To further advance assessment of patient-reported outcomes, the European Organisation of Research and Treatment of Cancer (EORTC) Quality of Life Group has developed computerized adaptive test (CAT) versions of all EORTC Quality of Life Core Questionnaire (QLQ-C30) scales/items. The aim of this study was to develop and evaluate an item bank for CAT measurement of insomnia (CAT-SL). In line with the EORTC guidelines, the developmental process comprised four phases: (I) defining the concept insomnia and literature search, (II) selection and formulation of new items, (III) pre-testing and (IV) field-testing, including psychometric analyses of the final item bank. In phase I, the literature search identified 155 items that were compatible with our conceptualisation of insomnia, including both quantity and quality of sleep. In phase II, following a multistep-approach, this number was reduced to 15 candidate items. Pre-testing of these items in cancer patients (phase III) resulted in an item list of 14 items, which were field-tested among 1094 patients in phase IV. Psychometric evaluations showed that eight items could be retained in a unidimensional model. The final item bank yielded greater measurement precision than the original QLQ-C30 insomnia item. It was estimated that administering two or more items from the insomnia item bank with CAT results in a saving in sample size between approximately 15–25%. The 8-item EORTC CAT-SL item bank facilitates precise and efficient measurement of insomnia as part of the EORTC CAT system of health-related quality life assessment in both clinical research and practice.

A series of experiments was performed to determine how the duration of a word spoken in a sentence is influenced by: (1) the grammatical category to which it belongs, and (2) the position of the word in a constituent. Experiment I contained Noun-Verb homophones (e.g. “I saw the coach…” — noun; “I saw him coach…” — verb), in sentences matched for phonetic environment and stress pattern. Results support the notion that Nouns are longer than Verbs in typical sentences. Experiment II, however, demonstrated that the duration of Noun-Verb homophones in matched clause-final position is approximately equal. Experiment III contained transitive and intransitive Verbs in matched sentences. The results of Experiments II and III provided support for a lengthening account based on constituent-final position, while ruling out an account based on a debatable deletion site. Results of Experiment IV generalized the account of constituent-final lengthening for Nouns and Verbs to additional major categories, by comparing the duration of the phrase-initial Adjective two with the phrase-final Adverb too. Finally, Experiment V tested the distinction between minor and major categories. Results demonstrated that the Preposition to is shorter than the Adjective two by approximately 50 percent. Taken in sum, these findings indicate that it is sufficient to make a binary distinction between major and minor categories for purposes of a theory of speech timing and speech synthesis. Durational effects traditionally ascribed to differences within the class of major categories can be accounted for solely in terms of constituent boundaries, already required in the theory to account for three other classes of phenomena.  相似文献

Cross-correlation between successive stresses and time intervals in periodic tapping was analyzed at 15 different frequencies. Three types of rhythmic pattern of stress and timing in the sequencing of taps were identified in their intrinsic regions of frequency above 2.7 Hz. The tapping sequences were temporally grouped into units of two taps in which relatively stressed taps at the top (type I) or end (type II) of the group were preceded or followed, respectively, by the longer time intervals, while type III was a rhythm with 4 meters. These three types transformed to each other reversibly with shift of the frequencies in a way such as ←(f1) type I←(f2) type II←(f3) type III, where critical frequencies were f1 = 2.7, f2 = 4.0 and f3 = 5.0 Hz. The changes of the rhythmic type with the frequencies are discussed in terms of phase transition in which structures of rhythm are reorganized in order to maintain stable control of sequential tapping in each of the three frequency regions.  相似文献

We examined the psychometric properties and factor structure of a Cambodian translation of the Anxiety Sensitivity Index (ASI) and an Augmented ASI (the ASI supplemented with a 9-item addendum that assesses additional Cambodian concerns about anxiety-related sensations). Both the ASI and the Augmented ASI distinguished among three diagnostic groups: highest score, PTSD with panic disorder (PP group); next, panic disorder without PTSD (P group); and then, other disorders than PTSD or panic disorder (O group). In the discriminant function analysis using the Augmented ASI, the best classificatory predictor (PP vs. P vs. O) was an Addendum item (“It scares me when I stand up and feel dizzy”). The principal component analysis (oblimin rotation) of the ASI yielded a 3-factor solution (I, Weak Heart Concerns; II, Control Concerns; III, Social Concerns) and of the Augmented ASI, a 4-factor solution (I, Weak Heart Concerns; II, Control Concerns; III, Wind Attack Concerns; IV, Social Concerns). The item clustering within the factor solution of both the ASI and Augmented ASI illustrates the role of cultural syndromes in generating fear of mental and bodily events.  相似文献

