心理咨询与心理治疗效果的关键在于心理咨询与心理治疗师自身的特质,而非理论技巧。因此,心理咨询与心理治疗师在心理咨询与治疗的过程中,会非常直接、深刻和迅速地影响当事人的生活。通过对我国心理咨询师健康发展之要素的探讨,提出心理咨询师的培养应逐步走向正规专业教育和训练途径;同时关注心理咨询师自己的个人成长,健全督导制度是预防心理咨询师的职业耗竭,也是我国心理咨询业健康发展的根本保障。

从两种咨询模式看我国心理咨询师的培养

本文分析厂我国大陆目前开展的两种类型的心理咨询,即心理健康咨询和医学心理咨询,心理健康咨询遵循发展和教育模式。主要服务对象是有生活适应问题的来访者,其根本目的是促进他们健康成长和增强适应应对能力。医学心理咨询遵循生物-心理-社会医学模式,其主要服务对象是患有各种非精神病性精神障碍的病人,其主要目的是将病人治愈。作者从两种咨询模式出发,就目前我国心理咨询师和心理治疗师的培养和资格认证提出了一些问题和粗浅的看法。     

Medical crisis counseling: A new service delivery model

The diagnosis or exacerbation of a chronic illness, the aftermath of a serious accident, or worries about future in the context of illness or debilitating injury are all examples of medical crises that pose significant coping challenges. Too often, traditional approaches to psychotherapy have been unable to respond the most urgent needs of people confronting such crises. Medical crisis counseling is a specialized approach to addressing the needs of individuals and families confronted by the difficulties of coping with losses or changes, as well as the challenge of living with long-term illness. This paper describes the medical crisis counseling model in contrast with other more traditional intervention approaches.

随着卫生部《关于实行病人选医生促进医疗机构内部改革的意见》文件的下发,病人选医生已经成为加快医疗卫生机构改革的一个重大突破;具体分析它在深化医疗卫生机构改革过程中实现了几个方面的重大突破：一是病人选医生是建立新型的医患关系的重大突破;二是确立“以病人为中心”的服务模式的重大突破;三是建立一种新型的评价体系的重大突破;四是建立新型医疗劳动个人收入分配制度的重大突破。     

Genetic Counseling: A Transnational Perspective

Although the basic goal and components of genetic counseling appears to be the same across the globe, judged by my experiences there are significant differences in the provision of genetic counseling services in Australasia (Australia and New Zealand) and India. There is poor recognition of the professional status of a genetic counselor in India at present. This may be partly because genetic counseling itself is a relatively new discipline within the medical field in India, although some types of genetic services and research have been conducted since 1960s. In this paper, I aim to provide insight from my personal transnational experiences.     

Iceland—Genetic Counseling Services

This brief report aims to give an overview of the history and current status of clinical genetics services in Iceland and specific genetic counseling considerations for Iceland’s population. Presently, there are two part time medical geneticists and one full time genetic counselor with an MSc education from Cardiff, within the Department of Genetic and Molecular Medicine, based in Iceland’s only tertiary healthcare facility, Landspitali, the National University Hospital. An oncologist (20 %) also contributes to the cancer genetic counseling service. In addition, a pediatric medical geneticist has a 25 % appointment at the Children’s Hospital. No other health care organization offers genetic counseling, and there are no private genetic counseling services.     

Toward professionalism or voluntarism in pastoral care

In its modern evolution as a subprofession, pastoral counseling has increasingly developed professional criteria to determine who can and who cannot legitimately do pastoral counseling. This trend moves pastoral counseling toward the medical profession and away from the life and mission of the institutional church. As pastoral counseling seeks a legitimacy in close association with the medical profession it runs the risk of losing touch with the institutional church—the voluntary associations of Christians that have always been the ultimate source of legitimacy for the ministry in America. The present challenge facing the field of pastoral care is to shift attention from professional specialization outside the church to an effort within the church to train the laity to be effective agents of pastoral care.This article was first presented in the Seminary Series Lectures, The Graduate Seminary of Phillips University, Enid, Oklahoma, during the spring of 1975.     

对“四一三”医保模式破解双向转诊困境的思考

双向转诊困境成为制约社区卫生服务和医疗保障体系发展的关键问题。引入“四一三”医保模式，分析了该模式破解双向转诊困境的可能性，有助于优化城市卫生资源配置、调整医疗机构利益分配格局，有助于双向转诊制度的建立，有助于医疗信息的交流，以及风险分析。建议鼓励包括“四一三”医保模式在内的各种保障制度的发展。     

How special are the specialties? Workplace settings in counseling and clinical psychology in the United States

How special are the specialties? Although clinical and counseling psychology each have distinctive origins, past research suggests their potential convergence across time. In a survey of 5666 clinical and counseling psychologists, the similarities and differences between their workplace settings were examined during early-, mid-, and late-career phases to explore the distinctiveness of the two specialties. Overall, clinical and counseling psychologists reported markedly similar workplace settings. However, some significant differences remained; a greater proportion of counseling psychologists reported working in counseling centers, while a greater proportion of clinical psychologists reported working in medical settings. In addition, during late-career, substantially more counseling and clinical psychologists worked in independent practice contexts than in community mental health centers, medical settings, academia, or university counseling centers. Findings are discussed in relation to the ongoing distinctiveness of the two specialties and the implications of this for training and service in the field of professional psychology.     

Genetic Counseling Services and Development of Training Programs in Malaysia

Genetic counseling service is urgently required in developing countries. In Malaysia, the first medical genetic service was introduced in 1994 at one of the main teaching hospitals in Kuala Lumpur. Two decades later, the medical genetic services have improved with the availability of genetic counseling, genetic testing and diagnosis, for both paediatric conditions and adult-onset inherited conditions, at four main centers of medical genetic services in Malaysia. Prenatal diagnosis services and assisted reproductive technologies are available at tertiary centres and private medical facilities. Positive developments include governmental recognition of Clinical Genetics as a subspecialty, increased funding for genetics services, development of medical ethics guidelines, and establishment of support groups. However, the country lacked qualified genetic counselors. Proposals were presented to policy-makers to develop genetic counseling courses. Challenges encountered included limited resources and public awareness, ethical dilemmas such as religious and social issues and inadequate genetic health professionals especially genetic counselors.
