BackgroundOlder adults are at greater risk of falls while descending stairs. Cognitive deficits can further influence one's ability and mild cognitive impairments (MCI) specifically affect visual attention and dual tasking behavior. The present study aimed at comparing the attentional costs at different points during the approach to and descent of a staircase between older adults with and without MCI.MethodsEleven older adults with MCI and twenty-three healthy older individuals without cognitive impairments were recruited. Neuropsychological tests were carried out. In addition, participants approached and descended a 5-step staircase while a simultaneous visual Stroop dual-task was randomly introduced during the approach, transition or steady state descent phases across trials. Three-dimensional kinematics and accuracy on the Stroop task were analyzed and dual task costs were calculated.ResultsThe MCI group showed deficits for visuo-spatial attention, memory and multi-tasking abilities, as well as balance and decreased confidence for falls efficacy, but not for daily activity scores. Despite such changes, this group of community-dwelling individuals with MCI presented a functional capacity to descend stairs even during divided visual attention. However, there were subtle, but significant, group differences for movement fluidity and performance on the simultaneous cognitive task, particularly during the approach and transition to descent phases. The MCI group also tended to descend slower while using the handrails more than healthy older adults.ConclusionThe present cohort of community-dwelling older adults with MCI were functional, but appeared to prioritize locomotor demands over the simultaneous cognitive task in a possible “posture first” strategy to descend stairs. The present findings should be considered for developing more ecologically based clinical assessments of mobility deficits following cognitive impairments, with the approach and transition phases during stair descent as key points of focus.

Mild memory impairment was detected in 28% of a sample of healthy community-dwelling older adults using the delayed recall trial of a word list learning task. Statistical analysis revealed that individuals with memory impairment also demonstrated relative deficits on other measures of memory, and tests of executive function, processing speed and global cognition, as measured by the CERAD and CogState batteries and CANTAB paired associate learning task. These relative deficits cannot be explained by age-related changes, education, intelligence, mood, health-related factors, or the individuals' ApoEepsilon4 status. Memory-impaired individuals (n = 30) did not recognize the extent of their memory and cognitive difficulties beyond the general complaints expressed by normal elderly (n = 77) within the study and their apparent difficulties did not appear to impact on their participation in life activities. These findings suggest it is unlikely that the memory and cognitive difficulties demonstrated by individuals with mild memory impairment reflect normal aging. Rather it is possible that such impairment may signal early neurodegenerative processes worthy of further investigation.

This study investigated the effects of prior knowledge on recognition memory in patients with a mild Alzheimer's disease. Normal older adults and mildly demented patients were presented with dated and contemporary famous faces with name tags and were asked to generate unique statements about each person. Results indicated that both groups generated more statements about the dated than about the contemporary figures. Most important, both groups performed better with the dated than with the contemporary faces in an unexpected episodic face recognition task. This pattern of results suggests that both groups (a) possess more knowledge of dated than of contemporary famous individuals and (b) are able to utilize prior knowledge to enhance episodic remembering. Viewing these results in light of other recent work, it is concluded that differences between normal old and mildly demented individuals in the ability to utilize cognitive support for remembering may be differences in degree.

While the effect of listening to music on cognitive abilities is highly debated, studies reporting an enhancing effect of music in elderly populations appear to be more consistent. In this study, the effects of listening to music on attention in groups of cognitively normal older adults and those with mild cognitive impairment were considered. Participants were exposed to both a music and silence condition, and after each condition performed Digit Span and Coding tasks which require attention for maximal performance. The hypothesis that listening to music, compared to a silence condition, enhances performance was not supported for either group. Various explanations for these findings are considered.

