When trauma enters into the reality of the analyst and of the analysand, when it attacks the setting, what becomes of the analyst’s role? How can transformations be brought about? With reference to three clinical situations, the author attempts to explore how the articulation between transference and countertransference – the inter‐relation – structures the situation; the analyst must remain in his or her role as analyst through managing to create and to reflect upon the clinical aspects of that situation when faced with the unpredictability of what war brings in its wake. It then becomes possible to see how the work of the negative can be confined to the outer limits of the setting.  相似文献

Psychoanalysis recruits the power of the spoken word to modify the subject's relationship with his or her own unconscious psychic processes. It helps the analysand to reclaim for his or her words the psychic integrity that was lost or never achieved due to the power of defensive dissociation and repression. The psychoanalytic dialogue and the working through mediated by it lead to the elaboration of self-narratives and interpretive understandings, which contribute to the transformation of the subject's self-experience. Such transformation is conditioned by earlier integration of experiences of satisfaction in the context of bodily dialogues and speech with primary objects.  相似文献

The author views the analytic enterprise as centrally involving an effort on the part of the analyst to track the dialectical movement of individual subjectivity (of analyst and analysand) and intersubjectivity (the jointly created unconscious life of the analytic pair--the analytic third). In Part I of this paper, the author discusses clinical material in which he relies heavily on his reverie experiences to recognize and verbally symbolize what is occurring in the analytic relationship at an unconscious level. In Part II, the author conceives of projective identification as a form of the analytic third in which the individual subjectivities of analyst and analysand are subjugated to a co-created third subject of analysis. Successful analytic work involves a superseding of the subjugating third by means of mutual recognition of analyst and analysand as separate subjects and a reap-propriation of their (transformed) individual subjectivities.  相似文献

In this paper the author examines her own use of language as a psychoanalyst and asks: what is the best way to help analysands to find the words to express not only what they are thinking but also what they are feeling and experiencing? In common with other psychoanalysts, the author has observed that each of us simultaneously utilises both advanced psychic mechanisms that are accessible to symbolism and more archaic ones, which are less so. However, she draws a distinction between people who are able to tolerate the perception of their own heterogeneity, even if it is sometimes a source of suffering, and those whom she terms ‘heterogeneous patients’. Patients in the latter category, whose lack of internal cohesion causes them anxiety, are afraid of losing their sense of identity. The author asks how we can understand their language and how we should speak to them. She uses several clinical examples to demonstrate that ‘heterogeneous patients’ need to be touched with a language that does not confine itself to imparting thoughts verbally but also conveys feelings and the sensations that accompany those feelings. It is also an ‘incarnated’ language because the words pronounced by the analyst can awaken, or reawaken, bodily fantasies in the patient. These words may enable him to find an emotional meaning in forgotten sensory or bodily experiences, which may then become a starting point for his work of thinking and of symbolisation.  相似文献

The therapeutic relationship frequently comes under a strain during the course of an analysis. At these times, the analysand perceives and reacts to the analyst as if the analyst were a critical or seductive archaic object. When this occurs, the analyst has the choice of interpreting the unconscious source of the analysand's non-therapeutic reactions or of interacting with the analysand in a manner which re-establishes and potentially strengthens the therapeutic relationship. Interpretations during the early phases of an analysis are unlikely to mitigate the strain until the therapeutic relationship has become stronger and until considerable reality testing has promoted a context which allows the analysand to understand and to tolerate her or his unconscious wishes.  相似文献

The author describes a group of phenomena that can affect the analyst and become the motivating force that pushes the analyst to break confidentiality. He describes the inevitable dialectic of symmetry and asymmetry in the psychoanalytic dyad during the analytic process, and asserts that, every time the analyst is in symmetry with the emerging material of the analysand, identification will occur. This identification with the material offered by the analysand activates the system of ideals and produces a value judgment. The author explores the anxiety, the loneliness, the narcissistic deficiencies and the symptom of the analyst as phenomena that can force the analyst to rupture the promise of confidentiality given in good faith to the analysand at the beginning of the treatment.  相似文献

Children with attention-defi cit/hyperactivity disorder (ADHD) and disorder of attention, motility control, and perception (DAMP) are often sensitive to the analyst's interventions. This is not always due to the literal import of the intervention. The children sometimes react as if the words were dangerous concrete objects, which they must physically fend off. The author traces this phenomenon to the child's unstable internal situation. A bad, un-containing internal object is easily awakened and threatens to expel the analyst's words independently of their content. This results in violent clinical situations. Infant research and psychoanalytic work with infants and mothers evince how a complex semiotic process develops between mother and baby. The prerequisite for this process to get started and maintained is a secure external object, which gradually is internalized. Findings from developmental research and clinical infant work are used to illuminate analytic work with children with ADHD and DAMP. Vignettes demonstrate how important it is for the analyst to phrase interpretations after having gauged the state of the analysand's internal object as well as his/her own countertransference. If this is overlooked, the psychoanalytic dialogue easily capsizes. The author provides some technical recommendations on the psychoanalysis of these children. As part of the theoretical discussion he raises the general question of how the representations, which the baby forms in interaction with the mother, and the analysand forms in interaction with the analyst, should be classifi ed. Rather than dividing them into bipartite thing- or word-presentations (Freud), the author suggests C. S. Peirce's tripartite semiotic classifi cation in that the baby forms representations of icons, indices, and symbols.  相似文献

