Humour has occasionally been regarded as a valuable tool in counselling and psychotherapy. The specific uses of humour and its potential as a coping tactic are rarely described. The therapeutic use of humour is accordingly examined in the light of reversal theory (Apter, 1982a). It is suggested that the use of humour needs to be related to the motivational task implicit in the therapeutic interaction.  相似文献

This paper discusses the contribution of family systems theory and therapy to mainstream alcohol treatment programmes. Systemic psychotherapy is integrated with group analytic psychotherapy in a couples and family therapy service within a community alcohol service (CAS) which uses the Prochaska and Diclemente model of stages of change. Problems and solutions in putting these complex ideas into practice are discussed, with specific reference to issues of engagement. An example of our work with a couple is offered to illustrate our weave of theory and practice.  相似文献

This article discusses the challenge of contextualism to developmental theory as articulated by Lewis (1997), as well as the challenge of current changes in life contexts for developmentally oriented counseling practice. Counseling practice refers to activities of psychologists and counselors in relation to theory, research, and intervention, and in relation to both career and psychotherapy domains of practice. It is hoped that a consideration of both career and psychotherapy practice will contribute to dialogue and integration across these domains. In the second part of the paper, a metaperspective for counseling practice, conceived as a response to this challenge, is presented. It consists of a conception of life contexts inclusive of work and relationships across both public and private domains of life and posits a generic developmental process goal of effective functioning as relevant to both career and psychotherapy domains of counseling practice.  相似文献

There is now vast and multi-faceted empirical literature on the impact of relational dynamics in psychotherapy, and relationality is a central theme within contemporary psychoanalytic theories. Yet, there is virtually no literature addressing the relational dynamics around outpatient psychotherapy that influence the wider therapeutic alliance, that is the relational dynamics that unfold between clients and administrative office staff in making initial contacts and appointments, completing paperwork, negotiating payment, discussing insurance coverage, working through crisis calls, and the myriad of other forms of interaction which serve to form the relational ecology of psychotherapy. Milieu therapy or the use of supportive and structured therapeutic environments has been given significant attention in inpatient settings; however, administrative relational factors or administrative staff characteristics have seemingly been ignored in outpatient psychotherapy and psychoanalysis literatures. Many outpatient clinical practice settings involve multiple levels and dimensions of relationality between clients, administrative personnel, and therapists which shape the potential for secure continuous containment and various forms of attachment to psychotherapeutic space. In this paper, we describe several clinical illustrations of relational dynamics between clients and office staff which influence therapeutic alliance and outline the contours of our emerging idea of administrative hospitality based on relational psychoanalysis, attachment theory, symbolic anthropology, and stigma research. In doing so, we draw on our collective experience working in several outpatient clinics and insights from our current context in an urban outpatient training clinic which emphasises psychodynamic practice. Our goal is not a comprehensive analysis of the relational ecology of psychotherapy but to offer an initial exploratory account of some of the administrative relational dynamics in and around outpatient psychotherapy which go beyond the therapist-client dyad. The anthropological concept of liminality is connected to relational psychoanalytic theories to describe the important dynamics of transitional space in an overall process of relational transformation.  相似文献

SUMMARY

This paper is concerned with the relationship between psychotherapy and general psychiatry. This relationship is examined from historical, clinical and theoretical perspectives. Psychotherapy is an integral part of psychiatric practice imparting to it a means of participating in, as well as observing, the patient's subjective experiences. Mental illnesses, whatever their nature have fundamental elements in common. This common ground is most apparent when mental illnesses are conceptualised in psychological terms. Such an approach in no way undervalues the importance of the hereditary and somatic influences which contribute to the predisposition to mental illnesses. Viewed in this psychobiological way it is not difficult to discern what is common to neuroses, psychoses and disturbances of the personality. The clinical phenomena provide the basis for a developmental theory which portrays the essential unity of mental illnesses. The paper concludes with a discussion of why psychotherapy is being gradually divorced from clinical psychiatry. The dangers which this separation hold for the clinical tradition in psychiatry are emphasised.  相似文献

Empirical evidence supports the commonly held belief that empathy facilitates psychotherapy outcomes. Complete analysis of the literature identified only six studies of empathy and counseling outcomes. Contrary to expectations, empathy does not appear important in counseling. This article discusses reasons for this finding as well as implications for counseling practice, theory, and research.  相似文献

Pastoral care givers are called to provide care and counseling to parishioners that are suffering in many ways. A core question is what does a pastor rely on to provide this care? Pastors usually call on models of psychotherapy to inform their care giving. A framework of theological anthropology, cosmology, soteriology and harmatiology is developed for analyzing the worldviews inherent in models of psychotherapy. The specific models of Adler’s Individual Psychology, Narrative therapy, Horney’s personality theory, and Intersubjective theory are used to demonstrate the application of this framework. This paper reviews the literature as it relates to psychotherapy and creativity. The focus is on the creative process and the role of the therapist as “artist” in the practice of couple therapy. The theory on which the paper is based in problem-centered with influences from the writing of Satir, humanistic psychology and psychodynamic psychotherapy. The somewhat crude question of whether we need neuropsychotherapy means more precisely whether its neuroscientific perspective can be useful for the psychotherapeutic practice. To answer this question the traditional scientific approaches do not completely suffice. The ??empirical-technical approach?? of quantitative psychotherapy research is of no service here as neuropsychotherapy is only its continuation. Within the framework of this methodology it is not possible to critically reflect on the relevance of neuroscientific research. Such a reflection is, however, the prerequisite for an evaluation of the benefits of neuropsychotherapy. The question about the meaning of neurobiological processes thus leads to the field of epistemology. This ??theoretical approach?? is nonetheless only helpful within limits. It provides an important background of reflection, however it needs to remain abstract compared to psychotherapeutic practice because it does not integrate the specific goals of such practice. What is presented here instead is an ??approach of pragmatic reflection??. This reflection departs from a general purpose of psychotherapy from which the value of neuropsychotherapy can be appropriately estimated. After this clarification the approach of pragmatic reflection can be preliminarily demonstrated with concrete applications of neuroscience in psychotherapy. What is shown hereby is that neuropsychotherapy represents a very specific practice of argumention and action, which implements a logic into psychotherapy that does not fit its contents and goals.  相似文献

