The clinical ethics propounded by Richard Zaner is unique. Partly because of his phenomenological orientation and partly because of his own daily practice as a clinical ethicist in a large university hospital, Zaner focuses on the particular concrete situations in which patients and their families confront illness and injury and struggle toward workable ways for dealing with them. He locates ethical reality in the clinical encounter. This encounter encompasses not only patient and physician but also the patients family and friends and indeed the entire lifeworld in which the patient is still striving to live. In order to illuminate the central moral constituents of such human predicaments, Zaner discusses the often-overlooked features of disruption and crisis, the changed self, the patients dependence and the physicians power, the violation of personal boundaries and their necessary reconfiguring, and the art of listening.  相似文献

This essay examines the relation of Darwin's orchids book to a central persuasive flaw in theOrigin: Its inability to give variation sufficient presence to break the hold of design in the mind of the reader. Darwin characterized the orchids book as a flank movement on the enemy; this essay identifies the enemy as Paley's natural theology and the flank as thetopoi, maxims, and habits of perception that led Darwin's colleagues and contemporaries to see design in nature. Moreover, this essay examines three aspects of rhetorical timing pertinent toOrchids - time askairos, time as adequate duration, and time as transformation - and then relates those features to Robert Cox's Heideggerian logic of repetition, disavowal, and transcendence. The essay concludes with implications of the tactical and temporal aspects of Darwin's reasoning for understanding both the logic of science and of Darwin as a rhetorical artist.An earlier version of this essay appeared in the Proceedings of the Sixth SCA/AFA Conference on Argumentation.  相似文献

Latent trait models for binary responses to a set of test items are considered from the point of view of estimating latent trait parameters=( 1, , n ) and item parameters=( 1, , k ), where j may be vector valued. With considered a random sample from a prior distribution with parameter, the estimation of (, ) is studied under the theory of the EM algorithm. This article distinguishes between the ego of Freudian analysis and the ego of spiritual tradition. Using his own personal psychic and spiritual development, the writer demonstrates how the Freudian ego must grow and transform, bringing into consciousness and healing all psychic wounds. The notion of monadic three-valued ukasiewicz algebras was introduced by L. Monteiro ([12], [14]) as a generalization of monadic Boolean algebras. A. Monteiro ([9], [10]) and later L. Monteiro and L. Gonzalez Coppola [17] obtained a method for the construction of a three-valued ukasiewicz algebra from a monadic Boolea algebra. In this note we give the construction of a monadic three-valued ukasiewicz algebra from a Boolean algebra B where we have defined two quantification operations and * such that *x=*x (where *x=-*-x). In this case we shall say that and * commutes. If B is finite and is an existential quantifier over B, we shall show how to obtain all the existential quantifiers * which commute with .Taking into account R. Mayet [3] we also construct a monadic three-valued ukasiewicz algebra from a monadic Boolean algebra B and a monadic ideal I of B. The most essential results of the present paper will be submitted to the XXXIX Annual Meeting of the Unión Matemática Argentina (October 1989, Rosario, Argentina).  相似文献

The definition of spirituality poses a variety of problems for the development of theory and research, as well as practical problems for persons interested in promoting the spiritual well-being of older adults. Although any definition of spirituality is problematic, a definition is proposed that comes out of the writer's clinical experience and is relevant to his understanding of the aging process in different cultural and religious contexts. Two case studies are presented to illustrate the relevance of the definition to the experience of older persons from different cultural and religious backgrounds. Reflections on the case studies suggest ways that an appropriately trained advocate might have helped the persons in these illustrations make changes in their situations that might have improved the quality of their lives. Material from the case studies is also used to clarify differences among terms such as spirituality, religion, religiosity, and piety. The paper proposes to be a contribution to a theoretical foundation for studying and working with spirituality in older adults.  相似文献

A significant proportion of the cases that come for therapy, perhaps as high as 25 percent, do so at the initiative of some outside person or agency. A brief literature survey describing the particular motivational set established by such extrinsic motivation is examined, and its potential as the gathering point-the summation of the internal along with the external resistance-is discussed. Clinical vignettes are presented to illustrate how important it is for the therapist to step away from the patient's designation (of him or her) as the enemy and available scapegoat in the saga of the unacceptable mandate. Only by the therapist's acknowledging the legitimacy of the patient's negativism towards the mandate, and dissociating both therapist and therapy from this external resistance can the internal resistance to change be successfully addressed. To not make this distinction is to risk the patient's characteristic rebelliousness and authority conflicts, perseverating on the mandate and subverting the quiet cry for help that is audible in every troubled individual, if we allow ourselves to hear it.  相似文献

