人工关节是指用生物相容性与机械性能良好的金属材料制成的一种类似人体骨关节的假体.近年的关节外科发展迅速,人工膝关节置换术每年都在改进,治疗效果已有明显的进步,各种形式的假体不断推出.随着生物医学模式向生物-心理-社会医学模式的转变,患者日益要求人工关节质量好、费用少、与术后生活质量并举.回顾膝关节假体发展的历史,剖析其各阶段的重大变革,骨科专业医务工作者从以下几个方面进行哲学思考:(1)人类对人工膝关节置换的认识受到主客观条件的限制,经过多次往复才得以完成到目前水平;(2)人工膝关节置换选择中既要理解它的共性原则,更要把握它的个性原则,才能达到人工与自然的协调;(3)最优化是人工膝关节置换发展中的最终目标.  相似文献

近年来,随着我国开展人工关节置换术,临床取得了较好的效果,为老年股骨颈骨折、股骨头无菌性坏死、成人先天性髋关节脱位等患者解除痛苦。随着人工髋关节置换术的广泛开展,因手术人员的技术原因,以及假体的长期使用,术后并发症也随之增多,翻修术也成为必然。针对我院30例人工髋关节术后翻修适应证选择及假体选择。谈一些治疗体会。  相似文献

探讨老年糖尿病患者人工髋关节置换术围手术期的治疗方法。对2000年1月~2006年3月54例糖尿病患者行人工髋关节置换术进行术后随访,利用临床关节功能及放射学检查进行分析。54例手术患者血糖控制满意且顺利度过围手术期。按Harris评分标准,术后髋关节功能优良率分别为人工股骨头置换组74%(14/19),人工全髋关节置换组91%(32/35)。未发现感染及假体松动现象等并发症。老年糖尿病患者如果有髋关节置换手术指征,没有严重的并发疾病,在重视围手术期处理的情况下,可以行人工髋关节置换术,且疗效肯定。  相似文献

长柄假体置换治疗高龄粉碎性粗隆间骨折的随访研究   总被引:1,自引:0,他引:1  
为了探讨长柄假体置换治疗高龄粉碎性粗隆间骨折的短期疗效,我们对31例符合要求的研究对象进行了平均22个月的随访,指标包括Charnley及Harris评分以及X-ray平片,结果最近一次X—ray观察3例出现异位骨化,无一例假体松动,Charnley分级Ⅳ级及以上90.0%,Harris评分优良率88.3%,因此长柄假体置换是治疗高龄粉碎性粗隆间骨折的理想选择之一。  相似文献

了解该院人工全髋关节置换术开展情况,明确其住院费用的结构和影响住院费用的主要因素,为合理控制住院费用提供参考依据。本研究收集2012年~2013年人工全髋关节置换术患者的住院病历信息,分析住院费用的变化趋势和结构比例,并分析其主要影响因素。结果显示该院住院费用总体呈上升趋势,且结构不尽合理,人工假体的使用是影响住院费用的主要因素。应该控制人工假体价格虚高,合理使用不同类型的人工假体,积极探索按病种付费模式,从而控制住院费用增长。  相似文献

股骨头坏死、髋关节脱位是骨科常见病,随着人们生活模式的改变发病率有所上升。对该病的成因、流行病学、病理转归、临床治疗策略与髋关节严重病损后功能重建等问题,学术界存在很大争议。作者提出从生物进化的角度、用自然重建理念做指导,应用髋关节分离牵拉术治疗股骨头坏死、骨盆补充截骨术重建青少年严重病损的髋关节功能人、组合性矫形手术治疗年长儿先天性髋关节脱位,获得了优良效果。因此呼吁:用自然重建理念治疗青少年股骨头坏死、髋关节脱位,降低人工假体置换对此类疾病的手术指证。  相似文献

Young infants produce a variety of spontaneous arm and leg movements in the first few months of life. Coordination of leg joints has been extensively investigated, whereas arm joint coordination has mainly been investigated in the sitting position in the context of early reaching and grasping. The current study investigated arm and leg joint coordination of movements produced in the supine position in 10 fullterm infants aged 6, 12 and 18 weeks. Longitudinal comparisons within limbs (intralimb) as well as between limbs (interlimb, ipsilateral and contralateral) were made as well as an exploration of differences in the development for boys and girls. The relationship between the joint angles was examined by measuring pair-wise cross-correlation functions for the angular displacement curves of the leg (hip, knee and ankle) and arm (shoulder, elbow and wrist) joints of both the right and left side. Both the arms and legs were found to follow a similar pattern of intralimb coordination, although the leg joints were more tightly coupled than the arm joints, particularly the proximal with the middle joint. In support of earlier findings, differences in the development of the right and left side were identified. In addition, gender differences in joint coordination were found for both intralimb and interlimb coordination. This contrasts with the view that gender differences in motor development may be primarily a result of environmental influences.  相似文献

Learning in complex tasks is usually conceived as the problem of mastering the multiple and redundant degrees of freedom of the system. To reduce control requirements, two different strategies are conceivable. The first one consists of a "freezing-freeing" process for most articular joints to reduce the number of active biomechanical degrees of freedom to be managed. The second strategy consists of introducing rigid couplings between the oscillators building the system. In this case, learning implies the dissolution of initial couplings and the emergence of new, more task-specific couplings. The goal of our study was to analyze the spontaneous coordination of beginners and its development in a complex balancing task on a stabilometer, and to examine the emergence of these two strategies. Our results showed that beginners were characterized by strong couplings between the joints of the lower limbs. During learning, new and more task-specific couplings emerged that reflected a new organization of the trunk and a decoupling of some joints of the lower limbs that were initially coupled during the first few trials.  相似文献

The explicit timing demands were lower in continuous drawing in comparison with the discontinuous task. The authors manipulated limb dynamics by changing the number of joints involved, with line drawing requiring fewer joints than circle drawing. Results showed that young children had high temporal variability in discontinuous circling but not in other conditions. The authors argue that both explicit timing and dynamic complexity of limb control may be determinants of temporal consistency and may thus play an important role in the development of drawing and writing skills in children.  相似文献

Spontaneous, supine kicking in newborn (2- and 4-week-old) infants is described in terms of its temporal structure, interjoint coordination, and muscle activation characteristics as measured by surface electromyography. Phasic kick movements shoed a constrained temporal organization in the movement, but not the pause phases. Hip, knee, and ankle joints moved in temporal and spatial synchrony, and all three joints showed a rhythmical or periodic organization over time. EMGs revealed antagonist coactivation at the initiation of the flexor movement, but little or not extensor activity. The dorsal muscles, the gastrocnemius and hamstrings, showed less activity than the ventral pair, tibialis anterior and quadriceps. Burst and onset-to-peak durations were also constrained. As a result of neural mechanisms and biomechanical forces, newborn leg movements are structured muscle synergies. This organization has implications both for newborn functioning and for later development.  相似文献