While cognitive changes in aging and neurodegenerative disease have been widely studied, language changes in these populations are less well understood. Inflecting novel words in a language with complex inflectional paradigms provides a good opportunity to observe how language processes change in normal and abnormal aging. Studies of language acquisition suggest that children inflect novel words based on their phonological similarity to real words they already know. It is unclear whether speakers continue to use the same strategy when encountering novel words throughout the lifespan or whether adult speakers apply symbolic rules. We administered a simple speech elicitation task involving Finnish-conforming pseudo-words and real Finnish words to healthy older adults, individuals with mild cognitive impairment, and individuals with Alzheimer's disease (AD) to investigate inflectional choices in these groups and how linguistic variables and disease severity predict inflection patterns. Phonological resemblance of novel words to both a regular and an irregular inflectional type, as well as bigram frequency of the novel words, significantly influenced participants' inflectional choices for novel words among the healthy elderly group and people with AD. The results support theories of inflection by phonological analogy (single-route models) and contradict theories advocating for formal symbolic rules (dual-route models).

Collaborative problem solving occurs in situations in which two or more individuals cooperate in appraising, representing, and solving a variety of cognitive tasks. Collaborative groups are the context for much everyday cognitive activity in adulthood. Collaboration has been explored as a means through which older adults may maintain high levels of performance, perhaps compensating for individual-level cognitive and neurological decline. This study explored the effects of collaboration (group size) and adult age on solving both fixed- and unrestricted-alternatives 20 Questions tasks. Younger (M=24.3 years) and older (M=67.9 years) adults were randomly assigned to one of three homogeneous group size conditions: individuals, dyads, and tetrads. Results indicated some dissociation between individual-level performance (poorer for older adults) and collaborative performance (better for older adults). For the fixed-alternatives task, older adults produced more of the relatively inefficient hypothesis-scanning questions than did younger adults. In contrast, older collaborative groups produced more of the efficient constraint-seeking questions than hhpothesis-scanning questions, and an amount equivalent to that of younger adults. Overall performance for the difficult unrestricted-alternatives task was less efficient for both younger and older adults. The roles of task type, group characteristics, and adult age are discussed.

This paper investigates the role of cognitive resources in everyday functioning, comparing visually impaired, hearing impaired, and sensory unimpaired older adults. According to arguments that cognitive resources are of increased importance and a greater awareness of cognitive restrictions exists among sensory impaired individuals, in particular among visually impaired individuals, we hypothesized differential relationships between resources and outcomes when comparing sensory impaired and sensory unimpaired older adults. Findings are based on samples of 121 visually impaired, 116 hearing impaired, and 150 sensory unimpaired older adults (M = 82 years). Results from a sample of 43 dual sensory impaired older adults are reported for comparison. Assessment relied on established instruments (e.g., WAIS-R, ADL/IADL). Structural equation modeling showed that cognitive resources and behavior-related everyday functioning were more strongly related in the sensory impaired groups as compared to the sensory unimpaired group. Cognitive resources and evaluation of everyday functioning were significantly linked only among the sensory impaired groups. When medical condition was controlled for, these effects persisted. It is concluded that both cognitive training as well as psychosocial support may serve as important additions to classic vision and hearing loss rehabilitation.

The authors compared patients with mild cognitive impairment with healthy older adults and young control participants in a free recall test in order to locate potential qualitative differences in normal and pathological memory decline. Analysis with an extended multitrial version of W. H. Batchelder and D. M. Riefer's (1980) pair-clustering model revealed globally decelerated learning and an additional retrieval deficit in patients with mild cognitive impairment but not in healthy older adults. Results thus suggest differences in memory decline between normal and pathological aging that may be useful for the detection of risk groups for dementia, and they illustrate the value of model-based disentangling of processes and of multitrial tests for early detection of dementia.

