Although there has been wide dissemination of research-based psychosocial prevention programs, a similarly strong research base to guide program implementation has been lacking. Program implementation has been particularly difficult for schools, due partly to insufficient understanding of how school ecologies interact with these programs. This study examined the effects of multiple dimensions of school climate on level and rate of change in implementation of a violence prevention intervention across three school years. Using multi-level modeling, the study found that teacher-reported support between staff and among teachers and students predicted higher average levels of implementation. Teacher-reported administrative leadership predicted greater growth in implementation across 3 years. Findings offer implications for an ecological model of program implementation that considers school-level contextual effects on adoption and sustainability of new programs in schools. This research examined the sustainability of Canada's At Home/Chez Soi Housing First (HF) programs for homeless persons with mental illness 2 years after the end of the demonstration phase of a large (more than 2000 participants enrolled), five‐site, randomized controlled trial. Qualitative interviews were conducted with 142 participants (key informants, HF staff, and persons with lived experience) to understand sustainability outcomes and factors that influenced those outcomes. Also, a self‐report HF fidelity measure was completed for nine HF programs that continued after the demonstration project. A cross‐site analysis was performed, using the five sites as case studies. The findings revealed that nine of the 12 HF programs (75%) were sustained, and that seven of the nine programs reported a high level of fidelity (achieving an overall score of 3.5 or higher on a 4‐point scale). The sites varied in terms of the level of systems integration and expansion of HF that were achieved. Factors that promoted or impeded sustainability were observed at multiple ecological levels: broad contextual (i.e., dissemination of research evidence, the policy context), community (i.e., partnerships, the presence of HF champions), organizational (i.e., leadership, ongoing training, and technical assistance), and individual (i.e., staff turnover, changes, and capacity). The findings are discussed in terms of the implementation science literature and their implications for how evidence‐based programs like HF can be sustained.  相似文献

Despite the popularity and noted utility of Wandersman and colleagues' (2008) Interactive Systems Framework, the literature currently provides a primary focus on delivery organizations’ and supportive stakeholders’ capacities and strategies to implement innovations, presenting a critical gap in understanding. Unfortunately, reflective of a larger void in community dissemination and implementation efforts, there is a more limited focus on the dissemination of innovations. This paper presents the social marketing literature as a supplement to the Prevention Synthesis and Translation System (PSTS), the system responsible for dissemination. The study and practice of innovation synthesis and translation is examined in the literature; and based on the conclusions drawn, social marketing theory is used to provide a systematic approach to improving dissemination within the Interactive Systems Framework. Specifically, three gaps related to the PSTS are identified in the literature that align with and can be filled using social marketing. Social marketing is defined and presented as a supplement by providing theory and practices, within a systems context, for effectively communicating and influencing change. By blending social marketing with the Interactive Systems Framework, the aim is to improve the understanding of strategic communication and its role in the effective dissemination, and subsequent implementation, of innovations.  相似文献

As efforts to develop models for suicide prevention and intervention in the juvenile justice (JJ) system continue to grow, research to understand the feasibility and acceptability of implementing these models is critical. Examining organizational readiness for implementation, ensuring leadership and staff buy‐in for delivering the intervention, and planning for sustainability of staff participation in implementation efforts is essential. The current study involved semi‐structured formative evaluation interviews with key JJ stakeholders (n = 10) to determine perspectives on the acceptability (perceived need and fit of the intervention) and feasibility (organizational readiness for change) of a proposed brief safety planning intervention for youth with suicidal ideation delivered by nonclinical staff and integrated into the existing system. Qualitative data revealed stakeholders' perceived need for the intervention in the family court context and their agreement that the aims of the intervention were congruent with the goals of the family court. Some barriers to successful implementation were noted, which, addressed through selection of appropriate implementation strategies, can be overcome in a future test of the safety intervention.  相似文献

A variety of predictors of parent participation in prevention programming have been identified in past research, but few studies have investigated how those predictors may vary by implementation context. Patterns of parent participation were examined in the Early Risers Conduct Problems Prevention Program using two family-focused service delivery models: a community center model (Center) and an in-home outreach-based model (Outreach). An ethnically diverse sample of Kindergarten through second grade students (n = 246) displaying elevated levels of aggression were recruited for the study and randomly assigned to either the Center (n = 121) or Outreach (n = 125) models. In both delivery models, participants and their families completed an assortment of baseline measures and received family skills and child skills intervention components and family- and school-based case management. Parents in the Center model demonstrated greater overall participation in family-focused components of the intervention. Parent motivation with parent-focused expectancies for the intervention represented the strongest predictor of parent participation across both delivery models. Family income differentially predicted parent participation across the two models, with low income predicting greater participation in the Center model and lower participation in the Outreach model. A qualitative finding emerged showing that parents receiving parent skills in the Center model via groups preferred to learn skills related to facilitating overall family relationships, whereas parents receiving parent skills via individual Outreach meetings preferred to improve a child’s behavior and emotion skills. Implications are discussed for the design of prevention programming in order to maximize parent participation in high risk populations.  相似文献

