (age, sex, and parent education levels); other diet variables (consumption of sugar containing beverages, coffee or tea, and fat); and other health behaviors (exercise, sleep quality and smoking). Life satisfaction and negative affect were not significantly related to fruit and vegetable consumption. Analysis of single-item measures similar to those used in past large scale surveys yielded similar results. Possible reasons for the association of fruits and vegetable consumption with well-being are discussed.  相似文献

Healthy Body Healthy Spirit was a multicomponent intervention to increase fruit and vegetable (F & V) consumption and physical activity (PA) delivered through Black churches. Sixteen churches were randomly assigned to 3 intervention conditions. At baseline, 1,056 individuals were recruited across the 16 churches, of which 906 (86%) were assessed at 1-year follow-up. Group 1 received standard educational materials, Group 2 received culturally targeted self-help nutrition and PA materials, and Group 3 received the same intervention as did Group 2 as well as 4 telephone counseling calls based on motivational interviewing (MI) delivered over the course of 1 year. At 1-year follow-up, Groups 2 and 3 showed significant changes in both F & V intake and PA. Changes were somewhat larger for F & V. For F & V, but not PA, there was a clear additive effect for the MI intervention.  相似文献

The present study examined a dietary approach – lowering energy density – for producing weight loss in obese patients with binge eating disorder (BED) who also received cognitive-behavioral therapy (CBT) to address binge eating. Fifty consecutive participants were randomly assigned to either a six-month individual treatment of CBT plus a low-energy-density diet (CBT + ED) or CBT plus General Nutrition counseling not related to weight loss (CBT + GN). Assessments occurred at six- and twelve-months. Eighty-six percent of participants completed treatment, and of these, 30% achieved at least a 5% weight loss with rates of binge remission ranging from 55% to 75%. The two treatments did not differ significantly in weight loss or binge remission outcomes. Significant improvements were found for key dietary and metabolic outcomes, with CBT + ED producing significantly better dietary outcomes on energy density, and fruit and vegetable consumption, than CBT + GN. Reductions in energy density and weight loss were significantly associated providing evidence for the specificity of the treatment effect. These favorable outcomes, and that CBT + ED was significantly better at reducing energy density and increasing fruit and vegetable consumption compared to CBT + GN, suggest that low-energy-density dietary counseling has promise as an effective method for enhancing CBT for obese individuals with BED.  相似文献

Social disadvantage is associated with being overweight, a poor diet and physical inactivity. The NHS Health Trainer Service (HTS) is a national initiative designed to promote behaviour change among socially disadvantaged people in England and Wales. This study reports pre-post changes in body mass index (BMI), associated behaviours and cognitions among service users who set dietary or physical activity goals during a 12-month period (2008-2009; N?=?4418). Sixty-nine percent of clients were from the two most deprived population quintiles and 94.7% were overweight or obese. Mean BMI decreased from 34.03 to 32.26, with overweight/obesity prevalence decreasing by 3.7%. There were increases in fruit and vegetable consumption, reductions in fried snack consumption, increases in frequency of moderate or intensive activity and gains in self-efficacy and perceived health and wellbeing. Clients with higher BMI, poorer diet or less activity at baseline achieved greater change. Findings suggest that the NHS HTS has the potential to improve population health and reduce health inequalities through behaviour change.  相似文献

OBJECTIVE: Little is known about temporal patterns of diet change within interventions, nor about predictors of early and sustained successful change. Social-cognitive theory asserts that early successes in achieving behavior change increase self-efficacy, leading to longer-term success. DESIGN: The authors conducted exploratory cluster analyses using dietary data from the first month of the telephone counseling intervention of the Women's Healthy Eating and Living (WHEL) Study. MAIN OUTCOME MEASURE: Change in dietary pattern at three early intervention timepoints. RESULTS: Three clusters were identified: Cluster 1 (25%) was close to meeting study goals at baseline, but still made major changes; Cluster 2 (49%) and Cluster 3 (26%) were not achieving study goals at baseline, but Cluster 2 made substantial immediate changes, while Cluster 3 changed their diet more gradually. Baseline demographic and behavioral variables were associated with cluster membership; however, the strongest predictors of cluster were self-efficacy, motivation, and approaches to study goals. Cluster membership predicted dietary pattern at 12 months. CONCLUSION: These data suggest that a one-on-one telephone counseling intervention that is intensive in the early weeks may maximize the level of change achieved in a study.  相似文献

