The effects of racial and ethnic diversity on the detection and management of behavioral and emotional problems in pediatric primary care are addressed. Service access and utilization as a function of race and culture are initially examined. Important barriers within pediatric training including minimal emphases on behavioral training and the limited supply of minority physicians are underscored. The impact of ethnicity on the development and maintenance of the clinical relationship that is critical to accurately and sensitively identifying and managing behavioral and developmental problems is briefly addressed. Recommendations for further integration of racial/ethnic issues into mental health service provision within the pediatric primary care arena are presented.  相似文献

Recent research projects have looked for social innovations, i.e., people creating solutions outside the mainstream patterns of production and consumption. An analysis of these innovations indicates the emergence of a particular kind of service configuration—defined here as relational services—which requires intensive interpersonal relations to operate. Based on a comparative analysis between standard and relational services, we propose to the Service Design discipline an interpretative framework able to reinforce its ability to deal with the interpersonal relational qualities in services, indicating how these qualities can be understood and favored by design activities, as well as the limits of this design intervention. Martin Buber’s conceptual framework is presented as the main interpretative basis. Buber describes two ways of interacting (“I-Thou” and “I-It”). Relational services are those most favoring “I-Thou” interpersonal encounters.

A goal of short-term intensive family preservation services (IFPS) is to facilitate access to other services. However, service use following IFPS has rarely been studied. I describe the types of aftercare services that IFPS therapists recommended and use of these services in the two months following IFPS termination. Since families were using services that were not on the recommended aftercare plans, use of these services is also described. Primary caregivers of families who received family preservation through either child welfare or mental health were interviewed at two months after family preservation for the purpose of ascertaining service use. Different types of services were recommended and used based on whether families received IFPS through child welfare or mental health. However, there were also differences in the presenting problems and demographics between families in child welfare and mental health. Future research should include an independent assessment of need in order to determine the relationship between need and the types of services recommended and used. Even though families used services prior to IFPS, the findings indicated that IFPS facilitated use of new services. More research is needed on the process of how therapists decide what services to recommend and what they do to help families access services. It is also important to examine whether use of less restrictive services helps prevent out-of-home placements and the relationship between informal support and use of formal services.  相似文献

The main goal of this research is to explore the organizational climate perceived by administrative and healthcare personnel working in Spanish healthcare services, analyzing the differences according to their health specialization, sex, age and professional status. The sample was made up of 3,787 individuals working in the administrative and healthcare services of the Public Health System of the Principality of Asturias, 88.7 % were working in specialist care and 11.3 % in primary care. Mean age was 51.88 (standard deviation of 6.28); 79.9 % were women and 20.1 % men. The organizational climate was assessed with the CLIOR scale. The organizational climate perceived is moderately positive, with a global mean of 3.03 on a scale of 1 to 5 points. The differences are statistically significant (p < .01) according to specialty, age and profession. A better working climate is perceived in primary care than in specialist care, and among older as compared to younger workers. The results indicate that the working climate perceived by administration and services staff employees in the Spanish healthcare context is moderately positive, with a better perceived climate in primary care than in specialist care.  相似文献

Examined developmental theory and its relevance for the practice of clinical child psychology. Following a brief review of basic principles of developmental psychology and developmental psychopathology, implications of a developmental perspective are explored for the diagnosis, assessment, and treatment of childhood disorders. Although it is obvious that many developmental issues confront the clinical child psychologist and that we have learned much about translating developmental theory into clinical practice, we conclude we have a long way to go before we can assert that a true developmental-clinical child interface has been realized.  相似文献

A mixed-data inquiry and analysis of the current state-level comprehensive developmental guidance models in the United States is presented. The 24 states with current programmes are commended in their initial attempts at model development, especially in the personal/social developmental domain. The most significant gap across models is in attention to ethnic and cultural developmental issues. Current programmes are also lacking in defining how developmental components are integrated with one another. Authors of state programmes are encouraged to be more specific in the use of developmental language and constructs so that comprehensive developmental guidance programmes will address the needs of all students. A challenge is given to other nations to consider the developmental nature of their programmes.  相似文献

Social skills training (SST) has emerged as a frontline treatment approach for aggressive children and adolescents. The present review evaluates this sizable literature from a developmental–clinical perspective. More specifically, the review summarizes key developmental findings, assesses the status of existing efforts to integrate these developmental findings into clinical research, and discusses intervention implications. Summaries of developmental findings are divided into six major areas: age, gender, race, identification of intervention samples, social cognition, and peer group influences. The review indicates that efforts to incorporate developmental findings and principles into clinical research have fallen woefully short. Even the most fundamental developmental considerations were frequently overlooked. Despite these general limitations, the review highlights a number of noteworthy developmental–clinical integration attempts and concludes with a discussion of directions for future research.  相似文献

Summary

This paper describes a formative evaluation of a school-linked service delivery system for immigrant and low income families in a large suburban county in Maryland. Initially we discuss local needs that prompted the development of the program and we place problems with services for children and families with multiple needs in a broad context. Next we discuss the methods and results of the evaluation. Findings indicate the program provides a range of appropriate services to intended beneficiaries but that lack of infrastructure and articulated understandings among participating organizations present potential problems.  相似文献

