This study investigated the relationship between physical and mental health and psychosocial variables and recent (within
the last 12 months) mental health service use among 240 medical patients recruited from general and specialty outpatient clinics
at an academic medical center. Results indicated 43.3% of the participants had recently received mental health services in
the form of psychotropic medication (75%), psychotherapy (2%), or a combination of these treatments (20.2%). Among patients
with moderate to severe symptoms of anxiety or depression, approximately two-thirds were receiving mental health treatment.
Moreover, four variables (healthcare provider referral for mental health services, perceived need for mental health services,
prior use of mental health services, and frequency of medical appointments) were significant unique predictors of recent mental
health service use. This suggests collaborative/integrated medical care may increase needed mental health service use. 相似文献

Psychologists in academic health systems are in a unique position to impact the dynamic interplay among research, clinical practice, and policy domains. In addition to their specialty expertise in psychopathology, particular types of physical illness and disease, and health promotion, psychologists have substantial knowledge of human behavior, refined scientific skills, knowledge and experience in the effective delivery of clinical services, strengths in written expression, and a collaborative orientation. This combination of strengths makes psychologists extremely well-suited for (a) interdisciplinary efforts to develop and implement evidence-based assessment and intervention strategies with biological, psychological, behavioral, and social components, and (b) leadership and advocacy efforts that impact the quality and availability of health care services. 相似文献

The American Occupational Therapy Association's Centennial Vision advocates for an emphasis on intervention and prevention services for children, young people, the aging, and those with severe and persistent mental illness. This article discusses mental health issues among adolescents in foster care, a population that would greatly benefit from occupational therapy services emphasizing skill development in the areas of independent living, vocation, and health. Additionally, current intervention modalities used by other disciplines with adolescents in foster care are explored. Finally, a pilot study demonstrating the opportunities for occupational therapists to influence the long-term outcomes of adolescents in foster care is described. 相似文献

Mounting evidence indicates that there are mental health disparities in the United States that disadvantage racial/ethnic minorities in medical and mental health settings. Less is known, however, about how these findings apply to a particularly vulnerable population, individuals with severe mental illness (SMI). The aim of this paper is to (1) provide a critical review of the literature on racial/ethnic disparities in mental health care among individuals with SMI; (2) identify factors which may contribute to the observed disparities; and (3) generate recommendations on how best to address these disparities. Specifically, this article provides an in-depth review of sociocultural factors that may contribute to differences in treatment engagement and rates of attrition from treatment among racial/ethnic minorities with SMI who present at medical and mental health facilities. This review is followed by a discussion of specific strategies that may promote engagement in mental health services and therefore reduce racial/ethnic disparities in SMI. 相似文献

During the 1950s and 1960s professionals intensely debated the prospect of changes in the mental health system, largely as a result of the introduction of new therapies, revised theories of mental disorder, and shifting policies governing mental health accommodation. As well as giving rise to new subspecialties within medicine, psychiatrists in some jurisdictions at this time worked closely with psychologists in an attempt to offer a more comprehensive set of options that merged theory with practice. In Saskatchewan, mental health professionals worked closely with government officials and bureaucrats and produced a variety of innovative strategies that addressed changing priorities in this system. This article examines the role played by psychologists in Saskatchewan during this period as they worked cooperatively with psychiatrists and bureaucrats to merge medical, psychological, and political perspectives in a system aimed at accommodating mental illness in the wake of new theories and treatments that questioned the efficacy of care in institutionalized settings in the wake of growing suggestions for care in the community. 相似文献