Evidence-based prevention and intervention programs are increasingly being implemented in schools and it therefore is becoming increasingly important to understand the complexities of program implementation under real-world conditions. Much research has focused on the contextual factors that influence program implementation but less work has attempted to provide an integrated understanding of mechanisms (e.g., teacher-training processes) that affect teachers' program implementation. In this paper, we review literature on factors related to teachers' implementation of school-based prevention and intervention programs, then from this review abstract what we believe are four basic ingredients that characterize potentially sustainable teacher-implemented classroom programs. Finally, we present a sequential model, based on these ingredients, of the naturalistic processes underlying sustainability of teachers' program implementation and describe how this sustainability can be enhanced through provision of teacher training and performance feedback from a classroom consultant.  相似文献

A comprehensive mission for schools is to educate students to be knowledgeable, responsible, socially skilled, healthy, caring, and contributing citizens. This mission is supported by the growing number of school-based prevention and youth development programs. Yet, the current impact of these programs is limited because of insufficient coordination with other components of school operations and inattention to implementation and evaluation factors necessary for strong program impact and sustainability. Widespread implementation of beneficial prevention programming requires further development of research-based, comprehensive school reform models that improve social, health, and academic outcomes; educational policies that demand accountability for fostering children's full development; professional development that prepares and supports educators to implement programs effectively; and systematic monitoring and evaluation to guide school improvement.  相似文献

This study explored multiple attachment relationships and examined four conceptual models of child–mother/father attachment—monotropy, hierarchy, independence, integration—to explain executive functioning (EF) in attention deficit hyperactivity disorder (ADHD) versus typical development (TD; n = 50 each; age: M = 11.45, SD = .50). Significant ADHD versus TD differences emerged on all EF measures and on distribution into four clusters (secure attachment to both parents, neither parent, only father, only mother). For both groups, results supported two attachment models: (a) monotropy: child–mother attachment predicted all EF measures; child–father attachment predicted none and (b) integration: clusters differed significantly on all EF measures. Children with ADHD comprised ~74% of the high‐risk cluster (concordant insecure attachment, highest EF difficulties). Discussion focused on unique risk/protective roles played by each parental attachment for understanding EF in children with ADHD or TD.  相似文献

Heterogeneity in cognitive performance, once regarded as noise, is now considered a causal mechanism or core deficit of ADHD and its related symptoms in most etiological models of the disorder. Previous research on cognitive performance variability has documented increased heterogeneity in response latencies using reaction time data. In contrast, variability in response accuracy remains understudied. The present study is the first to examine Response Accuracy Variability (RAV) among children with ADHD. Children with ADHD (N?=?54) and typically developing children (N?=?50) completed phonological working memory tasks with four set size conditions. RAV was calculated for each set size using the adjusted coefficient of variation (ζ). Results from a mixed model ANOVA indicated that children with ADHD evinced significantly greater variation in working memory performance relative to typically developing children when engaged in tasks within their cognitive capacity (i.e., set sizes 3 and 4), whereas all children exhibit similar, high levels of variability on tasks that exceeded their cognitive capacity (i.e., set sizes 5 and 6). Findings are aligned with the extant literature in documenting consistently inconsistent cognitive performance among children ADHD.

In this paper, we discuss the relations among internal mental models, attentional cues, and knowledge structure in solving elementary physics problems. The notion of semantic sensitivity is introduced as a qualitative measure of problem-solver receptivity to cues for generating, or making a shift to, a new internal model that may lead to the correct answer. Using three physics problems and their variations, the following results were obtained. First, think-aloud protocol data of two experts and one novice, working on those problems, were analyzed to provide empirical examples of internal models, and also to show that generation and shift of internal models generally results from the interplay of two kinds of processes, model construction and model development. Second, protocol data from 15 novices and seven professionals were used to present a more extensive empirical taxonomy of internal models for those problems. Third, seven experiments were used, along with the results from the protocol data, to suggest the following conditions on semantic sensitivity. (1) If the problem solver's present internal model is independent of his knowledge of physics principles underlying the given problem, but if attentional cues are related directly to the knowledge, then his present model is semantically sensitive to the cues, that is, it can be shifted easily to another model that is based on the knowledge. (2) His present model is semantically insensitive, if given cues are encoded based on the same knowledge as used for generating the model. (3) The relevance of cues to knowledge structure is critical for semantic sensitivity of the presently encoded model: Which cues to attend to is guided by the problem solver's knowledge, and if he can focus on those most relevant to the physics principles underlying the problem, then his present model is semantically sensitive to those cues, that is, he is more able to shift the present model to another that would lead to the correct answer.  相似文献

