Ethical Issues in Counseling Adult Survivors of Incest

Most counselors working with an adult clientele are faced with negotiating the many ethical dilemmas that occur when counseling adult survivors of child sexual abuse. In this article, the primary ethical concerns encountered in the treatment of adult survivors are addressed. Meta-ethical principles are identified for guiding ethical decision making in dealing with issues that arise in the survivor's relationships with the counselor, the abuser, and other family members.     

Therapeutic Considerations of Wilderness Experiences for Incest and Rape Survivors

The purpose of this article is to examine the relevancy of traditional wilderness programs, such as Outward Bound, to women who have been violated by experiences of childhood incest and/or adult sexual assault. Applications to African-American women, women with physical challenges, and women who are economically disenfranchised will be emphasized. The overview is from a feminist perspective, one that advocates empowerment over struggle and resisting revictimization on any level. The Rape Assistance and Awareness Program (RAAP) is a non-profit agency in Denver, Colorado dedicated to providing low-cost group psychotherapy for women who have been recently sexually assaulted as well as for women with histories of incest. About five years ago, a three-day Colorado Outward Bound School (COBS) experience was incorporated into RAAP's treatment program. While the COBS weekend generally has been viewed positively by most of the clients and group facilitators, aspects of this element of the program have become increasingly problematic. Its applicability to women with physical disabilities and women with conflicting social and economic priorities is questioned. When RAAP received a special grant to provide sexual assault services specifically targeting women of color, groups for Latinas and African-American women were established and facilitated by therapists of color. These women of color, both clients and therapists, consistently challenge the relevance and practicality for them of the COBS experience. In this article, we contend that wilderness experiences, in their traditional application, do not support the feminist, woman-centered approach to life which is advocated and nurtured at RAAP, nor does it specifically honor cultural diversity or ethnic multiplicity. Recommendations for wilderness experiences attracting a broader range of the feminine experience are made.     

Politicizing Survivors of Incest and Sexual Abuse

Abstract

As both a feminist therapist and a survivor of incest, moving myself beyond therapeutic issues into an area of public education has been crucial to my own healing process. Receiving positive responses to my workshops and discussion groups, it became apparent that using a similar approach with clients who were survivors might be beneficial to their healing. As a member of The Counseling Center for Women, a feminist therapy collective in Israel, I suggested we sponsor a public exhibition and a series of programs of the topic of incest and abuse. The Municipality of Tel Aviv was approached to be its governmental sponsor. Entitled “Silent No More,” the program moved therapists and clients from working in the privacy of therapy to a public forum. What began as an uncomplicated educational program became an emotional process and a politicizing of individuals to help others as well as themselves. Moving from a feeling of shame to a sense of empowerment, the exhibition gave us a new sense of self-esteem and strength in our own abilities and our recovery process.     

A Current Perspective on Short-term Groups for Incest Survivors

Short-term groups are a vital resource for adult women who were incestuously abused in childhood or adolescence. The authors describe a model developed by a hospital-based Rape Crises Program. The groups are designed to help members focus on specific goals and experience growth through identifying with other incest survivors. The group format addresses many aspects of the recovery process for survivors. Group heterogeneity, designation of groups for younger and older women respectively, and individual follow-up meetings with members are therapeutically valuable.

Effects of Incest in Aging Survivors: Psychopathology and Treatment Issues

Most older people who have been sexually abused in childhood have probably never been treated for the consequences of their destructive experiences. The character pathology and long-term effects of child sexual abuse are described. Parallels were drawn between delayed Posttraumatic Stress Disorder seen in older veterans and World War II victims and later-life onset of posttraumatic psychopathology in aging incest survivors. Special geropsychological treatment issues, such as the appropriateness of explicit inquiry regarding a history of abuse and the role of power, gender, and shame in the psychotherapeutic relationship, are presented. The dilemma of uncovering repressed malignant childhood trauma in aging individuals is discussed. With proper medical and psychological preparation, demanding curative interventions with older survivors may be considered.     

