We evaluated hemispheric functions ipsilateral to the side of carotid endarterectomy (CEA) in patients with a severe stenosis in the left or right carotid artery. Assessments took place 1 day before and 3 months after CEA. Only right-handed males were included. Nineteen patients underwent surgery of the left carotid artery and 17 of the right. Valid instruments for hemispheric function were included, such as verbal dichotic listening, finger tapping, and a lateralised test for motor planning. Results showed that, preoperatively, patients had lower scores compared to norms on the laterality tests, and on a visuoconstructive test. There was no evidence of ipsilateral improvement related to side of surgery. Left ear dichotic listening improved, which was seen in both left and right surgery groups. Also in both groups, left- and right-hand movement speed in the motor planning test decreased. Conclusion is that beneficial ipsilateral cognitive change after CEA in patients with severe stenosis in one of the carotid arteries may not be demonstrated, even if valid instruments for hemispheric function are included.  相似文献

评估症状性颈动脉狭窄(SCAS)患者颈动脉内膜剥脱术(CEA)手术时机选择及伴发症状对预后的影响.通过相关检索策略查找探讨CEA手术时机选择对于患者预后影响的随机对照试验.所有数据均采用Revman5.0分析软件进行Meta分析.纳入22篇文献,总计9 037名患者.Meta分析结果显示:SCAS患者4w、2w、1w 前后行CEA术后卒中再发率或死亡率组间无统计学差异.合并脑卒中的患者4w、2w 前后及合并短暂性脑缺血发作(TIA)或心肌梗死(MI)的患者4w前后行CEA术后卒中再发率或死亡率无统计学差异.症状稳定的SCAS患者应尽早行CEA术,伴发症状脑卒中、TIA 和MI不影响手术预后.  相似文献

In a subgroup of patients with mild traumatic brain injury (TBI) residual symptoms, interfering with outcome and return to work, are found. With neuropsychological assessment cognitive deficits can be demonstrated although the pathological underpinnings of these cognitive deficits are not fully understood. As the admission computed tomography (CT) often is normal, perfusion CT imaging may be a useful indicator of brain dysfunction in the acute phase after injury in these patients.In the present study, directly after admission perfusion CT imaging was performed in mild TBI patients with follow-up neuropsychological assessment in those with complaints and a normal non-contrast CT. Neuropsychological tests comprised the 15 Words test Immediate Recall, Trailmaking test part B, Zoo Map test and the FEEST, which were dichotomized into normal and abnormal. Perfusion CT results of patients with normal neuropsychological test scores were compared to those with abnormal test scores.In total eighteen patients were included. Those with an abnormal score on the Zoo Map test had a significant lower CBV in the right frontal and the bilateral parieto-temporal white matter. Patients with an abnormal score on the FEEST had a significant higher MTT in the bilateral frontal white matter and a significant decreased CBF in the left parieto-temporal grey matter. No significant relation between the perfusion CT parameters and the 15 Words test and the Trailmaking test part B was present.In conclusion, impairments in executive functioning and emotion perception assessed with neuropsychological tests during follow up were related to differences in cerebral perfusion at admission in mild TBI. The pathophysiological concept of these findings is discussed.  相似文献

While, prima facie, virtue/credit approaches in epistemology would appear to be in tension with distributed/extended approaches in cognitive science, Pritchard ( 2010 ) has recently argued that the tension here is only apparent, at least given a weak version of distributed cognition, which claims merely that external resources often make critical contributions to the formation of true belief, and a weak virtue theory, which claims merely that, whenever a subject achieves knowledge, his cognitive agency makes a significant contribution to the formation of a true belief. But the significance of the role played by the subject's cognitive agency in distributed cognitive systems is in fact highly variable: at one extreme, formation of a true belief seems clearly to be significantly creditable to the subject's agency; at the other extreme, however, the subject's agency plays such a peripheral role that it is at best unclear whether it should receive significant credit for formation of a true belief. The compatibility of distributed cognition and virtue epistemology thus turns on what it takes for a contribution to the formation of true belief to count as significant. This article argues that the inevitable vagueness of this notion suggests retreating from virtue epistemology to a form of process reliabilism and explores the prospects for a distributed reliabilist epistemology designed to fit smoothly with distributed cognition. In effect, distributed reliabilism radicalizes Goldberg's recent extended reliabilist view (Goldberg 2010 ) by allowing the process the reliability of which determines the epistemic status of a subject's belief to extend to include not only processing performed by other subjects but also processing performed by non‐human technological resources.  相似文献

This article explores the view that computational models of cognition may constitute valid theories of cognition, often in the full sense of the term “theory”. In this discussion, this article examines various (existent or possible) positions on this issue and argues in favor of the view above. It also connects this issue with a number of other relevant issues, such as the general relationship between theory and data, the validation of models, and the practical benefits of computational modeling. All the discussions point to the position that computational cognitive models can be true theories of cognition.  相似文献

