A pilot intervention that emphasized training and technical assistance to promote warm, sensitive, and responsive one‐on‐one caregiver–child interactions primarily during feeding and bathing/changing was implemented using regular staff in a depressed orphanage for children birth to approximately 8 years of age in Latin America. Despite a variety of unanticipated irregularities in the implementation of the intervention, many beyond the researchers' control, ward environments improved; caregivers displayed more warm, sensitive, and responsive interactions with children; and children improved an average of 13.5 developmental quotient (DQ) points after 4+ months' exposure to the completed intervention. Furthermore, 82% of the children had DQs greater than 70 before the intervention, but only 27.8% did so afterward. Although the training for all caregivers was aimed at children birth to 3 years, the number of different caregivers was reduced, and technical assistance was provided only to caregivers serving children less than 3 years, younger and older children (3–8 years) improved approximately the same amount. However, children who were transitioned from a younger to an older ward during the intervention improved less than did children who remained in either a younger or an older ward, the first evidence suggesting that the common orphanage practice of periodically graduating children from one homogeneous age group to another may impede their development. The study is consistent with others that have shown that orphanages can be changed, and increases primarily in warm, sensitive, responsive caregiver–child interactions can produce improvements in children's development.  相似文献

Children in two institutions in St. Petersburg (Russian Federation) experienced ward transitions, one in which caregivers were trained to provide sensitive, responsive caregiving and one that conducted business as usual. A third institution eliminated transitions, received the same training, and implemented a variety of structural and employment changes designed to promote improved caregiver–child interactions and relationships. While the no‐transition comprehensive intervention group of children steadily improved in Battelle Developmental Inventory (LINC Associates, 1988 ) scores across all age intervals, the children in the institution who encouraged some positive caregiver–child interactions improved before and after, but not during, an age period that involved a transition. In contrast, the no‐treatment group displayed no developmental changes across any age period with or without a transition. These results suggest that the common institutional practice of ward transitions to new peers and caregivers is potentially disruptive to infants' and toddlers' general development, but primarily in a context in which some degree of caregiver–child sensitive and responsive interactions are encouraged.  相似文献

This study examined the effects of the Bayley Scales of Infant Development (BSID) version on developmental outcomes among drug-exposed children, some of whom received an intervention. Developmental outcome was evaluated with the BSID at 12 and 18 months and with the BSID-II at 24 and 30 months. In the repeated measures analyses, children scored higher on the BSID Mental Developmental Index (MDI; p < .01) and Psychomotor Developmental Index (PDI; p < .01) than on the BSID-II MDI and PDI. Version x Time (p < .01) and Version x Group (p < .01) interactions were also found for the MDI but not the PDI. PDI scores decreased on both the BSID and the BSID-II (p < .01). Over the first 2 years postpartum, mean MDI and PDI scores decreased among these high-risk, drug-exposed children.  相似文献

The study focused on stability and prediction of parenting stress experiences over a 6‐year period. Mothers (N=93) who had received a clinical intervention for feeding or sleeping problems during infancy (Time 1; T 1) were followed‐up when the children were 5–10 years old (Time 2; T 2). An age‐ and sex‐of‐child matched normal group was used for comparison of stress levels at T 2. Parenting stress was measured by the Swedish Parenthood Stress Questionnaire, which consists of a general parenting stress scale and sub‐scales tapping different aspects of parenting stress experiences. T 1 predictors were clinical assessments of child problem load, maternal unresponsiveness, and family psychosocial problems. T 2 predictors were mother‐reported concurrent child problem load and psychosocial problems. The individual stability in stress experiences was moderate. Effect sizes indicated that mothers with early clinical contacts had reduced their stress to levels close to those in the normal sample. Parenting stress at T 2 could be predicted from early and from concurrent child and family problems. The results point to the relevance of early clinical assessments and to the importance of a sub‐area approach in parenting stress research, as there were differences between stress sub‐areas regarding both prediction and stability. We tested the capacity to perceive visual expressions of emotion, and to use those expressions as guides to social decisions, in three groups of 8‐ to 10‐year‐old Romanian children: children abandoned to institutions then randomly assigned to remain in ‘care as usual’ (institutional care); children abandoned to institutions then randomly assigned to a foster care intervention; and community children who had never been institutionalized. Experiment 1 examined children's recognition of happy, sad, fearful, and angry facial expressions that varied in intensity. Children assigned to institutional care had higher thresholds for identifying happy expressions than foster care or community children, but did not differ in their thresholds for identifying the other facial expressions. Moreover, the error rates of the three groups of children were the same for all of the facial expressions. Experiment 2 examined children's ability to use facial expressions of emotion to guide social decisions about whom to befriend and whom to help. Children assigned to institutional care were less accurate than foster care or community children at deciding whom to befriend; however, the groups did not differ in their ability to decide whom to help. Overall, although there were group differences in some abilities, all three groups of children performed well across tasks. The results are discussed in the context of theoretical accounts of the development of emotion processing.  相似文献