A Group Design for Adult Female Survivors of Childhood Incest

This paper will describe short term groups for women sexually molested as children. The goals of such a group were to provide an atmosphere in which women could discuss the experience of sexual abuse, explore the consequences of incest, be exposed to the probably phases of recovery and discuss practical coping skills. The design of the group is detailed, as are recommendations and relevant issues for therapists. Women's responses to the group, such as increase in self-esteem, trust level, assertiveness and confrontation with the abuser, are also discussed.     

Dyadic Influence of Adverse Childhood Experiences: Counseling Implications for Mental and Relational Health

Adverse childhood experiences (ACEs) predict physical and mental health. We examined dyadic differences in intimate partner relationship health by ACE score among couples with low to moderate income enrolled in a relationship education program. Overall, we found a high prevalence of ACEs, disproportionate representation in the highest risk ACE group, and a significant effect between ACEs and indicators of mental and relational health. Implications for counselors include support for an ACE‐informed approach to couples counseling services, as well as a holistic and systemic view of mental and relational health.     

The Individual Career Counseling Needs and Experiences of Female Survivors of Partner Violence

The impact of partner violence on women's employment and career development is profound. Career counselors may contribute substantially to these women's rehabilitation. This study examined employment and career counseling needs, barriers experienced, and counseling satisfaction of female survivors of partner violence (N = 71). The women participated in community‐ and research‐based individual career counseling services designed to promote their short‐ and long‐term career development. The authors conducted correlational, t‐test, and logistic regression analyses and found that barriers differed by race/ethnicity, age, number of children, and career service needs. Results provide important information about survivors' career counseling service access and needs.     

Impairment: Implications for the Counseling Profession

The issue of impairment has not been adequately addressed in the professional counseling literature. The authors explore the issue of impairment in the counseling profession, drawing from the literatures of medicine and psychology in discussing the prevalence of impairment, several views of the topic, responses to the problem, and related ethical considerations. Recommendations for the analysis and resolution of impairment issues in counseling are presented.     

Themes in Group Counseling with Adolescents

A group of counselors met in seminar over a period of a year, critiquing tapes of their group counseling sessions. It was found through analysis of these recordings that certain themes could be identified which occurred and reoccurred in the content of group counseling with adolescents. These themes were entitled: “My Vices,” “Outwitting the Adults,” “Problem Parents,” “My Brother, the Brat,” “My Public Image,” “Nobody Loves Me,” and “Let's Change the Rules.” Illustrative material is presented that was transcribed from the recordings, after all identifying data were removed.

Suicidality in Schizophrenia: Implications for the Counseling Profession

Schizophrenia is a common mental disorder treated by many counseling professionals, and it is associated with a high rate of early mortality. The most common reason for early mortality in schizophrenia is suicide. To date, the most recent comprehensive study on correlates of suicidality among clients with schizophrenia is titled “Suicidal Behavior in Schizophrenia: Characteristics of Individuals Who Had and Had Not Attempted Suicide” (J. M. Harkavy‐Friedman et al., 1999). It is the purpose of this Trends article to review this study and discuss its implications for counselors.     

Treating the Sexual Concerns of Adult Incest Survivors and Their Partners

SUMMARY

Negative attitudes about sex and sexuality and many types of sexual dysfunction are common aftereffects of sexual child abuse and incest. The traumatic sexualization and the aversive sexual conditioning resulting from childhood abuse have long gone unrecognized, however. This article presents research findings regarding the impact of incest on sexual attitudes and functioning along with treatment recommendations.     

Motivational Counseling: Implications for Counseling Male Juvenile Sex Offenders

Juvenile sex offenders (JSOs) often appear unmotivated to change, which thus necessitates a therapeutic approach that matches “resistant” client characteristics. In this article, the authors review common traits of JSOs, introduce motivational counseling as an effective treatment modality, and offer a case illustration.