This article describes an approach to counselling and psychotherapy which complements dominant discourse approaches to clinical evaluation based on New Public Management systems which underpin the Improved Access to Psychological Therapies scheme within the field of therapy. It is based on an approach to research and practice development which I will call practitioner microphenomena research. In order to demonstrate the method, I will examine an extract from a single case study of a client with major depressive disorder from the point of view of intersubjective theory and a little-known approach to therapy called anthroposophic psychotherapy based on the work of Rudolf Steiner. I will show how the two clinical methods are well suited to examining the microphenomena of practice and can be integrated into a coherent whole. A central feature of the account will be the anthroposophical view about the central importance of our individuality or ‘I’ which has similarities with the Jungian self but is still distinct from it.  相似文献

This brief contribution argues for the importance of a social justice perspective in counselling and psychotherapy, particularly within the current international socio‐political context. Much has been said about social justice over recent years in the fields of psychology, psychotherapy and counselling. Professionals and academics have expressed some concerns about what it might actually mean, or how difficult it might be in practice to engage with this perspective. In this paper, the activist phrase “the personal is political” (Hanisch, 1970) is used to illustrate the way in which a social justice approach to counselling and psychotherapy does not need to be complicated, but rather a foregrounding of a particular understanding of well‐being and the people we work with. Following this, some concrete and straightforward suggestions for how therapists might begin to act on their social justice values are made.  相似文献

In this paper we draw on feminist and post-colonial theory to situate an account of a six-month therapy between a black, Asian woman therapist and a woman patient of mixed parentage, within contemporary Britain. We draw on this analysis to further the debates between psychotherapeutics and social theory, challenging each in three ways. First, in relation to the literature on culture, class, gender and sexuality in psychotherapy, we indicate how these enter into the therapeutic relation in distinct and intersecting ways, which, second, challenge the separation between inner and outer worlds frequently maintained within psychotherapy circles. Third, in terms of the correlative challenge psychotherapeutic perspectives pose for social theorists and activists, we highlight how the therapist's constructive and explicit use of her power and socio-cultural position was central to the progress of this client's therapy. Finally, we reflect on the broader questions around the theorizing of culture and gender within psychotherapy, including the available positions and representations of black therapists as well as black patients.  相似文献

This paper presents an argument against A D Smith’s Direct Realist theory of perception, which attempts to defend Direct Realism against the argument from illusion by appealing to conscious perceptual states that are structured by the perceptual constancies. Smith’s contention is that the immediate objects of perceptual awareness are characterised by these constancies, which removes any difficulty there may be in identifying them with the external, or normal, objects of awareness. It is here argued that Smith’s theory does not provide an adequate defence of Direct Realism because it does not adequately deal with the difficulties posed by the possibility of perceptual illusion. It is argued that there remain possible illusory experiences where the immediate objects of awareness, which in Smith’s account are those characterised by perceptual constancies, cannot be identified with the external objects of awareness, contrary to Direct Realism. A further argument is offered to extend this conclusion to all non-illusory cases, by adapting an argument of Smith’s own for the generalising step of the Argument from Illusion. The result is that Smith’s theory does not provide an adequate Direct Realist account of the possibility of perceptual illusion.  相似文献

The present paper discusses the phenomenological approach to art-therapy and suggests an operational terminology for its main theoretical concepts. First we present the general assumptions that underline art therapy. Definitions, leading approaches, and the therapeutic effect of artwork are considered. Next we indicate the need for relevant and particular theories of art therapy. Phenomenological theory is examined as a case in point because of its popularity among art therapists. Despite its philosophical appeal, however, we find phenomenological theory lacking the operational terminology that is essential to form a coherent and distinct school of therapy. In the present paper, we extract from phenomenological theory operational principles, which can be followed and applied by art therapists. In this analysis, we take into consideration Betensky's pioneer attempt to create the bridge between phenomenological theory as applied in verbal psychotherapy and art therapy. We end by discussing the urgent need to operationalize other verbal psychotherapeutic theories so they are relevant for art-therapy practice and may help create greater similarities among art therapists of the same school of psychotherapy.  相似文献

The aim of this paper is to provide epistemic reasons for investigating the notions of informal rigour and informal provability. I argue that the standard view of mathematical proof and rigour yields an implausible account of mathematical knowledge, and falls short of explaining the success of mathematical practice. I conclude that careful consideration of mathematical practice urges us to pursue a theory of informal provability.  相似文献

This commentary on six papers about assimilative integration considers the place assimilative integration, as defined by Messer, ought to have in the discussion on psychotherapy integration. After briefly summarizing the contribution of each paper, with particular attention to whether the paper takes a position on the end point of psychotherapy integration, the author compares the relative benefits of assimilative integration to those of a unified grand theory. He argues that assimilative integration, as an end point, is preferable to a unified grand theory because it has a more generative influence on both theory evolution and clinical practice.  相似文献