Current research in child development has espoused the benefit of family-like routines in institutional orphanage care. However, the institutional framework evident in large-group orphanage care often hampers the creation of nurturing, family-like environments. This qualitative study is part of a larger case study exploring how one private Chinese orphanage infuses a family-like structure into an institutional setting. Data is examined from an ecocultural viewpoint, looking at how caregivers and directors’ attitudes and behaviors reflect universal caregiving practices and how they reflect the distinct cultural forces within this private Chinese orphanage. Data came from conversation-like interviews with the director, co-director, and two direct caregivers. This information was combined with observations that resulted from me living within the orphanage for a period of ten days. Results indicate the family-like environment was created primarily through the staff’s articulation of and identification with family roles (father, mothers, brothers, sisters rather than director, co-director, caregivers, and children). Family-like roles often involved levels of intimacy not typically found in institutional settings such as the culturally accepted practice of co-sleeping with younger children. Results are presented in textual format followed by implications for institutional frameworks.  相似文献

Early, intensive behavioral intervention is effective in treating children with autism spectrum disorders (ASDs), but can be cost prohibitive. Expenses may be defrayed if children can benefit from parents acting as therapists. This quantitative case series examines the efficacy of the Group Intensive Family Training (GIFT) program, a 12‐week (180 h, delivered 3 h each weekday) parent‐training for preschoolers with ASDs. Parents were individually mentored in the hands‐on application of behavior analytic techniques, implementing these skills in vivo within a group of six parent–child dyads. Seventy‐two parents and children (ages 25–68 months) with ASDs participated in this study. Children's cognitive and adaptive functioning was assessed before and after the intervention program. Analyses revealed average gains of eight standard score points on the Mullen Early Learning Composite and five standard score points on the Vineland Adaptive Behavior Composite after 12 weeks of treatment. Additionally, 14% and 11% of the children moved from the ‘impaired’ to ‘non‐impaired’ range on Mullen and Vineland composite scores, respectively. This preliminary investigation suggests that GIFT's behavioral, group parent‐training can lead to significant, yet cost‐ and time‐efficient gains for children with ASDs. Eighty 4‐ to 9‐year‐old children answered factual knowledge questions in math, science and social studies during one‐on‐one interviews. Children indicated whether they had known or guessed each answer, and whether they (a) remembered the moment they learned the answer (episodic response) or (b) did not remember. For episodic responses, children provided memory narratives of learning episodes. One third of children's responses identified a learning episode. There was a developmental trend in which older children were more episodic than younger children, and when children knew and provided correct answers, there was a gender difference in which females were more episodic than males. The impact of child abuse on the developmental functioning of infants was investigated. Thirty verified cases of physically abused children were compared to a reference group of 30 nonabused children matched for age, sex, race, and socioeconomic status. Abused children scored significantly lower in terms of cognitive and motor development as measured by the Bayley Scales of Infant Development. Developmental delays on three of the four sectors of the Denver Developmental Screening Test, personal-social, language, and gross motor, were also found in the abused children. There were, however, relatively few item differences between the two groups on the 30 more general behavioral variables constituting the Bayley Infant Behavior Record. Results appear to confirm clinical observation of abused children as developmentally retarded with specific delays in the language and gross motor areas. Although methodologically complex, longitudinal studies are clearly indicated to assess the stability and/or reversibility of the present findings.The author is indebted to John E. This study examined the effects of adult imitation and adult playfulness on the imitation, social attention and initiation of new behaviours by non‐verbal preschoolers with autism. Videotapes taken from a previous study were recoded for the adult's imitation and playful behaviour and the children's imitation, social attention (looking at the adult's actions) and initiation of new behaviours. In the original study, twenty non‐verbal, 4‐ to 6‐year‐old children with autism were randomly assigned to an imitation or a contingent responsivity group. Both groups of children engaged in an intervention phase (during which the adult imitated the children or contingently responded to them) and a subsequent spontaneous play phase (during which the adult interacted spontaneously with the children). ANOVA for the current study revealed that the imitation group children versus the contingent responsivity group children spent a greater percent time showing social attention and initiating new behaviours during the intervention phase and showing social attention and imitating the adult's behaviours during the subsequent spontaneous play phase. A correlation analysis yielded significant correlations between the percent time the adult imitated the child during the intervention phase and the percent time the child showed social attention during the same intervention phase and imitating the adult during the subsequent spontaneous play phase. Background. Development of socio‐emotional competencies is key to children's successful social interaction at home and at school. Aims. This study examines the efficacy of a UK primary school‐based intervention, the Pyramid project, in strengthening children's socio‐emotional competencies. Sample. Participants were 385 children from seven schools in two UK cities. All children were aged 7–8 years and in school Year 3. Children were screened for socio‐emotional difficulties through the Strengths and Difficulties Questionnaire (SDQ; Goodman, 1997 ) and a multi‐agency meeting of relevant professionals before being allocated to attend a Pyramid Club intervention (n= 103) or a comparison group (n= 282). Method. A 2 × 2 mixed‐model design was used: group (intervention group vs. comparison group) × 2 time points (pre‐ vs. 12 weeks post‐intervention) with repeated measures on the time factor to investigate the impact of the Pyramid Year 3 intervention. Teachers completed the SDQ‐T4‐16 for all children pre‐ and post‐intervention to measure participants’ socio‐emotional health status. Results. As measured by the two SDQ difficulty sub‐scales of Emotional and Peer problems and the SDQ strength sub‐scale of Prosocial behaviour, post‐intervention improvements in the Pyramid attendee group were of greater magnitude than those of the comparison group. Conclusions. The Pyramid project intervention improves the socio‐emotional health of vulnerable children through promoting positive outcomes as well as reducing socio‐emotional deficits. These findings further support the inclusion of a salutogenic approach in promoting children's socio‐emotional well‐being.  相似文献

