Specialization in Psychology and Health Care Reform

This article begins by contextualizing specialization and board certification of psychologists, with attention paid to relevant definitions and expectations of other health care professionals. A brief history of specialization and board certification in professional psychology is offered. The benefits of board certification through the American Board of Professional Psychology are highlighted. Consideration is then given to the primary reasons for psychologists working in academic health sciences centers to specialize in the current health care climate and to obtain board certification as a mark of such specialization.

Board Certification in Psychology: Insights from Medicine and Hospital Psychology

For physicians board certification is an accepted tradition that research suggests improves services and outcomes. In contrast, relatively few psychologists pursue board certification suggesting ambivalence or limited contingencies reinforcing it. The authors report on medical school and hospital-based psychologists’ attitudes toward board certification and current certification status. About one-fifth (21.7%) of the sample were certified by the American Board of Professional Psychology, a greater proportion than psychologists generally: Highest rates were seen in neuropsychology (7.5%), clinical psychology (6.4%), clinical child and adolescent psychology (3.2%) and clinical health psychology (2.8%). Few (<2%) reported their hospitals required board certification. Half recognized benefits to the profession for psychologists pursuing board certification, yet 70% opposed requiring it for their hospital-based practice. Forces seeking to promote healthcare quality ultimately may increase expectations for board certification. If consumers, employers, hospitals and managed care organizations demand board certification for health professionals, greater numbers of psychologists would likely seek it.

Nurse psychologists: A unique group within health psychology

Since the mid-1980s commonalities between nursing and psychology have been explored, with subsequent establishment of a Nursing and Health Psychology Committee within the American Psychological Association's Division 38. A survey was undertaken to study this little-known, unique cadre of individuals with dual preparation in both nursing and psychology. Participants were queried regarding their educational preparations in both disciplines, current employment, theoretical and/or research interests, and current projects. Results of the survey provide information about the characteristics of this group. Dual preparation appears to offer certain advantages for the practice of health psychology.     

The legacy of Kenneth B. Clark to the APA. The Board of Social and Ethical Responsibility for Psychology

The American Psychological Association faced many challenges beginning around 1965, including the challenge to make psychology more inclusive of traditionally underrepresented groups. The larger context of social unrest and public and political focus on social problems framed these challenges. This article describes the events that led to the establishment of the Board of Social and Ethical Responsibility for Psychology in 1972 and highlights the role of Kenneth B. Clark in those events.

Psychology in Academic Health Centers: A True Healthcare Home

This article is based on the invited presentation by the author at the American Psychological Association??s Annual Convention, August 4?C7, 2011, upon his receipt of the Joseph D. Matarazzo Award for Distinguished Contributions to Psychology in Academic Health Centers presented by the Association of Psychologists in Academic Health Centers. This article relates the history, roles, and responsibilities of psychologists in academic health centers to the ultimate survival and success of professional psychology. It describes implications of the Patient Protection and Affordable Care Act (ACA) on the institutional practice of psychology including how psychology??s place in academic health centers positions the field well for the future of healthcare reform. The article provides several recommendations to help professional psychology prepare for that future of integrated, interprofessional healthcare.     

Of faculties, fallacies, and freedom: dilemma and irony in the secularization of American psychology

In The Principles of Psychology, William James (1890) articulated an influential, boundary-setting argument against faculty psychology, subsequently dubbed the Fallacy of the Faculty Psychology. This argument was reiterated in American psychology textbooks for the next several decades, arguably solidifying and simplifying American perceptions of the "old" faculty psychology and establishing belief in the superiority of the "New Psychology." When placed in the context of American theological and philosophical history, however, the New Psychology argument appears unoriginal, somewhat unfair, and deeply (and even tragically) ironic. Despite their best intentions, a fallacy did emerge in the old psychology as they sought psychological foundations for libertarian free will. For those members of the New Psychology still committed to free will, then, the Fallacy argument cut both ways - refuting the fallacy also meant tearing down a long-standing foundation for free will in American psychology. Offering no viable alternative to fill the moral void, the New Psychology appeared at times conflicted with its new deterministic identity.     

