Little research has focused on the ethical issues around using social media for HIV prevention in low- and middle-income countries, such as Peru. This study surveyed participants from the HOPE social media HIV intervention in Peru to assess their experiences and perceptions of ethical issues in the study and the impact of age on their experiences and perceptions. This study found that, compared to younger participants, older participants were more likely to express higher levels of understanding of the consent form and trust that other participants were real. Older participants also reported being less likely to benefit from learning about their HIV status. Findings suggest that age plays a role in participants’ experiences in a social-media-based HIV intervention. 相似文献

In this essay I explore the heuristic value of the concept of ethics of complexity, chaos, and contingency by applying its framework to the analysis of the HIV/AIDS pandemic. Everyday human moral choices are outcomes of a moral impulse, and such an impulse is grounded in moral competence shaped by moral literacy. This literacy is constructed on the basis of a body of knowledge of culture, social context, environment, and the universe. It also includes the knowledge of religions and religious and secular ethical codes. I also distinguish between the social and cultural aspects of ethical systems. Both societies and cultures provide resources and constraints for the development of literacy and competence. An intentionally developed multifaith and multidisciplinary coalition may help us move away from various forms of social speciation and toward sociological mindfulness. This could help us remake the world into one that has more courage to care. 相似文献

Connect to Protect (C2P), a 10‐year community mobilization effort, pursued the dual aims of creating communities competent to address youth's HIV‐related risks and removing structural barriers to youth health. We used Community Coalition Action Theory (CCAT) to examine the perceived contributions and accomplishments of 14 C2P coalitions. We interviewed 318 key informants, including youth and community leaders, to identify the features of coalitions’ context and operation that facilitated and undermined their ability to achieve structural change and build communities’ capability to manage their local adolescent HIV epidemic effectively. We coded the interviews using an a priori coding scheme informed by CCAT and scholarship on AIDS‐competent communities. We found community mobilization efforts like C2P can contribute to addressing the structural factors that promote HIV‐risk among youth and to community development. We describe how coalition leadership, collaborative synergy, capacity building, and local community context influence coalitions’ ability to successfully implement HIV‐related structural change, demonstrating empirical support for many of CCAT's propositions. We discuss implications for how community mobilization efforts might succeed in laying the foundation for an AIDS‐competent community. 相似文献

In this paper we discuss our conceptualisation of a 'health-enabling social environment', and some of the strategies we are currently using to build social contexts most likely to support effective HIV/AIDS management in southern Africa. In developing these ideas, we draw on our on-going collaboration with residents of Entabeni, a remote rural community in South Africa where 43% of pregnant women are HIV positive. The aim of this collaboration is to facilitate contextual changes that will enable more effective community-led HIV/AIDS management in an isolated area where people have little or no access to formal health or welfare support, and where HIV/AIDS is heavily stigmatised. We give an account of the three phases of collaboration to date. These include research; the dissemination of findings and community consultation about the way forward; preliminary project activities (skills training for volunteer health workers; partnership building and a youth rally) as a way of illustrating what we believe are six key strategies for facilitating the development of 'AIDS-competent' communities: building knowledge and basic skills; creating social spaces for dialogue and critical thinking; promoting a sense of local ownership of the problem and incentives for action; emphasising community strengths and resources; mobilising existing formal and informal local networks; and building partnerships between marginalized communities and more powerful outside actors and agencies, locally, nationally and internationally. We discuss some of the triumphs and trials of this work, concluding with a discussion of the need to set realistic goals when working at the community level in highly conservative patriarchal communities to tackle problems which may be shaped by economic and political processes over which local people have little control. 相似文献