The emerging viewpoint of embodied cognition holds that cognitive processes are deeply rooted in the body’s interactions with the world. This position actually houses a number of distinct claims, some of which are more controversial than others. This paper distinguishes and evaluates the following six claims: (1) cognition is situated; (2) cognition is time-pressured; (3) we off-load cognitive work onto the environment; (4) the environment is part of the cognitive system; (5) cognition is for action; (6) offline cognition is body based. Of these, the first three and the fifth appear to be at least partially true, and their usefulness is best evaluated in terms of the range of their applicability. The fourth claim, I argue, is deeply problematic. The sixth claim has received the least attention in the literature on embodied cognition, but it may in fact be the best documented and most powerful of the six claims.  相似文献

MR methods have for some years been used to assess cognitive performance. Recently, studies have shown that diffusion-tensor imaging (DTI), which provides noninvasive maps of microscopic structural information of oriented tissue in vivo, is finding utility in studies of cognition in the normal and abnormal aging population. These studies suggest that water proton nonrandom, anisotropic diffusion measured by DTI is highly sensitive to otherwise subtle disease processes not easily seen with conventional MRI tissue contrast mechanisms and raises new issues of the role of MR in assessing cognitive potential.  相似文献

A central issue in cognitive neuroscience of aging research is pinpointing precise neural mechanisms that determine cognitive outcome in late adulthood as well as identifying early markers of less successful cognitive aging. One promising biomarker is beta amyloid (Aβ) deposition. Several new radiotracers have been developed that bind to fibrillar Aβ providing sensitive estimates of amyloid deposition in various brain regions. Aβ imaging has been primarily used to study patients with Alzheimer’s Disease (AD) and individuals with Mild Cognitive Impairment (MCI); however, there is now building data on Aβ deposition in healthy controls that suggest at least 20% and perhaps as much as a third of healthy older adults show significant deposition. Considerable evidence suggests amyloid deposition precedes declines in cognition and may be the initiator in a cascade of events that indirectly leads to age-related cognitive decline. We review studies of Aβ deposition imaging in AD, MCI, and normal adults, its cognitive consequences, and the role of genetic risk and cognitive reserve.  相似文献

This paper provides a brief review of various uses of magnetic resonance perfusion imaging in the investigation of brain/language relationships. The reviewed studies illustrate how perfusion imaging can reveal areas of brain where dysfunction due to low blood flow is associated with specific language deficits, and where restoration of blood flow to improve the tissue function results in recovery of those deficits. This sort of evidence helps to reveal areas of the brain that are essential to a particular language task. Other studies have provided clues regarding the mechanisms of early language recovery, or have shown how perfusion imaging can identify patients with chronic hypoperfusion due to cerebrovascular stenosis in whom the BOLD effect in fMRI may be absent or reduced in areas of neural activation.  相似文献

This study examined intellectual and memory functioning in a sample of sexually abused children compared to demographically and age-matched controls. The severity of abuse and other pertinent factors were also examined in relation to cognitive performance. Elevated levels of psychopathology were present in the abused children, as well as diminished performance on tasks influenced by attention/concentration. However, after controlling for differences in IQ and socioeconomic status (SES), significant differences in memory function were not found. Results are discussed in the context of stress effects on cognition and the potential resiliency of cognitive function in children undergoing treatment for sexual abuse.  相似文献

Background

Progressive myelination during adolescence implicates an increased vulnerability to neurotoxic substances and enduring neurocognitive consequences. This study examined the cognitive manifestations of altered white matter microstructure in chronic marijuana and alcohol-using (MJ + ALC) adolescents.

Methods

Thirty-six MJ + ALC adolescents (ages 16–19) and 36 demographically similar controls were evaluated with diffusion tensor imaging (Bava et al., 2009) and neurocognitive tests. Regions of group difference in fractional anisotropy (FA) and mean diffusivity (MD) were analyzed in relation to cognitive performance.

Results

In users, lower FA in temporal areas related to poorer performance on attention, working memory, and speeded processing tasks. Among regions where users had higher FA than controls, occipital FA was positively associated with working memory and complex visuomotor sequencing, whereas FA in anterior regions was negatively associated with verbal memory performance.

Conclusions

Findings suggest differential influences of white matter development on cognition in MJ + ALC using adolescents than in non-using peers. Neuroadaptation may reflect additive and subtractive responses to substance use that are complicated by competing maturational processes.  相似文献

Aging is associated with decline in a multitude of cognitive processes and brain functions. However, a growing body of literature suggests that age-related decline in cognition can sometimes be reduced through experience, cognitive training, and other interventions such as fitness training. Research on cognitive training and expertise has suggested that age-related cognitive sparing is often quite narrow, being observed only on tasks and skills similar to those on which individuals have been trained. Furthermore, training and expertise benefits are often realized only after extensive practice with specific training strategies. Like cognitive training, fitness training has narrow effects on cognitive processes, but in the case of fitness training, the most substantial effects are observed for executive-control processes.  相似文献