Non-amnestic mild cognitive impairment (naMCI) is one of the clinical subtypes of mild cognitive impairment (MCI). However, the characteristics of memory deficits in naMCI as assessed by clinical neuropsychological evaluations are not clear. In this study, a battery of neuropsychological tests was administered to 122 cognitively normal controls (NC), 133 amnestic mild cognitive impairment (aMCI) patients, and 72 naMCI patients. The results showed that in individuals with naMCI, episodic memory, and other cognitive domains were impaired. The Prospective Memory Test (PMT) event-based prospective memory (EBPM), the Symbol Digit Modalities Test (SDMT) Accidental Memory, Stick test (ST) visuoconstructional memory, and ST Working Memory were impaired, yet did not reach the level of aMCI. Semantic memory was affected to a degree comparable with aMCI. Some functions like Auditory Verbal Learning Test (AVLT) recognition, and Judgment of Confidence (JOC) were maintained, as well as PMT Time-Based Prospective Memory (TBPM). This study verified that memory impairment among individuals with naMCI was mainly in memory functions mediated by the frontotemporal cortex.

ABSTRACT

Thirty younger (Mean Age = 19.9 years) and 20 older adults (Mean Age = 74.7 years) performed Physical and Name Identity letter-matching tasks (matches were either within or between hemispheres) to study age-related changes in 1) the efficiency with which the two hemispheres interact with each other and 2) hemispheric asymmetry. In order to determine whether age-related effects were associated with differences in cognitive resources, the same individuals completed a set of memory span tasks. Performance on the letter-matching tasks indicated that the costs of interhemispheric collaboration were greater for older than for younger participants. However, within the older group, the advantage of spreading processing across both hemispheres increased as memory span decreased, suggesting that older individuals who are challenged by cognitive complexity are more likely to show increased benefits from between-hemisphere processing than individuals who are not so challenged. There was also an overall left visual field/right hemisphere advantage for the younger but not for the older group, suggesting greater age-related declines in right- than left-hemisphere function.

Memory and language tests are usually used to differentiate healthy elderly individuals and individuals with cognitive impairment (CI). In the latter case, there are usually no tests to assess grammatical comprehension. The aim of this paper is to explore the differences in grammatical comprehension between healthy older adults and older adults with CI, identifying the sentences that best discriminate these groups, as well as the underlying dimensions that are most relevant to the individuals. Participants were 71 elderly people, divided into two groups according to their performance on the Spanish version of the Mini Mental State Exam: normal and CI. All were given a comprehension test that included 12 kinds of sentence structures. The CI group performed significantly worse than healthy elderly group. The greatest differences were observed in the sentences adjusted to canonical order in Spanish. The sentences with higher weights in the discriminant function were the ones adjusted to the one-proposition canonical order. Propositional density was the most salient dimension in both groups. Individuals with CI had difficulty assigning thematic roles to constituents, even when it could be done by following a strategy based on the linear order of the roles.

The present study compared the effects of age of acquisition (AoA) on object naming across groups of older individuals with cognitive impairments, healthy older controls, and young healthy controls. All participants named a set of 80 pictures, within which both AoA and frequency were manipulated orthogonally. Early-acquired objects were named faster than late-acquired objects across all groups. Response time also declined with age and with cognitive impairment between the groups. The effect of AoA differed across groups, with AoA effects being largest for the older group with cognitive impairments and smallest for the young control group. The present study adds strength to the suggestion that AoA of picture names is one of the factors that influence survival or loss of memories in dementia and cognitive decline, and this could therefore be used as a potential screening test for cognitive impairment disorders in the future.

Cumulative exposure to glucocorticoid hormones (GC) over the lifespan has been associated with cognitive impairment and may contribute to physical and cognitive degeneration in aging. The objective of the present study was to examine whether the pattern of cognitive deficits in patients with Cushing’s syndrome (CS), a disorder characterized by chronic exposure to elevated levels of glucocorticoids (GC), is similar to that observed in older individuals. Ten subjects with CS were compared to sex-, age-, and education-matched healthy controls and older subjects (age of CS subjects + 15 yr). All participants were administered tests to assess attention, visuospatial processing, learning and memory, reasoning, concept formation and verbal fluency. MANCOVAs with depression scores as covariate and polynomial contrasts revealed that the age-matched control group performed better than the CS and older subject groups in visual target detection, trail making test, stroop task, digit symbol substitution, block design, object assembly, visual reproduction, spatial memory and similarities. The CS and older subjects performed similarly on these tasks. Further, a principal component analysis revealed two significant factors, representing general cognitive function and verbal memory explaining 39.9% and 10.0% of the variance, respectively. Additional MANCOVAs with depression as a covariate revealed that CS and older control subjects showed impaired performance on general cognitive function compared to age-matched controls. These results suggest that hypersecretion of GCs has “aging-like” effects on cognitive performance in individuals with CS.