Concerns for the integrity of psychology as an independent discipline have caused some psychologists to object to introducing
any knowledge from the biological sciences into the training of psychologists. However, calls for the greater incorporation
of the behavioral sciences in medical education, increased attention to research on the mechanisms of bio-behavioral interaction,
and initiatives in translational medical research and clinical care, have prompted increased interest in interdisciplinary
research, health care, and teaching. These changes, in turn, are resulting in a re-conceptualization of the structure of academic
medicine with increasing emphasis upon multidisciplinary knowledge and interdisciplinary collaboration, and less emphasis
upon disciplinary insularity and competitiveness. If clinical health psychology is to play a role in this evolving concept
of academic health care, it must adequately prepare its trainees to function in interdisciplinary academic health care settings.
This will require not only expertise in the role of behavioral factors relevant to medical disorders, but also some basic
familiarity with the biological processes to which those behavioral factors relate. With the evolution of its fund of knowledge,
clinical health psychology has the potential to utilize its science to discover, describe, interpret, teach and clinically
apply knowledge of the mechanisms of interaction between biological functions and behavioral, learning, cognitive, socio-cultural
and environmental processes. By failing to seize this initiative, clinical health psychology risks becoming irrelevant to
the evolving model of medical research, education and health care.

Addictive disorders among the elderly have emerged as a growing public health concern. As the proportion of the elderly population increases, more and more older adults will either develop addictions as a dysfunctional means of coping with the psychosocial consequences of aging, or will carry their long-standing addictive behaviors with them into later life. Among the most common of these addictions are smoking, excess consumption of alcohol, and gambling. This article briefly reviews these three addictive disorders and examines assessment and treatment options. The current cohort of older adults tends not to seek help for addiction problems in specialty mental health or substance abuse treatment. To improve rates of cessation and abstinence, assessment and intervention should be delivered in general medical settings such as primary care. With the addition of a behavioral health specialist, primary care has the potential to offer improved interventions in a cost-effective and time-efficient manner. 相似文献

This article explores the development of psychology as a profession in Thailand, its challenges, and future trends. There are currently very few Thai psychologists who are active in the medical fields in comparison to other health care professions. Being held back by limited education and training, many psychologists see their future career as less than promising. The lack of professional licensing standards and dominance of psychiatry further limit their career advancement. However, tremendous opportunities await those who persevere to expand their roles beyond the traditional assessment, treatment, and research models. A small number of them have ventured out into unexplored territories such as behavioral medicine, AIDS counseling, health promotion, and prevention of mental health problems. There is a strong potential for Thai psychologists to make significant contributions in these areas. It is clear that higher standards of education and more rigorous training are needed if this goal is to be realized in the near future. 相似文献

Citizenship is an approach to supporting the social inclusion and participation in society of people with mental illnesses. It is receiving greater attention in community mental health discourse and literature in parallel with increased awareness of social determinants of health and concern over the continued marginalization of persons with mental illness in the United States. In this article, we review the definition and principles of our citizenship framework with attention to social participation and access to resources as well as rights and responsibilities that society confers on its members. We then discuss our citizenship research at both individual and social‐environmental levels, including previous, current, and planned efforts. We also discuss the role of community psychology and psychologists in advancing citizenship and other themes relevant to a citizenship perspective on mental health care and persons with mental illness. 相似文献

Professor Frank Dattilio's article “The Self‐care of Psychologists and Mental Health Professionals” provides an overview of stress and related mental health problems among psychologists, and a proposition that psychologists are not vigilant in regard to self‐care. Dattilio offers a range of self‐care strategies and recommendations, and highlights self‐care practices within various psychology frameworks, and concludes with some “healthy tips” for managing stress. In my commentary I underscore Dattilio's message that self‐care is of critical importance in psychology practice, given the responsibility of caring for others inherent in the work psychologists undertake. However, I raise additional points of consideration and suggest an alternative approach to addressing the self‐care needs of the profession. My commentary makes the following points: (a) the need to distinguish between psychology trainees and practising qualified psychologists when addressing stress and self‐care requirements in the profession; (b) the importance of developing a culture of self‐care among psychologists by providing self‐care instruction during training; (c) the need to temper research findings on stress and mental health among psychologists by the methodological weakness of the studies in this area; (d) adhering to the recent call from colleagues to shift from a focus on pathology and punishment to a positive acceptance, mindfulness, and values‐based approach for encouraging self‐care among psychologists; (e) the use of a systematic framework for organising the presentation of self‐care strategies that makes them more accessible; and (f) an appeal to professional bodies to take their responsibility in promoting self‐care in the profession. 相似文献

