Interdivisional Healthcare Committee: Speaking with One Voice on Cross-Cutting Issues in Health Care Psychology

One of the greatest challenges facing psychology in health settings today is the need for unification on critical issues of client access, workforce oversupply, education and training, and outcomes evalutions. Health care psychology specialty areas can no longer afford to operate in parallel. A unified vision is needed to overcome threats to the viability of the profession and to maximize the potential for achieving common goals. The current paper describes the formation of the Interdivisional Healthcare Committee (IHC), whose purpose is to establish a common agenda for promoting the professional, educational, and scientific goals of health care psychology. The IHC's initiative to develop new CPT codes for increasing access to psychologists' services in health care settings is outlined.     

Adolescent Health Care Training in Predoctoral Psychology Internship Programs: A Model of Interdisciplinary Training Needs in Health Care

This needs assessment of predoctoral psychology programs in a large southeastern state reveals that although many programs include adolescent health issues and often involve the psychiatry discipline in their curricula, many trainees do not have exposure to medicine, nutrition, or nursing disciplines. Opportunities for greater interdisciplinary adolescent health care training exist in psychology training programs in this southeastern state. Integrating such training into psychology programs may enhance psychology's ability to function as a vital component of adolescent health care teams within academic medical centers across the country.     

Culture and Community Psychology: Toward a Renewed and Reimagined Vision

Interest is growing in community psychology to look more closely at culture. Culture has resided in community psychology in its emphasis on context, ecology, and diversity, however we believe that the field will benefit from a more explicit focus on culture. We suggest a cultural approach that values the community's points of view and an understanding of shared and divergent meanings, goals, and norms within a theory of empowerment. Furthermore, we posit the importance of pluralistic, multi-method programs of research and action encompassing both idiographic and nomothetic approaches, and critical reflexivity of our roles and agendas. Culture can be further incorporated into all the branches and fibers of community psychology.     

The Future of Psychology in Academic Health Care Centers

A promising opportunity exists to continue psychology's move beyond its traditional mental health emphasis into a general health care discipline. Abundant empirical evidence supports that psychologists treat a wide array of physical disorders as well as psychological components of medical problems. The immediate issues confronting psychology's full emergence into the health care world are discussed and recommendations are offered to assure the success of this initiative. Self Psychology and End of Life Pastoral Care

This paper explores salient concepts of Heinz Kohut's self psychology in relation to pastoral caregiving with dying persons and family members. Examination of a principle case study and other relevant clinical examples illumines the discussion. It is argued that the principles of Kohut's psychology can critically inform pastoral theological reflection. Strengths and limitations are considered throughout the analysis, and gestures are made toward beneficial end of life pastoral theological practices for clients and practitioners.     

Health Psychology in Catalonia in 2000: Current View and Future Perspective

This report describes the current status of health psychology in the nation of Catalonia (Spain). Emphasis is placed upon autonomy and self-governance, which have progressively influenced the structure and functioning of the health care system and the professional colleges. The current university educational model of training and practice in health psychology is reviewed. The most important characteristics of professional psychologists are presented. A critical view of health psychology research in medical settings and future perspectives of health psychology in Catalonia are also discussed.     

Interfaith Relations in the United States: Toward a Multilevel Community Psychology Approach

Interest in religion within the field of community psychology has steadily emerged within the last three decades. This interest has focused almost exclusively on the social benefits of religion, glossing over the often‐contentious nature of religious life and the ways in which religious individuals and institutions can disrupt healthy human and community development. Considering the recent surge of interfaith conflicts and discriminatory practices targeting religious minorities in communities across the United States, it is imperative that community psychologists begin to examine relevant trends in interfaith relations and potential directions for action research and intervention. This paper serves as the beginning point of just such an examination, proposing a multilevel model for addressing the microsystemic, mesosystemic, and macrosystemic levels of interfaith phenomena. More specifically, I present interfaith contact, congregation‐based community partnerships, and theological belief systems as particularly relevant to interfaith community research and intervention. Finally, I detail an interfaith organization that embodies these dimensions of interfaith relations and provides a concrete example of how a multilevel action research model may be effectively employed. Behavioral Interventions and Families: A Medical Rehabilitation Perspective

Behavioral interventions are pertinent to many issues faced by medical rehabilitation patients and their families in late adulthood. Despite their utility in managing chronic illnesses and reducing problematic behaviors related to cognitive impairment, behavioral interventions can be difficult to implement and maintain in a family setting. Problems resulting from countercontrol, caregiver fatigue, conflicting priorities, and impact on family relationships must be anticipated and addressed. These issues are illustrated through references to pertinent research and case examples. Recommendations are offered to reduce caregiver stress and to minimize shifts in relationships when family caregivers act to alter a member's behavior.     

Health Care Reform and Psychologists in Medical Schools: Stress, Challenge, and Change

Psychologists from 45 academic health science centers throughout the United States and Canada gathered at Georgetown University for a national conference November 2–5, 1995 organized by the Association of Medical School Psychologists. This paper introduces the proceedings of that conference described in the next four articles in this issue of the Journal of Clinical Psychology in Medical Settings[Volume 4, number 1]. Papers prepared by the working groups focus upon clinical services, education and training, research, and governance and administration within the academic medical setting.     

Toward a Critical Community Psychological Perspective on Unemployment and Mental Health Research

Psychological research has established that unemployment causes widespread psychological distress and ill health in communities but, arguably, little of this research is truly community psychological. In this paper we sketch out a critical community psychological perspective and use it to contribute to understanding of the role of psychosocial aspects of income in the experience and mental health of employed and unemployed members of low-income families in a severely deprived community context; to the development of innovative participatory methodology, and to promote the interests of impoverished unemployed people through the research process as well as through the research outcome.     

Medical Crisis Counseling on a Pediatric Intensive Care Unit: Case Examples and Clinical Utility

The intensive care unit is one of the most stressful and demanding work sites of the pediatric psychologist. The rapid changes in health status that necessitate stays on the intensive care unit often constitute an emotional roller coaster ride for both family members and the staff caring for their child. The Medical Crisis Counseling (MCC) intervention model is a useful approach to providing psychological support in the pediatric intensive care setting. A representative year's worth of consultation requests from the multidisciplinary intensive care unit of a large urban pediatric hospital is described and discussed in the context of the MCC model.     

The Future of Psychology in Medicaid's Health Delivery System for People with Disabilities

State Medicaid programs are rapidly converting fee-for-service health delivery systems to managed care for people with disabilities. In theory, managed care models of health delivery will substantially improve the quality of care for people with disabilities, but in reality, few successful models exist. This period of transition holds both opportunities and challenges for psychologists in medical settings. Because Medicaid reforms for people with disabilities may herald similar reforms for both the public and the private sectors, psychology's response will determine its role in the future of health care delivery for this population. Changes in training and increased attention to outcomes research, innovations in practice, and advocacy will be the keys to success.