The multidimensional model of broaching behavior (MMBB) is introduced as a heuristic that addresses multicultural counseling and social justice competence. The MMBB includes humanistic skills that allow counselors to broach clients’ racial, ethnic, and cultural (REC) concerns in an effort to develop strong therapeutic alliances and alleviate distress. This article provides illustrative examples of the 4 MMBB dimensions: intracounseling, intraindividual, intra-REC, and inter-REC. Background
The importance of addressing social determinants of mental health in therapy is well-documented. However, there appears to be limited research on what this actually involves in practice and how best to prepare therapists to work with individuals experiencing adverse social determinants.Aims
The aim of this review was to draw implications for practice and training from reviewing research on addressing social determinants of mental health in therapy.Methodology
A qualitative systematic review of the research on addressing social determinants of mental health in therapy was conducted, extracting papers from eight databases: Cochrane Library, ASSIA, CINAHL Plus, PsycInfo, Psychoanalytic Electronic Publishing, Science Direct, Scopus and Web of Science. Out of 127 papers found, five met the eligibility criteria and were quality-assessed and analysed thematically.Findings
The thematic synthesis generated three main themes: “the therapeutic relationship,” “adapting to client context” and “community as a resource,” with eight subthemes.Conclusion
This thematic synthesis highlights the importance of the therapeutic relationship and adapting mental health support to client frame of reference and context to address social determinants of mental health in therapy. It emphasises community as a helpful resource for informing training and therapeutic interventions, as well as a significant resource for people experiencing adverse social determinants. 相似文献7.

The ambivalent response of many black churches to current social issues has caused some scholars to question the centrality of black churches within African-American communities. Using a nationally representative sample of black congregations, this study engages the debate about the institutional centrality of black churches by focusing on their response to HIV/AIDS. Although many congregational studies treat black churches as a monolithic whole, this analysis identifies heterogeneity among black churches that shapes their responsiveness to social issues. Contrary to prior claims, a congregation's liberal-conservative ideological orientation does not significantly affect its likelihood of having an HIV/AIDS program. Beyond assessing churches’ internal characteristics, this study uses institutional theory to analyze churches as open systems that can be influenced by their surrounding environment. It demonstrates that externally engaged congregations are significantly more likely to have a program. These results indicate that black churches maintain institutional centrality by engaging their external environment. 相似文献

Data collected as part of a psychosocial study of gay and bisexual men's experiences of living with HIV infection as a chronic illness were examined to investigate the psychological impact of the perceived availability of illness-related support and negative illness-related network interactions in a sample of men from this population. The sample was comprised of 144 HIV-infected non-Hispanic white, African American, and Puerto Rican men living in the New York City metropolitan area. Analyses found evidence of a conjoint (interactive) effect between perceived support and negative network interactions. There was no evidence of either perceived availability of illness-related network support buffering or negative illness-related network interactions amplifying the effect of HIV/AIDS-related physical symptomatology on depressive symptomatology. 相似文献

Gay men living with HIV/AIDS (GMLHA) in China face the dual discrimination from stigmatization of HIV/AIDS and the marginalization of sexual orientation. Online social communities have become an increasingly important channel for them to seek and share health information. This study aims to examine the actual impact of online social support exchange on GMLHA's health self-management in China. In this study, results of a questionnaire survey conducted in China's Weibo community (N = 303) showed that offering online social support can positively predict both physical and psychological health self-management. Moreover, receiving online social support in the GMLHA online community can enhance the HIV/AIDS patient's mental health self-management but does not forecast an improvement in the physical health self-management level. This study examined the relationships among multicultural competency, task-oriented coping, spirituality, political involvement, and experiences of discrimination on students’ engagement in social justice advocacy in a sample of 412 clinical, counseling, and school psychology doctoral students. Specifically, this study examined the mediating role of political involvement and task-oriented coping between the predictor variables and social justice advocacy. The results showed that the relationship between experiencing discrimination and social justice advocacy was only present when participants reported more political involvement and more task-oriented coping. The same was true for the relationship between spirituality and social justice advocacy; this relationship was only present when going through task-oriented coping. Further while there was a relationship between students’ multicultural knowledge and social justice advocacy, it was partly accounted for by political involvement. Similarly, the relationship between multicultural awareness and social justice advocacy was partly accounted for by task-oriented coping. Implications and recommendations for future research are addressed. 相似文献