Alvin Plantinga has argued that evolutionary naturalism (the idea that God does not tinker with evolution) undermines its own rationality. Natural selection is concerned with survival and reproduction, and false beliefs conjoined with complementary motivational drives could serve the same aims as true beliefs. Thus, argues Plantinga, if we believe we evolved naturally, we should not think our beliefs are, on average, likely to be true, including our beliefs in evolution and naturalism. I argue herein that our cognitive faculties are less reliable than we often take them to be, that it is theism which has difficulty explaining the nature of our cognition, that much of our knowledge is not passed through biological evolution but learned and transferred through culture, and that the unreliability of our cognition helps explain the usefulness of science.  相似文献

Background

The mineralocorticoid receptor (MR) is highly expressed in the hippocampus and prefrontal cortex and is involved in social cognition. We recently found that pharmacological stimulation of the MR enhances emotional empathy but does not affect cognitive empathy. In the current study, we examined whether blockade of the MR impairs empathy in patients with major depressive disorder (MDD) and healthy individuals.

Methods

In a placebo-controlled study, we randomized 28 patients with MDD without psychotropic medication and 43 healthy individuals to either placebo or 300 mg spironolactone, a MR antagonist. Subsequently, all participants underwent two tests of social cognition, the Multifaceted Empathy Test (MET) and the Movie for the Assessment of Social Cognition (MASC), measuring cognitive and emotional facets of empathy.

Results

In the MET, we found no significant main effect of treatment or main effect of group for cognitive empathy but a highly significant treatment by group interaction (p < 0.01). Patients had higher cognitive empathy scores compared to controls in the placebo condition but not after spironolactone. Furthermore, in the spironolactone condition reduced cognitive empathy was seen in MDD patients but not in controls. Emotional empathy was not affected by MR blockade. In the MASC, no effect of spironolactone could be revealed.

Conclusion

Depressed patients appear to exhibit greater cognitive empathy compared to healthy individuals. Blockade of MR reduced cognitive empathy in MDD patients to the level of healthy individuals. Future studies should further clarify the impact of MR functioning on different domains of social cognition in psychiatric patients.

This study demonstrates the negative moderating effect of general self-efficacy on the relationship between need for cognition, which refers to stable individual differences in people's tendencies to engage in and enjoy cognitive activity, and cognitive effort. This negative moderating effect of general self-efficacy has been termed "plasticity." Scholars assume the relationship between need for cognition and cognitive effort is true by definition. The study uses data from 144 U.S. college students and employs moderated regression analysis followed by subgroup analysis to demonstrate plasticity. The results set a boundary condition to the generally presumed relationship between need for cognition and cognitive effort, thereby improving the understanding of how these phenomena are related.  相似文献

Atherosclerotic disease accounts for approximately 25% of ischemic strokes. Atherosclerotic stroke is caused mainly by embolic events from the carotid artery bifurcation or the aortic arch, although intracranial thrombosis can occur, more often in African Americans, Asians, and diabetes patients. Primary prevention of stroke is critical for patients with risk factors for atherosclerosis, including hypertension, diabetes, smoking and hypercholesterolemia. Stroke can be prevented in patients with established atherosclerotic disease by identification and management of patients with carotid artery stenosis by non-invasive testing. Cognitive impairments are frequent in patients with Multiple Sclerosis (MS). Yet, the influence of MS-related symptoms on cognitive status is not clear. Studies investigating the impact of trait fatigue along with anxio-depressive symptoms on cognition are seldom, and even less considered fatigue as multidimensional. Moreover, these studies provided conflicting results.Twenty-nine MS patients and 28 healthy controls, matched on age, gender and education underwent a full comprehensive neuropsychological assessment. Anxio-depressive and fatigue symptoms were assessed using the HAD scale and the MFIS, respectively. Six composite scores were derived from the neuropsychological assessment, reflecting the cognitive domains of working memory, verbal and visual learning, executive functions, attention and processing speed. Stepwise regression analyses were conducted in each group to investigate if trait cognitive and physical fatigue, depression and anxiety are relevant predictors of performance in each cognitive domain. In order to control for disease progression, patient’s EDSS score was also entered as predictor variable.In the MS group, trait physical fatigue was the only significant predictor of working memory score. Cognitive fatigue was a predictor for executive functioning performance and for processing speed (as well as EDSS score for processing speed). In the healthy controls group, only an association between executive functioning and depression was observed.Fatigue predicted cognition in MS patients only, beyond anxio-depressive symptoms and disease progression. Considering fatigue as a multidimensional symptom is paramount to better understand its association with cognition, as physical and cognitive fatigue are predictors of different cognitive processes.  相似文献

Interventions relating to cognition without the intention of interpretation occur in classical psychodynamic therapy; however, the application is less elaborated than in cognitive behavior therapy. It can often also be observed that “dysfunctional” cognition is accepted over a long time by psychodynamic therapists without any reflection.