Four classes of possible mechanisms for short-term item recognition are distinguished: (I) pure list-search, (II) direct-access activation (or trace strength) discrimination, (III) mixtures (of I and II), and (IV) response-association. Manipulations of recency, particularly of negative probe items, provide critical tests between them. Two experiments are reported using Sternberg' s 1966 varied-set reaction time paradigm, coupled with procedures intended to minimize rehearsal and control the recency of probes and memory set items. RT and error rate were greater for negative probe items that had recently been presented than for items less recently presented, and this effect increased with positive set size. In contrast, positive RT was, except for the initial item, a decreasing function of recency (= serial position), and there was no additional effect of set size per se. A brief filled delay between list and probe increased positive RT but not the slope of the set size function. These and other findings appear to reject models of Classes I and IV and, while implying some direct discrimination of an item's recency, require modification of the models of Classes II and III. The implications are discussed with respect to the relation between the two versions of Sternberg's paradigm and also in connection with facilitatory “priming” effects in memory.  相似文献

Objective: The majority of psychological studies with organ transplant recipients have examined negative psychological effects. This study aimed to further investigate the positive effects of organ transplantation and to construct a specific measurement instrument.

Design: The initial pool of 14 items for the Positive Effects of Transplant Scale (PETS) was derived from organ recipient interviews. A cross-sectional postal study included 87 heart, 46 lung and 193 liver transplant recipients. The PETS was subjected to principal components analysis (PCA) using varimax rotation, and associations with other measures investigated.

Main outcome measures: PETS and an open-ended item about positive effects.

Results: Coding of the open-ended item revealed that the majority of recipients attributed positive life changes to the transplant experience. PCA of the PETS indicated three factors that accounted for 58.82% of the variance. The 12-item questionnaire assesses improvements in: (1) life philosophy, (2) gratitude and (3) health. The total PETS scores exhibited adequate internal consistency and validity.

Discussion: Most transplant patients report positive psychological effects, which suggests this may be an understudied area. The initial development of an assessment tool provides researchers and clinicians a way to assess the degree and nature of these life changes.  相似文献

ObjectivesMost activities involve co-activation of several muscles and epimuscular myofascial force transmission (EMFT) can affect their mechanics. This can be relevant for spastic muscles of cerebral palsy (CP) patients. Isometric spastic semitendinosus (ST) forces vs. knee angle (KA-FST) data were collected intra-operatively to test the following hypotheses: (i) Inter-antagonistic EMFT elevates FST, (ii) changes the shape of KA-FST characteristics, (iii) reduces the muscle’s joint range of force exertion (Range-FST) and (iv) combined inter-antagonistic and synergistic EMFT further changes those effects.Methods11 limbs of 6 patients with CP (mean (SD) = 7.7 (4.7) years; GMFCS levels = II–IV) were tested in 3 conditions from 120° to full extension: ST activated (I) exclusively, (II) simultaneously with an antagonist, and (III) with added activation of synergists.ResultsCondition II increased FST (e.g., peak force = 87.6 N (30.5 N)) significantly (by 33.6%), but condition III caused no further change. No condition changed the muscle’s wide Range-FST (100.7° (15.9°)) significantly. Therefore, only the first hypothesis was confirmed.ConclusionsCo-activating its antagonist elevates forces of activated spastic ST substantially, but does not change its joint range of force exertion. Added activation of its synergists causes no further effects. Therefore, EMFT effects in CP can be relevant and need to be tested in other knee flexors.  相似文献

Religious connectedness is common phenomenon in Saudi Arabia and adjacent Gulf countries. An observational case control study was designed, enrolling 180 adult patients to report the association between religious connectedness and health-related quality of life (HRQL) in people with and without diabetes and foot ulcers. Sixty diabetic patients with foot ulcers (Group I) were compared with sixty diabetic patients without foot ulcer (Group II) and sixty healthy subjects (Group III) for assessment of their HRQL by using SF-36 questionnaire. The effect of religious connectedness was assessed using intrinsic/extrinsic religious connectedness scale. HRQL was found to be significantly lower in Group I compared with Group II and III as well as in group II compared with group III (P < 0.001). Group I patients showed a poorer HRQL with increased severity, duration and multiplicity of foot ulcers. There was a strong positive relationship between religious connectedness and HRQL as indicated by a positive correlation between religious connectedness scale and mental, physical component summary scores (r = 0.66 and 0.59 respectively and P < 0.001). While quality of life is generally poor in people with diabetic foot ulcers, there exists a strong positive relationship between religious connectedness and higher HRQL. These findings may have implications on improving outcomes.  相似文献

