Behavior at 10 and 13 years of age for children with low birth weight   总被引:1,自引:0,他引:1  
This study was based on data from a longitudinal research program. The cohort consisted of 874 normal children in an entire school grade in a Swedish community. The aim of the study was to investigate the relation between birth weight and behavior at school, for all children and for each sex separately. The results identified specific aspects of behaviour disorder significantly related to low birth weight (LBW) for children at the age of 10 but not at the age of 13. When the sexes were separated, there were no relations between birth weight and deviant behaviour for boys of low birth weight as compared to boys of normal birth weight, while girls of low birth weight showed specific behavioural disorders at age 10 as compared to girls of normal birth weight. For girls reared in families of low parental socioeconomic status, aggressiveness and motor restlessness at age 10 but not at age 13 was also present. Further analyses showed that girls born small-for-gestational age showed lack of school motivation and concentration difficulties both at age 10 and age 13.  相似文献

There is a consensus that prematurity could increase the risk of attachment impairment. We studied 90 premature children with very low birth weights (<1,500 g) and 96 healthy children born at term with similar age and sociodemographic characteristics. Our objective was to assess maternal stress and attachment representations, and compare development indices on both groups at 2 years of age. Premature infants had a medium‐to‐severe degree of immaturity and biological risk (M gestational age = 29.98 weeks; birth weight = 1,159.76 g), with 57% staying in the ICU ≥1 week. These children born prematurely had lower scores on the development indices within the normal range. Mothers of premature infants (n = 74) reported higher levels of stress during their first year of life (59%) than did controls (41%), but no significant differences were detected in maternal attachment discourse between the two groups. The findings confirm that premature birth has a great influence on maternal emotional responses and the health and development of children in the first years of life, and indicate that stress responses in parents and warning signs for attachment problems should be assessed as early as possible.  相似文献

Spatial location memory has rarely been assessed in young children due to a scarcity of developmentally appropriate tests. This study sought to compare nonverbal learning and recall in children born extremely low birth weight (ELBW; <1000 g) and less than 33 gestational weeks (GW) with term-born children at early school age using a recently developed and adapted test. We administered a modification of the Hopkins Board to 210 children at age six; 84 born ELBW (35 born < 26 GW; 49 born 26–33 GW) and 126 term-born. Six measures were obtained: naming, trials-to-criterion, errors-to-criterion, delayed item recall, delayed location recall, and percent retention. After age correction, ELBW children had worse general cognition, item naming, delayed item recall, delayed location recall, and percent retention than term-born children. Delayed item recall and percent retention performances of ELBW children remained worse after correction for general cognition. ELBW groups (< 26 GW and 26–33 GW) groups performed worse than term-born children in naming and delayed item recall with chronological age as covariate. Those born before 26 GW, but not 26–33 GW, performed worse than term-born children in delayed location recall and percent retention. Differences remained significant after controlling for gender, maternal education, and delivery type. All three groups' performance declined from final learning trial to delayed location recall, with a decline greater for less than 26 GW than term-born children. Extreme prematurity (< 26 GW) and ELBW are significant risk factors for spatial location memory deficit. The modified Hopkins Board discriminated high-risk preterm and term-born children at early school age and appears to be a useful test to measure this rarely studied cognitive capacity.  相似文献

Cognitive impairment often follows preterm birth but its early underlying nature is not well understood. We used a novel approach by investigating the development of colour cognition in 54 very preterm children born ?30 weeks gestational age without severe neurosensory impairment and 37 age‐matched term‐born controls, aged 2–5 years. Quantitative and qualitative differences in the development of colour cognition are well charted throughout the preschool years, enabling delayed from deviant development to be determined. Standardized domain‐general and experimental colour‐specific tests of language, attention, and memory were employed. Very preterm children showed significantly depressed language than term controls, with very preterm group girls significantly outperforming boys. Very preterm children also showed poorer attention and memory than term controls, but not significantly so. Importantly, colour‐specific tests showed qualitatively similar performance, but for naming and executive planning quantitatively poorer performance, across groups, indicating typical but delayed development. Hence, even before school entry, compared with term‐born peers, very preterm children show delayed development of cognitive processes that underpin later scholastic abilities, but the nature by which these processes operate appears to be typical of term children. If left untreated these early developmental delays may underpin later deviations from the typical developmental trajectory. Copyright © 2010 John Wiley & Sons, Ltd.  相似文献