U.S. population demographics are undergoing striking changes that will impact health care and the research and practice of health psychology. An increase in the number of people who are older; belong to an ethnic minority group; have disabilities; identify as lesbian, gay, bisexual, or transgendered; or live in poverty will influence definitions of aging, health, and illness, and will challenge current psychological and medical treatment models. The authors argue that health psychologists need to become context competent for the field to be relevant and viable over the course of this new century. Health psychologists need to become aware of the multiple, overlapping contexts in which people live and apply this knowledge on a regular basis to research, practice, education and training, and policy in health psychology. 相似文献

ABSTRACTIncreasingly, there is evidence of the potential benefits of an integrated care model. In fact, the American Psychological Association (APA) supports the role of psychologists in integrated healthcare given the positive outcomes for patients in primary care settings such as increased access to mental health services, reduced mental illness stigma, and improved health associated with recognizing the impact of psychosocial factors on physical wellbeing. One of the reasons integrated care has not become a dominant service delivery model is the unmet training agenda. This article
argues that the typical mental health professional is not trained to adequately address the challenges of integrated care.
To insure competency both a macro and clinical training agenda are needed. At the macro-level, mental health professionals
need to understand healthcare economics and basic business principles as any integrated care service delivery system is embedded
and driven by economic forces. Integrated care practitioners also need some basic business skills to understand these forces
and to create and manage a financially viable system, given the future flux of the system. Traditional mental health professionals
also do not have the clinical skills to implement integrated care. Integrated care is not simply placing a traditionally trained
mental health professional and letting them practice specialty mental health in a medical setting. Thus, the special skills
needed in integrated care are enumerated and discussed. Finally, a new degree program is described as it is time given the

This paper describes the Bureau of Health Professions (BHPr) Graduate Psychology Education program (GPE), which supports projects that train health service psychologists for work with underserved populations. BHPr history and funding criteria are discussed, as are those of BHPr's parent organization, the Health Resources Service Administration. BHPr objectives and methods for support of clinical psychology training parallel those that BHPr has used to support training in other heath professions. The paper also describes three psychology internship training programs in academic medical settings that competed successfully for BHPr GPE funding in 2002. The three training projects differ significantly in training rotation sites, target populations with which trainees work, and the other health care professions that partner with psychology in interdisciplinary training—but they are similar in that each project provides an example of a program that effectively satisfied BHPr criteria for expanding psychology's scope of practice with underserved populations. 相似文献

A number of studies have consistently reported that there is a greater prevalence of mental illness among the most socioeconomically disadvantaged. At the same time, there is evidence that services are not optimally accessed by the most socioeconomically disadvantaged; the most in need of care are also the most likely to have unmet healthcare needs. Of people with mental illnesses, those with severe mental illnesses (SMI) are the most at risk of poverty and the least likely to have optimal care. In the past, specialized community mental health services have been identified as the primary provider for people with SMI. However, there is growing interest in using the primary care setting as the main source of mental health care where both medical treatment and psychotherapy can be accessed. In this paper, we examine factors related to primary care use (and in turn, pharmacologic and psychotherapies) for people who are socioeconomically disadvantaged and who have a SMI. 相似文献

The origin and development of counselling psychology in South Africa has been profoundly influenced by the country’s sociopolitical history and the impact of apartheid. As a result of this, counselling psychologists in the country face a number of challenges and opportunities for the future. In this paper we provide a portrait of counselling psychology in South Africa by describing the current character of the specialty and the context in which South African psychologists work. We critically discuss the challenges that the specialty faces to meet the country’s mental health care needs, contest the current Scope of Practice; affirm multiculturalism without essentialising or reifying race and ethnicity, and build an evidence base for community interventions in the country. We also consider how, in the future, counselling psychologists in South Africa may make a more meaningful contribution within public health and the country’s health care and education systems. 相似文献