The place of employment and the diversity of its relations to other roles within the life structure of midlife women were investigated. Three groups were compared: Group I (n = 44) had a continuous low level of involvement in employment from their early 20s to their early 40s; Group II (n = 20) changed from low to high involvement; and Group III (n = 32) maintained a continuous high level of involvement. Retrospective reports regarding subjects' commitment to a number of different roles and feelings during their early 30s and early 40s constituted the data. Findings indicated that: (1) women in Group I had a positive, confident view of their involvement in the traditional roles of mother, wife, and volunteer; (2) women in Group II, initially quite similar to those in Group I, restructured their role commitments by substituting involvement in employment for more traditional pursuits; and (3) women in Group III successfully integrated involvement in the multiple roles of worker, mother, wife, and volunteer. Implications for a more dynamic view of identity, which takes into account significant reorganization of the life structure after adolescence and variability among diverse groups of women, are discussed.  相似文献

ObjectivesWe assessed the effects of a cognitive-behavioral intervention on English field hockey players’ appraisals of organizational stressors, emotions, and performance satisfaction.DesignA concurrent, across-participants, multiple-baseline, single-case research design with a three months post-intervention follow-up.MethodFour high-level female field hockey players participated in a four phase intervention that lasted between 24 and 26 weeks: rapport-building and observation (phase I), baseline monitoring (phase II), educating the players and facilitating acquisition of a cognitive restructuring technique (phase III), and encouraging integration of the technique during sport performance (phase IV). Questionnaires and social validation were used to record the participants' appraisals, emotions, and performance satisfaction throughout the intervention. A three months post-intervention follow-up was conducted to assess the participants’ retention of the intervention effects.ResultsReduced threat and loss appraisals and elevated challenge appraisals were reported immediately after Phase III had been introduced. Pleasant emotions and performance satisfaction increased while unpleasant emotions decreased throughout the intervention. Social validation immediately post-intervention and at the end of the follow-up period indicated sustained adaptive changes in each of the outcome variables.ConclusionsCognitive restructuring represents a promising technique for optimizing high-level hockey players’ appraisals. Challenge appraisals and pleasant emotions appear to be linked with increased performance satisfaction and positive intervention effects can be retained for a period of three months post-intervention. Researchers should examine the effectiveness and efficacy of the cognitive restructuring technique with other populations to develop a robust evidence base for appraisal optimization in sport.  相似文献

Skinner (1938) found that rats given discrimination training (Phase I) and then reinforced to “satiation” for responses in the presence of the negative stimulus (S?) (Phase II), began to respond again when the positive stimulus (S+) was reintroduced (Phase III). Experiment I replicated Skinner's finding with pigeons, alternating S+ and S? presentations during Phase III. In Experiment II, Phase II was extended, and Phase III results were similar to those of Experiment I, demonstrating that the recovery of S+ responding could not be attributed to a lax Phase II satiation criterion. In Experiment III, a uniform schedule of reinforcement was maintained throughout the three phases, and results similar to those of Experiment I were found, indicating that renewed S+ responding was not due to the shift in schedule between phases. In Experiment IV, Phase I consisted of discrimination training with two positive stimuli (S+s and S+n), and Phase II consisted of reinforcement for responses in the presence of S? and S+s. During Phase III, significantly more responding was found to S+s and S+n than to S?, but no difference in responding was found between the two positive stimuli. In Experiment V, Phase I consisted of simple discrimination training, and during Phase II, responses in the presence of both S? and a novel stimulus (So) were reinforced. During Phase III, significantly more responding was found to S+ than to either S? or So, with no difference found between S? and So responding. Renewed responding to S+ during Phase III in the present experiments is best explained by behavioral contrast developed during Phase I.  相似文献

Recognition memory for lists of items was investigated in pigeons using a YES-NO recognition technique. Experiment I showed that increasing the exposure duration of the first item of a two-item list improved recognition for that item without impairing recognition of the second item. Experiment II showed that decreasing the inter-trial interval had no effect on correct YES responses but significantly increased the number of false YES responses. Experiment III showed that recognition for the last two items of a three-item list was no poorer than that for lists of only two items. Experiment IV showed that increasing the delay between presentation and test of a two-item list (from 0·25-1 s) had a more disruptive effect on recognition for the second than for the first item. The data from these four experiments support a model proposed by Roberts and Grant, according to which memory traces are independent, and decay as a negatively accelerated function of time. Experiments V, VI, and VII investigated recognition for lists of three, four, and five items, and found no evidence for a primacy effect, performance being a linear function of time since sample offset.  相似文献

Abstract

Fifty five people, either currently sick or having recovered from their illness, were recruited if they reported positive consequences of illness. They were questioned about their experiences of illness, 41 by semi-structured interview and 14 by open-ended questionnaire and responses were classified into 17 categories. The categories were similar but slightly more extensive than previous accounts of positive consequences reported in the literature. The content of the interviews and questionnaires was used to construct a 66 item questionnaire about positive consequences of illness which was then completed by 97 patients. A principal components analysis indicated a large first factor accounting for 27% of the variance. Endorsement of items varied between 87% and zero for chronic lung disease patients attending pulmonary rehabilitation. However, all patients endorsed at least one item and the median number of items endorsed was 31. Positive consequences of illness are highly varied and more common than often realised, and this has implications for the concept and measurement of quality of life.  相似文献

