A conflict between neurology and psychiatry and controversies about psychoanalysis played an important role in the establishment of psychiatry in the general hospital. This article reviews the impact of this conflict on the establishment of an independent Department of Psychiatry in the Mount Sinai Hospital in New York during the first half of the twentieth century. A unique opportunity is provided to consider the relationships and personalities of the individuals involved, as well as economic, social, and scientific factors, including the introduction of psychoanalysis as a major psychiatric treatment.  相似文献

In this article, we describe the Naturalistic University of Alberta Nonlinear Correlation Explorer (NUANCE), a computer program for data exploration and analysis. NUANCE is specialized for finding nonlinear relations between any number of predictors and a dependent value to be predicted. It searches the space of possible relations between the predictors and the dependent value by using natural selection to evolve equations that maximize the correlation between their output and the dependent value. In this article, we introduce the program, describe how to use it, and provide illustrative examples. NUANCE is written in Java, which runs on most computer platforms. We have contributed NUANCE to the archival Web site of the Psychonomic Society (www.psychonomic.org/archive), from which it may be freely downloaded.  相似文献

This paper reports a 2 year audit of a family therapy clinic in a psychiatric department, focusing especially on the relationship between diagnosis and outcome. Patients with affective disorders–especially where anxiety predominated–and eating disorders had good outcome. Structural change occurred in families with psychotic members, even though psychotic symptoms were often unchanged. Difficulties with an acute onset were more responsive than long-term problems, but a substantial number of the latter did improve. Families receiving between four and 10 sessions had the best outcome, and improvement was generally maintained at 2 year follow-up.  相似文献

We describe the assessment of a troubled adolescent boy to demonstrate the use of the Shedler-Westen Assessment Profile for Adolescents (Westen, Dutra, & Shedler, 2005) and the Social Cognition and Object Relations Scale-Global (Westen, 1995) to formulate the Mental Functioning for Children and Adolescents Axis (MCA) of the Psychodynamic Diagnostic Manual (PDM Task Force, 2006). This article focuses on the role of the supervisory relationship in the training of psychodynamic group therapists. The supervisor's teaching responsibilities include identifying what the novice leader needs to learn about running a group, and designing and implementing supervisory strategies in a safe, collaborative, and creative setting. Of particular importance to psychodynamic supervision is the recognition of emerging impediments to learning and discerning their significance for understanding the underlying dynamics of the supervisees' treatment groups. Examples of two supervision groups are provided, one in which a misalliance had developed, and another in which a more promising supervisory relationship was forming.  相似文献

This paper discusses how thinking about a playroom involves our thinking about the setting in psychoanalytical work with children in general - the physical as well as the human aspects - and how this encourages us to rethink some of the basic aims in psychoanalytic work. The paper describes the importance of the setting being such as to help the therapist to have the freedom to think and to feel what is going on in the child and himself.