Psychoanalysis in theory and clinical practice is a developmental domain. Psychoanalysts think about their patients from a developmental point of view. The analytic relationship promotes development in both analyst and patient. Two concepts central to this author’s developmental point of view are epigenetics—as used in biology and philosophy—and that of the analyst as “developmental object.” Optimally, the analyst as developmental object facilitates what Rita Tähkä terms the “developmental illusion,” which intersubjectively transforms psychic structures, enabling alternatives to the repetition compulsion. Two vignettes with adult patients illustrate how empathic intimacy in psychoanalysis with an emphasis on latency and toddler phases as reconstructed in adult analysis presaged psychic growth. Transference as a vehicle for a developmental history taking is also considered.  相似文献

Developmental ordering is a fundamental prediction of developmental theories and a central issue in developmental research. However, logically sound evidence of developmental ordering is difficult to obtain. This article analyzes the logical basis of testing developmental order hypotheses with categorical measures. Depending on whether saltatory (i.e., discrete) or continuous developmental changes are being assessed, the observed relationship between categorical measures yields very different types of information about developmental ordering. When change is continuous, the relationship between the measures does not confirm any one ordering hypothesis, but rather, disconfirms one or more hypotheses. Whether an underlying variable undergoes saltatory or continuous development has long been recognized as an important theoretical issue, but its impact on the interpretation of developmental ordering has not previously been explicated.  相似文献

This paper highlights some developmental characteristics of adult psychotherapy groups and the relationships among them. It argues that members of a therapy group will experience and have an opportunity to resolve within the group itself normal developmental conflicts of adult life such as learning to establish intimate relationships. In addition, members will reexperience unresolved developmental conflicts of childhood and adolescence that frequently prevent adequate resolution of adult developmental tasks, and have an opportunity to rework them. Lastly, because of the essential isomorphism of therapy groups, members' developmental conflicts will be experienced and dealt with when the group-qua-group is dealing with parallel developmental struggles in its own life.  相似文献

This paper is an effort to propose a moderatel optimistic future for personal care services. It is proposed that the "best available future" scenario for personal care servlces between now and the year 2019 is to concentrate upon "privatization" of service delivery on a small-scale basis. Government, in this model should be llimited largely to three roles: (1) source of "venture capital"; (2) regulation of service delivery; and (3) ,income maintenance for the poorest ,populations. In this future, the main burden of personal care services will be born by the private sector. Dramatic improvements in the financing and marketing of human services will be called for, including expanded private practice, technology-intensive services and new forms of private insurance.  相似文献

In a four-wave longitudinal study, 228 adolescents from seven age cohorts were investigated annually regarding the importance and subjective attainment of age-specific developmental tasks. Distal outcomes (educational trajectory and residential independence) were examined when the adolescents were 21 years old. The results of latent growth models (LGM) showed that there was no mean level change in the importance of developmental tasks, whereas perceived attainment of developmental tasks increased over time. In general, whereas the importance of developmental tasks was more impacted by family factors, the attainment of tasks was more influenced by individual factors. Adolescents with more parental support for autonomy had higher developmental aspirations. Adolescents with unemployed fathers showed greater increases in the importance of developmental tasks, which subsequently predicted a higher educational trajectory. Only a small proportion of adolescents perceived a high initial level of attainment of diverse tasks; for 85% of the sample, the typical growth pattern with respect to overall attainment of developmental tasks was a moderate initial level, which increased over time. Girls, early maturers, and first-born adolescents reported higher perceived attainment of developmental tasks.  相似文献

In this paper developmental guidance and existentialism are viewed as parallel but relatively independent modes of thought. The major theoretical dimensions of developmental guidance are summarized and a brief comparison between developmental guidance and existentialism is made. Certain aspects of existential thought are related to some principles of developmental guidance. Existentialism can provide an idiographic dimension to the fundamentally nomothetic theoretical orientation of developmental guidance by complementing an objective understanding of the individual with a subjective understanding.  相似文献

Research in developmental psychology requires sampling at different time points. Accurate depictions of developmental change provide a foundation for further empirical studies and theories about developmental mechanisms. However, overreliance on widely spaced sampling intervals in cross-sectional and longitudinal designs threatens the validity of the enterprise. This article discusses how to sample development in order to accurately discern the shape of developmental change. The ideal solution is daunting: to summarize behavior over 24-hour intervals and collect daily samples over the critical periods of change. We discuss the magnitude of errors due to undersampling, and the risks associated with oversampling. When daily sampling is not feasible, we offer suggestions for sampling methods that can provide preliminary reference points and provisional sketches of the general shape of a developmental trajectory. Denser sampling then can be applied strategically during periods of enhanced variability, inflections in the rate of developmental change, or in relation to key events or processes that may affect the course of change. Despite the challenges of dense repeated sampling, researchers must take seriously the problem of sampling on a developmental time scale if we are to know the true shape of developmental change.  相似文献

We examine children, childhood, and development from an evolutionary perspective. We begin by reviewing major assumptions of evolutionary–developmental psychology, including the integration of “soft” developmental systems theory with ideas from mainstream evolutionary psychology. We then discuss the concept of adaptive developmental plasticity and describe the core evolutionary concept of developmental programming and some of its applications to human development, as instantiated in life history theory and the theory of differential susceptibility to environmental influence. We then discuss the concept of adaptation from an evolutionary–developmental perspective, including ontogenetic and deferred adaptations, and examine the development of some adaptations of infancy and childhood from the domains of folk psychology and folk physics. We conclude that evolutionary theory can serve as a metatheory for developmental science.  相似文献