On–off phenomena in Parkinson's disease (PD) are unpredictable motor fluctuations associated with long-term levodopa use. Mood fluctuations have been found to coincide with the motor fluctuations in that depression and anxiety increase while the person with PD is in the off state. What has been relatively unexplored is whether those persons with PD who have on–off phenomena differ psychologically in fundamental ways from those who do not have on–off phenomena. In the present study, depression and anxiety symptoms were assessed in 36 persons with PD (n = 14 with on–off phenomena, n = 22 without on–off phenomena). All those with on–off phenomena were assessed in their on state. Those persons with PD with on–off phenomena had significantly higher levels of anxiety than those without on–off phenomena. However, both groups, regardless of on–off status, were mildly depressed. Neurobiological interpretations of the results implicate the locus coeruleus in the pathogenesis of both on–off phenomena and anxiety, whereas psychological interpretations of the results involve the issues of learned helplessness and control over health symptoms in PD.  相似文献

Mel Gibsons film is a creative reworking of opportunities provided by migrating to Australia as a boy and a life-long desire to be reconciled to his father, Hutton Gibson. If the film is read as a study in psychology and biography, then biblical and theological critiques, along with contemporary politically correct stances towards what Gibson did, can be set temporarily aside in search of a larger trajectory of meaning implicit in the film. The embodied nature of the film, along with other violent films in which Gibson starred in the past three decades, points to wellsprings of power in the blood. Violence becomes an idiom of reconciliation with authority. Gibsons relationship to his father plays a major role in such a reading of the film. A continuous reconciliation with his father is evident not only in thematic continuities over the course of Mel Gibsons professional acting career and starring roles. It also structures the directors construction of Christs Passion. A desire not to be forsaken gives rise to this idiosyncratic and controversial film, itself a cinematic statement about Gibsons own personal spiritual torment.Richard A. Several of the so-called fallacies in Aristotle are not in fact mistaken inference-types, but mistakes or breaches of rules in the questioning games which were practiced in the Academy and in the Lyceum. Hence the entire Aristotelian theory of fallacies ought to be studied by reference to the author's interrogative model of inquiry, based on his theory of questions and answers, rather than as a part of the theory of inference. Most of the fallacies mentioned by Aristotle can in fact be diagnosed by means of the interrogative model, including petitio principii, multiple questions, babbling, etc., and so can Aristotle's alleged anticipation of the fallacy of argumentum ad hominem. The entire Aristotelian conception of inquiry is an interrogative one. Deductive conclusions caught Aristotle's attention in the form of answers that every rational interlocutor must give, assuming only his own earlier answers. Several features of Aristotle's methodology can be understood by means of the interrogative model, including the role of endoxa in it. Theoretically, there is also considerable leeway as to whether fallacies are conceived of as mistakes in questioning or as breaches of the rules that govern questioning games.  相似文献

Burgess' Attitude and Belief Scale, a measure of Ellis' irrational beliefs, was administered to a large sample of outpatients. All 13 subscales demonstrate very adequate internal consistency. A factor analysis yielded one factor accounting for 83% of the variance, which was labeled irrationality. Clients endorsed focused items more than overgeneralized items, self-referential items more than non-self-referential items, and preferential items more than irrational items. Clients also received higher scores on the irrational process of demandingness than they did on the irrational process of awfulizing, self-worth and low frustration tolerance. The results were consistent with new formulations in Rational-Emotive theory. Suggestions were made for the construction of measures of irrational beliefs.  相似文献

David Lewis, Stewart Cohen, and Keith DeRose have proposed that sentences of the form S knows P are indexical, and therefore differ in truth value from one context to another.1 On their indexical contextualism, the truth value of S knows P is determined by whether S meets the epistemic standards of the speakers context. I will not be concerned with relational forms of contextualism, according to which the truth value of S knows P is determined by the standards of the subject Ss context, regardless of the standards applying to the speaker making the knowledge claim. Relational contextualism is a form of normative relativism. Indexical contextualism is a semantic theory. When the subject is the speaker, as when S is the first person pronoun I, the two forms of contextualism coincide. But otherwise, they diverge. I critically examine the principal arguments for indexicalism, detail linguistic evidence against it, and suggest a pragmatic alternative.  相似文献