It is the art of psychoanalysis in the making, a process inventing itself as it goes, that is the subject of this paper. The author articulates succinctly how he conceives of psychoanalysis, and offers a detailed clinical illustration. He suggests that each analysand unconsciously (and ambivalently) is seeking help in dreaming his 'night terrors' (his undreamt and undreamable dreams) and his 'nightmares' (his dreams that are interrupted when the pain of the emotional experience being dreamt exceeds his capacity for dreaming). Undreamable dreams are understood as manifestations of psychotic and psychically foreclosed aspects of the personality; interrupted dreams are viewed as reflections of neurotic and other non-psychotic parts of the personality. The analyst's task is to generate conditions that may allow the analysand-with the analyst's participation-to dream the patient's previously undreamable and interrupted dreams. A significant part of the analyst's participation in the patient's dreaming takes the form of the analyst's reverie experience. In the course of this conjoint work of dreaming in the analytic setting, the analyst may get to know the analysand sufficiently well for the analyst to be able to say something that is true to what is occurring at an unconscious level in the analytic relationship. The analyst's use of language contributes significantly to the possibility that the patient will be able to make use of what the analyst has said for purposes of dreaming his own experience, thereby dreaming himself more fully into existence.  相似文献

From a perspective in recognition of my unconscious attachment to their theoretical conceptualizations as well as intrinsic blind spots to both their and my conceptualizing, I offer a consideration of the clinical assistance and difficulty created by the Boston Change Process Study Group's formulation of the reflective, the implicit and the disjunction between the two as an “intention unfolding process.” I first consider how their conceptualizations can either clarify or obscure organizing experience for the analyst trying to make sense of what is going on between her and her analysand. Then I consider how these ideas might guide or mislead the analyst's active participation in the micromoment process flow of clinical interaction. The impact of dissociation is recognized as constitutive of a messiness and uncertainty that accompanies, and often can undermine, the adoption of any theoretical “idea,” including these, for clinical organizing and intervening activity.  相似文献

Abstract

This paper considers questions of danger and safety in the analytic relationship in light of the contemporary recognition of analysis as a co-participatory process. In the interest of safety, the psychoanalyst has the responsibility to be persistently curious, particularly about the problems derived from his contact with the analysand. Information about the analyst's impact must be taken to heart; it must be experientially considered. As the process unfolds, the analyst presumes that a portion of its effect will be negative. The analyst aspires not to preempt all negative impact but to create an analytic environment in which the analysand's conscious and unconscious communications about impact may be attended to. The analyst's ability to receive such information is crucial in the establishment of a reliable process capable of addressing and surviving the unanticipated dangers that inevitably emerge and securing the analysand for further self articulation. The analyst can simultaneously attend to being the analyst and being a subject of analysis by regarding all communications from the analysand as representing, at least in part, interpretations of the analyst and the analyst's participation. The classical opinion encountered in psychoanalytic psychotherapy is that the main curative agent is language, i.e. the information proffered by the analysand and the interpretations of the analyst. Over and against this view the author draws attention to the non-verbal structures in the twosome relationship between analysand and therapist. Essential elements of this working alliance, he suggests, operate via non-verbal behaviour. Hence this behaviour needs to be systematically observed and evaluated. His conclusion is that it requires the consideration of non-verbalized fantasies and affects for the therapeutic relationship to develop into a "good relationship".  相似文献

The author provides both a theoretical context for, and clinical illustrations of, the way in which he thinks and works as a psychoanalytic supervisor. The analytic supervisory experience is conceived of as a form of 'guided dreaming'. In the supervisory relationship, the supervisor helps the analyst to dream (to do conscious and unconscious psychological work with) aspects of the analytic relationship that the analyst is unable to dream or is only partially able to dream. It is the task of the supervisory pair to 'dream up' the patient, that is, to create a 'fi ction' that is true to the supervisee's emotional experience with the analysand. To carry out this work, the supervisor must provide a frame that ensures the supervisee's freedom to think and dream and be alive to what is occurring in the analytic and the supervisory relationship, as well as in the interplay between the two. In one of the clinical illustrations presented, the author illustrates his conception of the importance of the feeling on the part of supervisor and supervisee that (at least occasionally) they have 'time to waste'. Such a state of mind may provide an opportunity for a type of freely associative thinking that enhances the range and depth of what can be learned from the supervisory experience. In another clinical example, the author describes his own experience in supervision with Harold Searles, which contributed to his conception of the supervisory process.  相似文献

