桡骨小头粉碎性骨折的两种治疗方法比较分析

摘    要:分析比较切开复位内固定和人工桡骨小头置换方法治疗桡骨小头粉碎性骨折(MasonⅢ型)的临床疗效。回顾分析2012年10月~2014年3月收治的24例桡骨小头粉碎性骨折(MasonⅢ型)的临床资料,其中切开复位内固定治疗17例;桡骨小头置换7例,平均随访12个月(6个月~18个月)。根据Broberg Morrey评分系统以及术后影像进行评价比较,同时比较两组住院天数以及费用。结果评价比较显示两者疗效均佳,术后两者活动度和功能评分相近,两组住院天数相近,内固定组费用更低。桡骨小头粉碎性骨折须尽早进行解剖复住功能重建,切开复位内固定和桡骨小头置换疗效相当。

The Analysis and Comparison of Two Methods in the Treatment of Comminuted Radial Head Fractures

Abstract:The purpose of this study was to evaluate the treatment of patients with Mason type HI radial head fracture by open reduction with internal fixation or artificial joint replacement. A small retrospective study was done on data of 24 patients with radial head fractures(Mason Ⅲ) treated in our department from October 2012 to March 2014. Fractures with open reduction and internal fixation (ORIF) in 17 cases (group 1), compared with 7 cases where radial head prosthesis was performed (group 2). The mean clinical and radiological follow-up was 12 months (6 to 18). Also group 1 was compared with group 2 in the number of days of hospitalization and costs. The Broberg-Morrey index showed excellent results in both groups. Post-operative evaluation of these two different techniques revealed similar ranges of movement and functional scores. Both groups showed equal time of hospitalization, and the costs of group 1 were lower than group 2. The radial head fractures should be given anatomical reduction for early functional exercise. ORIF and arthroplasty have the equal therapeutic effect.
