In a recent article, Weisz, Weiss, and Donenberg (1992) compared the effects of child and adolescent psychotherapy in experimental studies and in studies of clinic practice. Here we update that report with new information and we explore 10 possible reasons why, to date, therapy in experiments appears to have shown larger effect sizes than therapy in clinics. We find that beneficial therapy effects are associated with three factors which are more common in research therapy than in clinic therapy: (a) the use of behavioral (including cognitive-behavioral) methods, (b) reliance on specific, focused therapy methods rather than mixed and eclectic approaches, and (c) provision of structure (e.g., through treatment manuals) and monitoring (e.g., through review of therapy tapes) to foster adherence to treatment plans. These three factors all involve dimensions along which clinic procedures could be altered.  相似文献

In order to clarify the place of research in psychotherapy and counselling, I look at the interrelationship of two ‘organizations of knowledge’. Organization can represent a process or an institution. My thesis is that there are particular characteristics of psychotherapy and counselling, considered as an intellectual discipline, that mean it organizes knowledge in peculiar ways that are reflected in the difficulty of reconciling them within the organization of the modern university. I shall propose that a different conception of research, based upon a third ‘organization of knowledge’, is necessary to resolve these tensions. Once psychotherapy/counselling finds more cohesion as a discipline, it is more likely to establish an independent niche within the university.  相似文献

The ability to recognize and respond to the ethical dimension of medicine is integral to providing health care that is comprehensive and humane. However, this aspect of medical practice is underemphasized in clinical and academic medicine, despite attempts to devise curricula in this field. This paper examines the origins and consequences of this deficiency through a case history of a Jehovah's Witness who reluctantly accepted a blood transfusion. It emphasizes the ubiquity of the ethical context in medicine and argues that blindness to this context stems from the prevailing scientific and technological paradigm in medicine. Innovations in medical education are called for to enhance health care providers' abilities to appreciate and cope with these complex situations.  相似文献

This paper will explore emerging issues in the practice of counselling and psychotherapy in the outdoors, which the authors encountered when they took their clients outside of the traditional therapy room. The outdoors is defined as natural areas and spaces, such as woods and parks which have been termed ‘nearby nature’ (Kaplan &; Kaplan, 1989 Kaplan, R. Kaplan, S. (1989). The experience of nature a psychological perspective. New York: Cambridge University Press [Google Scholar]) and also more remote areas such as mountains and moors which are more isolated from civilisation, what some have termed wilderness (Mcfarlane, 2007). Particular emphasis will be given to the ‘frame’ of psychotherapy and how aspects of this are affected by moving outdoors, in particular contracting in relation to confidentiality and timing. The relationship in psychotherapy will be explored in relation to issues of mutuality and asymmetry alongside the role of nature in the therapeutic process. Lastly the challenges and therapeutic potential of psychotherapy in nature will be explored.  相似文献

There is a long‐lasting dispute about development of students' creativity in the course of their university education. Both the duration and major field of study may represent the educational effects. To address the issue, the present study collected data (N = 859) from a series of surveys of students in Hong Kong to clarify educational effects by controlling a number of background characteristics and prior scores on creativity. Apart from measuring self‐reported creative traits and creative products, it measured divergent thinking with five tasks to elicit students' creative ideas, which led to scores of fluency, flexibility, novelty, innovativeness, and originality. Results indicate the trend of monotonic decline in creativity with years of study at university and the general superiority of verbal creativity among students of humanities and social sciences, whereas business students had the highest scores on self‐assessed creative traits and products.  相似文献

Drawing on literature from different psychotherapeutic traditions, the meaning of money is reviewed, and the psychological and practical issues which arise from the setting, changing and payment of fees in counselling and psychotherapy are discussed. Through a comparative analysis of the key ontological notions in Merleau‐Ponty and Nagarjuna, I develop a relational social ontology that is grounded in their respective implicit and explicit ethics. Both thinkers take heed of our being‐in‐the‐world; this is evident in their views on intersubjective sociality and language. Recognizing the limitations in these views points us toward a greater understanding of the meaningfulness of our situated existences. In this vein, I propose a number of ideas to guide the work of comparative philosophy.  相似文献

Some information processing models consider the selection of an action plan (deciding what to do) and the parameterization of the action (deciding how to do it) as separate stages in movement preparation. Further, these models consider the stages to occur in a fixed order, whereby the selection of an action precedes the parameterization of the action. Five experiments are reported that tested this position. The first four experiments adopted a variation of Rosenbaum's (1980) precuing technique such that, under varying conditions, different amounts and types of advance information were provided to the subject, leaving only the unspecified action or parameter to be supplied following the imperative signal. The critical finding from each study was that the time to specify an action and the time to specify a parameter were equivalent. In experiment 5, similar results were obtained using a response priming paradigm (Rosenbaum and Kornblum 1982). The results of all five studies failed to support a fixed-order model of movement preparation and are more consistent with a variable-order model. The implications of these findings for other models of movement preparation are discussed.  相似文献