The study examined the association between low birth weight (LBW) (2,500 g) and attention deficit hyperactivity disorder (ADHD) in two socioeconomically disparate populations. LBW and normal birth weight (NBW) children from the 1983 to 1985 newborn lists of an urban and a suburban hospital in Southeast Michigan were randomly selected. A total of 823 children, 473 LBW and 350 NBW, participated. Data were gathered in 1990 to 1992, when the children were 6 to 7 years of age. The National Institute of Mental Health Diagnostic Interview Schedule for children—Parent version (DISC-P) was used to elicit information on DSM-III-R diagnoses of simple phobia, overanxious, separation anxiety, oppositional defiant, and ADHD. Teachers' ratings of behavior problems were obtained. LBW was associated with ADHD but not with childhood anxiety disorders or oppositional defiant disorder. The association was stronger in the urban than in the suburban population. Data from teachers' ratings revealed an association between LBW and attention problems. We examine the extent to which deficits in academic achievement in low birthweight (LBW) children at age 11 are explained by deficits in cognitive abilities at school entry. Data come from a longitudinal study of a stratified sample of LBW and normal birthweight (NBW) children from an innercity and middle class suburbs in the Detroit area. Woodcock-Johnson Psychoeducational Battery-Revised was used to measure reading and math at age 11. WISC-R and specific neuropsychologic tests were administered at age 6. On reading, the LBW-NBW difference was -3.6 points (SE = 1.2). The difference was explained almost entirely by IQ at age 6. On math, the LBW-NBW difference was -6.1 points (SE = 1.1). The difference on math was trivial and not significant, when IQ and neuropsychological tests at age 6 were controlled. Level of LBW was unrelated to reading, but it had a gradient relationship with math, with birthweight < or = 1,500 g associated with a greater deficit than heavier LBW. The results imply that most of the LBW-NBW gap in academic achievement at age 11 could be eliminated by eliminating differences in cognitive abilities at age 6. Interventions to improve academic performance of LBW children should focus on the preschool years.  相似文献

Background. In this study, 2 main hypotheses have been put forward to explain the variation in childhood intelligence or school performance by season of birth. In the first hypothesis, it is suggested that it is due to school policy concerning school entry, whereas the second suggests that a seasonally patterned exposure such as temperature, maternal nutrition, or infection during critical periods of brain development have a lasting effect on intelligence. Aims. To determine whether childhood performance on tests of different domains of intelligence is patterned by season of birth and to examine possible mechanisms for any associations. Sample. 12,150 individuals born in Aberdeen, Scotland between 1950 and 1956. Methods. Birth cohort study in which the variation in different domains of childhood intelligence measured at ages 7, 9, and 11 by season of birth were examined. Results. Reading ability at age 9 and arithmetic ability at age 11 varied by season of birth, with lowest scores among those born in autumn or early winter (September–December) and highest scores among those born in later winter or spring (February–April); p = .002 for joint sine‐cosine functions for reading ability at age 9 and p = .05 for sine‐cosine function for arithmetic ability at age 11. The child's perception and understanding of pictorial differences at age 7, verbal reasoning at 11, and English language ability at 11 did not vary by season of birth. Age at starting primary school and age relative to class peers were both associated with the different measurements of childhood intelligence and both attenuated the association between month of birth and reading ability at age 9 and arithmetic ability at age 11 towards the null. Both adjusted and unadjusted differences in reading ability at age 9 and arithmetic ability at age 11 between those born from September to December compared with other times of the year were less than 0.1 of a standard deviation of the test scores. Ambient temperature around the time of conception, during gestation, and around the time of birth did not affect intelligence. Conclusion. Any variation in mean childhood intelligence by season of birth is weak and largely explained by age at school entry and age relative to class peers.  相似文献

