This paper explores the therapeutic value of considering psychoanalytic diagnoses as co-constructions, and emphasizes two interrelated points: First, there are distinctions between medical, psychiatric, and psychoanalytic diagnoses, and similarly, a distinction may be drawn between diagnoses as nouns and the activity of diagnosing. Second, the author stresses that psychoanalytic diagnoses are theory bound. Various competing theories facilitate or interfere with the maintenance of an optimal analytic attitude. Some theories facilitate an analyst's urge to engage in the activity of diagnosing, which may reflect a destructive countertransference enactment. Analytic data in support of these premises is presented and discussed.  相似文献

An earlier publication ( Halpert, 1994 ) dealt with patients' magical expectations of physicians. In this presentation, aspects of physicians' expectations of themselves as doctors will be explored. This exploration will include material from the history of medicine, a series of interviews with non-patients and the analyses of two physicians. One element in the psychology of physicians not previously appreciated that can be drawn out of this material is the role and importance of knowledge and knowing and their relation to illness, death, rescue fantasies and reality in the psychology of physians.  相似文献

Certain ambivalent reactions to undergoing analysis add content and intensity to the analysand's transference and to some extent to the analyst's countertransference. A significant share of these reactions may be attributed to the structure of the psychoanalytic situation. Among other things, analysands' reactions feature fantasized and sometimes realistic experiences of being coerced by their analyst's counter-transferences. However, to achieve a clear view of these coercive experiences, they must be considered in relation to another, equally weighty structural feature: ambivalent reactions to the analyst's concerned care. Observation suggests that an intervention can be experienced, simultaneously or sequentially, as both coercive and caring, and that, when analyzed, each of these experiences is usually found to be saturated with mixed feelings.  相似文献

The competing theories in the psychoanalytic marketplace today should be judged on their merits, not on the basis of the authority of whoever first proposed them. What is valid in each theory should be included in any formulation of a psychoanalytic theory of mental development and functioning. Since psychoanalysis, as part of psychology, is a branch of natural science, pluralism in theory is to be avoided in psychoanalysis as in every branch of science. The psychoanalytic method is a valid one of studying a particular aspect of brain functioning. The method and the theories based upon it are as "organic" as is the case with any of the other neurosciences. Any valid psychoanalytic theory of mental functioning and development should include the following conclusions: (1) Unconscious mental processes are omnipresent and of great importance in mental functioning; (2) Thoughts are as causally related to one another as are other events in the universe; (3) Mental functioning is a developmental phenomenon with describable, sequential features; and (4) A major role in mental functioning and development is played by conflicts over the sexual and aggressive wishes that characterize mental life during the period from three to six years of age, and by the compromise formations that result from those conflicts. The last of these conclusions, though disputed by many, is abundantly supported by evidence that is not dependent on the use of the psychoanalytic method, as well as by evidence furnished by the use of the psychoanalytic method. There is also much evidence to support the assertion that any psychoanalytic theory that attributes language-dependent thoughts to a child whose brain is not yet mature enough to be capable of language is to be considered invalid, as are any observations made by the psychoanalytic method (= clinical observations) that are influenced by such an invalid theory. In psychoanalysis, as in every other branch of science, an observer--no matter how astute and how experienced--who subscribes to an invalid theory will be led astray by that theory, sooner or later, in one way or another.  相似文献

Across many languages from unrelated families, spoken-word recognition is subject to a constraint whereby potential word candidates must contain a vowel. This constraint minimizes competition from embedded words (e.g., in English, disfavoring win in twin because t cannot be a word). However, the constraint would be counter-productive in certain languages that allow stand-alone vowelless open-class words. One such language is Berber (where t is indeed a word). Berber listeners here detected words affixed to nonsense contexts with or without vowels. Length effects seen in other languages replicated in Berber, but in contrast to prior findings, word detection was not hindered by vowelless contexts. When words can be vowelless, otherwise universal constraints disfavoring vowelless words do not feature in spoken-word recognition.  相似文献

Natural dialogue does not proceed haphazardly; it has an easily recognized episodic structure and coherence which conform to a well developed set of conventions. This paper represents these conventions formally in terms related to speech act theory and to a theory of action.The major formal unit, the dialogue game, specifies aspects of the communication of both participants in a dialogue. We define the formal notion of dialogue games, and describe some of the important games of English. Dialogue games are conventions of interactive goal pursuit. Using them, each participant pursues his own goals in a way which sometimes serves the goals of the other. The idea of dialogue games can thus be seen as a part of a broader theoretical perspective which characterizes virtually all communication as goal pursuit activity.We also define and exemplify the property of Motivational Coherence of dialogues. Motivational Coherence can be used as an interpretive principle in explaining language comprehension.Actual dialogue games have a kind of causal connectedness which is not a consequence of their formal properties. This is explained in terms of a theory of action, which is also seen to explain a similar attribute of speech acts.  相似文献

