Next generation sequencing (NGS) is dramatically increasing the number of clinically available genetic tests and thus the number of patients in which such testing may be indicated. The complex nature and volume of the reported results requires professional interpretation of the testing in order to translate and synthesize the meaning and potential benefit to patients, and genetic counselors are uniquely suited to provide this service. The increased need for genetic counselors in this role, coupled with the time required and a limited number of trained and available counselors presents a challenge to current models for making genetic testing available to patients and their healthcare providers effectively and efficiently. The employment of genetic counselors at genetic/genomic laboratories is one model to expand the resources for providing this service. In this article, we briefly review the advent of NGS and its clinical applications, examine the core skills of genetic counselors and delineate the expanding roles and responsibilities of laboratory-based genetic counselors. We also propose changes to the genetic counseling training program curriculum to account for the increasing opportunities for genetic counselors to contribute and thrive within genetic testing laboratories.  相似文献

To address the need for research regarding online counseling, the authors explore the experiences of 4 female and 2 male Caucasian counseling students who facilitated 5 chat‐based online counseling sessions. Conducting semistructured interviews based in grounded theory methods, the authors discuss technological barriers, counseling without visual and verbal cues, online counseling techniques, counseling from home, and factors related to the pace of the sessions. They suggest that online counseling requires specific technical, ethical, and clinical considerations.  相似文献

Although adjustment after trauma is often positively associated with meaning, some studies challenge this connection (Bonanno, Memory, 21(1), 150–156, 2013; Silver and Updegraff 2013). In this article we elaborate on the relation between existential meaning and resilience. First, we conceptualize existential meaning—searching for and finding meaning in life—in terms of “orienting in moral space”, using the philosophical ideas of Taylor (1989), the psychological meaning-making model of Park (Psychological Bulletin, 136(2), 257–301, 2010), and existential theory. We argue that orienting systems in moral space are “believable visions of the good”. We then search recent literature on resilience—in particular literature in which the connection with meaning is challenged—for indications of a connection with existential meaning. We conclude that resilience necessarily comprises a “moral dimension” that is an adaptive process of (eventually) finding meaning in life. Finally, we discuss implications for the role that pastoral counselors, as professionals in the domain of existential meaning, may play in promoting resilience in organizations where employees regularly face existential issues like violence, suffering, and death.  相似文献

Couple counseling is widely practiced by Christian counselors, but there are almost no empirical data investigating it. The present study presents data from a national web-based survey of counselors from the American Association of Christian Counselors (AACC). We describe the nature of Christian couple counseling, as well as counselors' religiousness and their attitudes toward integrating religion and spirituality into couple counseling. Christian couple counselors were highly religious and their personal religiosity affected their attitude toward incorporating religion in counseling. There were differences between professional, pastoral, and lay counselors suggesting that each be treated separately rather than be lumped generically as Christian couple counseling.  相似文献

A major question in labour market research is the extent to which discrimination in employments causes the disadvantages experienced by children of immigrants. This article contributes to the debate by utilising a correspondence test study in which pairs of equivalent résumés and cover letters—one with a Pakistani name and one with a Norwegian name—were sent in response to 900 job openings in the greater Oslo area. The results show that applicants with Norwegian names on average are 25 % more likely to receive a call back for a job interview than equally qualified applicants with Pakistani names. More refined analyses demonstrate that the effect of ethnic background on employment probabilities is larger among men than women and larger in the private sector than in the public sector, and important variations among the occupations included in the study are revealed. In an effort to separate the potentially conflating effects of gender and sector, all applications to gender-segregated occupations were removed from the analyses. Interestingly, the gender differences disappear when exclusively analysing discrimination in gender-integrated occupations by sector. In gender-integrated occupations in the private sector, the gender difference in fact is reversed, indicating that women with minority background are treated less favourably than are minority men in the private sector. These results suggest that the intersection of gender, ethnicity, and sector should be scrutinised more carefully in future field experiments.  相似文献

Literature on the counseling needs of special populations typically has focused on the inadequate treatment those populations receive. Counselor education programs have responded by developing courses that target specific populations such as women, minorities, or disabled people. There are serious drawbacks, however, to this proliferation of specialized courses. In this article the authors discuss those drawbacks and argue that counselor education programs should include an integrated course that facilitates student self-awareness in conjunction with practical experience with a variety of clients.  相似文献

The well-being of relocated, male Florida Annual Conference United Methodist clergy and clergy spouses (N = 124) was compared with that of nonrelocated male United Methodist clergy and clergy spouses (N = 153). Results revealed no significant differences in well-being between relocators and nonrelocators. Clergy spouses exhibited significantly lower well-being than did clergy. Perceptions of relocation, stress level, and coping resources were significant predictors of clergy well-being. However, only stress level and coping resources predicted spouse well-being. Qualitative analysis of participants' responses revealed that pastoral counselors should address issues such as grief, powerlessness, loneliness, and clergy family reluctance to seek counseling services.  相似文献

Innovations in testing emerging from the competency assessment movement offer counselors new capabilities in helping their clients to understand aspects of themselves and their problems, as well as to establish directions for development and improvement efforts. New types of tests and measures sample actual behavior more closely than testing instruments previously used: The characteristics they examine are closely linked to performance in a wide variety of jobs, and therefore provide increased focus of assessment on life outcomes. With this new degree of specificity and criterion referencing, implications for counseling, training, and development efforts emerge more clearly than with other forms of testing.  相似文献

Outcomes in the field of genetic counseling have not been well-defined or categorized, despite pressures to provide evidence-based measures in all areas of healthcare. This study describes a process to elucidate and categorize a wide-ranging set of outcomes as characterized by diverse groups of practicing genetic counselors. Semi-structured focus groups were conducted at the National Society of Genetic Counselors 2013 NSGC Annual Education Conference during an educational breakout session. A general inductive qualitative research approach was utilized to code focus group notes, categorize them into themes, and compare them across specialty groups. A total of 107 individuals participated in 14 focus groups, consisting of specialists in cancer (n = 20), general genetics (n = 40), prenatal genetics (n = 11), and “other” (n = 36). Of the twelve genetic counseling outcomes themes identified, the most common across focus groups included: 1) appropriateness of testing and accuracy of results interpretation; 2) psychosocial outcomes; 3) adherence to or receipt of appropriate medical management; and 4) patient and provider knowledge. Data assessed by specialty demonstrated similarities in outcomes themes, suggesting that a common set of genetic counseling outcomes would likely be appropriate to cover the majority of needs for the profession. Results can serve as a platform from which to build a more well-defined and comprehensive set of outcomes.  相似文献