Underlying deficits in self‐regulation and sensory processing are seen in children with regulation disorders (RD) and might lead to emotional and behavioral problems as the child develops. However, little is known about the specific developmental course of RD. This follow‐up study was conducted to investigate the development of a clinical sample of RD children, diagnosed by means of the Diagnostic Classification of Mental Health and Developmental Disorders of Infancy and Early Childhood, Revised (DC:0–3R; ZERO TO THREE, 1994), toward specific psychopathology 4 to 10 years later based on parent‐ and teacher‐reports on the Child Behavior Checklist (T.M. Achenbach & L.A. Rescorla, 2007). Parental reports showed that 39 to 69% of children with RD had internalizing, externalizing, and total problems above borderline cutoffs, as compared to norm group data (16%). In addition, higher rates of affective, anxiety, attention deficit/hyperactivity, oppositional defiant, and conduct problems were reported. Analyses between RD subtypes showed significant differences on future attention problems and rule‐breaking behavior, wherein less favorable results were found for the Hypersensitive subtype Type A (fearful/cautious) in comparison to the sensory stimulation‐seeking/impulsive subtype. The current results indicate persistence of emotional and behavioral problems into middle childhood and adolescence in children with preschool RD diagnoses. More attention should be paid to differentiation of psychopathology in these children since developmental outcomes may differ between RD subtypes.  相似文献

Omega‐3 supplementation has been found to reduce externalizing behavior in children. Reciprocal models of parent‐child behavior suggest that improving child behavior could lead to improvements in parent behavior, however no study has examined whether omega‐3 supplementation in children could reduce intimate partner violence or child maltreatment by their adult caregivers. In this randomized, double‐blind, placebo‐controlled, stratified, parallel group trial, a community sample of children were randomized to receive either a fruit drink containing 1 gm of omega‐3 fats (Smartfish Recharge; Omega‐3 group, n = 100) or the same fruit drink without omega‐3's (Placebo group, n = 100). Child participants, adult caregivers, and research staff were blinded to group assignment. Adult caregivers reported inter‐partner and child‐directed physical assault and psychological aggression at baseline, 6 months (end of treatment) and 12 months (6 months post‐treatment) using the Conflicts Tactics Scale. Caregivers of children in the omega‐3 group reported long‐term reductions in psychological aggression in a group × time interaction. Improvements in adult psychological aggression were correlated with improvements in child externalizing behavior scores. No differences were reported for child maltreatment. This study is the first to show that omega‐3 supplementation in children can reduce inter‐partner psychological aggression among adult caregivers not receiving supplements. Findings suggest that improving child behavior through omega‐3 supplementation could have long‐term benefits to the family system as a whole.