Application of cognitive neuroscience tasks offers the opportunity to characterize early executive functions in young children born preterm. In a preliminary sample of 29 preschool children born preterm (M birth gestational age = 32.4 weeks), executive function outcome was compared to that of fullterm controls by contrasting performance on two prototypic delayed-response-type paradigms, Delayed Alternation and Spatial Reversal. Preschoolers born preterm correctly retrieved the reward on fewer trials on Delayed Alternation than did matched controls. Furthermore, preschool children born preterm used problem-solving strategies that included more perseverative errors than controls. These preliminary findings highlight the utility of cognitive neuroscience paradigms to understand neuropsychological outcome in preschool children born preterm and suggest areas of developmental vulnerability that may include dorsolateral prefrontal circuits.  相似文献

It is well recognized that reaction time performance of term-born children with a normal birth weight (NBW > 2500 g) who fulfill the DSM-IV criteria for attention deficit/hyperactivity disorder (ADHD) in the primary school age is sensitive for the presentation rate of stimuli. They have been found to perform more poorly in conditions of relatively slow event rates as compared with fast event rates. The purpose of the current study was to explore whether reaction time performance of children with very low birth weight (VLBW < 1500 g) with or without ADHD showed the same sensitivity for the factor presentation rate of stimuli compared to children with a normal birth weight plus ADHD. To this end, reaction time performance of four groups of children was compared on a Go/No-Go test with a fast presentation rate of 1 second and a slow presentation rate of 6 seconds. Groups were: children with VLBW plus ADHD, children with VLBW only, children born full term with normal birth weight (NBW >2500 g) plus ADHD, and children born full term without ADHD (the control group). Findings indicated that, compared to the non-ADHD groups, the groups with ADHD (NBW and VLBW) showed a more dramatic decline in their reaction time performance in the slow condition: a state regulation deficit. In addition, both groups showed a response inhibition deficit. No difference was found in reaction time performance between the groups of children with VLBW only and the control group.  相似文献

Within preterm-born children, being born male and at a lower gestational age (GA) have both been associated with a heightened risk for developmental difficulties. However, in this population little is known about the combined effect and the influence of these risk factors on cortical structures and executive control. In the present study, 58 preterm-born children (GA ranging from 24.0 to 35.1 weeks) were administered the computerized Child Attention Network Task at 6 years of age. Brain magnetic resonance imaging was performed and analyzed using Voxel-Based Morphometry (VBM) in all children. At a behavioral level, boys born <28 weeks of GA had significantly less executive control than preterm-born girls <28 weeks (p = .001) and preterm-born boys ≥28 (= .003). The reduced executive control in preterm-born boys <28 weeks gestation was related to lower cortical densities in the inferior frontal gyrus (IFG) and dorsolateral prefrontal cortex (DLPFC). The current study links the higher incidence of reduced executive control in preterm-born boys to a higher degree of prematurity (low GA) and identifies brain structural abnormalities in the prefrontal cortex related to these deficits. Interest in monitoring long‐term neurodevelopmental outcomes of children born moderate‐to‐late preterm (32–36 weeks gestation) is increasing. Moderate‐to‐late preterm birth has a negative impact on academic achievement, which may relate to differential development of executive function (EF). Prior studies reporting deficits in EF in preterm children have almost exclusively assessed EF in affectively neutral contexts in high‐risk preterm children (< 32 weeks gestation). Disrupted function in motivational or emotionally charged contexts (hot EF) following preterm birth remains uninvestigated, despite evidence that preterm children show differential development of neural circuitry subserving hot EF, including reduced orbitofrontal cortex volume. The present study is the first to examine whether low‐risk, healthy children born moderate‐to‐late preterm exhibit impairments in the development of hot EF. Preterm children at age 4.5 years were less likely to choose larger, delayed rewards across all levels of reward magnitude on a delay discounting task using tangible rewards, but performed more similarly to their full‐term peers on a delay aversion task involving abstract rewards and on measures of cool EF. The relationship between gestational age at birth and selection of delayed rewards extended across the entire gestational age range of the sample (32–42 weeks), and remained significant after controlling for intelligence and processing speed. Results imply that there is not a finite cut‐off point at which children are spared from potential long‐term neurodevelopmental effects of PT birth. Further investigation of reward processing and hot EF in individuals with a history of PT birth is warranted given the susceptibility of prefrontal cortex development to early environmental variations.  相似文献