Live identification procedures in England and Wales have been replaced by use of video, which provides a sequential presentation of facial images. Sequential presentation of photographs provides some protection to innocent suspects from mistaken identification when used with strict instructions designed to prevent relative judgements (Lindsay, Lea, & Fulford, 1991 ). However, the current procedure in England and Wales is incompatible with these strict instructions. The reported research investigated whether strict instructions would enhance the reliability of identification from video. The effect of using moving rather than still video clips was also investigated. Participants witnessed a live staged incident, and attempted to identify the culprit later from police video lineups, which were run double blind. Strict instructions produced a significantly lower rate of correct identifications in culprit present lineups, but did not significantly reduce the rate of mistaken identification in culprit absent lineups. Moving images yielded fewer mistaken identifications in culprit absent lineups. Copyright © 2006 John Wiley & Sons, Ltd.  相似文献

This paper presents findings from an intensive, mixed methods case study of one session of psychoanalytic parent–infant psychotherapy (PPIP) addressing early relational trauma, and aims to shed light on the multimodal interactive processes that take place in the moment-to-moment exchanges comprising the therapeutic encounter. Different research methods were used on video material from PPIP sessions, including microanalysis of adult–infant interactions, discourse analysis of talk, and coding systems developed to study parent–infant interaction. These different perspectives were brought together with the clinical narrative to illuminate the complex, dynamic processes of parent–infant–therapist interaction. More specifically, the detailed analysis of one interactive episode revealed brief behavioral manifestations of fearful and disoriented states of mind, reflecting dysregulated interaction between mother and infant, which also powerfully affected the therapist. The processes through which the therapist gradually resolves this rupture are also described in detail. Through this pilot study, we were able to show that it is possible to systematically study the process of PPIP. The study contributes to the growing psychotherapy research literature that takes into account both the verbal domain and implicit, interactional processes in therapeutic practice, and underscores the therapist's comprehensive engagement in the therapeutic process.  相似文献

This study examined the extent to which curriculum-based measurement (CBM) procedures could be implemented in a nonbasal reading curricula. Participants included 160 students from 31 second, third, fourth, and fifth grades, located in two school districts. Half of the students (20 from each grade) were instructed primarily in a literature-based reading series, while the remaining 80 participants (20 from each grade) were instructed primarily in a traditional skills-based reading program. CBM passage probes from each reading series were administered to all students twice weekly over an 8-week period. Students' rate of progress in each reading series was indexed using the slope of their data series' calculated by ordinary least squares regression. Results showed small yet significant main effects for the type of probe used for progress monitoring; however, this effect was not consistent across grades. In addition, significant main effects were found for grade. Students' growth in oral reading rate was found to increase linearly with grade until fifth grade where a leveling off in growth rate was observed. Results are discussed in relation to CBM as an applied measurement methodology for use in both practical and research applications.  相似文献

Although shame is a central affect running through all phases of psychosexual and social development, it is usually masked by guilt and therefore it is not readily recognised, explored, and understood within the therapeutic situation. Moreover, there is a tendency to treat all shame manifestations as if they operate at the same level. The author proposes the need to distinguish between two qualitatively discrete manifestations of shame states which, albeit intertwined, operate at different levels and require different understanding and technique: a primary, unconscious kind based on psychobiological survival and triggered by a condition of psychic and physical danger, and a secondary, social shame, mainly conscious, based heavily on vision and evoked in social situations. The natural, primary form of shame becomes pathological after catastrophic chronic exposure of the primitive ego to unthinkable anxieties. Such premature rupture of primary skin containment may result in omnipotence-based pathological organisations impeding or precluding acceptance of guilt and need for reparation. Pathological primary shame predisposes the individual to states of pathological secondary shame. When initial traumatic conditions are re-activated and re-experienced in therapy, they may trigger re-enactments and, possibly, a negative therapeutic reaction. Recognising variations and mixed states of primary and secondary shame states, especially when shame is compounded with guilt, can provide guidance in the assessment of the fragility of the ego, and therefore inform our technique and the therapeutic process. This theoretical position is discussed with the help of clinical material from a twice-weekly psychotherapy of a 16-year-old boy imbued with shame compounded with guilt, related to transgenerational objectification and dehumanising experiences.  相似文献
