Harlequin Ichthyosis among the Navajo: Counseling issues

Harlequin Ichthyosis is a rare, fatal congenital disorder of keratinization characterized by thickened, scale-like plaques of skin with a diamond configuration. Autosomal recessive inheritance has been established, and prenatal diagnosis for this disorder remains controversial. Five infants with this disorder were born among approximately 25,000 Navajo women who delivered in Gallup, New Mexico from 1970 to 1989. The incidence of 1 in 5000 among the Navajo in Gallup is high compared to previous reports in other populations. Two families with affected infants were seen in a genetics outreach clinic. Two important counseling issues were raised: (1) reluctance to discuss the possibility of recurrence for fear of affecting the outcome, and (2) feelings of guilt caused by cultural beliefs which attributed the birth defect to paternal behavior during pregnancy. Such issues are encountered in many traditional cultures, and they can be addressed by tailoring the timing and content of counseling.The opinions expressed are those of the authors, and are not to be construed as reflecting the views of the Public Health Service.     

Maddening Desire: Understanding Lust in Pastoral Counseling

Christian theologians have argued that lust is a compelling sexual desire that distorts reason and judgment, leads to exploitation and injustice, and corrupts the erotic dimensions of human experience and relationships. In this article, I expand on this tradition, arguing that lust signifies the collapse or attenuation of amative space. This collapse is described as either the subjugation of the other or pseudo surrender to the other, which is accompanied by misrecognition of the other's unique and inviolable subjectivity, as well as a heightened sense of distrust. The diverse communicative actions associated with lust, which lead to the collapse of amative space, have several sources, namely, trauma, physiology, and culture. The attempt to restore amative space requires that the counselor and client join together in identifying and understanding the meanings and sources of lustful behaviors, which assists them in considering various strategies for the sake of increasing self-awareness and self-control that loosen the bonds of lust.

Understanding and Counseling Narcissistic Clients

This article is intended to provide counselors with an overview of narcissism and its treatment. In the first section, dysfunctional narcissism is described, drawing on the diagnostic indicators presented in the DSM-III and the contemporary object relations theories of Heinz Kohut and Otto Kernberg. Counseling Muslim Americans: Cultural and Spiritual Assessments

In this article, the authors identify the cultural and spiritual assessments needed to conduct counseling with Muslim Americans and Muslim immigrants to the United States. Assessment processes are outlined that include cultural identity (which subsumes several variables); worldview; spiritual assessment along with acculturation level and migration concerns; impact of languages spoken; social, occupational, and educational status of the client in host culture and in home culture (if the client is an immigrant); and family composition and social supports.

Understanding Workaholism: A Review of Causes and Counseling Approaches

Is workaholism an addiction or a rewarding work style and life-style that may be harmful to one's family and co-workers? This central question directed the review of definitions and causes of workaholism. Thereafter, the authors discuss the role of “Type A” and obsessive-compulsive traits on the behavior of some workaholics, review the effects of workaholism on workaholics themselves and on others, and suggest counseling approaches that might be helpful to workaholics.     

The Counselor and Change: Counseling for Cultural Transition

Cultural analysts have indicated that American culture is going through a significant transition in values. The views of three of these analysts are reviewed and implications are drawn for counselors.     

Counseling and the Law: A Cross‐Cultural Perspective

Counselors must demonstrate ethical standards of the profession and abide by the law. This task is made challenging by the fact that the mental health and legal systems represent 2 different, sometimes conflicting cultures. Using a cross‐cultural perspective, overlapping or common elements of each culture are noted, differing elements of each culture are identified, and recommendations are provided for the 2 cultures to work together as effectively as possible.     

Counseling Muslim Women: Navigating Cultural and Religious Challenges

In this article, the authors consider strategies for counseling female Muslim clients. First, they review general beliefs and practices of Muslims in the United States. Through the use of a case study, they illustrate a collaborative method of counseling Muslim women that is based on a trusting client‐counselor relationship.     

Understanding and Incorporating God Representations Within Counseling

The religious and spiritual identity of a client may be relevant to the counseling process, and thus counselors should be prepared to integrate these components within treatment. The authors consider the assessment and exploration of God representations (i.e., God images and concepts) to understand and integrate religious and spiritual meanings within the context of a client's presenting concerns. The authors provide examples, strategies, and considerations for how to apply God representations within counseling.     

Cultural identity, explanatory style, and depression in Navajo adolescents

This study investigated the interrelationships among cultural identity, explanatory style, and depression in Navajo adolescents. A total of 332 (197 female, 135 male) Navajo adolescents completed 7 self-report measures. These data were used to create, via structural equation modeling, a series of factor models and full structural models. Analyses indicated that current factor structures for explanatory style and depression are adequate for use with Navajo adolescents. Increased control and predictability and limited duration of stressful encounters were both predictive of decreased symptoms of depression. Higher levels of Navajo cultural identity had a modest effect in terms of reducing depression. Other factors, such as perceived discrimination and urban/reservation domicile, are important to study to provide an increased understanding of depression among Navajo adolescents.     

Toward a Developmental Understanding of the Counseling Specialty: Lessons From our Students

Trends in professional practice and training have led to a debate concerning the need for an increasing remedial emphasis within the counseling field. Proposals for an integration of the counseling and clinical specialties have been opposed by those arguing that a specialty differentiation captures meaningful differences. This article hypothesizes that the counseling specialty is distinguished by its reliance on a developmental metatheory, which makes unique assumptions regarding distress, change, and the helping process. This metatheory is rooted in a rich theoretical and empirical tradition and has generated innovations in practice that transcend usual remedial forms of helping. It is proposed that the value of developmentalism and the counseling specialty are most evident in the developmental interventions of college counseling.     

Living in Harmony with Navajo Indian Traditional Religious Beliefs: Honesty,Acceptance, and Understanding

ABSTRACT

This paper reports on the religious beliefs and traditions of Navajo American Indian elders. It reinforces the importance of religion in their everyday lives, and the commitment they have to fulfilling important roles within their culture as American Indian elders. The paper also discusses developing an appreciation of Navajo religious beliefs and particularly honesty, acceptance and understanding. Information for this paper was gleaned from poetry group sessions with older Navajo Indians who were residents of an extended care facility.     

Group Counseling Children of Divorce in the Elementary Schools: Understanding Process and Technique

One approach to counseling the children of divorced or separated parents includes the children in peer-group counseling sessions. The authors offer guidelines and techniques for elementary school counselors in establishing such a group-counseling program.