HIV testing and counseling expends considerable HIV prevention resources and offers great opportunities for HIV risk reduction.
Individuals who are at risk for HIV and have not been HIV tested are the focus of current targeted testing campaigns and yet
persons who are repeatedly tested for HIV often continue engaging in high-risk practices. This study examined HIV testing,
risk behaviors, and other medical diagnostic testing practices of men (N = 231) and women (N = 86) attending an inner-city sexually transmitted infections (STI) clinic. Results showed that 75 (23%) participants had
not yet been tested for HIV, 45 (14%) had been tested once, and 197 (63%) had been tested two or more times. Patients that
had not been tested and those who were repeatedly tested were similar in their risk behaviors; both demonstrated significantly
greater risks for HIV than persons tested just once, although repeat testers were more likely to have had a past STI. HIV
testing history was minimally associated with other medical testing and health protective practices, such as testicular self-examination,
mammography, and having had PAP tests. Results support targeting high-risk untested persons for HIV testing and suggest an
urgent need for interventions to reduce risk behaviors among STI clinic patients who repeatedly test for HIV. 相似文献

This article offers a reflective review of 12 articles and 4 brief reports included in this special issue of the International Journal of Psychology in Africa with the theme, ‘HIV/AIDS, education and childhood in the African context’. Muthukrishna and Ramsuran (2007) have emphasised that the consequences of HIV/AIDS can be far-reaching for children and young people resulting in unequal life chances. It is thus fitting that this special issue should be edited by them in KwaZulu-Natal, South Africa, in the country (and province) with the highest levels of incidence of HIV/AIDS in Africa. This study explored infant feeding practice meanings of HIV-positive mothers. Informants were eight black South African mothers (age range 18–29 years) enrolled in a mother-to-child transmission (MTCT) prevention programme. In-depth interviews on factors that influence exclusive breast-feeding were conducted with the mothers. Thematic analysis of the data yielded meanings around practices of knowledge contestation with health care providers and family, work demands, and personal preferences. Moreover, successful breast-feeding by mothers with HIV status may depend on addressing contradictory messages by health workers on infant feeding and community and family pressure to mix-feed the infant children. 相似文献

The following is a commentary on “Ethical Considerations for Psychologists Providing Treatment for Arab Americans” (Cho, this issue). Cho (this issue) brings to light important issues on providing ethical treatment with the Arab American community. However, there is currently no formal mechanism to identify the number of Americans of Arab descent, thus impacting the ability of researchers to study psychological treatment with Arab Americans. In addition, many Arab Americans are refugees with exposure to trauma, highlighting the need for understanding the role of culture in trauma when working with this community. Implications of these issues pertinent to the Arab American community and the need for psychologists to act in advocacy roles are discussed. 相似文献

AIDS is a debilitating and fatal disease that was first identified as an infectious disease syndrome in the 1970s. The discovery of a nearly universally fatal infectious and rapidly spreading disease in the post–antibiotics era created apprehension in the medical community and alarm in the general population. Questions about how patients should be handled in medical and nonmedical settings resulted in the ostracizing of many AIDS patients and inappropriate patient management. Scientific investigation into modes of disease transmission and control helped to shape the management of AIDS patient care in such a way that ethical and protective practices could be developed. Theoretical research asserts that social justice advocacy (SJA) represents an extension of multicultural competence (MC). However, limited empirical literature examines if and how SJA manifests and is developed as an aspect of trainee MC. The current study used path modeling to examine direct and indirect links between aspects of training environment and trainee MC and SJA for a sample of 197 trainees embedded in counseling-oriented training programs. MC demonstrated the strongest positive association with SJA. In addition, multicultural training environment, but not social justice training environment, influenced SJA indirectly through MC. Results are encouraging of a differentiation between multicultural and social justice training emphases, with practical implications for training programs seeking to enhance and foster both MC and SJA among their trainees. 相似文献