Background

Low birth weight and low 5-min Apgar scores have been associated with developmental delay, while older maternal age is a protective factor. Little is known about trajectories and predictors of developmental skills in infant twins, who are generally born with lower birth weights, lower Apgar scores and to older mothers.

Methods

Developmental skills were assessed at 3, 6, 9, 12, 18 and 24 months using the Ages and Stages Questionnaires in 152 twins from the Birmingham Registry for Twin and Heritability Studies. Multilevel spline and linear regression models (adjusted for gestational age, gender, maternal age) were used to estimate developmental trajectories and the associations between birth weight, maternal age and Apgar scores on developmental skills.

Results

Twins performed worse than singletons on communication, gross motor, fine motor, problem solving and personal-social skills (p < 0.001). Twins caught up around 6 months (score within −1 standard deviation of norm), except on gross motor skills, which did not catch up until after the age of 12 months. A one-year increase in maternal age was significantly associated with decreases in gross motor and personal–social z-scores of up to −0.09, whereas one unit increases in Apgar score increased z-scores up to 0.90 (p < 0.01).

Conclusions

Healthy twins should be considered at a higher risk for developmental delay. Whether these results are comparable to preterm singletons, or whether there are twin-specific issues involved, should be further investigated in a study that uses a matched singleton control group.  相似文献

The Posner cue-target paradigm for the study of shifts in visual attention was used to assess focused attention and shifts in attention in 43 young adults, 19 to 28 years of age, who had been in need of cardiopulmonary resuscitation at birth because of perinatal asphyxia (n=31) or who were born small for gestational age (n= 12). Their performances were compared to that of 18 control subjects. The subjects resuscitated at birth were divided into two groups, mild and moderate, according to neurological symptoms during the neonatal course. An expected significant main effect by block of trials and conditions was found, with general decrease in reaction times from Blocks 1 to 5 and with slowest reaction times to an invalid cue and fastest to a valid one. No group differences were statistically significant, but the moderate asphyxia and the groups small for gestational age had longer average reaction times on all test conditions than control subjects, implying that those who showed the most severe pre- or neonatal complications also were most affected in attentional functions.  相似文献

Data on growth status at birth in a French sample of gifted children identified at school age were analysed in a retrospective survey. Birth weight, gestational age, head circumference, stature, gender, parental socio-economic status, information on the pregnancy, and motor development were recorded. On the basis of standard growth curves, the sample was characterised according to the 10th, 50th, 75th and 90th percentiles. Three groups of gestational age were determined: preterm births (18.6%), full term births (63.2%) and post-term births (18.1%) with a high frequency of hypertrophy in preterm births (≥ 90th). Intellectual quotient (IQ) did not differ among the groups, showing a notable advance in motor development. This research shows that, among children later identified as gifted, compared to those born full term, preterm infants showed a significant relationship between homogeneity in anthropometric variables and future motor and intellectual development when these children were exposed to a favourable perinatal environment (few pregnancy complications) and a favourable postnatal parental socio-economic status.  相似文献