The present study tested whether baseline perceived social support and social integration predicted baseline and follow-up measures of health-related quality of life for 364 older adults with osteoarthritis. The findings are secondary analyses of a randomized controlled trial of an exercise intervention. Multiple regression analyses indicate that perceived social support was related to baseline measures of functioning in psychological (depressive symptoms, social functioning, and life satisfaction) and physical domains (self-rated disability, observed physical function, and perceived health), after accounting for demographic and clinical status factors. At 18-month follow-up (additionally controlling for exercise intervention and baseline outcomes), social support significantly predicted changes in psychosocial functioning, but was unrelated to changes in self-reported and observed physical health. The findings indicate that social support is an important predictor of long-term psychosocial outcomes, but is less important than baseline clinical status for physical health endpoints in this cohort of older adults. In contrast, social integration was not a consistent predictor of outcomes.  相似文献

The Dash of Faith pilot used a community-based participatory research approach to design an experiential dietary intervention based on two African-American churches, one intervention and one comparison. Congregation members identified components that were incorporated into 12 weekly and 4 monthly sessions, with a goal of increasing fruit and vegetable and lowering fat intake. At 2 months, a marginally significant (p = 0.07) increase in fruit and vegetable consumption was observed in the intervention group but was not maintained at study conclusion. We propose that these mixed findings may be attributable, in part, to bias introduced by the participatory nature of the design.  相似文献

Abstract

Differences in fat consumption and psychosocial factors between subjects in different trans-theoretical stages of change for fat reduction were studied. Data on fat consumption, self-rated fat intake, attitudes, social support and self-efficacy were collected among a sample of 507 adults. Analysis of variance with Scheffe's multiple-comparison test was used to study differences in these factors between stages of change. Attitudes and social support were most positive among subjects in preparation and action. Self-efficacy expectations were lowest among subjects in contemplation and preparation. Fat consumption was highest among precontemplators hut no substantial differences between other stages were found. It is concluded that subjects in different stages of change differ in psychosocial factors that are regarded as important determinants of behavioral change. Based on these differences it is recommended to develop stage-tailored nutrition education. A majority of subjects in maintenance consumed more fat than is recommended. Therefore, it is suggested that the usually applied staging algorithms might need adjustment for a valid application in nutrition research.  相似文献

This study identified mediators of a school-based nutrition intervention for 4th graders and their parents. Nine variables were tested for satisfying 4 conclusions necessary to establish mediation of intervention effects on changes in fruit and vegetable consumption (FVC) in 4th graders (N = 1,676). FVC was measured in children by the use of 24-hr dietary recalls. Mediators were assessed by the use of questionnaires completed by children and parents. All 4 conclusions were met for positive outcome expectancies. Knowledge, self-efficacy, and parent consumption satisfied the first 3 conclusions for mediation but the mediating effects were not statistically significant (Conclusion 4). Future mediational analyses and the creation of a national database of mediators are crucial for increasing strength and efficiency of school-based nutrition programs.  相似文献

In a prospective study, two nutrition behaviors, meat and sodium consumption, were examined utilizing a social psychological model of health behavior suggested by Wallston and Wallston. The model conceptually integrates elements from the Health Belief Model, Fishbein's attitude-behavior model, and Triandis' theory of social behavior. According to the model, behavior is a function of behavioral intent, habit, and facilitating conditions. Behavioral intent is a function of affect, perceived consequences, and social and personal norms. In this study, the model explained a significant proportion of the variance for each self-reported nutrition behavior and behavioral intent. Hierarchical regression analyses indicated that all three factors contributed to the prediction of behavior, and differences between meat and sodium consumption were found. For behavioral intent, each of the components made a significant contribution, with affect being the best predictor.  相似文献