The first part of this paper explains how the transgenerational mandate influences the mental activity of the child. When a child acts the denied suffering of the parental couple, the analyst risks being imprisoned by the transgenerational mandate. Frequently the analyst is unconsciously asked by the parents to cure without curing because the child's psychological birth, still in the making, threatens the defensive equilibrium of the parents. The analyst is thus caught between the parents and the child and must find his/her own way forward in order to free the child from the burden of the mandate. In the second part of the paper, an analysis of a 5-year-old child is presented. The author shows how a transgenerational mandate may hold a traumatic potential because it can impair heavily the child's capacity to think. The author also describes the way in which she works with the parents and how she manages the setting and the style of the interpretation. She insists in particular on the need to sustain the child's perceptions in order to gradually allow the child to take roots in his/her experiences and therefore develop his/her own identity.  相似文献

For a surprising number of analysands, many with above‐average intelligence and facility with language, words do not adequately capture or convey emotion or symbolize experience. This often subtle difficulty can have a powerful impact on the ability to verbally communicate emotions, process affect, and utilize traditional psychoanalytic interventions. Analysands with these problems therefore often have difficulty using analysis to explore and understand their own experience, yet the reasons for such difficulties are often not recognized by either analyst or analysand. In this article, I suggest that the concept of alexithymia can provide a valuable tool for understanding this gap between affects and language and, in its broadest interpretation, can aid therapists in their pursuit of meaningful approaches to the analytic process. Using the concept as a starting point, I describe and illustrate an approach in which paying attention to apparently insignificant aspects of an analysand's experience gradually helps analysands identify and explore personal symbols and meanings in their lives.  相似文献

In a 1933 paper Ferenczi implied that conflict between the adult's and the child's construction of reality is traumatic for the child. As all individuals construct their own view of reality, it is inevitable that there will be conflicting constructions between child and adult--and between analyst and analysand. This may be biologically rooted; recent ethological studies suggest that parent-child conflict is ubiquitous because of a divergence of needs. When a child perceives a marked divergence between his or her construction of reality and that of the caretaker, the child may tend to reject the information proffered by the caretaker. This may appear later as a resistance to learning from the analyst. The divergence of needs between child and caretaker may have a profound influence on the child's cognitive development. The capacity to share other constructions of reality is a developmental achievement which may be facilitated by the psychoanalytic process.  相似文献

The author presents her experience as the analysand of a training analyst who was investigated and expelled for ethical violations with another patient, including sexual-boundary violations, during her analytic training. While boundary violations by training analysts are not uncommon, the particular trauma experienced by 'bystanders' such as candidates and supervisees is not discussed in the literature, nor the response of institutes to the educational problems that are generated. The author illustrates the complications for candidates that arise from the dual roles of training analyst as educator and analyst when he or she faces investigation or censure, including isolation and secrecy, which promote various splits in the candidate, analytic dyad and group, as well as loyalty conflicts. The discussion covers three phases of the author's experience as a candidate-analysand, namely the period encompassing the institute's ethics investigation, the announcement of findings to her and to the institute as a group, and the ensuing individual and group dynamics generated by her analyst's expulsion from the institute and revocation of his medical license. Theoretical perspectives are utilized to understand the group regression, including contamination and contagion fears, which occurred in the wake of the training analyst's expulsion, and the impact of these processes on the candidate, including the pressure to function as a 'container' for projections of the group. Implications and recommendations for candidates and institutes are made for dealing helpfully with trainees who are affected by the process of dealing with a training analyst's ethical violations. Short-term and longer-term outcomes of the experience are considered.  相似文献

An upper-middle-class black woman, who grew up adapting to a white-dominated environment, entered the consulting room of a multicultural ‘white-passing’ analyst, and here unique emotional experiences were reflected in dream images of racial disorganization, internal racism, and identity confusion. While sorting through the analysand’s internal dynamics, the external world erupted in May 2020 with the murder of George Floyd, catapulting both analysand and analyst (and the nation) into a transformative confrontation with their mutual, deep-seated woundings of American racial and cultural inequities. The analysand’s racial complexity directly impacted the analyst’s ‘white-passing’ privilege, bringing into question established classifications of American whiteness. Overlapping dynamics and experiences as ‘in-betweeners’ and ‘outsiders’ – a black woman subsumed by a white-dominated society and an immigrant refugee acculturated to American life – provided a common exilic ground for mutual understanding and mirroring. The analyst explores the racial and multicultural straddling that served as a lens into the analysand’s fragmented racial identity during the eruption of American racial unrest.  相似文献

Instead of viewing body and psyche as two "substances," the author proposes that they be seen as two ways of experiencing the complex reality of the human being. Clinically, this translates into an exploration of the possibility of including the somatic in the territory of what can be represented--that is, recognizing in somatic and sensory elements and in bodily functions the potential for meaning and language, appreciating their status as precursors of representation. These somatic signals may be detected in a dialogue involving not only the minds of analyst and analysand, but also their bodies.