The effects of aerobic exercise training in a sample of 85 older adults were investigated. Ss were assigned randomly to either an aerobic exercise group, a nonaerobic exercise (yoga) group, or a waiting-list control group. Following 16 weeks of the group-specific protocol, all of the older Ss received 16 weeks of aerobic exercise training. The older adults demonstrated a significant increase in aerobic capacity (cardiorespiratory fitness). Performance on reaction-time tests of attention and memory retrieval was slower for the older adults than for a comparison group of 24 young adults, and there was no improvement in the older adults' performance on these tests as a function of aerobic exercise training. Results suggest that exercise-related changes in older adults' cognitive performance are due either to extended periods of training or to cohort differences between physically active and sedentary individuals.

The purpose of this study was to evaluate the association between functional mobility and mild cognitive impairment in older adults. A total of 800 older adults were recruited (653 controls and 147 subjects with MCI [88 subjects with aMCI and 59 subjects with naMCI]). Motor performance was measured with the Timed Up and Go test (TUG). The demographic factors associated with MCI were: age (OR = 1.05; 95% CI: 1.01–1.09) and the level of education (OR = 0.73; 95% CI: 0.68–0.79). An independent clinical factor associated with MCI was the TUG (OR = 1.14; 95% CI: 1.03–1.27). In the aMCI group, the relation between the TUG and cognitive status occurred (OR = 1.15; 95% CI: 1.02–1.31), whereas in the naMCI group this relationship was not observed. There is an association between cognitive dysfunction and impaired motor performance in older adults with MCI.

Complex inhibitory control, defined as the ability to inhibit a planned or ongoing action, was assessed in a sample of individuals with a history of mild head injury, case-matched with normal control subjects for age and gender. This central act of control was assessed using a modification of the stop-signal paradigm. The group with mild head injury took longer to inhibit their on going action and reported more accidents than the normal control subjects. The group that reported having had a mild head injury did not differ in terms of their go reaction time, number of correct responses, handedness, education level, or reported learning disabilities. Limitations of this design and directions for future research are discussed.

In this study, we compared executive functioning in 32 mild cognitive impairment (MCI) individuals with that of normally aging controls. Cognitive planning tests (Trail Making, Porteus Maze Test, verbal fluency tests) show a group difference favoring the normal controls, but tests for inhibition of prepotent responses (no-go accuracy, two measures of the Stroop effect, and negative priming) failed to uncover a significant group difference. The results indicate that there is no general executive control function impairment in MCI; rather, the deficits found are compatible with the hypothesis that MCI is an accelerated form of normal aging.

The authors investigated the psychometric properties of the Beck Anxiety Inventory (BAI) in a sample of 75 older generalized anxiety disorder (GAD) patients and a comparison group of 32 older adults without significant psychopathology. Internal consistency was above .80, and the BAI showed evidence of convergent validity in both groups. Evidence for discriminant validity with respect to measures of depression was weaker. Two items, fearing the worst and nervousness, correctly distinguished 86.5% of patients with GAD and 93.8% of the normal controls. Medical comorbidity was associated with somatic but not cognitive anxiety symptoms in the normal older sample. Overall, results indicate the limitations of the BAI in assessing anxiety symptoms in older adults and suggest the need for use of an instrument focusing on cognitive aspects of anxiety.
