This article is the third part of an originally unplanned trilogy caused by the internal logic of the research process on psychosomatic theories. On the background of Freud’s difficulties with the illness and disease theory the fragmentation of the field of psychosomatic medicine has been described and presented in the two previous parts as an attempt to explain its tortuous developmental course. In the first part of this current paper the author tries to understand both the ideas and sociopolitical forces that were involved in the historical development of psychosomatic medicine. The scope of the field as well as the models used oscillated greatly. Psychoanalysis has played an important role in this development even if the mainstream psychoanalysts showed relatively little interest. Psychosomatics also required therapeutic approaches that were foreign to the accustomed psychoanalytical setting. Occasionally the influence of various physiological models was strongly displayed (e.g. Selye’s stress model and the influence of the immune system). The deepening of the psychological understanding is nevertheless due to original marginal psychoanalytic thinkers, such as Ferenczi and Groddeck. Under the influence of Ferenczi the unconventional search was completed by Balint to some extent either through development of the psychotherapeutic technique or by the influence of listening to the doctors in their interaction with patients. 相似文献

It is held that specific ingredients are responsible for the effectiveness of various psychotherapies and it is supposed that for this reason particular therapeutic approaches are more efficient than others. First, this article presents Bruce E. Wampold’s research on psychotherapeutic efficacy, which gives evidence that psychotherapies efficiency is due to general, not to specific factors. It is shown that in the end the common factors like alliance, compliance, empathy and the fact, that therapists generally make best use of their personality, are interpersonal factors. The impact of these empirical results on psychotherapy, on theory and practice is described in the light of two metatheories, the “medical modell”and the “contextual model”. The author delineates some consequences, for instance that in psychotherapeutic practice the diagnosis-based indication for therapy has to be replaced by fitting-indication. Regarding outcome research, the randomized goldstandard research design has to be replaced by a fitting indication concerning therapist /patient and patient/paticular treatment. Further it is shown that although specific ingredients in a strict sense are not active, therapists have to realize that they are nessessary even if active only as a component of the healing context. How specific ingredients might be realized within a healing context is shown with regard to the adaptive approach to psychoanalysis. 相似文献

The narrow concepts of illness insight and compliance in psychiatry and psychotherapy will be discussed, not least because they contradict the dynamics of psychosis. Demands on therapeutic relations will be deduced from the therapeutic relationship between psychosis and ownness as well as of the experiences of untreated patients. The particular prospects of the accompaniment of a trained peer counsellor during convalescence will be represented and explained by the experiences of the Hamburg psychenet project. Psychotherapy and psychoanalysis form a contrast – in particular in German-speaking countries – comparable to that between education and “Bildung”: psychotherapy and education intentionally and purposefully influence the patient or pupil whereas psychoanalysis and “Bildung” pursue the ideal of a development process which is to a great extent free from outside influences. The present paper aims to show that these seemingly contrasting pairs have been artificially differentiated into extremes. Both are supposed to solve the following paradoxes: the educational paradox “How is freedom possible under constraint?” and the psychoanalytical paradox “How is independence possible under dependency?” It might, however, be possible to overcome the unfruitful contrast in both cases and to keep the feared paradox in abeyance. 相似文献

After a long time of growth current studies on representative samples of adolescents indicate that the number of adolescent cannabis users in Germany is possibly decreasing. However, it may be presumptuous to expect a similar trend concerning the number of clients with cannabis-related disorders. Even if there is in fact a downward trend which is not yet proven, the rapidly growing number of clients presenting with these disorders in recent years cannot be explained by a similar growth in the number of regular cannabis users. Treatment centres should therefore expect a further inreasing number of especially male adolescents and young adults seeking help for problems related to cannabis use. The development, evaluation and implementation of specific treatment options concerning cannabis use disorders will be increasingly needed in the future. Cognitive-behavioral and motivational interventions have been shown to be evidence-based treatments. The paper describes some promising or already successful treatment approaches and related research projects in Germany. 相似文献

Zusammenfassung Vierergruppen von Vpn arbeiteten in Kommunikationsnetzen, in denen es infolge des Fehlens einer (direkten oder indirekten) Wechselseitigkeit gewisser Verbindungen einigen Vpn — nämlich denjenigen, die ganz bestimmte (ungünstige) Positionen innerhalb des Netzes einnahmen — verwehrt war, die Lösung eines Problems durch Austausch von Informationen zu finden. Den Vpn-Gruppen wurde diese Eigenschaft der Kommunikationssysteme bei Beginn der Versuche nicht mitgeteilt. In einer derartigen Lage suchen jene Vpn den Grund für den unbefriedigenden Informationsaustausch (zumindest zunächst) nicht in der Struktur des Kommunikationsnetzes, sondern unterstellen den anderen Gruppenmitgliedern mangelnde Loyalität oder/und mangelndes Verständnis. Die Folge sind starke affektive Spannungen, die zur Verschlechterung der Kooperation und zur Verringerung der Gruppenleistung führen. Die kognitive Seite des Erlebens der Vpn wurde u. a. mit Hilfe der Begriffe phänomenale Variabilität und phänomenale Konstanz näher analysiert. 相似文献