Small shifts in choice occur even without discussion, when individuals merely know each other's preference. This appears to support an interpersonal comparison explanation of group induced shifts in choice and to refute explanations based on persuasive argumentation. The present study demonstrates the contrary, that such effects are consistent with the persuasive-arguments formulation and are obtained under particular conditions specified only by the latter theory, to wit: Knowledge of other's choices is assumed to lead a person to think of reasons (arguments) others might have had for their choices-reasons which ordinarily would not come to mind without this knowledge. Such reasoning functions in the same way as persuasive argumentation during group discussion; it causes the person to persuade himself that an alternative course of action now has greater merit than the one he initially preferred. To test this analysis, an experiment was performed in which subjects responded to choice-dilemma items under three different conditions: Following their own choice (I) they learned what several others had chosen and then wrote arguments in support of alternatives given in that same item; (II) they learned what several others had chosen and then wrote arguments in support of alternatives given in a different item; and (III) they received no information about others' choices but merely wrote arguments on that item. As predicted, shifts in choice occurred only if the person knew what others chose and had an opportunity to think about the latter (condition I); they did not occur if an opportunity to think of others' choices was denied (condition II), nor if knowledge of others' choices was withheld (condition III). Content analysis of the arguments subjects produced in conditions I and II completely supported the hypothesis, as did analyses of responses to postexperimental questionnaire which directly asked the subjects about their feelings and thoughts upon learning what others had chosen.  相似文献

Experiencing unpredictability in the environment has a variety of negative outcomes. However, these are difficult to ascertain due to the lack of a psychometrically sound measure of unpredictability beliefs. This article summarizes the development of the Scale of Unpredictability Beliefs (SUB), which assesses perceptions about unpredictability in one's life, in other people, and in the world. In Study I, college students (N = 305) responded to 68 potential items as well as other scales. Exploratory factor analysis yielded three internally consistent subscales (Self, People, and World; 16 items total). Higher SUB scores correlated with more childhood family unpredictability, greater likelihood of parental alcohol abuse, stronger causal uncertainty, and lower self-efficacy. In Study II, a confirmatory factor analysis supported the three-factor solution (N = 186 college students). SUB scores correlated with personality, childhood family unpredictability, and control beliefs. In most instances the SUB predicted family unpredictability and control beliefs beyond existing unpredictability measures. Study III confirmed the factor structure and replicated family unpredictability associations in an adult sample (N = 483). This article provides preliminary support for this new multi-dimensional, self-report assessment of unpredictability beliefs, and ideas for future research are discussed.  相似文献

ABSTRACT

Transgender individuals have a gender identity or expression that is not congruent with their anatomy (Bullough, 2000). Limited empirical data are available regarding the mental health and general well-being of the transgender population. In addition, few standardized measures have been developed specifically for transgender individuals. The main purpose of this study was item and scale development of a measure of adjustment for transgender adults, as well as the assessment of the internal consistency and validity of the measure. Responses to the Transgender Adaptation and Integration Measure (TG AIM) were collected from a sample of 108 self-identified male-to-female transsexual adults. Standardized measures of quality of life and self-esteem were also administered to participants in order to assess construct-related validity. Factor analysis of the measure identified four factors: Gender-Related Fears, Psychosocial Impact of Gender Status, Coping and Gender Reorientation Efforts, and Gender Locus of Control. Internal consistency for each of the four factors ranged from r = .59 to r = .81. Psychosocial Impact of Gender Status and Gender-Related Fears were positively correlated with self-esteem and quality of life. Coping and Gender Reorientation Efforts were significantly correlated to both Psychosocial Impact of Gender Status and Gender-Related Fears. Preliminary psychometric analysis resulted in a 15 item, multidimensional measure with three reliable and valid subscales for use with male to female (MtF) transsexual adults. Further psychometric evaluation of the measure for use with other segments of the transgender population is needed.  相似文献

In all three studies described in this article, novices were trained to associate traits from the DSM-IV Cluster B personality disorder categories with four letters of the alphabet. Novices were also taught various gender associations (i.e., "base rate" information) with the categories (none, stereotype-consistent, or stereotype-inconsistent). Results showed that when no gender associations were taught (Study I), case gender did not affect ratings of case vignettes. When associations were consistent with social stereotypes (Study II), case gender influenced ratings in the expected direction. When associations were the opposite of social stereotypes (Study III), only the ratings for narcissistic and histrionic were affected. Across the three studies, these results suggested that subjects rated cases in accordance with known "base rates," but that the correspondence between base rates and stereotypic associations affected the consistency and magnitude of this base rate effect.  相似文献
