This paper presents an interactional approach to the problem of dysfunctional silencing in family therapy. Silencing is classified as dysfunctional if it satisfies two conditions: (a) it occurs repeatedly and independently of content, and (b) it functions as negative feedback that limits change in the family system. Dysfunctional silencing is defined as those efforts of one or more family members to limit change by repeatedly blocking the communication of another family member, who in turn colludes by tacitly agreeing to remain silent. The interactional approach presented utilizes conflict-resolution techniques and videotape feedback; it is illustrated by a case example.  相似文献

To increase the effectiveness of parent education interventions, family therapists can address those factors that shape the way in which parents interact with their children. This article presents ecological theory as a framework that can help clinicians expand the range of their parent training efforts. In order to assess the utility of ecological theory for clinical practice, the present study empirically explored the ways in which selected characteristics of parents, children, and the family context related to mothers' parenting behavior. Specific recommendations for family therapists who employ parent training techniques are offered.  相似文献

Although family therapy recognizes the importance of the social context as a determiner of behavior, family therapists have not examined the consequences of traditional socialization practices that primarily disadvantage women. The unquestioned reinforcement of stereotyped sex roles takes place in much of family therapy. A feminist therapy orientation that considers the consequences of stereotyped sex roles and the statuses prescribed by society for females and males should be part of family therapy practice. This paper describes the ways in which family therapists who are aware of their own biases and those of the family can change sexist patterns through applying feminist principles to such areas as the contract, shifting tasks in the family, communication, generational boundaries, relabeling deviance, modeling, and therapeutic alliances.  相似文献

Although cognitive behavioral spectrum approaches with individual children are plentiful and demonstrate effectiveness, cognitive behaviorally oriented clinicians are frequently left to their own devices when it comes to treating families. Cognitive behavioral family therapy is a relatively recent development and there are precious few reports of its clinical use. This article presents a conceptual foundation and clinical rubrics for the practice of cognitive behavioral family therapy. Basic theoretical background information is presented and places the therapeutic processes and procedures in a proper context. Session structure in cognitive behavioral therapy is illustrated and the way it propels therapeutic momentum and adds focus to each session is explained. Rudimentary processes of self-monitoring, self-instruction, rational analysis, and behavioral enactment are described and augmented with case material. Finally, the conclusion offers directions for further theory building, research, and clinical practice.  相似文献

In the era of transnationalism, family therapists working with immigrant women are provided with the space to be innovative in their search for alternatives to Eurocentric approaches. Doing so can help therapists to best serve working-class immigrants and refugees who arrive in North America to escape violence and poverty. The case presented in this article highlights the cross-border interconnectedness between a Salvadoran refugee woman in Canada and other people in her life. The woman's sources of resilience were her emotional and spiritual connections with her extended family and community members who lived in her country of origin and in the United States. A knowledge base of culturally sensitive psychotropic pharmacotherapy is encouraged to help patients receive accurate diagnoses and beneficial treatment. Research is best informed if the complexity of individuals is directly addressed beyond main effects in research designs. The interactions of age, sex/gender, race/ethnicity, and cultural practices, as well as medication effects, need to be studied. Reporting data with distinctive characteristics exhibited by different ethnic-gender subgroups provides rich information with which psychologists can treat patients more effectively as well as ethically. Conceptual models of prescribing need to include perspectives of the whole person. An interactional model is one model that would address physiological, psychological, and sociocultural dimensions.  相似文献

Experiential family therapy is an intuitive approach that utilizes active, multisensory techniques. These techniques, such as role plays and drawings, increase the family's expression of affect and uncover new information. Increased affect and uncovered information stimulate change and growth in the family system. Experiential techniques are especially useful when more traditional, verbal-based communication is not effective. In this article, I will present a pediatric case in which the patient, a 7-year-old boy diagnosed with autism, was referred to the genetics clinic to rule out the presence of an associated genetic disorder. I will then describe a hypothetical second counseling session with the same family and suggest how three experiential family therapy techniques: family drawing, empty chair technique, and continuums might be used in the session to help resolve a marital conflict between the patient's parents.  相似文献

Although Viktor Frankl's Logotherapy concepts have been described as a basis for support groups and for small sharing groups, they have seldom been reported in the literature as a basis for the practice of interpersonal group psychotherapy. In this article the author overviews and outlines the treatment elements and dynamics of a Logotherapy approach to interpersonal group therapy. This paper presents a model for a therapeutic approach to the cultural systems of families. Using anthropologically derived concepts of material and ideational planes of culture, magic, and ritualistic intervention, the inducement of culture change in frozen familial systems is framed in dialectical terms. Four brief case studies are presented describing the systems engaged, the material-ideational rituals employed, and the cultural transformations induced. The paper concludes with a brief discussion of some of the theoretical and practical implications of this cultural approach to the family in therapy.  相似文献

This paper describes a model of family therapy that draws on experiences and theories of psychoanalysis as applied to individuals and groups. It suggests concepts that help to understand and work with the family as a system. A behavioral interactional model (B.I.M.) of the family is described, which is based upon Bertalanffy's definition of a “system.” The model has a logical structure derived from the formal properties of dyads. The features of dyadic interaction, combined with the “constituent analysis” of protocols, provide a method of analyzing and comparing views about family relationships. This subjective viewpoint outlines a plan of attack for those who, like the present writer, would like to pirate some of the useful models and orientation developed at the Philadelphia Child Guidance Clinic. Specifically focused publications abound from the talented people at the Philadelphia Clinic, but, to date, no unifying work has appeared in the literature to define just what is meant when they use the rubric "structural family therapy." The present article defines one person's understanding of the components included under that rubric. As a definition and overview, no effort is made to be comprehensive, and few illustrations show how specific strategies would be implemented. Rather, this is an effort to draw from the literature, discussions, talks, and videotapes originating at the Philadelphia Child Guidance Clinic, a central kernel that could be adapted by other agencies desiring to utilize this orientation.  相似文献

The Solution-Focused Brief Therapy model has been applied to support families with various problems commonly met at the department of family medicine in a large, urban teaching hospital in Seoul. Korea. Many cases there show a different distribution of problems when compared with a tertiary family therapy center. The majority involve psychosomatic problems, family distress around a chronically-ill member, the loss of a family member, and severe illness, only a few involving drinking problems, family violence, or abuse. The SFBT model meets the seven requirements of Doherty and Baird (1983) for a family therapy model to be useful in a medicine setting.  相似文献