Although children born preterm or low birth weight (PT LBW) are more likely to exhibit behavior problems compared to children born at term, developmental and family processes associated with these problems are unclear. We examined trajectories of maternal depressive symptoms in relation to toddler compliance and behavior problems in families with PT LBW infants. A total of 177 infants (93 boys, 84 girls) and their mothers enrolled in the study during the infant's NICU stay. Data were collected at five time points across 2 years. Assessments of maternal depressive symptoms were conducted at all time points, and toddler compliance and opposition to maternal requests and behavior problems were assessed at 2 years. Toddlers born earlier with more health problems to mothers whose depressive symptoms increased over time exhibited the most opposition to maternal requests during a cleanup task at 24 months, consistent with multiple risk models. Mothers with elevated depression symptoms reported more behavior problems in their toddlers. The study has implications for family‐based early intervention programs seeking to identify PT LBW infants at highest risk for problem behaviors.  相似文献

The relationship between severe early institutional deprivation and scholastic attainment at age 11 in 127 children (68 girls and 59 boys) adopted from institutions in Romania was compared to the attainment of 49 children (17 girls and 32 boys) adopted within the UK from a non-institutional background. Overall, children adopted from Romania had significantly lower attainment scores than those adopted within the UK; the children within the Romanian sample who had spent 6 months or more in an institution had significantly lower attainment scores than those who had spent less than 6 months in an institution, but there was no additional risk of low attainment associated with longer institutional care after 6 months. The lower scholastic attainment in the children adopted from Romanian institutions, as compared with domestic adoptees, was mediated by IQ, and to a lesser degree, inattention/overactivity. When these factors were taken into account, only small between-group differences in attainment remained.  相似文献

The aim of this study was to determine whether information obtained from measures of motor performance taken from birth to 4 years of age predicted motor and cognitive performance of children once they reached school age. Participants included 33 children aged from 6 years to 11 years and 6 months who had been assessed at ages 4 months to 4 years using the ages and stages questionnaires (ASQ: [Squires, J. K., Potter, L., & Bricker, D. (1995). The ages and stages questionnaire users guide. Baltimore: Brookes]). These scores were used to obtain trajectory information consisting of the age of asymptote, maximum or minimum score, and the variance of ASQ scores. At school age, both motor and cognitive ability were assessed using the McCarron Assessment of Neuromuscular Development (MAND: [McCarron, L. (1997). McCarron assessment of neuromuscular development: Fine and gross motor abilities (revised ed.). Dallas, TX: Common Market Press.]), and the Wechsler Intelligence Scale for Children-Version IV (WISC-IV: [Wechsler, D. (2004). WISC-IV integrated technical and interpretive manual. San Antonio, Texas: Harcourt Assessment]). In contrast to previous research, results demonstrated that, although socio-economic status (SES) predicted fine motor performance and three of four cognitive domains at school age, gestational age was not a significant predictor of later development. This may have been due to the low-risk nature of the sample. After controlling for SES, fine motor trajectory information did not account for a significant proportion of the variance in school aged fine motor performance or cognitive performance. The ASQ gross motor trajectory set of predictors accounted for a significant proportion of the variance for cognitive performance once SES was controlled for. Further analysis showed a significant predictive relationship for gross motor trajectory information and the subtests of working memory and processing speed. These results provide evidence for detecting children at risk of developmental delays or disorders with a parent report questionnaire prior to school age. The findings also add to recent investigations into the relationship between early motor development and later cognitive function, and support the need for ongoing research into a potential etiological relationship.  相似文献

The association between attachment and school-related cognitive functioning was longitudinally examined for a French Canadian sample of 108 school-age children. The affective quality of mother-child interaction patterns, child cognitive engagement, and quality of child attachment to mother were evaluated during a laboratory visit that included a separation-reunion procedure occurring when the children were approximately 6 years of age. Children's mastery motivation and academic performance were assessed 2 years later (at age 8). Analyses indicated that secure children had higher scores than their insecure peers on communication, cognitive engagement, and mastery motivation. Controlling children were at greatest risk for school underachievement, with the poorest performance on all measures except mastery motivation. Avoidant and ambivalent children were lowest on mastery motivation. Results of mediational analyses support the salience of mother-child interactional processes and child cognitive engagement at school age in explaining relations between attachment and cognitive functioning in school.  相似文献