Procedures for identifying evidence-based psychological treatments for older adults

The authors describe the methods used to identify evidence-based psychological treatments for older adults in this contribution to the special section. Coding teams were assembled to review the literature on several problems relevant to mental health and aging. These teams used the manual developed by the Committee on Science and Practice of the Society for Clinical Psychology (Division 12) of the American Psychological Association that provided definitions of key constructs used in coding. The authors provide an overview of the process followed by the review teams and of some of the issues that emerged to illustrate the steps involved in the coding procedure. Identifying evidence-based treatments is a fundamental aspect of promoting evidence-based practice with older adults; such practice is advocated by most health care disciplines, including psychology.

President's Award for Significant Contributions to Psychology

The contributions upon which this award was based were made primarily in two domains, one local and the other international. The former had two aspects, one comprised research and teaching, and the other, through being Treasurer and then President of the Australian Psychological Society, was directed at building a strong and viable Society, financially, collegiately, and professionally. The second domain of involvement resulted in holding the posts of Secretary General and then President of the International Association of Applied Psychology in which, among other things, an underlying interest was in strengthening and deepening the participation of Australian psychologists in the activities of the Association. Due progressively to the efforts of many this has led to a range of major outcomes including an increasing membership of Australian psychologists in both the Association and its Board of Directors, their increasing participation in the Association's congresses, and in the Society being granted the right for the first time to host an International Congress of Applied Psychology (the 2010 ICAP in Melbourne). Collectively the benefits for Australian psychology have been many. These include raising the profile of Australian psychology internationally, giving Australia a voice in the governance of international psychology, and working towards Australian psychology contributing to mainstream psychological theory and practice. One of the distinguishing characteristics of the discipline of psychology is that it is both scientifically and professionally oriented, and while the balance between the two may vary from one organisation to another their interdependence is a necessity. Because all national societies and international associations of psychology are children of the discipline the affinity is natural, yet while this may be so the connection needs to be actively maintained to prevent it withering, to the detriment of both. Thus, in keeping with tradition, while this article will outline the major contributions upon which the award was made, its primary focus is the nature and imperative of the relationship between science and professional practice, and this will be a recurring theme in each of the following principal sections dealing with research and teaching, the Australian Psychological Society (APS), the International Association of Applied Psychology (IAAP), the 2010 International Congress of Applied Psychology (ICAP), and mainstream psychology.     

The state of the psychology health service provider workforce

Numerous efforts to describe the health service provider or clinical workforce in psychology have been conducted during the past 30 years. The American Psychological Association (APA) has studied trends in the doctoral education pathway and the resultant effects on the broader psychology workforce. During this period, the creation and growth of the PsyD degree and the formalization of the predoctoral internship placement system (the APPIC Match) have been well noted, but efforts to gain a complete understanding of professional practice are lacking. Specifically, piecemeal research on the provider workforce has led to the study of specific subpopulations using varying approaches and definitions of those providing direct clinical service. Consequently, estimates of the supply and need for health service providers are distinctly divergent and generate protracted debate in organized psychology. The APA membership directory and the APA Doctorate Employment Surveys have traditionally been relied on for workforce analyses. Yet, these data have become characterized by limited generalizability in recent years because of declining survey response rates and the fact that APA member data may not be as representative of the entire psychology health service provider population as they were previously. The 2008 APA Survey of Psychology Health Service Providers targeted these limitations by including nonmember psychologists in the sampling frame. Results revealed emerging themes in the demographics, work settings, and delivery of health services of the psychology health service provider workforce. Future areas of research for APA and organized psychology to undertake in addressing need and demand are suggested.

Science in Mental Health Training and Practice,With Special Reference to School Psychology

The first words in the inaugural version of the American Psychological Association Ethical Standards of Psychologists (1953) declared, “Psychology is a science” (p. v). Professional ethics for all of the mental health disciplines support science (and objectivity) for knowledge and practice. Using school psychology as an example, consideration is given to the presence of science and research in the scientist-practitioner, professional practitioner, and psychoeducational training and practice models. Although none of the three models truly ignores a commitment to science, the potential Achilles heel comes from the individual practitioner who fails to rely on science in planning, implementing, and evaluating interventions. Similarly, idiosyncratic preferences within training programs could lead to a diminution of science in the curriculum.