Murray et al. (2014--this issue) present a fascinating account of their international dissemination and implementation (D&I) research focused on training therapists in Thailand and Iraq to provide a modular treatment approach called Common Elements Treatment Approach to youth. In this commentary, we use Murray et al. as a springboard to discuss a few general conclusions about the current direction of D&I research. Specifically, we reflect on current D&I models, highlighting their ecological focus and their emphasis on stakeholder involvement. Next, we discuss the central importance of implementation supports such as treatment programs, training approaches, assessment and outcome monitoring tools, and organizational interventions. We conclude with a consideration of how D&I work that aims to adapt implementation supports for local needs represent a key path to our goal of sustainability.  相似文献

Community practitioners can face difficulty in achieving outcomes demonstrated by prevention science. Building a community practitioner's prevention capacity—the knowledge and skills needed to conduct critical prevention practices—could improve the quality of prevention and its outcomes. The purpose of this article is to: (1) describe how an intervention called Assets‐Getting To Outcomes (AGTO) was used to establish the key functions of the ISF and present early lessons learned from that intervention's first 6 months and (2) examine whether there is an empirical relationship between practitioner capacity at the individual level and the performance of prevention at the program level—a relationship predicted by the ISF but untested. The article describes an operationalization of the ISF in the context of a five‐year randomized controlled efficacy trial that combines two complementary models designed to build capacity: Getting To Outcomes (GTO) and Developmental Assets. The trial compares programs and individual practitioners from six community‐based coalitions using AGTO with programs and practitioners from six similar coalitions that are not. In this article, we primarily focus on what the ISF calls innovation specific capacity and discuss how the combined AGTO innovation structures and uses feedback about its capacity‐building activities, which can serve as a model for implementing the ISF. Focus group discussions used to gather lessons learned from the first 6 months of the AGTO intervention suggest that while the ISF may have been conceptualized as three distinct systems, in practice they are less distinct. Findings from the baseline wave of data collection of individual capacity and program performance suggest that practitioner capacity predicts, in part, performance of prevention programs. Empirically linking practitioner capacity and performance of prevention provides empirical support for both the ISF and AGTO. We examined the sustainability of the KiVa antibullying program in Finland from its nationwide roll-out in 2009 to 2016. Using latent class analyses, we identified four different patterns of implementation. The persistent schools (43%) maintained a high likelihood of participation throughout the study period. The awakened (14%) had a decreasing trend during the first years, but then increased the likelihood of program participation. The tail-offs (20%) decreased in the likelihood of participating after the third year, and the drop-offs (23%) already after the first year. The findings suggest that many schools need support during the initial years to launch and maintain the implementation of evidence-based programs; yet a large proportion of schools manage to sustain the program implementation for several years. The logistic regression analyses showed that large schools persisted more likely than small schools. Lower initial level of victimization was also related to the sustainability of the program. Finally, persistent program participation was predicted by several school-level actions during the initial years of implementing the program. These results imply that the sustainability of evidence-based programs could be enhanced by supporting and guiding schools when setting up the program during the initial implementation.  相似文献

An increasing number of culturally adapted family-level interventions address mental health disparities with marginalized populations in the United States. However, with these developments many barriers have arisen, such as challenges with degree of cultural fit, engagement, and sustainability. We conducted 12 elite phenomenological interviews with mental health scholars involved in prevention and intervention family research with various Latinx communities within and outside of the United States. These scholars discussed their experiences of overcoming barriers in their research. We used thematic analysis to code and analyze participant responses, and our findings support the gaps in previous literature and highlight potential pathways to overcoming barriers in cultural adaptation research. Themes included the need for: (a) better understanding of the intersection between culture and context; (b) community-centered approaches to addressing implementation challenges; and (c) structural changes within institutional, governmental, and political levels. We discuss implications for researchers and practitioners working with Latinx families.  相似文献