Children with attention-defi cit/hyperactivity disorder (ADHD) and disorder of attention, motility control, and perception (DAMP) are often sensitive to the analyst's interventions. This is not always due to the literal import of the intervention. The children sometimes react as if the words were dangerous concrete objects, which they must physically fend off. The author traces this phenomenon to the child's unstable internal situation. A bad, un-containing internal object is easily awakened and threatens to expel the analyst's words independently of their content. This results in violent clinical situations. Infant research and psychoanalytic work with infants and mothers evince how a complex semiotic process develops between mother and baby. The prerequisite for this process to get started and maintained is a secure external object, which gradually is internalized. Findings from developmental research and clinical infant work are used to illuminate analytic work with children with ADHD and DAMP. Vignettes demonstrate how important it is for the analyst to phrase interpretations after having gauged the state of the analysand's internal object as well as his/her own countertransference. If this is overlooked, the psychoanalytic dialogue easily capsizes. The author provides some technical recommendations on the psychoanalysis of these children. As part of the theoretical discussion he raises the general question of how the representations, which the baby forms in interaction with the mother, and the analysand forms in interaction with the analyst, should be classifi ed. Rather than dividing them into bipartite thing- or word-presentations (Freud), the author suggests C. S. Peirce's tripartite semiotic classifi cation in that the baby forms representations of icons, indices, and symbols.  相似文献

To understand a sentence, the meanings of the words in the sentence must be retrieved and combined. Are these meanings represented within the language system (the lexical hypothesis) or are they represented in a general conceptual system that is not restricted to language (the conceptual hypothesis)? To evaluate these hypotheses, sentences were presented in which a pictured object replaced a word (rebus sentences). Previous research has shown that isolated pictures and words are processed equally rapidly in conceptual tasks, but that pictures are markedly slower than words in tasks requiring lexical access. The lexical hypothesis would therefore lead one to expect that rebus sentences will be relatively difficult, whereas the conceptual hypothesis would predict that rebus sentences would be rather easy. Sentences were shown using rapid serial visual presentation (RSVP) at a rate of 10 or 12 words per second. With one set of materials (Experiments 1 and 2), readers took longer to judge the plausibility of rebus sentences than all-word sentences, although the accuracy of judgment and of recall were similar for the two formats. With two new sets of materials (Experiments 3 and 5), rebus and all-word sentences were virtually equivalent except in one circumstance: when a picture replaced the noun in a familiar phrase such as seedless grapes. In contrast, when the task required overt naming of the rebus picture in a sentence context, latency to name the picture was markedly longer than to name the corresponding word, and the appropriateness of the sentence context affected picture naming but not word naming (Experiment 4). The results fail to support theories that place word meanings in a specialized lexical entry. Instead, the results suggest that the lexical representation of a noun or familiar noun phrase provides a pointer to a nonlinguistic conceptual system, and it is in that system that the meaning of a sentence is constructed.  相似文献

Hope helps alleviate suffering. In the case of terminal illness, recent experience in palliative medicine has taught physicians that hope is durable and often thrives even in the face of imminent death. In this article, I examine the perspectives of philosophers, theologians, psychologists, clinicians, neuroscientists, and poets, and provide a series of observations, connections, and gestures about hope, particularly about what I call "deep hope." I end with some proposals about how such hope can be sustained and enhanced at the end of life. Studies of terminally ill patients have revealed clusters of personal and situational factors associated with enhancement or suppression of hope at the end of life. Interpersonal connectedness, attainable goals, spiritual beliefs and practices, personal attributes of determination, courage, and serenity, lightheartedness, uplifting memories, and affirmation of personal worth enhance hope, while uncontrollable pain and discomfort, abandonment and isolation, and devaluation of personhood suppress hope. I suggest that most of these factors can be modulated by good medical care, utilizing basic interpersonal techniques that demonstrate kindness, humanity, and respect.  相似文献

This study draws upon both Carlo Ginzburg's procedures for establishing and preserving (historical) evidence and Claude Lévi-Strauss' structuralist approach with its analysis of "primitive" mythologies to demonstrate that binary coding of the social domain is an all-pervasive structural principle. Heim's intention in this is to show that (present-day) xenophobia and racism are the products of a phantasm centering around the division of the world into pure and impure. The author brings into alignment collective fantasies about the homogeneity of the "body politic" with a form of primary narcissism which, if it is to preserve the illusion of original purity, is forced to externalize instinctual urges experienced as heterogeneous and unpleasurable and project them onto "foreigners" and things foreign.  相似文献