Objective: We describe a preventive short‐term group intervention with nine single‐by‐choice (SBC) mothers to provide maximal support for parental functioning and to minimize possible emotional and/or developmental difficulties in their children. Method: Dynamically oriented group work (fifteen one‐and‐a‐half‐hour sessions) focused on: elaboration of painful experiences in the peri‐natal period; reducing stress, tension and guilt; helping mothers with problematic aspects of parenting through work on parental self‐image and perceptions of the child and the dyadic interaction; and strengthening their acceptance of the chosen family model. Results: Therapeutic gains described by mothers and facilitators include: reduced tension, anxiety and guilt; improved integration of the mother's parental self‐image and perception of the child; reduced ambivalence in dyadic relationships; strengthening the mother's fantasized triadic relationships; better acceptance of chosen family pattern; mothers' willingness to tell children their birth story. Conclusion: Dynamically oriented preventive group intervention with SBC mothers can identify potential psychological risk factors and help mothers with sensitive aspects of parenting.  相似文献

A randomized trial compared effects of a Family Critical Time Intervention (FCTI) to usual care for children in 200 newly homeless families in which mothers had diagnosable mental illness or substance problems. Adapted from an evidence‐based practice to prevent chronic homelessness for adults with mental illnesses, FCTI combines housing and structured, time‐limited case management to connect families leaving shelter with community services. Families were followed at five time points over 24 months. Data on 311 children—99 ages 1.5–5 years, 113 ages 6–10 years, and 99 ages 11–16 years—included mother‐, teacher‐, and child‐reports of mental health, school experiences, and psychosocial well‐being. Analyses used hierarchical linear modeling to investigate intervention effects and changes in child functioning over time. Referral to FCTI reduced internalizing and externalizing problems in preschool‐aged children and externalizing for adolescents 11–16. The intervention led to declines in self‐reported school troubles for children 6–10 and 11–16. Both experimental and control children in all age groups showed reductions in symptoms over time. Although experimental results were scattered, they suggest that FCTI has the potential to improve mental health and school outcomes for children experiencing homelessness.  相似文献

Over time, developmental theories and empirical studies have gradually started to adopt a bidirectional viewpoint. The area of intervention research is, however, lagging behind in this respect. This longitudinal study examined whether bidirectional associations between (changes in) parenting and (changes in) aggressive child behavior over time differed in three conditions: a child intervention condition, a child + parent intervention condition and a control condition. Participants were 267 children (74 % boys, 26 % girls) with elevated levels of aggression, their mothers and their teachers. Reactive aggression, proactive aggression and perceived parenting were measured at four measurement times from pretest to one-year after intervention termination. Results showed that associations between aggressive child behavior and perceived parenting are different in an intervention context, compared to a general developmental context. Aggressive behavior and perceived parenting were unrelated over time for children who did not receive an intervention. In an intervention context, however, decreases in aggressive child behavior were related to increases in perceived positive parenting and decreases in perceived overreactivity. These findings underscore the importance of addressing child-driven processes in interventions aimed at children, but also in interventions aimed at both children and their parents.  相似文献

We report on rapid perceptual learning of intonation contour categories in adults and 9‐ to 11‐year‐old children. Intonation contours are temporally extended patterns, whose perception requires temporal integration and therefore poses significant working memory challenges. Both children and adults form relatively abstract representations of intonation contours: Previously encountered and novel exemplars are categorized together equally often, as long as distance from the prototype is controlled. However, age‐related differences in categorization performance also exist. Given the same experience, adults form narrower categories than children. In addition, adults pay more attention to the end of the contour, while children appear to pay equal attention to the beginning and the end. The age range we examine appears to capture the tail‐end of the developmental trajectory for learning intonation contour categories: There is a continuous effect of age on category breadth within the child group, but the oldest children (older than 10;3) are adult‐like.  相似文献

The paper examines the effect of malnutrition on the developmental functioning of young Indian children. A total of 117 children between the ages 0 to 6 years attending the anganwadis of Chandigarh city were recruited. The Indian Developmental Inventory was used to assess developmental functioning. Anthropometric evaluations were done by a pediatrician and these included height and weight. Nearly 30 % of children of the study population were found to be stunted and 22 % were wasted. There were significant differences between the 3 groups of children on developmental functioning: Social DQ (F?=?3.46, P?=?.036); Communication DQ (F?=?3.16, P?=?.047); and Total DQ (F?=?5.17, P?=?.007). Children with chronic malnutrition had the most impaired developmental scores. The problem of poor child development urgently needs to be tackled with child nutrition, early stimulation, and development programmes.  相似文献

Stepping Stones Triple P (SSTP) is a parenting program designed for families of a child with a disability. The current study involved a randomized controlled trial of Group Stepping Stones Triple P (GSSTP) for a mixed‐disability group. Participants were 52 families of children diagnosed with an Autism Spectrum Disorder, Down syndrome, Cerebral Palsy, or an intellectual disability. The results demonstrated significant improvements in parent‐reported child behavior, parenting styles, parental satisfaction, and conflict about parenting. Results among participants were similar despite children's differing impairments. The intervention effect was maintained at 6‐month follow‐up. The results indicate that GSSTP is a promising intervention for a mixed‐disability group. Limitations of the study, along with areas for future research, are also discussed.  相似文献
